1.Initiation of Punitive Compensation System in Drug Field in China

OBJECTIVE:To initiate punitive compensation system in drug field to make up the insufficiency in the legislation of legal liability of drug injury events,regulate unlawful act of pharmaceutical enterprises and enhance supervision efficiency in drug field. METHODS: The definition and function,etc. of the punitive compensation system were studied by reviewing the pertinent legal clauses both at home and abroad,furthermore,the necessity of initiating the punitive compensation system in China was analyzed and the application conditions of the punitive compensation system was put forward. RESULTS & CONCLUSIONS: It is urgent to initiate the punitive compensation system in drug field and establish a punitive system with Chinese characteristics. It is advisable to initiate the related contents in Pharmaceutical Administration Law of the People’s Republic of China and define the application conditions of the punitive compensation system as well as the way to establish the amount of compensation,etc.
2.Effects of paeoniflorin on cerebral blood flow and the balance of PGI2/TXA2 of rats with focal cerebral ischemia-reperfusion injury.

This study is to investigate the effects of paeoniflorin on cerebral blood flow and the balance of PGI2/TXA2 of rats with focal cerebral ischemia-reperfusion injury. A total of 72 SD rats (3) were randomly divided into 6 groups: sham operation group, cerebral ischemia-reperfusion model group (I/R gourp), low (10 mg.kg-1), middle (20 mg.kg-1) and high (40 mg.kg-1) doses of paeoniflorin groups and nimrnodipine group. Focal cerebral ischemia in rats was made by inserting a monofilament suture into internal carotid artery for 90 min and then reperfused for 24 h. The effects of paeoniflorin on neurological deficit scores and the infarction volume of brain were detected. Relative regional cerebral blood flow (rCBF) was continuously monitored over ischemic hemispheres by laser-Doppler flowmetry (LDF). The expression of COX-2 in hippocampal CAl region was estimated by immunohistochemistry and the contents of prostacyclin I2 (PGI2), thromboxane A2 (TXA2), and ratio of PGIJ2/TXA2 in serum were measured by ELISA kits. Paeoniflorin significantly ameliorated neurological scores, reduced the infarction volume, and increased regional cerebral blood flow relative to the I/R group. In addition, paeoniflorin could inhibit COX-2 expression and the release of TXA2 and prevent the downregulation of PGI2 induced by I/R injury. The neuroprotective effects of paeoniflorin against focal cerebral ischemia-reperfusion rats might be attributed to improve the supply of injured hemisphere blood flow and adjust the balance between PGI2/TXA2.

3.Surgical staging applications with antibiotic graft bone for the treatment of open calcaneal fractures.

OBJECTIVETo discuss the effect of surgical staging and using craft bone with vancomycin for the treatment of calcaneal fractures.
METHODSFrom January 2006 to December 2012,13 patients with open calcaneal fractures were treated including 9 males and 4 females with an average of 35.2 years old ranging from 23 to 66. All cases were emergency cases. According to Sanders classification of calcaneal fractures, 2 cases were type II, 7 cases were type III, 4 cases were type IV. According to Gustilo-Anderson soft tissue injury classification, 8 cases were type II, 2 cases were type III A, 2 cases were type III B, 1 case were type III C. Firstly a thorough debridement or VSD procedures were applied,secondly calcaneal fracture were treated with open reduction, plate fixation and bone graft complex with antibiotics. Based on clinical examination, radiographic evaluation, and American Foot and Ankle Surgery Society (AOFAS), ankle function were evaluated after operation.
RESULTSOpen wounds were headed after dressing and repairing,, lateral calcaneal wound were healed during the first period. All patients were followed up for 6 to 36 months (means 14.5 months). Fracture healing time was 14 to 20 weeks (means 16.2 weeks). Last follow-up AOFAS ankle-hindfoot score was (80.0 +/- 7.4) ranging from 55 to 95.
CONCLUSIONFor patients with open fractures, through reasonable clinical evaluation, staging operation, using bone graft with antibiotics can reduce the incidence of postoperative wound infection and promote fracture healing.

4.Effects of Shenmai injection on the regulation of heart CYP450 isozymes in rats

Aim To investigate the influence of Shen-mai injection ( SMI ) on the expression of cytochrome P450(CYP450) system in rat′s hearts. Methods Rat hearts were prepared after a fourteen-day continuous administration of SMI. The expression of several CYP genes, ANP, BNP and EPHX2 were measured by qPCR. Results SMI induced the increase in the ex-pression of other CYP genes except CYP2 B1、CYP4 A3 and CYP4 F6;HSI caused an induction of CYP2 E1 , CYP4A3,CYP4F1 and EHPX2 as compared with the control. In addition, there was a significant induction of ANP, BNP and EHPX2 and a significant inhibition of CYP2B1 and CYP2C11 after treated with MDI. Conclusion Although there is no significant change in the gene expression of CYP2 B1 after the treatment with SMI, but there is a general trend of induction, and MDI shows a significant inhibition of CYP2 B1 , therefore HSI has greater effect on CYP 2 B 1 than MDI . SMI causes a significant induction of CYP2 E1 , CYP4F1 and EHPX2 , similarly there is an induction of CYP2E1,CYP4F1 and EHPX2 by HSI and MDI, indi-cating that Hongshen and Maidong are both involved in the induction. MDI has a greater inductive effect than HSI on ANP and BNP. SMI is widely used for the treatment of cardiovascular diseases due to its regula-tion of CYP2J3、ANP and BNP mRNA expression.
5.Comparative study of dissolved matter of glycyrrhizic acid before and after supermicro-pulverization of Radix Glycyrrhizae.

OBJECTIVETo investigate various activities of dissolved matter of glycyrrhizic acid of Radix Glycyrrhizae and the powder by supermicro-pulverization.
METHODThe contents of glycyrrhizic acid in different samples were tested.
RESULTThe dissolved matter of glycyrrhizic acid was greatly increased by supermicro-pulverization. The more time used for grinding, the smaller the size of the powder, and the easier the glycyrrhizic acid would be dissolved.
CONCLUSIONSupermicro-pulverization is helpful to the dissolved matter of glycyrrhizic acid of Radix Glycyrrhizae, and the size of powder exerts great influence on dissolved matter of glycyrrhizic acid.

6.Accuracy of three different electronic apex locators in determination of perforation with various conditions in vitro.

OBJECTIVETo evaluate accuracy of Propex, Raypex 5, Root ZX electronic apex locator in positions of the perforation with different irrigations in the root canal.
METHODSPerforation lengths were measured with Propex, Raypex 5, Root ZX electronic apex locators in 19 extracted human teeth embedded in model after simulate perforation preparation by ultrasonic ET40 and compared with the actual canal length measurements taken before embedding the teeth in model. Measurements were taken with the different canal contents. RESULTS; Propex, Raypex 5, and Root ZX could locate the positions of the perforation with different irrigations in the root canal. The differences between actual root canal perforation length and measured root canal perforation length of same electronic apex locators in different irrigations were no statistically significant (P > 0.05). Meanwhile, the differences between actual root canal perforation length and measured root canal perforation length of three kinds of electronic apex locators in same irrigations were not statistically significant (P > 0.05).
CONCLUSIONPropex, Raypex 5, and Root ZX electronic apex locators can detect perforation accurately.

7.A matched case-control study on the risk factors of metabolic syndrome among policemen.

OBJECTIVETo investigate the life style, genetic and occupational risk factors of metabolic syndrome (MS) among policemen.
METHODS1:4 matched case-control study was used, based on physical examination data of Tianjin Policemen in 2010, 708 patients with MS were randomly selected as cases, which were matched with 2832 healthy controls on the basis of sex and age (+/- 1 year). An epidemiological investigations on the past exposure status of several possible risk factors was conducted, and the data were analyzed with conditional logistic regression.
RESULTSFifteen factors related to exposure were identified for MS through univariate conditional logistic regression analysis. Multivariate conditional logistic regression analysis suggested that, seven factors, such as family history of hypertension (OR = 2.406, 95% CI: 1.946-2.975), family history of diabetes (OR = 1.301, 95% CI: 1.043-1.623), smoking (OR = 1.357, 95%CI: 1.010-1.823), snoring (OR = 1.268, 95% CI: 1.043-1.543), work intensity (OR = 4.603, 95% CI: 3.767-5.623), occupational stressful events (OR = 1.524, 95% CI: 1.209-1.922), security policemen (OR = 1.453, 95% CI: 1.127-1.872) and criminal investigation policemen (OR = 2.792, 95% CI: 2.168-3.596), could significantly increase the risk of disease development, but dairy products (OR = 0.782, 95% CI: 0.619-0.989) was a protect factor for MS. The results from population attributable risk factors analysis showed that the control of smoking, snoring, work intensity, occupational stressful events can decreased the risk of MS to 16.26%, 11.71%, 56.87% and 8.97%, respectively.
CONCLUSIONMetabolic syndrome has became a significant public health problem among policemen, it's necessary to take measures on life style, occupational risk factors for reducing the incidence of MS, and improving the health level among policemen.

8.Digestive system manifestations in children infected with novel influenza A (H1N1) virus.

OBJECTIVETo study the digestive system manifestations in children infected with novel influenza A (H1N1) virus.
METHODSA prospective study of 153 children infected with novel influenza A (H1N1) virus in Shenzhen Children's Hospital from November 2009 to January 2010 was conducted. The clinical features and outcomes of 69 children with digestive system manifestations were analyzed.
RESULTSThe children presenting with digestive system manifestations accounted for 45% (69 cases) in the 153 hospitalized children with novel influenza A (H1N1) infection. Gastrointestinal manifestations were observed in 50 cases (33%) and liver function abnormality in 19 cases (12%). The incidence rate of coma, neurological complications, increase in creative kinase level, ICU admission, and death in the patients with digestive system manifestations were significantly higher than those without digestive system manifestations (P<0.05). In the 69 patients with digestive system manifestations, 5 died from severe complications and 64 recovered fully. Gastrointestinal manifestations disappeared through 1 to 3 days and abnormal liver function recovered through 4 to 7 days.
CONCLUSIONSDigestive system manifestations are common in children infected with novel influenza A (H1N1) virus. Neurological system involvements are more common in the patients with digestive system manifestations than those without.

9.Analysis of parents' compliance in non-hospital settings during operation of expressed breast milk bank.

OBJECTIVETo investigate the parents' compliance in non-hospital settings during the operation of expressed breast milk bank.
METHODSIn September 2014, a questionnaire survey was carried out to investigate the parents' willingness about feeding the inpatient neonates with maternal expressed breast milk, to evaluate the effectiveness of the breast milk feeding supporting system, and to monitor the compliance in non-hospital settings during the delivery of maternal expressed breast milk. Improvements in education were made according to the results. A second survey was done in September 2015.
RESULTSA total of 340 questionnaires were sent out, and 338 usable questionnaires were returned. According to the time when the questionnaires were sent out, they were divided into two groups: 2014 group (n=229) and 2015 group (n=109). The age of most mothers was 20-30 years in the 2014 group and 30-40 years in the 2015 group. Most mothers delivered at the West China Second Hospital of Sichuan University in both groups, but the 2015 group had a significantly higher proportion than the 2014 group (74.3% vs 61.6%; P<0.05). Guidance was given to mothers in the presence of insufficient breast milk production in both groups, but the 2015 group had a significantly higher proportion than the 2014 group (91.7% vs 79.9%; P<0.05). Both groups had good family compliance in the collection, storage, and transport of breast milk. There were no significant differences in their compliance with washing hands, sterilizing instruments, and using a clean special refrigerator between the two groups. The expressed breast milk was transported strictly according to the procedure in both groups, but the 2015 group had a significantly higher proportion than the 2014 group (100% vs 87.1%; P<0.05).
CONCLUSIONSBefore and after improvements in the health education, most parents have good compliance in the collection, storage, and transport of breast milk.

10.Methodological study on microbial limit test of zinc oxide and talcum powder lotion

Objective To establish a method for the microbial limit test of zinc oxide and talcum powder lotion in Chinese Pharmacopoeia 2015.Methods Microbial enumeration test and specified microorganisms test with instructions were conducted from Chinese Pharmacopoeia 2015,which involved 10 batches zinc oxide and talcum powder lotion and five species of bacteria in all.The samples were treated by centrifugation and membrane filtration.Microbial enumeration test:the total number of aerobic bacteria using TSA medium to examine,strains were Staphylococcus aureus,Pseudomonas aeruginosa,Bacillus subtilis,Candida albicans,and Aspergillus niger;The total number of molds and yeasts using SDA culture medium to examine,strains were Candida albicans and Aspergillus niger.Each strain was divided into two groups:Adding bacteria test group and bacteria control group.Samplecontrol group and negative control group of each culture medium was prepared respectively.Count the colonies and calculate the rate of recovery.Specified microorganisms test:Pseudomonas aeruginosa and Staphylococcus aureus were applicated and set the bacteria validation group,sample control group,and negative control group.The culture medium of each group was crossed on the corresponding medium plate,and Microflex LT mass spectrometer was used to identify the bacteria.Results The recoveries of all kinds of strains in microbial enumeration test,total yeast and mold count were 0.75-1.16 in all batches.In the validations of Staphylococcus aureus and Pseudomonas aeruginosa,all kinds of strains were respectively detected in bacteria validation group,and there were no bacterial growth in sample control group and negative control group.Conclusion The microbiological examination methods for zinc oxide and talcum powder lotion can meet the requirements of Chinese Pharmacopoeia 2015.
