1.Application of arthroscope-assisted surgical technique in treating knee or knee peripheral fractures

Objective To evaluate the effectiveness of arthroscope-assisted surgical technique in treating knee fractures and knee peripheral fractures.Methods From September 2000 to December 2005,there were 46 cases with knee or knee peripheral fractures,in whom artbroseope-assisted surgical technique was performed in 36 cases including 24 males and 12 females,with mean age of 30.5 years (18-52 years).Of 36 cases,26 cases with patella fractures were treated by internal fixation of tension band using cancellous bone screws,four with distal end femoral fractures by retrograde interlocking using intramedullary nail and six with fractures of tibial plateau and tibial condyle crista by internal fixation u- sing cancellous bone screws.Results The mean hospitalization was eight days.A follow up for 3-36 months(average 14 months)in 36 cases showed fracture healing.Of 36 cases,32 cases regained full knee range of motion,four left with terminal flexion,with mean loss of 15?,and had thigh circumference (10 cm above knee)1.1 cm(range 0.6-2.0 cm)shorter than health side and two had slight pain in the knee joint.Lysholm knee joint function score was 86.5+7.6 for patellar fractures,tibial plateau frac- tures and condylar crista fractures.Conclusion As for knee or knee peripheral fractures,arthroscope- assisted surgical technique can provide fine monitoring,decrease trauma and complications,fixate inter- nally and help early ground exercise.

4.Sustentaculum tali screw fixation for the treatment of Sanders type II and III calcaneal fractures.

OBJECTIVETo explore the clinical outcomes of open reduction and internal fixation with calcaneal locking plates in treating Sanders type II and III calcaneal fractures.
METHODSFrom January 2010 and October 2012, 38 calcaneal fractures with Sanders type II or III were treated with open reduction and internal fixation with calcaneal locking plate. According to the Sanders classification, 15 fractures were classified as type II, 23 fractures as type III. The patients were divided into two groups (group A and B) according to the different fixed methods. Sustentaculum tali was fixed with one screw in group A, including 13 males and 5 females, with a mean age of (38.56±8.03) years old (ranged, 25 to 55). And sustentaculum tali was not fixed in group B, including 16 males and 4 females, with a mean age of (42.35±8.29) years old (ranged, 29 to 53). Clinical effects were evaluated according to the changes of Böhler's angle and the Maryland Foot Score and VAS score.
RESULTSAll patients were followed up from 12 to 20 months with a mean of 14 months. Böhler's angles and subtalar joints obtained satisfactory reconstruction in all patients. One year after operation, the mean Maryland Foot Score was 88.61±7.59 in group A; and was 82.40±9.24 in group B; Maryland Foot Score of group A was higher and foot functional rehabilitation was better than group B. The mean VAS score was 13.39±11.47 in group A; and was 22.50±13.10 in group B; VAS score of group A was lower and foot pain was less than group B.
CONCLUSIONSustentaculum tall screw fixation has advantages of strong fixed strength, high stability, less postoperative pain, rapid functional recovery in treating Sanders type II and III calcaneal fractures.

5.Association of Epstein-Barr virus with lymphoepithelioma-like carcinoma of the parotid gland

Purpose To investigate the relationship between Epstein-Barr virus (EBV) and lymphoepithelioma-like carcinoma (LELC) of the parotid gland and detect the gene expression products of EBV harbouring in LELC cells. Methods Thirty-two parotid LELCs were collected from the Departments of Pathology, Sun Yat-sen University of Medical Sciences, Guangzhou, China during the period of January 1986 and December 1995. All the 32 formalin-fixed paraffin-embedded blocks had been consecutively re-sectioned again. Immunohistochemical and in situ nucleic acid hybridization methods for detection of EBV gene encoded products were performed. Results (1) 32 LELCs were found out of 125 parotid gland carcinomas, the frequency was 25.6% (32/125). (2) All of the 32 specimens contained a variable number of EBNA-1 and EBERs positive neoplastic cells. (3) Twenty-seven out of 32 specimens (27/32, 84.4%) had a portion of carcinoma cells expressing LMP-1. (4) No ZEBRA positive cell could be found. (5) EA-D, VCA and MA positivity rates for these 32 parotid LELCs reached to 71.9%(23/32), 68.8%(22/32), and 12.5%(4/32), respectively. Conclusions (1) The parotid gland LELC is frequently to be seen in Guangzhou locale of China, where is a high-incidence area of nasopharyngeal carcinoma (NPC). The parotid gland LELC and NPC are co-prevalent in Guangzhou locale. (2) This disease is also consistently associated with EBV infection. (3) The EBV infection of the parotid gland LELCs is essentially the type of latency Ⅱ, expressing EBNA-1, EBERs and LMP-1. (4) The latent infected EBV harbouring in LELC cells could in part be switched over to lytic cycle, producing EA-D, VCA or/and MA.
6.Autologous Hematopoietic Stem Cell Transplantation for Hematological Malignancies and Solid Tumors

Objective: This study was designed to observe the efficacy and side effects of autologous hematopoietic stem cell transplantation (AHSCT) in the treatment of hematological malignancies and solid tumors. Methods: In the 7 patients, two received autologous bone marrow transplantation (ABMT) and five received autologous peripheral blood stem cell transplantation (APBSCT). The conditioning regimen consisted of VCCA (Vincristine, Me-CCNU, Ara-C, CTX) and TLI(total lymph irradiation) for malignant lymphoma, VCCED (VCR, Me-CCNU, CTX, Vp-16, DNR) and TBI for acute myeloid leukemia-M4, VCCME (VCR, Me-CCNU, CTX, Mitoxantron,Vp-16) for breast cancer. All harvests of stem cells were stored at 4℃ except one at -80℃ ,than transfused back to the patients within 72 hours without purgation. Results: The number of mononuclear cells and CD34+ cells in harvest in APBSCT patients was (7.45± 5.89)× 108/kg and (18.62± 4.74)× 106/kg,respectively. The patients' granulocyte count reached ≥ 0.5× 109/L on day+ 9.6 and+ 11, platelet count reached ≥ 50× 109/L on day + 12.6 and+ 21.5 and reticulocyte reached ≥ 0.5% on day+ 11.2 and+ 16.5,respectively,after APBSCT and ABMT. The side effects were mainly the gastrointestinal reactions. Up to now, no relapse were seen but four women had developed secondary amenorrhea after AHSCT. Conclusion: AHSCT is an efficient way for malignant tumors and there were earlier hematopoietic reconstitution and fewer side effects in patient with APBSCT than with ABMT. The impairment of gonad by conditioning may affect the quality of life.
7.Clinical association of gingipain K-caspase like subdomain expression of Porphyromonas gingivalis with puberty gingivitis.

OBJECTIVETo detect and compare the activity and intensity of gingipain K (Kgp)-caspase like subdomain in culture medium and cell extract of Porphyromonas gingivalis (Pg) isolates in puberty gingivitis and to reveal the possible association of Kgp with puberty gingivitis.
METHODSThirty-six children of 14 to 17 years old were enrolled in this study. Clinical parameters including gingival index (GI), sulcus bleeding index (SBI) and probing depth (PD) were evaluated. Subgingival plaque samples were collected and Pg isolates were obtained. 16S rRNA PCR was used to confirm Pg clinical isolates. Bacteria were grown in batches of BHI base and harvested at the end of log-phase growth. Culture fractions (culture medium and cell extract) of 10 Pg isolates were performed with SDS-PAGE and Western blot technique using primary antibody against specific Kgp-caspase like subdomain. Activity of Kgp in both samples was detected as well. The data were statistically analyzed using SPSS 11.5 software. The relationship between the Kgp intensity/activity of Kgp and the clinical parameters was statistically analyzed using Spearman correlation coefficient.
RESULTSThere was positive correlation between the intensity/activity of Kgp and the clinical parameters (P < 0.05).
CONCLUSIONSThe Kgp in clinical isolates of Pg from puberty gingivitis is in complicated forms. Caspase-like molecules with low molecular weight may exist as intracellular functional protein molecules which can affect the interaction between Pg and host. Kgp was contributes in certain degree to the pathogenesis of puberty gingivitis.

8.Antimicrobial Peptides: Antibacterial Mechanism and Therapeutic Use

Antimicrobial peptides, a cluster of small peptides secreted by the majority of creatures, have been demonstrated with activity against a wide range of microorganisms including bacteria, protozoa, yeast, fungi, viruses and even tumor cells. These peptides have some features such as broad spectrum , high effi-cacy and stability, little drug resistance. A lack of new antibiotics combined with emerging multi-drug resis-tance issues demands that new antimicrobial strategies be explored for treating these infections. It has been proposed that the antimicrobial peptides might form the foundation for a new class of clinically useful an-timicrobials. We review the advantages of these molecules in construction features and bioactivity, with the focus on the mechanism and clinical applications.
9.Study on membrane injury mechanism of total alkaloids and berberine from Coptidis Rhizoma on Aeromonas hydrophila.

To explore the antibacterial activity and mechanism of total alkaloids and berberine from Coptidis Rhizoma on Aeromonas hydrophila, and determine the effect of total alkaloids and berberine from Coptidis Rhizoma on minimum inhibitory concentrations, permeability and fluidity of cell membrane, conformation of membrane proteins and virulence factors of A. hydrophila. The results showed that both total alkaloids and berberine from Coptidis Rhizoma had antibacterial activities on A. hydrophila, with minimum inhibitory concentrations of 62.5 and 125 mg · L(-1), respectively. Total alkaloids and berberine from Coptidis Rhizoma could increase the fluidity of membrane, change the conformation of membrane porteins and increase the permeability of bacteria membrane by 24.52% and 19.66%, respectively. Besides, total alkaloids and berberine from Coptidis Rhizoma significantly decreased the hemolysis of exotoxin and the mRNA expressions of aerA and hlyA (P < 0.05, P < 0.01), the secretion of endotoxin and the mRNA expression of LpxC (P < 0.05, P < 0.01). The results suggested that the antibacterial activity of total alkaloids and berberine from Coptidis Rhizoma on A. hydrophila may be related to the bacteria membrane injury. They inhibited the bacterial growth by increasing membrane lipid fluidity and changing conformation of membrane proteins, and reduced the secretion of virulence factors of A. hydrophila to weaken the pathogenicity.

10.Characterization of high-yield performance as affected by genotype and environment in rice.

We characterized yield-relevant characters and their variations over genotypes and environments (locations and years) by examining two rice varieties (9746 and Jinfeng) with high yield potential. 9746 and Jinfeng were planted in two locations of Shanghai, China, during 2005 and 2006. The results show that there was a large variation in grain yield between locations and years. The realization of high yield potential for the two types of rice was closely related to the improved sink size, such as more panicles per square meter or grains per panicle. Stem and leaf biomasses were mainly accumulated from tillering stage to heading stage, and showed slow decline during grain filling. Meanwhile, some photosynthetic characters including net photosynthesis rate (Pn), leaf area index (LAI), specific leaf area (SLA), fluorescence parameter (maximum quantum yield of PSII, Fv/Fm), chlorophyll content (expressed as SPAD value), as well as nutrient (N, P, K) uptake were also measured to determine their variations over genotypes and environments and their relationships with grain yield. Although there were significant differences between years or locations for most measurements, SLA at tillering and heading stages, Fv/Fm and LAI at heading stage, stem biomass at heading and maturity stages, and leaf nitrogen concentration at tillering and heading stages remained little changed, indicating their possible applications as selectable characters in breeding programs. It was also found that stem nitrogen accumulation at tillering stage is one of the most important and stable traits for high yield formation.
