1.Study on hepatoprotective effects of total flavonoids in Scorzonera austriaca Wild in vivo and in vitro

Objective To investigate the hepatoprotective effects of total flavonoids in Scorzonera austriaca Wild (TFSA) in vivo and in vitro. Methods In vivo, ICR mice were randomly divided into negative, model, positive, TFSA’s low-dose, medium-dose and high-dose groups,and acute chemical liver injury models were constructed with CCl4 and acute autoimmune liver injury models with Bacillus Calmette-Guerin vaccine ( BCG ) and lipopolysaccharide (LPS).The activity of serum aspartate aminotransferase (AST), alanine aminotransferase (ALT), lactate dehydrogenase (LDH) was detected and liver tissue was used as biopsy.In vitro, liver cells of Wistar rat were extracted and isolated by orthotopic collagenase digestion method, and liver cell damage was induced with CCl4.Then the liver cells were cultured with TFSA solution and the contents of AST, ALT, LDH, nitric oxide (NO), superoxide dismutase (SOD) in the supernatant and malondialdehyde (MDA) in rat hepatocyte were detected.Results The results of CCl4-and BCG+LPS-induced acute chemical liver injury models in mice showed that there were less microstructures damage of liver tissue in TFSA groups compared with model group in liver pathological sections (HE), AST, ALT and LDH levels in model group were significantly higher than those in negative group (P<0.01), the above indexes in positive drug group were significantly lower than those in negative group (P<0.01,P<0.05), and the above indexes in TFSA’s low-dose, medium-dose and high-dose groups were significantly lower than those in model group(P<0.01,P<0.05).The results of CCl4-induced rat hepatocyte injury in vitro showed that AST, ALT, LDH, MDA and NO levels were significantly higher and SOD level was lower in model group than those in negative group (P<0.01), AST, ALT, LDH, MDA and NO levels were significantly lower and SOD level was higher in positive drug group than those in model group (P<0.01, P<0.05), and AST, ALT, LDH, MDA and NO levels were significantly lower and SOD level was higher in TFSA ’ s low-dose, medium-dose and high-dose groups were significantly lower than those in model group ( P <0.01, P <0.05 ) .Conclusion TFSA have hepatoprotective effects on CCl4-induced chemical liver injury and BCG+LPS-induced immune liver injury in mice, and rat hepatocyte damage.This study provides experimental data for the development and utilization of Scorzonera austriaca Wild resources and new hepatoprotective medicines.
2.Preparation of 68Ga-PSMA-617 and its microPET imaging in BGC-823 cell bearing mice

Objective To prepare 68Ga-PSMA-617 and perform its microPET imaging on both normal BALB/c mice and BGC-823 (PSMA expression) tumor bearing mice.Methods 68GaCl3 was eluted from 68Ge-68Ga generator by 0.05 mol/L HCl,then added to the DKFZ-PSMA-617 and heated at 85 ℃ for 5 min.The labeling efficiency and in vitro stability of 68Ga-PSMA-617 in sodium chloride solution and HAS were analyzed by radio-HPLC.Water partition coefficient and plasma protein binding rate were also evaluated.MicroPET imaging was performed in normal female BALB/c mice and human gastric tumor (BGC-823) bearing mice at 60 min post-injection of 68Ga-PSMA-617.18F-FDG was also injected to BGC-823 tumor bearing mice to acquire microPET imaging for contrast.Results The labeling yield of 68Ga-PSMA-617 was 97.9%,and it could be used directly without purification.68Ga-PSMA-617 showed good in vitro stability in sodium chloride solution and 5% HAS,the radiochemical purities were 94.9% and 81.0% respectively at 80 min post-incubation.68Ga-PSMA-617 was water-solubility substance,and it cleared mainly through the kidneys.MicroPET imaging showed that 68Ga-PSMA-617 could be accumulated in tumor (T/NT=2.28),which was better than 18F-FDG.Conclusions Preparation of 68Ga-PSMA-617 is convenient and has a high labeling yield.It can specifically target to PSMA expression tumors and has a promising prospect in clinical application.
3.Correlation analysis on combined medication with of Xiyanping injection in treatment of lung infection in real world.

To analyze the regularity in combined medication with Xiyanping injection (Xiyanping for short) in the real world by as- sociation rules. Totally 5 822 patients using Xiyanping injection was collected from the 18 Class III Grade I hospitals nationwide to study the combined medication information of the patient with lung infection and make the analysis by using association rules and Apriori. According to the results, major drugs combined with Xiyanping in treatment of lung infection included compound amino acid, inosine, coenzyme A, cytidine triphosphate, vitamin C. Common drugs combined with Xiyanping can be divided into 5 categories: nutrition support therapy (vitamin C, compound amino acid) , coenzymes (coenzyme A, cytidine triphosphate, inosine), expectorants and antiasthmatics (ambroxol, salbutamol, doxofylline), hormones (dexamethasone, budesonide), antibiotics (mainly cefminox). The main combined medicines mostly conformed to the regularity for drugs treating lung infection. In addition, there were two most common medical combination models: the model for Xiyanping combined a single medicine is Xiyanping + nutrition support therapy, while the model for Xiyanping combined two or more than two medicines is Xiyanping + nutrition support therapy + coenzyme. Pharmacologically, Xiyanping is mostly combined with western medicines with similar pharmacological effects to substitute or supplement the antibiotic effect in treating lung infection. However, further studies shall be conducted for the safety and rationality of the combined medication based on clinical practices, in order to provide reference for clinical medication.

4.Study on medical pattern of traditional Chinese medicine and western medicine diagnosis and treatment of hypertension patients in 30 034 cases in real world.

Select patients diagnosed of hypertension in the first place in 16 hospitals and the patients were described by association rules analysis and distribution for the analysis in the study,in order to understand the information of diagnosis and treatment in hypertension patients in real world. The information include age, gender, admission condition, inpatient department, hospitalization expenses, western medicine comorbidities, the traditional Chinese medicine (TCM) syndrome and the medicine. The average age of patients was 64 years in the study. The proportion of men to women about 1. 10: 1. Admission condition is critical for about 10.12% of the total and more concentrated in the cardiovascular department. Hospital stay ranged from 8 to 14 d. Inpatients with medical insurance is 62.93%. Total hospitalization cost distribution most is the 5 000-10 000 RMB. Hypertension complicated with coronary heart disease, blood lipoprotein disorder disease; phlegm and blood stasis and Yin deficiency of liver and kidney are the most in TCM syndrome type. Promoting blood circulation and removing blood stasis is the major in Chinese medicine treatment. Western medicine treatment basically conforms to the guidelines. The most antihypertensive drugs is calcium antagonistst, accounting for about 81.2% of the total number. Class ACEI, class ARB, beta blockers, diuretics class are accounted for 43.0%, 43.4%, 42.4%, 42.4%, fixed compound is only 2 393, accounting for about 8%. the total frequency of five classes of antihypertensive drugs is 78 206 times. The principal conclusions of this analysis are as follows: the elderly people is the most in hypertension, more men than women; medical insurance is the majority type in hospitalization cost; nearly half of the population is combined with coronary heart disease; phlegm and blood stasis and yin deficiency of liver and kidney are the most in TCM syndrome type; western medicine treatment basically conforms to the guidelines. Combination therapy is more, less fixed compound preparation in western medicine. Chinese medicine is mainly to promoting blood circulation. Suggesting that the clinical treatment in strict blood pressure and at the same time, pay attention to combination therapy and the use of fixed compound preparation,it's useful to control the blood pressure better. Combined with traditional Chinese medicine and resolving phlegm and removing blood stasis, reduce the target organ damage, reduce complications, the prevention and control work of hypertension in China still faces enormous challenges at present. The result provides reference information for clinical medicine and medical research.

5.Synthesis and biodistribution of 99Tcm-Cetuximab antibody for lung tumor targeted molecular imaging

Objective To prepare and evaluate 99Tcm radiolabeled Cetuximab (C225) for imaging of EGFR specific binding.Methods Cetuximab antibody was reduced by 2-iminothiophene (2-IT).The radiolabeling of IT-Cetuximab with 99Tcm(99Tcm-IT-Cetuximab) was analyzed by HPLC,and was tested for in vitro stability and molecular integrity.The human lung cancer line (A549)-bearing nude mouse model was prepared for biodistribution and tumor targeted study.Tumor uptake was also measured by in vivo γ imaging.Results The labeling efficiency was 93.15%.The radiochemical purity was 96.46% after purification.The in vitro stability was good in 5% HSA,in which the radiochemical purity maintained above 80% at 4 ℃ for 24 h.99Tcm-IT-Cetuximab showed good specific binding to tumor with peak uptake of (3.417±0.769) %ID/g after 4 h.The T/NT ratio of blood increased to 1.454±0.174 at 24 h.γ imaging of A549-bearing nude mice xenografts also showed high T/NT ratio.Conclusions 99Tcm-IT-Cetuximab molecular probe could be prepared with high radiolabeling yield and radiochemical purity.It has excellent in vitro and in vivo stability,and shows specific uptake in A549 tumor.
6.Analysis of factors related to perforation of lateral wall by lower cervical vertebral pedicle screw

ObjectiveTo analyze the risk factors related to perforation of lateral wall by lower cervical pedicle screw instrumentation in the treatment of cervical spinal disorders. MethodsA retrospective review was made to analyze 214 cervical vertebral injury patients ( 1 024 pedicle screws were used) treated with pedicle screw instrumentation at C3-7 from July 2004 to July 2009. Lateral walls of 18 patients were perforated (a total of 28 pedicle screws). The surgeons assessed the position and the angle of the screw in the pedicle, and the relation and the distance between the screw and the pedicle walls by carefully probing intraoperative walls of cervical pedicles and studying postoperative thin-slice computed tomography (CT) scan of the fixed vertebral segments of the cervical spine. The data of patients with lateral wall perforation was recorded and analyzed statistically. ResultsPerforation of the lateral wall occurred in 18 patients (28 pedicle screws). Backward step by step Logistic regression analysis was used and two variables were selected in the end. ConclusionsRatio variance between inner and lateral wall is the risk factor of perforation in the lateral wall associated with lower cervical pedicle screw instrumentation, while the angle variance between implanted screws and CT measurements is the protection factor of perforation in the lateral wall.
7.Detection of Fonsecaea, Sporothrix and Penicillium marneffei in paraffin wax embedded tissues by nested PCR

Objective To evaluate the performance of nested PCR in the detection of different fungi in paraffin wax embedded tissues. Methods Forty-four tissue samples were resected from rats infected with Fonsecaea monophora, patients with chromoblastomycosis, sporotrichosis or penicilliposis marneffei followed by preparation of paraffin wax embedded tissue sections for pathological examination and DNA extraction. Nested PCR was performed by using specific primers targeting the ribosomal DNA of Fonsecaea, Sporothrix and Penicillium marneffei, respectively. The sensitivity and specificity of nested PCR were analyzed and compared with those of pathological examination. Results The nested PCR showed positive results in 8 of 20 samples from rats with chromoblastomycosis, 7 of 10 samples from patients with sporotrichosis and all of the 10 samples from patients with penicilliposis marneffei, but not in the control samples. In the detection of Fonsecaea,Sporothrix schenki and Penicillium marneffei, the sensitivity was 40% ,70% and 100%, respectively, and the specificity was consistently 100%, for the nested PCR. Pathological examination revealed fungal elements in 95%, 70% and 80% of the corresponding samples, respectively. Conclusion Detection of fungal DNA in paraffin wax embedded tissue by nested PCR can be applied to the diagnosis of deep mycosis, especially to the diagnosis of penicilliposis marneffei.
10.Clinical analysis on combined medication in patients of acute pancreatitis in real world.

OBJECTIVETo study the characteristics and regular pattern of the medicine in common use and combined medication in patients of acute pancreatitis in real world.
METHODCollect the information of 5 433 acute pancreatitis patients in 19 grade IIIA general hospitals in China, analysis by descriptive statistics and association rule.
RESULTIn the 5 433 patients of acute pancreatitis, the glycyrrhizic acid injection and somatostatin are the frequency top used Chinese traditional and western medication. Glycyrrhizic acid injection, somatostatin and insulin are the frequency top used drug combination pattern.
CONCLUSIONThe Chinese and western integrative medicine drug use pattern are accord with the clinical guideline of acute panceatitis. The hepatic and renal function, blood routine and coagulation function should be monitored when the medicines are used.
