OBJECTIVE:To construct surgery base drug management system,and to realize efficient and accurate drug man-agement in surgery room. METHODS:The functions of surgery base drug management system exploited by our hospital were intro-duced,and the effect of the system was evaluated in department of anesthesiology. RESULTS&CONCLUSIONS:The system pos-sesses the functions of surgery drug automated affiliated charge,standardized narcotics prescription autogeneration,discarded nar-cotics prescription auto-prescribing,and drug information summary statisticing and checking,etc. After the application of the sys-tem,nonstandard rate of prescription decreased from 12.7% of handwritten prescription to 0.1% of electronic prescription;the time of drug requisition and checking decreased from(8.5±1.6)min to 0 min and(7.6±1.0)min to(2.9±0.9)min(P<0.05 or P<0.01). The system standardizes medication behavior of physicians and improve their work efficiency,avoid the loophole of drug charge management,realize the consistency between the accounts and the real numbers of narcotics,improve the rate of quality pre-scriptions and narcotics management, and realize integration,automation,intellectualization and whole-course supervision of drug management in pharmacy and clinical departments.

Objective To assess the effectiveness of treatment of meniscal injuries of knee joints by arthroscopy.Methods 33 patients 35 joints were followed up and the parts,types and treatment under arthroscopy were analysed.Results 33 patients were followed up from six months to six years,the mean preoperative Lysholm score was 60.5 points,and the mean postoperative one was 86.7 points.Conclusion The advantage of treating meniscal injuries by arthroscopy was the result of correct examination and little wound of arthroscopy operation,and arthroscopic repair or partial menisectomy could effectively restore the function of the injured knee.

Background Researches showed that as the non-optical factors,cognitive has certain influence on the regulating system.So accurately experimental design is one of the key steps that evaluates the non-optical factors on regulating system.Objective The present study was to investigate the influence of presenting pattern of target and watching way on the level and amplitude of accommodative fluctuate and to analyze the effect of focus gaze of cognitive on regulating system and the relationship between focus gaze condition under near work and the development of myopia.Methods This study complied with Declaration of Helsinki,and the permission of Ethic Committee and written informed consent was obtained before entering in this trial.Thirty healthy volunteers were included with the mean age (24.80 ± 1.98) years old,equivalent refractive diopter (-1.92 ± 2.02) D and mean cylinder (-0.19±0.58) D.The presenting pattern of the targets was designed as focus gaze and relaxed gaze.The accommodative response and accommodative fluctuation in the complete corrected right eyes for the different targets at the 40 cm under the gazing state was recorded with Grand Seiko WAM 5500 automatic infrared refractor in the experiment.Results The mean accommodative response value was (1.86±0.26) D under the focus gaze and (1.27±0.39) D under the relax gaze,showing a statistically significant difference (t=-8.052,P=0.000).The mean fluctuate value was(0.17±0.06) D under the focus gaze,with a significant lowing in comparison with (0.28±0.17) D under the relax gaze (t =3.600,P =0.001).Conclusions These results demonstrate that the different presenting patterns of sighting target and watching ways of the subjects affect accommodation system.The accommodative response was relatively more accurate with a smaller microwavc moving under the focus gaze condition.

OBJECTIVETo compare clinical outcomes of swing shoulder and internal fixation in treating proximal humeral fractures.

METHODSFrom June 2007 to June 2012, totally 89 elderly patients with humeral proximal fractures were treated by swing of shoulder or internal fixation, and 81 patients were followed up. In swing shoulder group, there were 38 patients including 13 males and 25 females aged from 62 to 84 with an average of (67.11±6.18) years old; 27 cases were 2-part fractures and 11 cases were 3-part fractures according to Neer classfication. In internal fixation group, there were 43 patients including 16 males and 27 females aged from 60 to 80 with an average of (66.47±5.48) years old; and 29 cases were 2-part fractures and 14 cases were 3-part fractures according to Neer classfication. VAS score and complications were compared between two groups after treatment, and Constant-Murley functional scoring was used to evaluate shoulder function of patients.

RESULTSEighty-one patients were followed up from 13 to 26 months with an average of 18.3 months. There was no significant difference in preoperative VAS score between two groups. After treatment, VAS score in swing shoulder group was (3.11±0.95), and (3.88±1.14) in internal fixation group, and had significant difference between two groups (t=-3.313,P<0.05). There was no significant difference in Constant-Murley scores between swing shoulder group (79.53±3.73) and internal fixation group (77.98±4.11) (t=1.768,P>0.05). Postoperative complications in swing shoulder group was 18.4%(7/38), 39.5%(17/43) in internal fixation group, and had significant differences between two groups (χ2=4.313,P<0.05).

CONCLUSIONSwing shoulder for the treatment of proximal humeral fractures in elderly has advantages of low cost, less complications and good recovery of joint function; while internal fixation has a good therapeutic effect but increased complications.

Objective To assess the treatment effects of As_2O_3-PLGA microspheres on VX2 rabbits liver tumor models.Methods Thirty-two New Zealand white rabbits were involved with formation of VX2 rabbits liver tumor models,and randomly divided into 4 groups of 8 each.(a)As_2O_3 plus 0.9% NaCl solution group:3 F co-catheter was plugged into hepatic artery,and then As_2O_3(3 mg/kg)plus 10 ml 0.9%NaCl solution was injected into the tumor through the feeding artery;(b)As_2O_3-PLGA microspheres group :As_2O_3- PLGA microspheres(3 mg/kg)was injected into the tumor through the feeding artery;(c)PLGA mierospheres group:Injection of PLGA microspheres(3 mg/kg)injected into the tumor through the feeding artery;(d) control group:0.9% NaCl solution(10 ml)was injected into the feeding artery.Dual phase helical CT scans were performed the day before treatment,20 days after treatment and finally the rabbits were sacrificed.The sizes of the tumor were measured and followed by histopathological analysis and HE staining.Results The volumes of tumors post operatively of group a,b,c and d were(31.08?11.15)cm~3,(3.82?2.537)cm~3, (13.22?4.665)cm~3 and(115.8?62.01)cm~3 respectively.The most significant treatment effect was available in group b,with more coagulation necrosis and fibrotic tissue formation.Statistical analysis indicated that the treatment effects of group a,b,c were better than that of group d with significant difference which can be also seen between group a and b,group b and c.Conclusion As_2O_3-PLGA microspheres shows rather excellent chemoembolization effects on VX2 rabbit liver tumor model with safely.

Gastric adenocarcinoma (GC) is one of the most common malignancies in the world and one of the most frequent causes of cancer-related death.Autophagy is a highly regulated catabolic pathway responsible for the degradation of long-lived proteins and damaged intracellular organelles.However,the mechanism and guiding significance of autophagy in the development and progression of GC have remained to be elucidated.This study aimed to explore the clinicopathological significances and prognostic values of autophagy-related proteins AMBRA1 and Beclin-1 in GC.Quantum dots based immunofluorescence histochemistry (QDs-IHC) was performed to observe the expression of AMBRA1 and Beclin-1 proteins in the tissue rmicroarrays including 163 specimens of GC and 20 noncancerous gastric tissues.Simultaneously,AMBRA1 and Beclin-1 proteins were detected by Western blotting in the 10 fresh GC and corresponding normal gastric tissues.The results showed that the expression levels of both AMBRA1 and Beclin-1 proteins were higher in GC tissues than in noncancerous gastric tissues by QDs-IHC and Western blotting (P＜0.05).High AMBRA1 expression was detected in 90 of 163 (55.2％) GCs and high Beclin-1 expression was detected in 83 of 163 (50.9％) GCs.High AMBRA1 expression was closely related to depth of invasion,and lymph nodes metastasis (P＜0.05).High expression of Beclin-1 protein was correlated with tumor grade (P＜0.05).Positive correlation was observed between AMBRA1 and Beclin-1.Survival analysis indicated the high expression of AMBRA1 and Beclin-1 was an independent factor in predicting poor overall survival (OS) of GC patients.These findings suggest the high expression of AMBRA1 and Beclin-1 proteins is significantly correlated with GC progression.High AMBRA1 and Beclin-1 expression heralds worse outcome of GC patients,suggesting a novel candidate prognostic marker and a therapeutic target for GC.
