National Journal of Andrology 2014;20(8):675-678
Androgen deprivation therapy can effectively suppress the progression of prostate cancer, but accumulating evidence for the relationship of testosterone with prostate cancer challenges the conventional wisdom. High levels of testosterone are not risk factors for prostate cancer, nor promote its development. On the contrary, a low testosterone level indicates a worse pathological stage. So far there has been no strong evidence to prove the role of testosterone in the occurrence and progression of prostate cancer. Therefore, the relationship between testosterone and prostate cancer is quite complicated and deserves further investigation.

Objective To investigate the clinical significance of early lactate clearance rate in respect of the prognosis of patients with severe chronic obstructive pulmonary disease (COPD) . Methods A total of 108 COPD patients with respiratory failure were studied prospectively. The patients were divided into survival group ( n = 82) and the death group ( n = 26) , and the survival group further divided into the conventional treatment subgroup ( n = 55 ) and the invasive ventilation subgroup ( n = 27 ) . Arterial blood lactate, arterial gas analysis, APACHE Ⅱ score and the 6-hour clearance rate were measured before and 6 hours after treatment in 3 groups. The high lactate clearance rate group (n =71 ) was defined as a lactate clearance rate > 10% , and the low lactate clearance rate group (n = 37) with a lactate clearance rate < 10%. The correlation between 6-hour lactate clearance rate and prognosis in patients was analyzed. Results Compared the survival patients in invasive ventilation group with the death group, there were no statistical differences in arterial blood lactate, pH value, PaCO2, PaO2, oxygenation index and APACHE Ⅱ score before the treatment. The 6-hour lactate clearance rate in survival patients in conventional treatment group or invasive ventilation group was significantly higher than that in the death group (t =31.2, P<0. 01; t =34.6, P<0. 01, respectively), while there were no statistical differences in APACHE Ⅱ score and pH value between these 3 groups. Between the conventional ventilation treatment group and the invasive ventilation treatment group, there was no statistical difference in 6-hour lactate clearance rate ( P > 0. 05 ) . There was no statistical difference in APACHE Ⅱ score between before treatment and after treatment in 3 groups. Mortality in high lactate clearance rate group (4. 2% ) was significantly lower than that in low lactate clearance rate group (62. 2% ) (x2 = 121.9, P < 0. 01 ) .Conclusions The early (6h) lactate clearance rate could be used as a marker for evaluating the severity, therapeutic efficacy and prognosis of patients with severe COPD.

Plasma exchange is a commonly used blood purification method, h includes plasma exchange, double filtration plasmapheresis and immunoadsorption. Many nervous system diseases have the antibody. Guillian-Barre syndrome is caused by antibodies of the nervous system demyelinafing disease; Myasthenia grads is caused by the acetylcholinc receptor antibodies. CIDP also has autoimmune-mediated mechanism. Plasma exchange can be apphed effectively to remove antibodies, and to quickly improve the patient' s clinical symptoms and shorten the course of disease.

Objective To investigate the correlation between the plasma unconjugated bilirubin(UCB)and electrical activity of brain in term neonates with hyperbilirubinemia, and explore the parameters that possess the prognostic value and deduce a series of discriminant for prognostic judgment. Methods Thirty-two neonates with hyperbilirubinemia and 31 neonates without hyperbilirubinema(control group)were studied with standard EEG,and they were assessed for their neurological development in 3 and 6 months.A statistical method of Bayes Discriminant analysis was adopted to set up an equation with values of parameters of EEG,clinic datum and consequence.Stepdic and Dis-grim processes were used to pick out the parameters which had great significance for predicting the outcome of nervous system development.A series of discriminant was deduced for prognostic judgment and then cross validation was performed to assess the accuracy of the discriminant. Results 1.The standard EEG and brain electrical activity map records indicated that the hyperbilirubinemia group had longer maximum duration of interburst interval(IBI),longer mean duration of IBI,lower amplitude under all conditions, more anterior dysrhythmia(AD)in wake, and higher incidence of sleep-wake cycle disturbance.2.The plasma concentration of UCB was linear correlated with AD in wake.3.Gestational age, birth weight, plasma concentration of UCB,burst duration of IBI in trace alternate, amplitude in all sleep state,AD in wake, brief burst not in quiet sleep and sleep-wake cycle disturbance might play important roles in predicting the outcome of neonates with hyperbilirubinemia. Conclusions Cerebral electrical activity is an effective tool for predicting neurologic outcome of neonates with hyperbi-lirubinemia.Cerebral electrical activity and clinic data will provide a new method for accurately predicting neurological development and prognosis.

Background Congenital heart disease (CHD) is a diverse group of diseases determined by genetic and environmental factors.Considerable research has been done on genes associated with the development of the heart.Recently, focus is on the role of transcription factor NFATc1 in the development of proper valve and septa. As part of a larger study, high density single nucleotide polymorphism (SNP) scanning was used to explore the relationship between NFATc1 gene polymorphism and susceptibility to ventricular septal defect (VSD) in the Chinese Han population. Methods One hundred and ninety-two pediatric patients with congenital VSD and 192 matching healthy control subjects were studied. The haplotype reconstructions were calculated by PHASE2.0 software.Haploview software was used to perform linkage disequilibrium assessment and define haplotype blocks. The algorithm used for defining the blocks was the confidence interval method. Results The NFATc1 gene region can be divided into 11 haplotype blocks.Strong linkage disequilibrium existed within blocks 6,8,9,and 11.Three SNPs (rs7240256,rs11665469,and rs754505) within the NFATc1 gene had significant correlation with VSD by single marker association analysis. In addition, two haplotypes correlated with VSD.Conclusions NFATc1 is associated with the occurrence of VSD and it may be a predisposing gene to CHD in Han Chinese. This finding has set a direction for further genetic and functional studies.

OBJECTIVE:
To observe the chemical groups changing in rat kidney with regard to fatal hyperthermia by Fourier transform infrared microspectroscopy (FTIR-MSP) and to provide a new method to diagnose fatal hyperthermia.
METHODS:
Rats were sacrificed by hyperthermia, brainstem injury, massive hemorrhage and asphyxiation and divided into groups. The renal samples were dissected immediately after death. The data of infrared spectroscopy in glomerulus were measured by FTIR-MSP.
RESULTS:
The absorbances of 3290, 3070, 2850, 1540 and 1396 cm(-1) significantly increased (P < 0.05), and the ratios of Al650/A3290 and A1650/A1540 significantly decreased (P < 0.05) in group of hyperthermia.
CONCLUSION
FTIR-MSP can analyze the changes of chemical groups of kidney as an auxiliary diagnosis for discriminating hyperthermia with other causes of death.
