1.Effect of Busheng Huoxue Capsule on the quality of life of primary osteoporosis senile males.

OBJECTIVETo investigate the effect of Busheng Huoxue Capsule (BHC) on the quality of life (QOL) in senile male osteoporosis (OP) patients, and to explore its mechanisms.
METHODSTotally 200 senile primary OP patients were randomly assigned to two groups according to random digit table method, 100 in each group. Patients in the treatment group took BHC plus caltrate-D (600 mg CaCO3), while those in the control group took alendronate (70 mg per week) plus caltrate-D. The therapeutic course was 12 months for all. Chinese medical symptom score and quality of life (QUALEFFO-41) score, bone mineral density (BMD) of lumbar vertebra (L2 -L4) and left femoral neck were compared between the two groups before and after treatment. Serum free testosterone (FT) and estradiol (E2) were also measured.
RESULTSChinese medical symptom scores and QUALEFFO-41 scores, serum FT and E2 levels increased in the two groups after treatment (P < 0.05, P < 0.01). The therapeutic effect was superior in the treatment group (P < 0.05, P < 0.01). After treatment the BMD of lumbar vertebra (L2 -L4) and the left femoral neck were somewhat improved (P < 0.05), but with no statistical difference between the two groups (P > 0.05).
CONCLUSIONBHC could effectively improve the QOL of senile male OP patients, which might be correlated with elevating the BMD levels and regulating the levels of sex hormones.

3.Acortatarin A inhibits high glucose-induced extracellular matrix production in mesangial cells

Background Diabetic nephropathy (DN) is the leading cause of end-stage renal disease.Various treatment regimens and combinations of therapies provide only partial renoprotection.Therefore new approaches are needed to retard the progression of DN.The aim of the present study was to evaluate the role of a novel spiroalkaloid from Acorus tatarinowii named acortatarin A (AcorA) in inhibiting high glucose-induced extracellular matrix accumulation in mesangial cells (MCs).Methods The cytotoxity of AcorA on MCs was examined by 3-(4,5-dimethylthiazol-2-yl)-2.5-diphenyltetrazolium bromide (MTT) assay.The expression of fibronectin and collagen Ⅳ was examined by real time PCR and western blotting.The expression of p22phox and p47phox was detected by western blot.The interaction between p22phox and p47phox was examined by co-immunoprecipitation.The phosphorylation of p47phox was examined by immunoprecipitation.The phosphorylation of protein kinase C (PKC) α,PKCβ,phospholiase C gamma (PLCγ1),and the p85 subunit of PI3K was determined by Western blotting.Results AcorA significantly inhibited high glucose-induced activation of NADPH oxidase,a ROS-generating enzyme,by increasing phosphorylation of p47phox and enhancing interaction between p22phox and p47phox.Preincubation of AcorA with MCs inhibited high glucose-induced collagen Ⅳ and fibronectin production in a dose-dependent manner.Moreover,AcorA attenuated high glucose enhanced phosphorylation of PKCα,PKCβ,PLCγ1,and the p85 subunit of PI3K.Conclusion AcorA inhibits high glucose-induced extracellular matrix production via blocking NADPH oxidase activation.
4.Optimization of a floating osmotic pump system of dipyridamole using central composite design-response surface methodology.

A new type of floating osmotic pump of dipyridamole was designed in this paper. Apparatus three (Chinese Pharmacopeia 2005, appendix XD) was employed for in vitro dissolution test in order to evaluate the release and floating behavior in a same experiment. The system was optimized using central composite design-response surface methodology; where similarity factor (f2) between the dissolution profile of prepared formulation and the target dissolution profile was taken as dependent factor, usage amount of polyoxyethylene (X1, mg), NaCl (X2, mg) and pore former (PEG4000, X3, %) were taken as independent factors. The optimization model was f2 = -29.3 + 2.35X3 - 0.123X1(2) - 0.046X2(2) + 0.145X1X2 (R = 0.996). It was found that the dissolution profile could match the target dissolution profile under the condition of weight gain 8%-9%, X1 (20-34), X2 (30-57), X3 = 50. It is also found that the minimum usage percentage of pore former is 35.1%. The prediction results of the optimization model were good in the experiments.

5.Comparison between mini-traumatic bone-grafting and non-bone-grafting in percutaneous K-wire fixation to treat the calcaneal fractures.

OBJECTIVETo compare the effect between mini-traumatic bone-grafting and non-bone-grafting in percutaneous K-wire fixation for treating the calcaneal fractures.
METHODSFrom 2002 to 2006, 112 patients with the type II (Paley type) fractures of calcaneus were studied. There were 56 cases in bone-grafting group involving 36 males and 20 famales,aged from 21 to 65, averaged (42.0 +/- 2.3) years; 11 cases were in type II a and 45 were in type II b; the course was from 3 to 14 days, averaged (6.0 +/- 1.2) days. And there were 56 cases in non-bone-grafting group involving 38 males and 18 famales,aged from 22 to 67, averaged (43.0 +/- 2.5)years; 13 cases were in type II a and 43 were in type II b; the course was from 2 to 15 days, averaged (5.0-2.1) days. All the cases were treated by closed reduction and percutaneous K-wire fixation, and bone-grafting group(56 cases) were treated by mini-traumatic bone-grafting, but the other group (56 cases) were not. The collapsing rate and fineness rate were compared.
RESULTSAll the cases were followed up from 5 to 52 months. There were no collapsing cases in the bone-grafting group after operation, but 3 cases occurrenced re-collapsing in the non-bone-grafting group. According to the Zhang Tie-liang's evaluation criterion, in the bone-grafting group,the results were excellent in 43 cases, good in 12, fair in 1, the fineness rate was 98.2%. In the non-bone-grafting group,the results were excellent in 37 cases, good in 16, fair in 2, poor in 1, the fineness rate was 94.7%.
CONCLUSIONTreatment of the type II fracture of calcaneus with closed reduction, percutaneous K-wire fixation and mini-traumatic bone-grafting can prevent the posterior talar articular surface of caltaneus from collapsing again after operation, enhance the union of fracture, elevate the curative effect, thus it should be taken with the standard therapeutic regimen.

6.Effects of allitridin on the expression of human cytomegalovirus immediate early antigens-IE72 and IE86 in human embryonic lung cells.

OBJECTIVETo investigate the effect of allitridin injection on the expression of human cytomegalovirus (HCMV) immediate-early antigens (IEAs including IE72 and IE86) in human embryonic lung cells.
METHODHCMV AD 169 Virus strain infected cell model (MOI = 2.5 and 0.25, respectively) were established, and then treated with ICm5 and MTC doses of allitridin. Western blot was used to analyze the of IE72 and IE86 expression after the treatment, ganciclovir(GCV, IC50 and 2.3 x IC50) treatment as control.
RESULTNo matter what kind of MOI was used, both IE86 and IE72 antigens' expression was effectively suppressed by allitridin treatment, and the inhibitory rate of IE86 was almost twice of IE72's. Compared with GCV, allitridin had stronger inhibitory effect on IE86 expressing, although its efficacy on IE72 was weaker than GCV.
CONCLUSIONAllitridin could suppress the expression of IE72 and IE86, especially for IE86 expressing, maybe it is ore of key role in the mechanism of allitridin against HCMV.

7.Optimization of process for standardizing antimicrobial use opportunity in consecutive operations

Objective To optimize antimicrobial use process,ensure the rational use of preoperative antimicrobial prophylaxis during consecutive operations.Methods Antimicrobial use process in a hospital in December 2015 was optimized,6 072 cases of consecutive operations in May-November 2015 were selected as control group,5 832 cases of consecutive operations in December 2015-May 2016 were as trial group,the qualified rate of rational use of antimicrobial agents was compared between two groups,causes for delayed/prior use was analyzed.Results Before and after the optimization of antimicrobial use process,rates of antimicrobial use were 77.16% and 78.80% respectively,there was significant difference between two groups(x2 =8.305,P =0.004).After the optimization of antimicrobial use process,rate of antimicrobial use within 0.5-1 hour was significantly higher than that before the optimization (82.36% vs 41.11%);rate of antimicrobial use <0.5 hour before skin incision decreased from 57.11% before optimization to 4.32% after optimization;but rate of antimicrobial use >1 hour before skin incision increased from 1.78% to 13.32%.Causes for delay/prior use of antimicrobial agents was due to the lack of effective communication between doctors and nurses,which resulted in circuit nurses' inaccurate assessment on interval of consecutive operations(62.13%),the duration of intubation or puncture was too long for anesthesiologists (13.57%).Conclusion Optimizing antimicrobial use process in consecutive operations can improve prophylactic antimicrobial use rate within 0.5-1 hour,and is helpful for ensuring the efficacy of antimicrobial prophylaxis.
8.Change of platelet count and P-selectin in preserved platelet concentrates.

To observe the change of quantity and quality of platelets preserved in a full-sealed bag, and explore the difference of platelets preserved in oscillating and static conditions at (22 +/- 2) degrees C, the platelet concentrates were prepared with a CS-3000-plus blood cell separator, the platelet counts were performed with automatic blood cell analyzer and P-selectin in supernatant of platelet concentrates was detected by ELISA. The results showed that both of platelet count and P-selectin content in the platelet concentrates had no significant difference between oscillating and static preservation condition. With prolongation of preserved time, the platelet count decreased and P-selectin content increased gradually in both preserved conditions. There was no difference in the platelet counts during 0 - 72 hours preservation in both conditions, and significant difference was seen in 96 - 120 hours preservation. It was concluded that the expired date for platelet product preserved in CS-3000-plus blood cell separator full-sealed system should be 3 days. Under the condition of (22 +/- 2) degrees C, the quality of the platelet preserved in oscillating state is not superior to static preservation.

9.Oral health status in Sichuan Province:findings from the oral health survey of Sichuan, 2015–2016

To investigate oral health status in the residents of Sichuan Province, southwest China, a cross-sectional study was performed using the latest Oral Health Survey Basic Methods recommended by the World Health Organization. A multistage stratified random cluster-sampling method was used to enroll participants from the following three groups: children aged 3–5 years, adolescents aged 12 years, and people aged 65–74 years. In these three groups, the mean numbers of teeth that were affected by caries were 3.28, 0.86 and 5.13, respectively, resulting in a prevalence of 63.47%, 37.20%and 83.20%, respectively. Relative to the high rate of decayed teeth, the prevalence of fillings was very low in all age groups (0.97%, 7.24% and 5.43%, respectively). In the 12-year-old adolescent group, only 3.61%had good pit and fissure sealing. In addition, the rate of dental fluorosis was 24.80%, and the Community Fluorosis Index value was 0.39. In the elder group, the community periodontal index was 2.92. The prevalence in the elderly of having lost at least one tooth was 75.54%. Additionally, 4.44%of these participants were edentulous. The incidence of dental prosthesis was 51.75%, the proportion with a removable partial denture, a fixed denture, full dentures, dental implants and an informal fixed bridge was 21.59%, 11.45%, 4.64%, 0 and 16.67%, respectively. In this study, 8.2%of the elderly participants were affected by different types of oral mucosal lesions. Among such lesions, recurrent aphthous ulcers were most prevalent (2.69%) and oral lichen planuses were second (1.6%). The conclusion presented in this survey is that oral diseases, especially dental caries and periodontal disease, are frequent and common in Sichuan province, China. Moreover, the treatment rate is very low, and primary prevention and treatment options are therefore urgently needed in this population.
