BackgroundCataract is a leading blinding eye disease,and the prevalence of cataract varies in different regions.ObjectiveThe present study was to investigate the incidence of cataract,surgical coverage rate of cataract and postoperative visual acuity,and to propose a strategy for blindness prevention in Chifeng Keshiketengqi.Methods This is a cross-sectional survey of research.A random cluster sampling method was performed in 12 populated areas in Keshiketengqi.Inhabitants over 50 years in the region were included in this survey.This study was approved by the Ethic Committee of Inner Mongolia People' s Hospital.Informed consents were obtained orally from all the subjects.Disease history,visual acuity,intraocular pressure and regular ophthamological examinations were performed.The inclusion criteria of a pilot study and detailed protocol were employed to calculate the incidence rate of cataract.The questionnaire survey was simultaneously adopted to analyze the cataract-related factors.The surgical coverage rate of cataract in this area was calculated.ResultsIn a total 4234 inhabitants,3826 subjects participated in the survey with a response rate of 90.36％.Cataract was found in 1200 participants with an incidence rate of 31.39％.Age,the female gender and illiteracy were identified as risk factors for cataract (x2 =101.005,P =0.000 ;x2 =58.853,P =0.000 ;x2 =12.037,P =0.007 ).The prevalence rates of cataract were 36.98％ and 33.56％ for illiterates and women,respectively.The incidence rate in the Mongol population was significantly higher than that of the Han population ( 34.83％ vs.29.76％ ) (x2 =9.734,P =0.001 ).The cataract surgery rate increased yearly since 1998.When the vision acuity of the fellow eye was ＜0.1,the surgery coverage rate was 20.60％,but the rates were lower in female,illiterates and the Mongol population (x2 =1.993,P =0.038 ;x2 =16.463,P =0.044 ;x2 =8.523,P =0.029).When a pinhole visual acuity of ＜0.05 was determined as the criteria of blindness,the surgical coverage rate was 28.36％.The cataract surgical coverage rates for Han and Mongolian were 30.67％ and 24.53％,respectively,with a significant difference between them (x2 =12.951,P=0.000).The social burden rate was 3.34％ in this area,with a majority of females,illiterates and the Mongol population ( P＜0.05 ).ConclusionsIn agricultural and pastoral areas of Chifeng Keshiketengqi,the ≥50 years old population has a high rate of cataract blindness and low surgery coverage rate.Surgery is still the preferred way for cataract treatment.

OBJECTIVETo explore the clinical manifestations, diagnosis, treatment and prognosis of acute panautonomic neuropathy (APN).

METHODSWe reviewed the history, clinical findings, electrophysiological characteristics, laboratory features of CSF and treatment of 4 patients with APN.

RESULTSAll these patients showed acute onset with apparent involvement of the autonomic nervous system. The clinical features of autonomic involvement included fixed dilated pupils, abdominal pain, diarrhea, and anhidrosis. Electromyography showed evidence of peripheral neuropathy. Early treatment with corticosteroid and intravenous immunoglobulins resulted in improved prognosis of the patients.

CONCLUSIONAPN is a rare autonomic nervous system disorder, and understanding of the clinical features may help in early diagnosis and treatment of the patients.

OBJECTIVETo determine the incidence of work-related acute pesticide poisoning among farmers in Jiangsu Province and to identify the risk factors.

METHODSMulti-stage stratified sampling and cluster sampling methods were used to randomly select 1490 farmers from the south (Z village), centre (Y village) and north (X village) of Jiangsu Province. The database was established by EpiData 3.1. SPSS 17.0 and SAS 9.13 were used to do chi-square test, trend chi-square test, single factor and multi-factor logistic regression analysis.

RESULTSAmong 1490 farmers, 121 people had work-related acute pesticide poisoning in the past year. The incidence rate in Jiangsu Province was 8.1%. 118 farmers (97.5%) were mild pesticide poisoning.3 farmers (2.5%) were moderate pesticide poisoning. The incidence rate of Work-related acute pesticide poisoning in X village of North Jiangsu Province (13.5%, 68/505) is obviously higher than that in Y village of Central Jiangsu Province (4.8%, 19/399) and Z village of South Jiangsu Province (5.8%, 34/586), and the difference is statistically significant (χ(2) = 29.60, P < 0.01). The incidence rate of Work-related acute pesticide poisoning in female (10.4%, 58/557) is obviously higher than that in male (6.8%, 63/933), and the difference is statistically significant (χ(2) = 6.26, P < 0.05). There are lack of safety guidance, unsafety application methods (without alternate row spraying, without backward application, without down-wind application) and risk behaviors in the spraying (without read labels, preparing pesticides without gloves, wiping sweat with hand (s), equipment leakage, body pollution by pesticide, working when feeling sick, without bath after work). If farmers have risk behaviors above, the incidence rate of Work-related acute pesticide poisoning will be obviously higher than farmers who receive safety guidance and have no such dangerous behaviors, and all the differences are statistically significant (P < 0.05). The group of 24 ∼ 34 years old has the highest risk of work-related acute pesticide poisoning (OR = 7.15).

CONCLUSIONThe incidence rate of work-related acute pesticide poisoning among farmers in Jiangsu Province was higher. Effective measures are needed to prevent and reduce the occurrence of work-related acute pesticide poisoning. We can teach the farmers who spray pesticides about safety guidance, safety application methods and avoiding risky behaviors.

OBJECTIVEOver the last few decades, diabetic cardiomyopathy has been identified as a significant contributor in cardiac morbidity. However, the mechanisms of diabetic cardiomyopathy have not been clarified.

METHODSIn the present study, a diabetic rat model was induced by the intraperitoneal injection of streptozotocin. The myocardial CD147 expression and extent of glycosylation, as well as thematrixmetalloproteinases(MMPs) expression and activity, were observed in the diabetic and synchronous rats.

RESULTSThe results showed that CD147 located on sarcolemma of cardiomyocytes. The myocardial CD147 expression and glycosylation were significantly increased in the diabetic rats as compared with the control. Expression of MMP-2 protein, MMP-2 and MMP-9 activity were also increased in left ventricular myocardium in the diabetic rats. Tamoxifen only inhibited the enhanced expression of myocardial CD147 in the diabetic rats, but not in synchronous control rats. Tamoxifen inhibited glycosylation of myocardial CD147 in both diabetic and control rats. The inhibition of tamoxifen on CD147 glycosylation was stronger than on the expression in the myocardium. The extent of myocardial CD147glycosylation was positively related toMMP-2 and MMP-9 activity. Tamoxifen induced an inhibition of myocardial MMP-2 and MMP-9 activity in the control and diabetic rats.

CONCLUSIONThese results indicate that myocardial CD147 expression, especially the extent of glycosylation, regulates MMP-2 and MMP-9 activity, then accelerates cardiac pathological remodeling inducing diabetic cardiomyopathy. Tamoxifen inhibits myocardial CD147 glycosylation and further depress the activity of MMPs. Therefore, tamoxifen may protect the diabetic rats against diabetic myocardium.

Background The in vitro culture of retinal vascular endothelial cells is the foundation of experimental study of retinal vascular disease. Shortage of human donor eyeballs is a main limiting for the laboratory work. The culture method of rat-derived vascular endothelial cells has been established. However, this method is not enough effective because of severer cellullar injury. Objective Present study was to establish a simple and high effective method for the culture of vascular endothelial cells in vitro. Methods The retinas from 5 SPF SD rats was digested by 0. 1% collagenase and cultured with explant culture method. 20% fetal bovine serum, vascular endothelial growth factor ( VEGF) , insulin-transferrin-selenium( ITS) were composed into the endothelial cell culture medium, and enough blowing was performed to get the cells and fragments from retinal tissue. The cellular suspension was prepared and cultured consequently on human fibronectin-coated culture flasks. Cultured vascular endothelial cells were identified by anti-von Willebrand staining factor. Results The cells emerged from the tissue mass,and cells and some tissue fragments attached to the wall after 24 hours of seeding. The cells grew to show the fusiform in 4 days and merged together in 5 to 6 days,and a cell monolayer was seen in the 14th day after culture. The endothelial cells showed the positive response for von Willebrand factor. After passage, the merging-growth statue of the cells was regained in 2 hours after culture. Conclusion Use of retinal pieces and collagenase-digestion can get the vascular endothelial cells with better activity in vitro. The culture method based on highly selective endothelial cell culture medium associated to FN adhesion-promoting is helpful for gaining the purified of endothelial cells.

Objective To investigate the possible role of interstitial cell of Cajal (ICE)in the pathogenesis of diabetic gastroparesis and the protective effect of"Xiaopi prescription (消痞方)".Methods Fifty healthy male Spregue-Dawley (SD) rats were randomly divided into 5 groups (each n=10):normal, model,and 3 Xiaopi prescription groups:low,middle and high dosages.Diabetes was induced by intravenous injection of alloxan,and equal amount of normal saline was intravenously injected in the normal group.Gastric lavage method was used to administer the traditional Chinese medicine decoction of Xiaopi prescription in corresponding amount (5,10 and 20 g?kg~(-1)?d~(-1)) in respective low,middle and high dosage groups.In the normal control group and diabetic model group,only equal amount of normal saline was administered into the stomach.Gastric emptying rate was measured by method of nutritious semisolid paste;c-kit positive cells of ICC were quantitatively measured with immunohistochemistry assay and computer image analysis system;c-kit mRNA positive cells were quantitatively measured with in situ hybridization and computer image analysis system.Results①Gastric emptying rate:The rate was significantly lower in the model group than that in the normal control group (P0.05),but higher than those in the model group and the low dosage group (all P0.05).②c-kit immmunohistochemistry:c-kit positive cell presented yellow in color,and its membrane was stained yellow,this kind of cells primarily were distributed around the neural plexus in the inter-space between the circular and longitudinal muscular fibers, and around the ganglionic cells forming"sheath-like"structure.The results of numbers of c-kit positive cells in the various groups:the number of the cells in the model group was significantly lower than that in the normal group (P0.05),but the numbers in the former two dosage groups were obviously higher than those in the model group (all P0.05),being significantly lower than that in the normal group,middle and high dosage groups (all P0.05),but obviously higher than those in the model group (all P0.05),being significantly lower than that in the normal,middle and high dosage groups (all P

OBJECTIVE: To discuss X-ray findings and clinical analysis of neonatal respiratory distress syndrome (NRDS), and to compare the use of pulmonary surfactant and mechanical ventilation. METHODS: The X-ray findings, clinical characteristics, and therapy analysis of 32 babies with NRDS were studied retrospectively from Jan. 2004 to Dec. 2006. The correlation between X-ray findings and clinical characteristics was analyzed. The advantages of surfactant replacement therapy, compared with mechanical ventilation alone were discussed. RESULTS: According to X-ray findings all babies were divided into Stage I (n=7), Stage II (n=11), Stage III (n=8), Stage IV (n=3), and normal (n=3). Of them, 18 were very low-birth-weight neonates, and 11 were very low-birth-weight neonates. Twenty received surfactant replacement therapy, after the diagnosis of NRDS had been established clinically and radiographically. NRDS had classic chest radiographic findings. There was statistically significant difference between the fetal age or birth weight and the first time X-findings (P<0.05), and difference existed between the use of surfactant and nCPAP alone (P<0.01). CONCLUSION The younger the fetal age and the lower the birth weight, the more likely that the first X-ray findings would be below Stage II. Early surfactant replacement therapy with extubation to nCPAP for infants with NRDS was not only safe and beneficial, but also it significantly reduced both the need for mechanical ventilation and the briefer requirement for respiratory supports.

OBJECTIVETo summarize the experiences of ligating left subclavian artery (LSA) in total arch replacement and stented elephant trunk implantation for Stanford type A aortic dissection patients with difficulty in exposing the LSA.

METHODSTotal arch replacement and stented elephant trunk implantation were performed on 79 consecutive patients from January 2008 to June 2010. Twenty-nine cases of the cohort undertook LSA ligation due to bad exposure. There were 21 males and 8 females patients, aged from 19 to 55 years with a mean of (44 ± 12) years. There were 12 acute dissections, 4 sub-acute dissections and 13 chronic dissections. Based on thoroughly evaluation of the Willis' circle and bilateral vertebral arteries through pre-operative imaging and intra-operative circulative parameters, if the collateral circulation was considered sufficient, LSA was ligated directly and only the innominate artery and carotid artery were reconstructed; if considered insufficient, an additional bypass from ascending aorta to left axillary artery was performed.

RESULTSAll the 29 operations were completed successfully. There was one patient died from pulmonary infection and the others recovered well.Blood pressure of left arms were lower than right postoperatively [(78 ± 17) mmHg vs. (126 ± 24) mmHg, 1 mmHg = 0.133 kPa, P < 0.01], but oxygen saturation, skin temperature and strength of the left hand were normal compared to the right. All the survived patients have been followed 1 - 27 months and none of them presented with any symptoms of left subclavian artery steal syndrome and ischemia of left arms.

CONCLUSIONSLigation of LSA under strict evaluation of collateral circulation could be safe in Type A dissection patients with bad exposure due to big ascending aortic aneurysm and will simplify the procedure significantly.

OBJECTIVETo investigate the clinical value of extended radical resection with nerve- preservation for rectal cancer.

METHODNinety-eight patients with rectal cancer received extended radical resection with nerve- preservation in our hospital. The questionnaire were used to collect the data of the patients urination and sexual function. The survival was analyzed retrospectively.

RESULTS62.3% (61/98) of the patients could erect normally and 57.1% (56/98) of the patients had normal sexual function. The average time of catheterization in 57 patients was 60 hours, the residual urine volume (RUV) was 28 ml and the max-micturition-desire urine volume was 400 ml. The 5-year survival rate of those who underwent extended radical resection with nerve-preservation was 61.2%.

CONCLUSIONExtended radical resection with nerve-preservation,which could decrease the incidences of post-operative urination and sexual dysfunction, and have not affect the survival, was the most optimal operation for rectal cancer.
