Objective To evaluate the pathophysiology of paraplegia during transarterial chemoembolization in liver cancer and investigate effective management and prevention for improving clinical situation and relieve major symptoms.Method 2758 patients accepted TACE procedure because of liver cancer(and/or combined with remote metastasis),demonstrated 4 cases suffering from paraplegia(3 males,1 female)since Mar.2003 to Feb.2005,with mean age of(51?14)years old,The operative records and the clinical features after chemoembolization were summarized in detail. Results The incidence of paraplegia was 0.145%,with major symptoms of dysesthesia and hypokinesise emerging within 4 hours after TACE,and most symptoms aggravating gradually within 24-48 hours untill paraplegia appeared, and then turned to stabilization about 5-7 days, with their body functions partial recovery in 2 months. Conclusion More attention should be paied to prevent ectopic embolization of spinal cord vessels originating from extrahepatic collateral arteries during TACE or TAE.

Objective To investigate the clinical value of new transnasal Fujifilm EG-530-NW electronic gastroscope. Methods A total of 295 patients who underwent gastroscopy were randomly divided into 2 groups to receive examination by routine endoscope (n =172) or Fujifilm EG-530-NW transnasal endoscope (n =123).The fluctuation of heart rate, blood pressure and compliance during the examination were recorded and compared between the two groups. Results The new Fujifilm EG-530-NW could obtain clear images with same quality of routine gastroscope. The heart rate change and visual analog pain score in patients from EG-530-NW gastroscope group were significantly lower than those of routine gastroscope group (P < 0.05).No significant difference in blood pressure fluctuation was observed between the two groups. In patients older than 50 years, the fluctuations of heart rate and blood pressure in EG-530-NW group were significantly less than those of routine gastroscope group. Conclusion Fujifilm EG-530-NW gastroscope can get high quality image with less influence on heart rate and blood pressure fluctuation, which is more significant in older patients.

OBJECTIVETo investigate the efficacy mechanisme of chicory extract interventing abdominal obesity rat from the aspect of gut bacteria.
METHODMale SD rats were randomly divided into five groups, namely the normal group, model group, large and small dose group of chicory and the fenofibrate group. Normal group was given deionized water, the other group was given fructose water and give the medical treatment of chicory and fenofibrate. Assay triglycerides, total cholesterol, LDL and HDL by biochemical methods and measure body weight and abdominal circumference and microscopicly observe the count changes of gut bacteria through real-time PCR method.
RESULTCompared with normal group, the triglyceride level and abdominal circumference were significantly higher (P < 0.05), weight and high-density lipoprotein increased but no significant changes and E. coli, lactobacillus increased significantly. Compared with model group, chicory extract large and small dose group and the fenofibrate group can significantly reduce triglyceride levels (P < 0.05), reduce the number of E. coli and Lactobacillus and increase the number of bifidobacteria. The fenofibrate group can significantly reduce total cholesterol and high-density lipoprotein levels.
CONCLUSIONThe chicory's treatment effect on abdominal obesity is significant. The efficacy mechanisme intervention abdominal obesity may be related to the reduction of the number of lactic acid bacteria and E. coli and the increase of bifidobacteria.

Chinese Journal of Digestive Endoscopy 2012;29(5):255-258
ObjectiveTo evaluate the efficacy and safety of endoscopic submucosal dissection (ESD) for the treatment of laterally spreading tumors (LST).MethodsESD was applied to remove 32 colorectal LSTs larger than 2 cm.The characteristics of the tumors and clinical results including en bloc resection rate, procedure time, complication and recttrrence rates were retrospectively evaluated. Results The lesions ranged from 2.5 cm to 8.0 cm,with a mean diameter of 4.1 ± 2.1 cm.En bloc resection wasachieved in 29 patients (90.6% ) with a mean operation time of 75.7 ±66.0 min.Immediate arerial bleeding occurred in 5 ( 15.6% ) cases but was stopped successfully by clips or coagulations.Late bleeding occurred in 2 ( 6.2% ) and perforations in 3 (9.4% ),which were closed successfully by clips without surgery.Pathological diagnosis revealed low-grade intraepithelial dysplasia in 19 (59.4% ),high-grade intraepithelial dysplasia in 6 ( 18.8% ),and carcinomas in 7 (21.9%).The lesions were restricted in mucosal layer in 25 (78.1% ),infiltrating into sm1 layer in 5 ( 15.6% ) and sm2 layer in 2 (6.2% ),and the later 2 were referred to surgery.Twenty patients were followed up for 3-12 months, and no local recurrence was found..ConclusionESD was an effective and safe therapy for colorectal LST larger than 2 cm.
