Objective To analyze the status of multiple organ failure in patients with acute poisoning and to investigate the relationship between sequential organ failure assessment (SOFA) and prognosis. Method A retrospective review of 76 patients admitted to emergency intensive care unit in the First Affiliated Hospital of China Medical University for acute poisoning was carried out. Data about heart rate, blood pressure, blood oxygen saturation, blood gas analysis, rutine blood examination (WBC,platelet,albumin, ALT,total bilirubin, BUN,creatinine, CK, CK-MB, blood glucose), dosage of vascular active agents and mental status were documented at admission and 48 h after admission. SOFAs were counted at admission (SOFA ) and 48 h after admission (SOFA ) and then a comparison was made between survivors and victims. Results There was no significant difference in SOFA between survivors and victims ( P = 0.26) , but significant difference in SOFA48 as well as SOFA was shown between survivors and victims (P < 0.01). Multivariate logistic regression analysis indicated that the SOFA48 of renal and nervous systems were the independent predictors with acute poisoning (P = 0.003 and 0.012 respectively). Spearman correlation analysis implicated that ALT,CK-MB, WBC,base excess (BE) and blood glucose (BG) had correlation with SOFAs48. Conclusions SOFA shows its score eorrellated with the prognosis of poisoning with a-cute poisoning and it is a practical method in eveluating the outcome of patients with acute poisoning.

Adrenomedullin (ADM) has wide distribution in the body and has many biological effects,which participates in many physiological and pathological processes.What's more,it is closely related to the degree of malignancy,prognosis and metastasis of tumors.ADM has different effects on different types of cells via one or more signal pathways,so that can product various effects.It also provides a new treatment strategy for preventing or slowing the progress of the cancer.

Background Trachoma is a common infectious eye disease,and its incidence is associated with region,economy and sanitary condition.To survey the prevalence status of an area is very important for the prevention and treatment of trachoma.Objective Aim of this study was to survey the prevalence of trachoma in primary and secondary school students of Chifeng Keshiketengqi,and to investigate the relationship between the prevalence of trachoma with age,gender,ethnicity and living environment.Methods This was a cross-sectional study.According to cluster random sampling method,2067 students were drawn from 10 primary and secondary schools,including 1061male and 1006 female.The age of students ranged from 6 to 17 years old,with the average age (12.53 ± 2.31)years.In the taken samples,Han students were 1768,and Mongolian students were 299;the agricultural and pastoral students were 1209,and the urban students were 858.The general life factors associated with trachoma were surveyed by questionnaire.Trachoma was diagnosed and graded based to 1987 WHO Simplified Trachoma Grading Standards.Eye examinations included visual inspection,eyelids,conjunctiva and cornea neovascularization under the slit lamp microscope,and the clinical data were recorded and collected.The results were statistically analyzed by SPSS13.0 software.Results According to the sampling proportion of 10.05％,2153 students were included and 2067 students participated in this study with the response rate 96％.Trachoma was diagnosed in 972 students with a morbidity 47.0％.The prevalence rate of trachoma was increased with the ageing,and a statistically significant difference was seen among the 6-8 years group,9-11 years group,12-14 years group and 15-17 years group (x2 =11.043,P =0.011),and the prevalence of trachomatous infiltration was significantly higher than that of trachomatous folliele (x2=11.493,P＜0.05).The prevalence rate of trachoma in Han and Mongolian students was 45.8％ and 54.5％,showing a significant difference between them (x2 =6.228,P =0.013).In addition,statistically significant differences also were found in the prevalence rate of trachoma between the pastoral areas students and urban students (56.1％ vs.39.7％,x2=30.226,P =0.000),but no significant difference was seen between male and female (47.0％ vs.46.9％,x2 =0.000,P =0.995).Conclusions The trachoma prevalence rate is relatively higher in the primary and secondary students in Chifeng Keshiketeng.These results offer a basis for the prevention and treatment of trachoma.

Objective To study the expression and relationship between adrenomedullin (ADM )and integrinαvβ3 in different pathological grades ,clinical stages and histologic subtypes of human osteosarcoma .Methods The expression positiverate ,intensity , the average optical density (ALD) of ADM and integrinαvβ3 in 30 osteosarcomacases was detected with immunohistochemical SABC technique and image analysis system .Each pathological section observed by two pathologists respectively in the case of unknown clinical and pathological information .At least five representative high field of vision counting 100 cells observed at random ,in this way the degree of tumor grade ,stage and cell staining were determined by two pathologist who unknown clinical and pathological data .Results The positive rate and intensity of ADM and integrinαvβ3 were significantly increased with the increase of osteosarco-ma stage(P<0 .05) and the difference was significant in each stage(P<0 .05) .The expression of ADM was positive correlated with the expression of integrinαvβ3 .Conclusion The expression of ADM and integrinαvβ3 in different clinicopathological grades osteosar-coma are positive associated with malignant degree of tumor ,which suggest that both of them play an important role in malignant biological behaviors of human osteosarcoma .

Background Orthokeratology has been proved to temporarily alter the equivalent sphere,but its effects on visual quality deserve attention.Objective The present study was to investigate the visual quality after overnight orthokeratology in pre-adolescent myopes.Methods Written informed consent was obtained form each subject prior to entering into this series.A descriptive study design was used.One hundred and fifty eyes of 76 teenagers aged (14.90± 1.24)years with low and moderate myopia (-2.79 ± 0.82)D were included in the study.Fitted with Ortho-K contact lens,the lens was wore every night for over 8 hours.Subjective refraction,uncorrected visual acuity (UCVA),contrast sensitivity function,corneal-topography,and aberrometry were examined before,1 week and 3 months after the initiation of orthokeratology.Visual quality was generally evaluated by comparing before wearing,1 week and 3 months after wearing using the National Eye Institute refractive error quality of life instrument (NEI-RQL-42TM).All the procedures were performed by the same clinician.Results Spherical equivalent refractions were (-0.33± 1.02) D and (-0.26 ± 0.60) D 1 week and 3 months after orthokeratology,showing significant decline in comparison with (-2.79±0.82) D of before orthokeratology (P =0.001,0.001).However,no considerable difference was seen between 1 week and 3 months after orthokeratology (P=0.161).Contrast sensitivity function was significantly different in all spatial frequencies before wearing,1 week and 3 months after wearing,and those of 1 week and 3 months after wearing were significantly lower than those of before wearing (1 week:3 cpd P =0.001,6 cpd P=0.001,12 cpd P＜0.05,18 epd P＜0.05 ;3 months:3 cpd P=0.001,6 cpd P=0.001,12 cpd P＜0.05,18 cpd P＜0.05).There was no significant change in contrast sensitivity function between 1 week and 3 months of orthokeratology (P＞0.05).Flat K,steep K and corneal eccentricity (e) were significantly reduced (P =0.000),and surface asymmetry index (SAI) and surface regularity index (SRI) were significantly improved after orthokeratology in comparison with before orthokeratology (both P =0.001).Root mean square (RMS) of total higher-order,third-order and fourth-order aberrations were significantly increased (P＜0.05),but RMS of whole aberration and second-order aberrations significantly decreased after orthokeratology (P＜ 0.05).There were no significant changes in the fifth-order,sixth-order and seventh-order aberration among pre-wear,1 week and 3 months after orthokeratology.Scales of dark to bright,nocturnal driving,glare,visual fluctuate and halo were lower than those before orthokeratology (P＜0.05).The scales of visual fluctuate between 1 month and 3 months after orthokeratology had significant differences (P＜0.01),while the rest of the scales had no significant differences (P＞0.05).Conclusions Orthokeratology can reduce myopic diopter 1 week after orthokeratology.The overnight wearing of fitted orthokeratology can decrease contrast sensitivity and increase corneal surface irregularity and RMS of third-order and fourth-order.Compared with spectacles,orthokeratology reduces subjective visual quality,especially nocturnal quality.

Using colon adenocarcinoma cell line HT-29 as testing material, adhesion test was carried out for 24 strains of Bifidobacteria isolated from dejecta of longevous people aged one hundred years. The results showed that all strains could adhere to colon adenocarcinoma cells HT-29, among which the strains of TTF, Z2, TZ5 and (J-1) showed higher adhesive capability. Thus these four strains of Bifidobacteria were further tested for their endurance to stomach acid and bile acid. The production of vitamin B by these bacterial were also tested. All these tests indicated that the strain of TTF has high ability to produce Vitamin B_(1), B_(2), B_(6), B_(12), The survival rate after the treatment under pH 3.0 for 120 min was 93.11%, at the same time, the strain TTF can survive after being treated with 2% bile acid for 24h.

Aim To investigate the effects of monocrotaline pyrrole on production of nitric oxide and expression of eNOS protein of cultured pulmonary artery endothelial cells and on contractility of cultured pulmonary artery smooth muscle cells.Methods DAF-2 fluorescence technique was used to determine NO level,Western blot analysis was performed to determine the level of eNOS protein,and collagen gel contraction system was adopted to analyze muscle contractility.Results NO production induced by ACh and expression of eNOS protein were obviously inhibited by monocrotaline pyrrole compared with those of control group and gel contraction area in MCTP-treated cells induced by Thapsigargin obviously decreased.Conclusions monocrotaline pyrrole could inhibit the level of the ACh-induced production of NO and expression of eNOS protein,and enhance the contractility of pulmonary artery smooth muscle cells,which may be one of the possible mechanisms of MCTP-induced pulmonary artery hypertension.

Abstract： Objective To evaluate the filtration effects of various nanofiltration systems on intravenous human immunog⁃ lobulin（IVIG）in order to screen the optimal nanofiltration system. Methods Various nanofilters were used for IVIG filtration to determine the best one and then various prefilters were selected to combine with the optimal nanofilter for IVIG filtration to determine the optimal nanofiltration system. Results The tangential flow（cross flow）nanofilter showed better filtering effect than dead end（direct current）nanofilter，and nanofilter C was the best one. The effect of deep filtration prefilter was better than that of absolute filtration prefilter，and prefilter Y1 in series with nanofilter C was the optimal nanofiltration system. Conclusion The optimal nanofiltration system was determined through the effect evaluation of various nanofiltration systems filtering for IVIG.

Background With the development of phacoemulsification surgery,the minimization of the size of the incision in order to reduce the damaging of eye tissue and postoperative complications becomes a focus.Objective This study aimed to assess the clinical efficacy of coaxial phacoemulsification type cataract surgery through a 1.8 mm microincision with foldable intraocular lens (IOL) implantation in eyes with age-related cataract.Methods Informed consent was obtained from each patient at the beginning of this study.A serial case observation study was designed.Thirty-two eyes of 32 patients with age-related cataract were enrolled in the Inner Mongolia Autonomous Region People' s Hospital from April 2012 to May 2012,including 8 eyes of grade Ⅱ hard nucleus cataract,16 eyes of grade Ⅲ hard nucleus cataract and 8 eyes of grade Ⅳ hard nucleus cataract.Ocular axial length was measured by an A-mode ultrasonic apparatus,and IOL diopter was calculated using the SRK-Ⅱ formula.Under ocular surface anesthesia,a 1.8 mm clear corneal tunnel incision was made at the 10-11 o' clock position,and then an auxiliary incision was made at the 2 o' clock position.The opaque lens was extracted by routine phacoemulsification.An Akreos MI60 IOL was implanted through the 1.8 mm incision.The time and level of ultrasonic power required for phacoemulsification,postoperative visual acuity and incidence of postoperative complication were assessed.Postoperative examinations were scheduled at 1 day,1 week and 1 month after the surgery.Results All the operations proceeded smoothly.The mean phaco-time was (7.0±3.6) seconds and the mean ultrasonic power level was (15.3 ±6.1)％ among the different grades of cataract groups.The number of eyes presenting an uncorrected distance visual acuity of ≥ 0.5 was 18 (56.25％),28 (87.50％) and 30 (93.75％) 1 day,1 week and 1 month after surgery,respectively.The number of eyes with a best corrected distance visual acuity of ≥ 0.5 and 0.8 were 31(96.88％) and 26 (81.25％),respectively,1 month after surgery.Negligible changes were detected in the anterior chamber depths,and no thermal damage was found at the incision during the operation.Conclusions Coaxial phacoemulsification and IOL implantation through a 1.8 mm microincision is safe and effective.Thorough planning and precise execution are necessary.

Select patients diagnosed of hypertension in the first place in 16 hospitals and the patients were described by association rules analysis and distribution for the analysis in the study,in order to understand the information of diagnosis and treatment in hypertension patients in real world. The information include age, gender, admission condition, inpatient department, hospitalization expenses, western medicine comorbidities, the traditional Chinese medicine (TCM) syndrome and the medicine. The average age of patients was 64 years in the study. The proportion of men to women about 1. 10: 1. Admission condition is critical for about 10.12% of the total and more concentrated in the cardiovascular department. Hospital stay ranged from 8 to 14 d. Inpatients with medical insurance is 62.93%. Total hospitalization cost distribution most is the 5 000-10 000 RMB. Hypertension complicated with coronary heart disease, blood lipoprotein disorder disease; phlegm and blood stasis and Yin deficiency of liver and kidney are the most in TCM syndrome type. Promoting blood circulation and removing blood stasis is the major in Chinese medicine treatment. Western medicine treatment basically conforms to the guidelines. The most antihypertensive drugs is calcium antagonistst, accounting for about 81.2% of the total number. Class ACEI, class ARB, beta blockers, diuretics class are accounted for 43.0%, 43.4%, 42.4%, 42.4%, fixed compound is only 2 393, accounting for about 8%. the total frequency of five classes of antihypertensive drugs is 78 206 times. The principal conclusions of this analysis are as follows: the elderly people is the most in hypertension, more men than women; medical insurance is the majority type in hospitalization cost; nearly half of the population is combined with coronary heart disease; phlegm and blood stasis and yin deficiency of liver and kidney are the most in TCM syndrome type; western medicine treatment basically conforms to the guidelines. Combination therapy is more, less fixed compound preparation in western medicine. Chinese medicine is mainly to promoting blood circulation. Suggesting that the clinical treatment in strict blood pressure and at the same time, pay attention to combination therapy and the use of fixed compound preparation,it's useful to control the blood pressure better. Combined with traditional Chinese medicine and resolving phlegm and removing blood stasis, reduce the target organ damage, reduce complications, the prevention and control work of hypertension in China still faces enormous challenges at present. The result provides reference information for clinical medicine and medical research.
