Journal of Applied Clinical Pediatrics 2006;0(13):-
Objective To investigate the association between serum N-terminal pro brain natriuretic peptide(NT-ProBNP) and creatine kinase myocardial(CK-MB),cardiac troponin I(cTnI) in children who were diagnosed as viral myocarditis(VMC),and to explore the clinical significance. Methods Fifty children with VMC were selected as VMC group who were further divided into cTnI(+)group and cTnI(-)group.Fifteen healthy children were selected as healthy control group. The levels of myocardial enzyme of the 2 groups were tested by automatic biochemical analyzer and level of NT-proBNP was detected by specific NT-proBNP enzymelink immunosorbent assay(ELISA) kit.SPSS 13.0 software was used to analyze the data. Results 1.There were 23 cases with positive cTnI and 27 cases with negative cTnI.The levels of NT-proBNP in VMC group including cTnI positive and cTnI negative were significantly higher than those in healthy control group (P0.05).2.In the group with positive cTnI,the level of NT-proBNP was correlated with cTnI.The coefficient of determination was 0.17.3.In the group with negative cTnI,the level of NT-proBNP was correlated with CK-MB.The coefficient of determination was 0.34.NT-proBNP presented a increasing trend during interval of 25-100 U?L-1 and 175-255 U?L-1 of CK-MB.Conclusions The levels in the serum NT-proBNP in VMC children were higher than those in healthy children.Serum detection NT-proBNP may be applied as a new approach in diagnosis of VMC in children.

Objective To study the validity of furosemide test for predicting the hematoma enlargement in pa- tients with hypertensive cerebral hemorrhage. Methods Four hundred fifty-one patients with hypertensive cerebral hemorrhage were diagnosed using CT after oneset of the disease and 24 h reexamined 24 h after. The incidence of enlarged hematoma was evaluated by comparison the baseline and 24 h CT scanning.Furosemide(20 mg iv)was ad- ministered and blood pressure was measured 30 min after furosemide. Results The decreasing level of MAP after furosemide was significantly inversely related with incidence rate of hematoma enlargement{ r=-0.94,t=58.4,P 10 mmHg as the cut-off point, with the rate of hematoma enlargement as being 6.4 %,MAP decreased≤10 mmHg was associated with increases in prevalence of hematome to 33.2 %(?~2=51.82, P

Aim The relative bioavalability of hydrochloride eperisone granule in 10 healthy volunteers was studied. Methods The time-plasma concentrations of hydrochloride eperisone granule, as test drug, and myonal, as reference drug, were determined by GC-MS, with tolperisone senuing as internal standard. The pharmacokinetic parameters of both reference and test drug were calculated and analyzed with two-one side test and confidential interval test. Results The results showed that the AUC0-8, AUC0-∞, Cmax, Tpeak, t1/2(?) and t1/2(?) were (17.9?1.3)ng?h?ml-1 and(18.6?1.6)ng?h?ml-1, (19.1?1.2)ng?h?ml-1 and (20.2?1.6)ng?h?ml-1, (5.2?0.5)ng?ml-1 and (5.4?0.5) ng?ml-1, (1.05?0.18)h and (1.08?0.23)h, (0.78? 0.13)h and ( 0.82?0.14)h,( 1.8?0.3)h and (1.8?0.3)h, respectively. The relative bioavalability of test drug was (105? 5)%. Conclusion It can be concluded that the test and reference are bioequivalented between individuals, preparations and periods.

Objective To investigate the MR features of pituitary abscess. Methods The MR features of 14 cases of pituitary abscess proved by surgical pathology and clinical treatments were analyzed retrospectively. Results Pre-contrast MR showed hypointense heterogeneous intensity on T_1 WI in 12 cases and iso-hyperintense on T_1 WI in 2 cases, hyperintense on T_2 WI in all cases.Post-gadolinium MR showed the ring-like enhancement around the uneven edge of abscess and the surrounding enhanced meninges connecting to the focus. The normal pituitary could not be identified in all 14 cases. The MR specific findings include the fluid-fluid level, nodule on the edge and the enhanced patchy shadow. Conclusions The pituitary abscess has specific findings on MR examination, which can be used to combine with clinical symptoms to achieve the diagnosis before operation,so that the cases could be treated with antibiotic without operation.

Objective To analyze the feasibility of body surface electrodes instead of multipair esophageal electrodes for the evaluation of neural respiratory drive in patients with COPD. Methods Diaphragm electromyo-gram(EMG)from body surface electrodes and multipair esophageal electrodes, was recorded in 29 patients with stable COPD recruited from outpatient clinic. Changes of neural respiratory drive of two kinds of electrodes during resting and maximal isocapnic ventilation (MIV) were observed before and after inhalation of bronchodilators. Results Ventilation significantly improved ,RMS-sur and RMS-eso significantly decreased after the inhalation of bronchodilators during resting and MIV. RMS-sur and RMS-eso were significantly correlated(r=0.660,P<0.01). Conclusion EMG from the surface electrodes may be a useful and noninvasive technique to evaluate neural respi-ratory drive in patients with COPD.

Background Heat shock proteins (HSPs) are highly conserved proteins that are induced in cells when confronted with a wide variety of proteotoxic stresses.HSP27 has a high degree of similarity with α-crystallin protein. The abnormality of HSP27 structure and expression are closely related to the formation of cataracts. Our previous study showed sodium salieylate has the protective effect on H2O2-induced lens damage. Objective This study was to investigate the roles of MAPK signal pathway in sodium salicylate-induced the expression of HSP27 in human lens epithelial cells (LECs) in vitro. Methods Human LECs were incubated in the fresh media containing sodium salicylate at different concentrations (0-55 mmol/L) for different times (1-5 hours) and allowed to be recovered in fresh medium without sodium salicylate for 1-24 hours with or without pretreatment with P38MAPK inhibitor (SB203580), ERK1/2 inhibitor (PD98059) and JNK/SAPK inhibitor ( SP600125). The expressions of P38MAPK, EBK1/2, JNK/SAPK, phosphorylated P38MAPK, phosphorylated ERK1/2, phosphorylated JNK/SAPK and HSP27 were detected by Western blot. HSP27 mRNA was detected by RT-PCR. The expression of HSP27 was also detected by immunohistochemistry. Results There was only weak expression of HSP27 in normal human LECs.After stimulation of 35-55 mmol/L sodium salicylate was removed and human LECs were cultured again for 6 hours, the expression of HSP27 in LECs were significantly increased ( F= 509. 953,P<0. 01). HSP27 was absent expressed in human LECs in 55 mmol/L sodium salicylate stimulation for 1-5 hours groups, but LECs were re-cultured for 3,6 hours after removed the stimulation, the expression of HSP27 was elevated (F = 452. 534, P<0. 01). Activation of P38 M APK occurred after sodium salicylate stimulation 30 minutes and 1 hour ( F = 865.68, P<0. 01). However, ERK 1/2 was expressed after sodium salicylate was eliminated for 1-6 hours ( F = 388.84, P<0. 01). JNK/SAPK was inactived by sodium salicylate. The expression of HSP27 could be down-regulated with the pretreatment of SB203580 and PD98059. Conclusion Sodium salicylatc can induce the expression of HSP27 in human (LECs) . The effects are mediated,at least in part ,through the activation of P38MAPK and ERK1/2 signaling pathway .
