1.Initial experience of previously blocking both pulmonary artery and veins in true video assisted thoracic surgery for the treatment of NSCLC

ObjectiveWe evaluate our experience of previously blocking both pulmonary artery and veins in true video assisted thoracic surgery (VATS) for the treatment of non-small-cell lung carcinoma ( NSCLC ).MethodsSix patients undergoing lobectomy with previously blocking both pulmonary artery and veins in the VATS in our institution between December 2007 and June 2011 were reviewed.ResultsIn our series,six patients required previously blocking both pulmonary artery and veins in the VATS,The mean occlusion time of PA and PV was (42.0 ± 8.6 ) min and ( 39.3 ± 8.2 ) min,respectively.Four patients required partial PA reconstruction.The mean repair time of the PA was (21.25 ±9.91 ) min.No complications attributable to the technique or mortality were seen.There were no local recurrences on the pulmonary artery.ConclusionIt is feasible and safe that the technology of previously blocking of the pulmonary artery and veins was applied in VATS.This technique can reduce the risk of intraoperative bleeding and the chance of converting to open thoracotomy,and extend the inclusion criteria of VATS lobectomy.
2.Study on the nutritional status and determinants among rural stranded children in China

Objective To explore the nutritional status and determinants among rural stranded children under 7 years of age. Methods The group of stranded children (n=7585) and the children for control (n=7557) were identified by multi-stage stratified cluster sampling. The ascertainment methods mainly included questionnaire, anthropometric measurements and laboratory examination. Results In this study, there were three groups, including the group of stranded children whose single parent left home to look for better job somewhere, the group of stranded children whose parent both left home and the third group serving as control. Prevalent rates on the following items were: stunting -- 14.9%, 17.9% and 16.3%, underweight -- 7.2%, 8.3% and 7.6%, wasting -- 3.1%, 3.4% and 3.3% and anemia -- 18.9%, 20.6%, 18.7% respectively. The prevalent rate of stunting in the group of children with both parents away from home was significantly higher than that those with only one parent did. The prevalence rate of anemia in the group with both parents away from home was significantly higher than that in the control group. Based on multivariate non-conditional logistic regression analysis, the determinants of stunting among the stranded children were: age, with low birth weight, living alone, being the only child in the family, mother's education level, frequency of eating breakfast and snacks every week, motives of caretakers and the relationship between children and the caretakers. The determinants of underweight among those stranded children were: age of child, with low birth weight, the frequency of eating snacks every week, motives of the caretakers and having rickets. Conclusion The nutritional status among rural stranded children was relatively poor, especially in those whose parents were both away from home, which calls for urgent improvement.
3.Study on the classification of gonococcal island of different genotypes and effects of sac-4 gene on serum resistance of Neisseria gonorrhoeae

Objective Using molecular methods to study the relationship between genotypes and serum resmtance of Neisseria gonorrhoeae in Wuhan area.Methods NG-mast and serum bactericidal assays at the molecular level were used to differentiate the 46 strains which were isolated from the outpatients of sexually transmitted disease clinics and the relationship between different genotypes while phenotypes was also studied.Results 80.43% of the 46 strains contained the island and we were able to define three dillerent combinations of genes in the isolates.Results from serum bactericidal assays showed that all 9 sac-4 stralns did not provide any serum resistance.Conclusion Different isolates carried clifferent gonococcal genetm islands(pathogenicity island)and certain phenotypes.There were no sobious relationship between sac-4 gene and serum resistance of Neisseria gonorrhoeae.
4.Molecular epidemiological study on oxidative DNA damage among Hasake ethnic migrants in Shenzhen

objective To explore the relationship of migration and oxidative DNA damage by comparative study of oxidative DNA damage effects on people with difierent years of migration among Xinjiang Hasake ethnecity in Shenzhen.Methods Sixty Hasake residents in Shenzhen were selected,and were divided into three groups(n=20)according to the years of migration.Major changes of their life style were investigated.8-hydroxy-2'-deoxyguanosine(8-OH-dG)levels in urine were analyzed,and comet assay of peripheral blood lymphocytes conducted.Results When comparing with the group having a shorter than 1 year of stay,a significant decrease of oliveive tail moment and tail/head length in comet assay in the>3 years group(P<0.05)was observed 8-OH-dG level decreased significantly in 1-3 years group (P<0.05)and>3 years group(P<0.01).Conclusion Our results suggested that life style changes which related to migration might reduce DNA damage in Hasake nationalities.
5.Chidamide Combined with(+)-JQ-1 to Kill MLL-Rearrangement Acute Myeloid Leukemia Cells by Disrupting the DNA Damage Response Pathway

Objective:To investigate the mechanism of DNA damage and repair in MLL-rearranged acute myeloid leukemia(MLL-r AML)cells by the combination of Chidamide and the BRD4 inhibitor(+)-JQ-1.Methods:MLL-r AML cell lines Molm-13,MV4-11 and non-MLL-r AML cell line Kasumi were divided into control group(contr),Chidamide group(chida),(+)-JQ-1 group and Combination group(combi),respectively.Cell viability of Molm-13 was measured by CCK-8 to determine optimal the concentrations of Chidamide and(+)-JQ-1.The cell cycle was detected by flow cytometry,and apoptosis-related factors Bcl-2,Bax and caspase-3 were detected by Western blot.DNA damage marker γH2AX was detected by immunofluorescence.The protein expressions of DNA damage factor γH2AX,DNA damage checkpoint kinases p-ATR,p-CHK1,p-ATM,p-CHK2 and DNA damage repair factors Rad51 and 53BP1 were detected by Western blot.The expression of DNA damage repair factors Rad51 and 53BP1 mRNA was detected by qRT-PCR.Results:Under the treatment of Chidamide(300 nmol/L)and(+)-JQ-1(400 nmol/L),the proportion of G1 phase cells in MLL-r AML cell lines Molm-13 and MV4-11 was increased in combination group compared with control group.In non-MLL-r AML cell line Kasumi,compared with control group,the proportion of G1 phase cells in combination group was increased(P<0.05).In Molm-13 and MV4-11 cell lines,compared with control group,the expression level of DNA damage marker γH2AX in combination group was increased(P<0.05).The expression levels of DNA damage checkpoint and damage repair factors p-ATR,p-CHK1,p-ATM,p-CHK2,Rad51,53BP1 were decreased(P<0.05).In Kasumi cell line,compared with control group,there was no significant change in the expression of some of the above factors in combination group(P>0.05),but the expression trend of some factors was opposite.In MLL-r AML cell lines Molm-13 and MV4-11,compared with control group,the expression levels of Bax and caspase-3 protein were increased in combination group,while the expression levels of Bcl-2 protein were decreased(P<0.05).In non-MLL-r AML cell line Kasumi,there was no significant change in apoptotic factor protein expression in combination group compared with control group(P>0.05).Conclusion:Chidamide combined with(+)-JQ-1 can inhibit the proliferation of MLL-r AML cells,inhibit the initiation of protective self-repair of these leukemia cells by inhibiting the DNA damage response pathway,and ultimately increase the apoptosis of these cells,but non-MLL-r AML cells have no similar results.
6.Cryptosporidium spp., Giardia intestinalis, and Enterocytozoon bieneusi in Captive Non-Human Primates in Qinling Mountains.

Non-human primates (NHPs) are confirmed as reservoirs of Cryptosporidium spp., Giardia intestinalis, and Enterocytozoon bieneusi. In this study, 197 fresh fecal samples from 8 NHP species in Qinling Mountains, northwestern China, were collected and examined using multilocus sequence typing (MLST) method. The results showed that 35 (17.8%) samples were positive for tested parasites, including Cryptosporidium spp. (3.0%), G. intestinalis (2.0%), and E. bieneusi (12.7%). Cryptosporidium spp. were detected in 6 fecal samples of Macaca mulatta, and were identified as C. parvum (n=1) and C. andersoni (n=5). Subtyping analysis showed Cryptosporidium spp. belonged to the C. andersoni MLST subtype (A4, A4, A4, and A1) and C. parvum 60 kDa glycoprotein (gp60) subtype IId A15G2R1. G. intestinalis assemblage E was detected in 3 M. mulatta and 1 Saimiri sciureus. Intra-variations were observed at the triose phosphate isomerase (tpi), beta giardin (bg), and glutamate dehydrogenase (gdh) loci, with 3, 1, and 2 new subtypes found in respective locus. E. bieneusi was found in Cercopithecus neglectus (25.0%), Papio hamadrayas (16.7%), M. mulatta (16.3%), S. sciureus (10%), and Rhinopithecus roxellana (9.5%), with 5 ribosomal internal transcribed spacer (ITS) genotypes: 2 known genotypes (D and BEB6) and 3 novel genotypes (MH, XH, and BSH). These findings indicated the presence of zoonotic potential of Cryptosporidium spp. and E. bieneusi in NHPs in Qinling Mountains. This is the first report of C. andersoni in NHPs. The present study provided basic information for control of cryptosporidiosis, giardiasis, and microsporidiosis in human and animals in this area.

7.Lower Uterine Segment Involvement as a Predictor for Poor Prognosis in Patients Receiving Adjuvant Radiotherapy for Stage I Endometrial Carcinoma

Objective To explore the effect of lower uterine segment involvement (LUSI) in predicting the prognosis of stageⅠendometrial carcinoma patients treated with postoperative radiotherapy .Methods The data of a total of 265 patients with stage Ⅰendometrial carcinoma treated with hysterectomy and adjuvant radiotherapy be -tween January 1999 and December 2012 were retrospectively analyzed .The median age of the cohort was 53 years.The most common pathological type was endometrioid adenocarcinoma (226/265, 85.3%).The patients were divided into two groups according to the presence of LUSI:LUSI group and non-LUSI group .The prognostic factors and treatment outcomes were compared between the two groups , subgroup analysis was conducted in the high-risk and high-intermediate-risk patients in the two groups .Primary outcomes were overall survival ( OS), progression-free survival ( PFS) , loco-regional recurrence ( LR) , distant metastasis ( DM) , and treatment failure (TF).The survival rates were calculated using the Kaplan-Meier method.The survival rates between different groups were compared using the Log-rank test .Prognostic factors for survival were analyzed using a Cox propor-tional hazards regression model .Results The 5-year OS and PFS for all the patients were 92.8%and 89.7%, respectively;the 5-year LR, DM, and TF were 4.5%, 6.4%, and 7.8%, respectively .Univariate analysis re-vealed that LUSI was a significant predictor of OS and PFS ( P=0.015 , 0.035 ) .Cox proportional hazard model demonstrated a significantly decreased OS and PFS in the LUSI group compared with the non -LUSI group ( P=0.041 , RR=0.346 , 95%CI:0.125-0.959;P=0.041 , RR=0.411 , 95%CI:0.175-0.963 ) .Subgroup univariate analysis showed that for high-risk and high-intermediate-risk patients, LUSI was a significant predictor of TF ( P=0.034 ) .Conclusions LUSI may be a significant predictor of reduced OS and PFS in patients with stage Ⅰ endometrial carcinoma treated with adjuvant radiotherapy .In addition, LUSI may be associated with treatment failure in high-risk and high-intermediate-risk patients .
8.Two cases of neonatal Legionella pneumonia

Patient 1,a 12-day-old female infant,presented with fever,cough,dyspnea,and elevated infection markers,requiring respiratory support.Metagenomic next-generation sequencing(mNGS)of blood and bronchoalveolar lavage fluid revealed Legionella pneumophila(LP),leading to diagnoses of LP pneumonia and LP sepsis.The patient was treated with erythromycin for 15 days and azithromycin for 5 days,resulting in recovery and discharge.Patient 2,an 11-day-old female infant,presented with dyspnea,fever,elevated infection markers,and multiple organ dysfunction,requiring mechanical ventilation.mNGS of blood and cerebrospinal fluid indicated LP,leading to diagnoses of LP pneumonia,LP sepsis,and LP intracranial infection.The patient was treated with erythromycin for 19 days and was discharged after recovery.Neonatal LP pneumonia lacks specific clinical symptoms,and azithromycin is the preferred antimicrobial agent.The use of mNGS can provide early and definitive diagnosis for severe neonatal pneumonia of unknown origin.
