3.Association between prodromal headache and propensity of rebleeding after aneurysmal subarachnoid hemorrhage

Objective To investigate the association between prodromal headache and the propensity of rebleeding after aneurysmal subarachnoid hemorrhage.Method Prodromal headache was defined as a sudden attack of severe headache lasting at least one hour with or without accompanying symptoms,not leading to a diagnosis of SAH 4 weeks before the confirmation of subarachnoid hemorrhage.Two hundred and nine patients with aneurymal subarachnoid hemorrhage were enrolled in this research.Age,gender,Hunt-Hess grade,D-dimer, (FBG),site and number of aneurysms and prodromal headache were recorded.Patients were divided into rebleeding group and non-rebleeding group.Comparing the above survey components between two groups was carried out by using case control study method.Age,aneurysm number,D-dimer and fibrinogen of the two groups were analyzed by t test.Prodromal headache,gender,site of aneurysms were analyzed by chi square test.Hunt- Hess grade was analyzed by rank test.Results Twenty-two(10.5%)patients had rebleeding.In the rebleeding group,10 patients(45.5%)had prodromal headache,while in the non-rebleeding group(n=187),21 patients (11.2%)had prodromal headache.Patients with prodromal headache had greater propensity of rebleeding compared with those without prodromal headache(P
4. Analysis of risk factors of thyroid nodules in patients with type 2 diabetes in the community

Objective • To evaluate the prevalence, risk factors and the effects of macrovascular complications on the onset of thyroid nodules in patients with type 2 diabetes mellitus (T2DM) in the community. Methods • A total of 711 outpatients with T2DM from March 2015 to December 2017 in Community Family Doctor Outpatient Department and Department of Endocrinology and Metabolism of Fengxian District Central Hospital were selected. Anthropometric data and clinical test data were collected. Patients were divided into two groups based on the results of thyroid ultrasound. The Logistic regression analysis was used to analyze the risk factors for thyroid nodules in T2DM patients, such as age, sex, body mass index, glycolipid metabolism index, pancreatic function index, and thyroid function index. Furthermore, the effect of macrovascular disease on the onset of thyroid nodules was analyzed. Results • Of the 711 patients with T2DM, 438 (61.60%) had thyroid nodules, including 231 males (55.13% of the male population) and 207 females (70.89% of the female population). Comparison of anthropometry data, glycolipid metabolism index, pancreatic function index, and thyroid function index in groups with and without thyroid nodules revealed that there was a statistically significant difference in gender, age, duration of T2DM, 2-hour post-meal plasma glucose, total cholesterol, triglyceride, low density lipoprotein-cholesterol, free triiodothyronine, and thyroid peroxidase antibody between the two groups (P<0.05). Multivariate Logistic regression analysis showed that gender, age, 2-hour post-meal plasma glucose, low density lipoprotein-cholesterol, and thyroglobulin antibodies levels were the risk factors of thyroid nodules in T2DM patients (P<0.05). After adjusting for these risk factors, it was found that there was no significant correlation between macrovascular complication and thyroid nodules in T2DM patients (P>0.05). Conclusion • This study found that females, seniors, reduction of 2-hour post-meal plasma glucose, low density lipoprotein-cholesterol, and thyroglobulin antibody levels were the risk factors of thyroid nodules in T2DM patients in the community. There is no certain correlation between macrovascular complications and the occurrence of thyroid nodules in T2DM.

7.Treatment of meniscal injuries of knee joints by arthroscopy

Objective To assess the effectiveness of treatment of meniscal injuries of knee joints by arthroscopy.Methods 33 patients 35 joints were followed up and the parts,types and treatment under arthroscopy were analysed.Results 33 patients were followed up from six months to six years,the mean preoperative Lysholm score was 60.5 points,and the mean postoperative one was 86.7 points.Conclusion The advantage of treating meniscal injuries by arthroscopy was the result of correct examination and little wound of arthroscopy operation,and arthroscopic repair or partial menisectomy could effectively restore the function of the injured knee.
8.Trichostatin A Inhibits Proliferation and Induces Expression of p21WAF and p27 in Human Brain Tumor Cell Lines

Background and Objective: The histone deacetylase inhibitor,trichostatin A(TSA),was shown to induce apoptosis in transformed cells at submicromolar concentrations. However, the effect of TSA on brain tumor cells is still unknown. This study was designed to investigate whether TSA posses antitumor activity and if any, its mechanism. Materials and Methods: A p53 mutant human glioma cell line T98G and a p53 wild type human neuroblastoma cell line SKNSH were exposed to TSA. Cell proliferation was assessed by sulforhodamine B (SRB) cytotoxicity assay. Apoptosis was quantified by flow cytometry and confirmed by apoptotic ladder formation. Expression patterns of accumulation of highly acetylated histone H3,H4; p53 and cell cycle-associated p21waf,p27 which were induced by TSA were determined by using Western blot analysis. Results: TSA inhibited the proliferation of brain tumor cell lines at nanomolar concentrations and induced accumulation of highly acetylased histone moleculars. Treatment with TSA at 0.33μ M for 24h significantly induced cell apoptosis.In addition to the suppression of cell growth, the up regulation of p21waf and p27 expression was observed within 48h after the treatment.p21 protein levels were increased at early time points and reached maximal levels at 8h, while p27 protein levels were increased after 8h. However, there was no significant changes of acetylased p53 and endogenous p53 protein levels were observed. Conclusion:TSA may inhibit brain tumor cell growth in vitro, which is otherwise particularly resistant to chemotherapy. TSA acts as an anti-tumor agent could be through co-operation between p21 and p27 in growth inhibition, irrespective of endogenous p53 status.
9.Peripheral blood cell factors of Graves ophthalmopathy and effect of intervention with tripterygium glycosides.

To explore the effect of tripterygium glycosides on the level of peripheral blood cell factors of Graves ophthalmopathy (GO). In the study, 64 patients of GO in moderate-severe acute stage were selected, and randomly divided into the treatment group (32 cases) and the control group (32 cases). Both of the two groups were provided with basic treatment. The control group was added with prednisone(0. 75 mg kg-1 d-1 ), which gradually reduced (by 5-10 mg week-1 )to the minimum dose of 5 mg d-1. The treatment group was treated with 20 mg tripterygium glycosides, three times a day. One therapy course is three months. The levels of peripheral blood cells(TNF-alpha , IL-2, IL-10, IFN-gamma)of the two groups before and after the treatment and the clinical efficacy were observed. The study indicated that, before the treatment, TNF-alpha, IL-2, IFN-gamma in both groups were significantly higher than that in the health group, but with IL-10 notably lower than the healthy group. After the treatment, TNF-a, IL-2, IFN-gamma in the treatment group significantly decreased, but with IL-10 significantly increasing (P <0. 01). After the treatment, the two groups showed significant difference (P <0. 01). The total clinical efficacy in the treatment group was 88. 10% , and that in the control group was 57. 14% (P <0. 01). After the treatment, the two groups showed significant changes in the exophthalmos degree (P < 0. 01). The results showed that the level of peripheral blood cells (TNF-alpha,IL-2, IL-10, IFN-gamma)of GO patients was positively correlated with the severity of ocular disease. 10.Preparation and characterization of cucurbitacin B sodium deoxycholate/phospholipid-mixed oral fast dissolving film and antitumor activity study.

A novel drug delivery system combining oral fast dissolving film with sodium deoxycholate/phospholipid mixed micelles was prepared to increase the absorption of cucurbitacin B that is a poor aqueous solubility substance. Encapsulation efficiency, particle size, zeta potential, polydispersity coefficient, investigated the morphology, disintegration time of oral fast dissolving film and the pharmacodynamic properties of cucurbitacin B sodium deoxycholate/phospholipid-mixed micelles before and after solidified in mice were evaluated and compared. The oral fast dissolving film prepared in this study showed a homogeneous pale yellow and could completely disintegrated in the 30 s. It could meet the requirements of rapidly disintegrating fully. The encapsulation efficiency, particle size, zeta potential, polydispersity coefficient of cucurbitacin B sodium deoxycholate/phospholipid-mixed micelles loaded in oral fast dissolving film were (43.36 +/- 2.12)%, (108.82 +/- 5.2) nm, (-34.18 +/- 1.07) mV, 0.088 +/- 0.012, respectively. The encapsulation efficiency, particle size, zeta potential, polydispersity coefficient of cucurbitacin B sodium deoxycholate/phospholipid-mixed micelles in solution were (41.26 +/- 2.22)%, (181.82 +/- 4.48) nm, (-30.67 +/- 0.81) mV, 0.092 +/- 0.012, respectively. The difference of pharmacodynamics among film of cucurbitacin B-loaded micelles, cucurbitacin B-loaded micelles and free cucurbitacin B in vivo was compared. Solubility of cucurbitacin B loaded in sodium deoxycholate/phospholipid-mixed micelles has also been greatly improved. The tumor inhibition rate of cucurbitacin B loaded in sodium deoxycholate/phospholipid-mixed micelles was significantly improved and did not change significantly before and after solidified. These showed that the sodium deoxycholate/phospholipid-mixed micelles could enhance the antitumor activities of cucurbitacin B and the stability of cucurbitacin B sodium deoxycholate/phospholipid-mixed micelles was improved significantly after solidified by oral fast dissolving film technology without pharmacodynamic properties changed significantly.
