2.Application of two-dimensional speckle tracking on long-term prognosis of patients with acute infarcted myocardial

Objective To assess the effect of two-dimensional speckle tracking imaging (STI) on prognosis of patients with acute infarcted myocardial(AMI).Methods Eighty patients with AMI and 30 healthy subjects were selected as our subjects.Among the AMI patients,there was 30 cases were emergency percutaneous coronary intervention (PCI),25 cases were selective PCI group and 25 cases with conservative.All were performed a 4-6 months following up after discharge from hospital.Left ventricular ejection fraction(LVEF) was measured by Simpson's.Rotation were measured in the left ventricular basal and apical short-axis views using STI.LV Peak twist (Ptw),apical Peak rotation (PAR),the basal Peak rotation (PBR),twist at aortic valve closure(AVC tw),twist at mitral valve opening(MVO tw),untwisting rate (Untw R),and half time of untwisting (HTU) were calculated.And the correlation between Ptw and LVEF was analyzed.Results Compared with the control group,the value of Ptw,PAR,AVC tw and MVO tw were significantly decreased in myocardial infarction group(F =22.481,12.899,18.923 ;P < 0.05).Ptw,PAR in emergency PCI group at following up periods were (18.61 ± 8.08) ° and (12.16 ± 6.98) °,higher than that in hospital periods ((13.76 ± 6.31) °,(7.03 ±5.76)°).Ptw,PAR in selecting PCI group were (19.5 ±7.73)° and (13.4 ±7.02)°,higher than that in hospital periods((15.25 ± 6.83) °,(9.69 ±.6.72) ° ; t =-2.607,-3.104,-2.079,-1.955 ; P < 0.05).Significant correlation was found between Ptw and LVEF (r =0.527,P < 0.05).Conclusion Left ventricular twist can be measured using STI.LV rotation can be the quantitative index to evaluate the systolic function of LV.STI can be the new method to guide clinic diagnosis and therapy.
4.Construction of bio-micro-frontier based on theory of biotechnology supremacy

Biotechnology supremacy is a newly-advanced power theory. It is a superior dominance of military biotechnological application based on the microcosm of life structure within a certain period of time. The advancement of biotechnology supremacy and modern biotechnology has created the concept of bio-micro-frontier, which involves information and defense resources of all living ultra-micro-organisms with national and regional characteristics. Being feasible both in theory and practice, the implementation of bio-micro-frontier system is strategically important. This article explores the implementation of bio-micro-frontier in terms of strategy and tactics, which will add a unique dimension to future military transformation and active defense.
5.Protective effect of ulinastatin on acute lung injury induced by orthotopic liver transplantation

Objective To evaluate the effects of ulinastatin on acute lung injury during ortho- topic liver transplantation(OLT).Methods Twenty ASAⅢ-Ⅳpatients with end-stage liver disea- ses.undergoing OLT were randomly divided into two groups.Ulinastatin group received intravenous infusion of ulinastatin(3?10~4 IU in 100 normal saline)after skin incision and every 4 h thereafter(n =10).Control group received same amount of normal saline instead of ulinastatin(n=10).Blood sam- pies were taken before skin incision,120 min after skin incision,30 min after liver was removed,5 min and 60 min after reperfusion of the graft and at the end of operation for determination of plasma IL-6, IL-8,TNF-?,MDA concentrations and SOD activity.Respiratory index(RI)[P_(A-a)DO_2/PaO_2]was cal- culated before skin incision,30 min after liver was removed,5 min and 60 min after reperfusion of the graft and at the end of operation.After anesthesia was induced,cardiac output,mixed venous oxygen saturation and central venous temperature were continuously monitored during operation.ECG,CVP, SpO_2,P_(ET)CO_2,radial artery and MPAP were also continuously monitored during operation.P_(ET)CO_2 was maintained at 35-40 mm Hg during operation.Blood temperature was maintained above 35.5℃during operation.Results In group C plasma IL-6 and IL-8 concentrations were significantly increased from 120 min after skin incision to the end of operation as compared with the baseline values(P0.05).RI was significantly lower at 60 min after reperfusion of the graft and at the end of operation in group U than in group C(P

7.Treatment of femoral shaft fractures by closed reduction and interlocking intramedullary nailing.

OBJECTIVETo discuss the clinical effect of closed reduction and interlocking intramedullary nailing in the treatment of femoral shaft fracture.
METHODSFrom March 2006 to December 2011,103 patients with femoral shaft fracture were treated by closed reduction and interlocking intramedullary nailing including 76 males and 27 females with an average age of 36 years old ranging from 19 to 55 years old. According to AO classification,there were 64 cases with type A,27 with type B, 12 with type C. Thirteen cases were open fractures including 5 cases with Gustilo type I , 8 with Gustilo type II . The time of bone healing were observed after operation, the knee function recovery was evaluated by HSS scoring standard at 1 year after operation.
RESULTSThe intraoperative complications included femoral neck fracture in 1 case and proximal femoral fracture in 1 case,both of the patients were treated with reconstructive intramedullary interlocking nail and the fractures healed postoperatively. One patient was suffered from common peroneal nerve injury,which were fully recovered at 4 months later after medical treatment. All the patients were followed up from 12 to 28 months (averaged 22 months). All of the fractures were healed well and the average healing time was 3 to 9 months (averaged 5 months). All the hip joints were recovered to normal function. The average HSS was 90.89±5.06 at 1 year after operation.
CONCLUSIONInterlocking intramedullary nailing is the preferred treatment for patients with femoral shaft fracture. Carefully operating and closed reduction can reduce the complications.

9.Micromovement characteristic of distal tibiofibular joints in normal Chinese.

OBJECTIVETo analyze micromovement of distal tihiofihular joints in different motion range of ankle joint ,and define the micromovement characteristic and range of distal tibiofihular joints.
METHODSTwelve normal Chinese were chosen. There were 9 males and 3 females, aged from 19 to 37 years old with an average (26.5 +/- 0.5) years. Detection terminals of laser photographic scanner were installed near the highest point between medial malleolus and lateral malleolos, the change of detection terminals on the position of dorsiflex, extension, introversion and eversion of ankle joint were scanned by 3D-laser scanner. The displacement of two detection terminals on the X ,Y and Z-axis (X-axis stands for the vertical-axis between coronal plane and Z-axis Y-axis stands for the vertical-axis between sagittal plane and Z-axis Z-axis stands for macroaxis of tibia).
RESULTSAlong with increased range of motion on the position of dorsiflex ,extension, introversion and eversion of ankle joint, the range of micromovement of distal tibiofibolar joints increased too. The max-displacement of X, Y and Z were respectively (1.04 +/- 0.12) mm, (1.70 +/- 0.16) mm and--(0.87 +/- 0.10) mm.
CONCLUSION3D-laser scanner can be used to determine the detailed displacement of distal tibiofibolar joint on the X , Y and Z, and measure the motion of distal tibiofibular joint. The method can be used to study the pathologic change of distal tibiofibular joint ,and provide basic biomechnics data for internal fixtor fitting for the characteristic of distal tibiofibular joint.

10.Case-control study on two osteotomy techniques for the treatment of distal radial malunion.

OBJECTIVERadial corrective osteotomy is an established but challenging treatment for distal radial malunion. There is an ongoing discussion about whether an opening or closing-wedge osteotomy between should employed. The purpose of the present study was to retrospectively compare the clinical and radio graphic results between conventional opening-wedge osteotomy and closing-wedge technique.
METHODSFrom January 2004 and December 2012,42 patients with extra-articular distal radial malunion were managed with corrective osteotomy and were followed for a minimum of one year. Twenty-two patients (5 males and 17 females, ranging in age from 25 to 75 years old) were managed with radial opening-wedge osteotomy and implanting of interpositional bone graft or bone-graft substitute, and twenty patients (4 males and 16 females, ranging in age from 19 to 79 years) were managed with simultaneous radial closing-wedge and ulnar shortening osteotomy without bone graft. The selection of the surgical procedure was determined by the surgeon. Each patient was evaluated on the basis of objective radio graphic measurements, and functional outcomes were determined on the basis of clinical examinations, including range of wrist motion, grip strength, pain-rating score, Mayo wrist score, and Disabilities of the Arm, Shoulder and Hand (DASH) score.
RESULTSThe mean duration of follow-up was 36 months (ranged, 12 to 101 months) for the opening-wedge cohort and 28 months (ranged, 12 to 87 months) for the closing-wedge cohort. The two techniques were comparable in terms of complications. Post-operative volar tilt and ulnar variance were improved significantly in each cohort. The ulnar variance was more frequently restored to within defined criteria (22.5 to 0.5 mm) in the closing-wedge cohort than that in the opening-wedge cohort. The post-operative mean extension-flexion are of the wrist and Mayo wrist score were significantly better in the closing-wedge cohort. Differences in the pronation-supination arc, grip strength, pain-rating score, and DASH scores between these two cohorts were not significant.
CONCLUSIONThe closing wedge osteotomy technique is an effective reconstructive procedure for the treatment of extra-articular distal radial malunion. It is significantly better than the opening-wedge osteotomy technique in terms of the restoration of ulnar variance, the extension-flexion arc of wrist motion, and the Mayo wrist score.
