1.Physical and chemical factors affecting the allergenic potency of the house dust mite dermatophagoides farinae extract

The physical and chemical factors affecting the allergenic potency of the house bdust mite Dermatophagoides farinae extract were determined by skin test and ELISA technique in the asthmatic patients sensitive to the mite. The results showed that the allergenic potency of the extract could be reduced by heat sterilization, trypsin trea-tment and lyophization,but not lost completely,and that the potency could be increasedby supersonic treatment and repeated freeze-thaw(but less than 10 times).It was alsoshowed that the allergenic potency of the extract preserved at -20℃ was more stablethan those preserved at 4℃ or at room temperature,and that the mite extract preservedat 4℃ more than 9 years still had allergenic potency.

4.Microanatomy of intracranial segment of vertebral artery and its main branches in surgery adopt far lateral approach

Objective To discuss how to protect the intracranial vertebral artery and posterior inferior cerebellar artery by observing and measuring the intracranial vertebral artery in the surgery adopt far lateral approach. Methods Mimicking far lateral approach, 20 adult cadaveric heads connected to neck fixed with 10% formalin were dissected. Intracranial segment of the vertebral arteries and their main branches were exposed and measured under operating microscope. Results The intracranial vertebral artery joined with the contralateral one into the basilar artery after traveling through the atlanto-occipital sulcus. The relationship between the vertebral artery and the hypoglossal nerve is close. Thirty sides (75%) of the vertebral arteries traveled to pons medulla sulcus in front of the hypoglossal nerve roots and 2 sides (5%) behind the hypoglossal nerve roots, while 8 sides (20%) traveled among the hypoglossal nerve roots; 70% of the vertebral arteries were contacted to the hypoglossal nerve roots, 30% of which compressed the hypoglossal nerve. The main branches of intracranial segment of the vertebral arteries were the posterior inferior cerebellar arteries, the anterior spinal arteries, the posterior meningeal arteries,and some perforating arteries. Posterior inferior cerebellar arteries all originated from the intracranial vertebral artery were the largest vertebral artery's branches; their trip was mostly loop-shaped and they had close relationship with Ⅸ, Ⅹ, Ⅺ cranial nerves. The starting points of the posterior inferior cerebellar arteries were different, even in the same specimen, but most of them originated from the upper 1/3intracranial vertebral artery. No anterior inferior cerebellar artery was noted originated from the vertebral artery in our specimen. Anterior spinal arteries originated from the vertebral arteries joined with the branches of the bilateral vertebral arteries and traveled down through the tortuous anterior median fissure to supply the spinal cord. Conclusion Being familiar with the characteristics and anatomic vertebral arteries variations of the intracranial vertebral artery and its branches can contribute to identify and protect the intracranial segment of the vertebral artery and its main branches in the surgery adopt far-lateral approach.
5.Stable life predection of Hangbaiju and Gongju by classic constant temperature accelerated test method.

OBJECTIVETo study the stability of chlorogenic acid, cynaroside and 3,5-O-discaffeoylquinc acid in Hangbaiju and Gongju and to predict their term of validity.
METHODHangbaiju and Gongju were incubated in an environmental chamber at different temperatures and relative humidities. After the incubation, quantitative determination of chlorogenic acid, cynaroside, 3,5-O-discaffeoylquinc acid in Hangbaiju and Gongju were measured by HPLC. The effective period of the preparation was calculated according to Arrhinius index law. Quantitative determination of 3,5-O-discaffeoylquinc acid, cynaroside, chlorogenic acid in Hangju and Gongju were analyzed by HPLC.
RESULTThe stable life of Hangbaiju has been determined as 2.25 years. The stable life of Gongju has been determined as 4.31 years.
CONCLUSIONThe high temperature is not conducive to the stability of Hangbaiju and Gongju, which needs to be placed in a dark and cool place.

7.Percutaneous lumbar discectomy combined with medical ozone in the treatment of lumbar disc herniation

Objective To observe the distribution of O2-O3 mixture in the disc and the shrinkage of herniation after the application of percutaneous lumbar discectomy (PLD) combined with medical ozone (O3) in patients with lumbar disc herniation, and analyze the clinical effect of PLD combined with O3 and the complications of lumbar disc hemiation. Methods One hundred and twenty patients with lumbar disc herniation proved by imaging (CT or MRI) and physical examinations were divided randomly into 3 groups (n=40): PLD-treated group, O3-treated group and PLD combined with O3 treatment group.They were treated as planning and their clinical effects were observed. Results PLD combined with O3 treatment group showed a significantly lower incidence of lumbago and obviously better distribution of O3 in or outside the discs as compared with the other 2 groups. The shrinkage rate of herniation was not statistically different among the 3 groups. The effective rate in the PLD combined with O3 treatment group was 86% (34/40); that in the PLD-treated group was 83% (33/40); that in the O3-treated group was 35% (14/40). After 6-18 months follow-up, the total effective rate was as following: 87.5% (35/40) in the PLD-treated group, 77.5% (31/40) in the O3-treated group and 92.5% (37/40) in the PLD combined with O3 treatment group. No complications were found in all the 3 groups. Conclusion PLD combined with O3 is a safe and effective method in the treatment of lumbar disc herniation with less adverse reaction.
8.Clinical observation of soft tissue wire rivets in the treatment of large patellar cartilage fracture.

OBJECTIVE:
To explore the clinical curative effect of soft tissue wire rivet for the treatment of fracture of patella cartilage.
METHODS:
A retrospective study was conducted in 25 patients(25 knees) from June 2015 to February 2017 in patients with patellar cartilage fractures. Among them, 19 were male, 6 were female, 11 were left knee, 14 were right knee. The accident occurred in 2 cases, 23 cases of sprained athletes; the average age was(25.12±6.02) years old, the average weight was (62.48±7.91) kg, and the average body mass index was(23.25±1.51) kg/m². The average time from injury to admission was (1.96±1.51) d. The clinical manifestations were swelling of knee joint, positive floating patella sign, severe pain, occasional joint strangulation, limited flexion and extension, obvious tenderness of lateral patella, positive patellar extrapolation test and positive extrapolation fear test. X-ray and CT examination were performed before operation. X-ray showed the continuity of patellar bone and a loose body in the joint cavity. CT and 3D reconstruction showed patellar cartilage and facial defects. All 25 patients were fixed with soft tissue wire rivet. The clinical efficacy included preoperative Lysholm score and Insall evaluation.
RESULTS:
All the patients were followed up, and the duration ranged from 3 to 15 months, with an average of(9.72±4.07) months. The preoperative Lysholm score was 60.32±5.08, and the final follow-up was 88.24±4.37. The postoperative score was better than that before operation(-22.926, <0.05). According to Insall criteria, 21 cases were excellent, 4 cases were good, no fracture fragments were found and knee joint adhesion was found. Six weeks after operation, X-ray and CT showed that the fracture was well positioned, the cartilage surface was smooth without obvious steps, and the patella was in good shape. MRI showed the smooth articular surface of patellar cartilage at the 6th month after operation.
CONCLUSIONS
Soft tissue wire rivet for the treatment of massive patellar cartilage fracture has the advantages of simple operation, rapid postoperative recovery, no need for secondary operation pain, fewer complications, and definite effect, which is worthy of clinical promotion.

9.Suppressive effect of knockdown of miR-21 expression on U87 human glionto growth in vivo

Objective To study the suppressive effect of knockdown of miR-21 on the U87 human giioma xenograft growth and the possible mechanism. Methods Nude mice bearing U87 human glioblastoma subcutaneously were treated with miRNA-21 anfisense oligonucleotides(AS-miR-21)intratumomlly every 3 d until the observation peded ended.The tumor volume of the mice treated withAS-miR-21 was measured regularly as compared with that in the control untreated mice and in the mice treated with scramble oligonucelotides(ODN).Finally,the tumors were removed from nude mice for the examination.In-sire hybridization and real-time PCR were conducted to detect the miRNA expression of miR-21.The biological charaetedsties of the tumors were evaluated by HE and immunohistochemieal staining, and the cell apoptosis was detected by TUNEL method. Resulls During the observation period,the tumor growth was delayed and the final tumor volume of AS-miR-21 heated group was smaller than that in the control and scramble ODN treatedg roup(F=6-056,P=0.007).The expression of miRNA precursor was knocked down in As-miRNA treated tunlors compared with that in untreated or scramble ODN treated tumors.Histopathological examination exhibited the appearance of degraded malignancy.The expressions of PCNA and MMP-9 were down-regulated while Septin-7 and P21 were up-regulated and apoptotic index was increased significantly (F=141.021,P=000) as well.Conclusion The suppressive effect of anti-miR-21 ODNs on the growth of U87 human glioma xenogratts is significant and miR-21 Call be taken as a candidate for gene therapy ofhuman glioma.
