1.Effect of ulinastatin on lung function in pediatric patients undergoing cardiopulmonary bypass:a meta-a-nalysis

Objective To systemically review the effect of ulinastatin on lung function in pediatric patients undergoing cardiopulmonary bypass.Methods PubMed,Embase,Cochrane Controlled Trials Reg-istry,China National Knowledge infrastructure,China Biology Medicine disc,VIP and Wanfang databases were searched from their inception to October 2015.Articles regarding the use of ulinastatin on lung function in patients undergoing cardiopulmonary bypass were searched.Studies were screened by two independent re-viewers and then the data were extracted.The methodological quality was evaluated according to the inclusion and exclusion criteria.Meta-analysis was then performed using RevMan 5.1 software. Results Nineteen eligible studies (n = 657 patients)were identified.The results of meta analysis showed that ulinastatin could improve the oxygen partial pressure(SMD=0.90,95%CI 0.52-1.28,P <0.01)and oxygenation index (SMD=1.01,95%CI 0.45-1.56,P <0.01),decrease the PA-a O2 (SMD= -0.87, 95%CI -1.70--0.03,P =0.04),reduce the respiratory index (SMD=-0.81,95%CI -1.51--0.11, P =0.02),Lower the airway peak pressure (SMD=-0.83,95%CI -1.18--0.48,P <0.01),improve the dynamic compliance (Cd)(SMD=1.10,95%CI 0.57-1.62,P <0.01),and shorten the breathing ma-chine ventilation time (SMD=-0.98,95%CI -1.59--0.36,P <0.01).Conclusion This meta-analysis showed that ulinastatin treatment had a certain degree of protective effects on lung function in pediatric pa-tients undergoing cardiac surgery with CPB,but further research was needed for all these studies which were not multicenter,strictly controlled.

3.Single-stage revision total knee arthroplasty for infection resulting from methicillin-resistant Staphylococcus aureus in rabbits

ObjectiveTo compare the efficacy of single-stage and two-stage revision prosthesis-relative chronic infection causing by methicillin-resistant Staphylococcus aureus(MRSA) after total knee arthroplasty(TKA) in rabbits, and evaluate the clinical feasibility of single-stage revision TKA. MethodsA new kind of prosthesis was implanted into the right knee joints of 48 New Zealand white rabbits following proper anesthesia. After 4 weeks, the dose of 5×105 colony forming unit MRSA was inoculated into every knee to establish prosthesis joint infection model. The rabbits were divided into 2 groups randomly: experimental and control group. Four weeks after inoculation, the treatments of the experimental and control group were singlestage and two-stage revision respectively. The levels of serum C-reaction protein (CRP) and Erythrocyte sedimentation rate (ESR) were monitored in ten phase, i.e. prior to primary arthroplasty and revision, at 1, 3, 5, 7days, and 2, 4, 6, 12 weeks after revision. Twelve weeks after revision, animals were sacrificed and joint samples were collected for bacterial culture. The positive results were judged as reinfection, and the negative results were judged as successful healing. ResultsFive rabbits were excluded out of the group for some reasons. The recurrence rates of infection in the experimental group and control group were 22.7% (5/22) and 14.3%(3/21) respectively after revision. The difference between them was statistically insignificant(χ2=0.102,P=0.750). The levels of serum CRP of the two groups raised, and reached their peaks at 3 days, then dropped into the normal level prior to primary arthroplasty at 4 weeks after revision. The difference between them was statistically insignificant (F=0.157, P=0.694). The ESR levels of the two groups elevated after revision, and reached their peaks at 5 days, then declined slowly into the original level prior to primary arthroplasty at 12 weeks. The difference between them was statistically insignificant (F=0.936,P=0.339). ConclusionThough the prosthesis-relative chronic infection caused by high virulence organism after TKA, the short-term efficacy of single-stage revision is similar to that of two-stage if the stain of pathogenic bacteria and its spectrum are obtained.

Laser scanning confocal microscopy (LSCM) and whole-cell perforated patch-clamp techniques were combined to study simultaneously the changes of intracellular signal molecules and membrane currents. Intracellular calcium transients and spontaneous transient outward currents (STOCs) were recorded simultaneously in freshly isolated mouse cerebral artery smooth muscle cells. The cells loaded with fluo-4/AM were scanned with the confocal line-scan mode. Triggering voltage pulses derived from an EPC-10 patch clamp amplifier triggered the confocal line scan. The results showed that STOCs and intracellular calcium transients could be simultaneously recorded in the same cell. This technique will be useful in studies of diseases caused by impairments of intracellular Ca(2+) signaling and related ionic channel activities, or vice versa.

6.The retractive effects of different gingival retraction agents.

OBJECTIVETo evaluate the retractive effects of different gingival retraction agents.
METHODSThirty subjects with healthy gingival conditions were recruited to the trial, and the buccal gingival sulcus of the hibateral first max-illary premolars of each subject (n=60) were treated randomly with one of the six agents--Group A was tetrahydrozoline hydrochloride, Group B was oxymetazoline hydrochloride, Group C was epinephrine hydrochlorid, Group D was AlCl3, Group E was Fe2 (SO4)3, and Group F was normal saline as control. Both pre-treated and post-treated accurate impressions of buccal gingival sulcus of the experimental teeth were made, so were the anhydrite casts. The casts of experimental teeth were sectioned into 2 mm slices bucco-lingually at the buccal eminence. Then the width of the sulcus was measured as the distance from the tooth to the crest of the gingival under a low-power microscope to compare the effect of these gingival retraction agents.
RESULTSThe width of gingival sulcus became larger after the treatment in all groups (P < 0.05). Either Group A or Group B had greater gingival retraction effects than Group C and Group F (P < 0.05). Except Group F, both Group D and Group E had no significant differences from the other groups (P > 0.05). There was no statistically signification between Group D and Group E (P > 0.05), neither was there any statistically significance between Group A and Group B (P > 0.05).
CONCLUSIONTetrahydrozoline manifested excellent effect of gingival retraction.

Objective To investigate the imaging manifestations of lung neoplasms in pre- and post- treatment with CT-guided Argon-Helium cryoablation.Methods All the lung neoplasms in 96 patients have been treated with CT-guided percutaneous Argon-Helium targeted cryoablation.All patients have pre- and post-treatment CT scanning in measurement of lesion size and CT value.The CT scanning has been rerpeated afterl,3,6,12 months of treatment.Results Among total 96 cases,there are 82 cases of lung cancer and 14 cases of metastasis with 110 lesions(89 cases with single lesion,7 cases with multiple lesions).The Ar-He cryoablation has been given 103 times in total.The size of each lesion ranged from 1.2 cm to 15.0 cm in diameter with mean value of(4.0?2.5)cm,including 12 lesions less than 2 cm,51 lesions between 2— 4 cm,24 lesions between 4—6 cm,23 lesions over 6 cm.There are 25 patients whose lesions covered by iceball with 1 cm of overloaping it's margin.There are 63 lesions with diameter less than 4 cm gained 100% ablation rate,24 lesions with 4—6 cm diameter gained 95.8% ablation rate,and 23 lesions with over 6 cm diameter gained 69.6% ablation rate.The post-treatment CT show a progressively enlarged round,low density refrigerant area which clearly demarked with non- refrigerant area.The center of each refrigerant area has negative CT value,the mean decreased CT value of lesion instantly after the treatment are about 30— 50 HU with P
8.Surgical anatomy of transoral atlantoaxial reduction plate internal fixation.

OBJECTIVETo provide anatomical data for transoral atlantoaxial reduction plate internal fixation.
METHODMicrosurgical dissecting was performed on 10 fresh craniocervical specimens layer by layer according to transoral approach. Stratification of posterior pharyngeal wall, course of vertebral artery, adjacent relationships of atlas and axis and correlative anatomical parameters of internal fixation to atlantoaxial joint were observed.
RESULT(1) Posterior pharyngeal wall consisted of 2 layers and 2 interspace: mucosa, anterior fascia of vertebrae, posterior interspace of pharynx and anterior interspace of vertebrae. (2) The range from anterior rim of foramen magnum to C3 could be exposed by this approach. (3) The distance between the vertebral artery at atlas and midline was (25.2 +/- 2.3) mm and that between the vertebral artery at axis and midline was (18.4 +/- 2.6) mm. (4) The width of atlas and that of axis could be exposed respectively to (39.4 +/- 2.2) mm and (39.0 +/- 2.1) mm. The distance (a) between 2 atlas screw inserting points (center of anterior aspect of C-1 lateral mass) was (31.4 +/- 3.3) mm. The vertical distance (b) between the connecting line of 2 atlas screw inserting points and that of 2 axis screw inserting points (at the central part of the vertebrae which was 3 - 4 mm lateral to the midline of C-2 vertebrae) was (18.7 +/- 2.7) mm. The odds of a/b was 1.5 approximately 1.7.
CONCLUSIONSAnterior atlantoaxial plate internal fixation through transoral approach is suitable and feasible. The design of the plate should be based on the above data.

9.Low-dose urokinase combined with low-molecular weight heparin nadroparin calcium and ozagrel sodium in patients with acute cerebral infarction

[Objective]To explore an individualized treatment measure enjoying more practical,effective and safe characteristics through evaluating the efficacy and safety of combined medications of low-dose urokinase,low-molecular weight heparin nadroparin calcium and ozagrel sodium in treating patients with acute cerebral infarction.[Methods]One-hunderd patients with acute cerebral infarction patients were recruited in this trail,and grouped according to different treatment times:Group A (n=40,from January 2005 to February 2008,being selected into the group in accordance with standards of China Guideline for Cerebrovascular Disease Prevention and Treatment) and Group B (n=60,from March 2008 to June 2011,being selected into the group in accordance with indications for onset time within 24 h and allowing age more than 75 years).Standard thrombolytic therapy (high dose urokinase) was performed on Group A and combined medications of low-dose urokinase,low-molecular weight heparin nadroparin calcium and ozagrel sodium (triple antithrombotic therapy) were performed on group B.National Institute of Health Neurological Deficit Scale (NIHSS) and Evaluation Standard of Clinical Efficacy were used to evaluate the recovery of neurological function before treatment and 24 h,7 and 14 d after treatment.[Results] NIHSS scores after therapy rapidly decreased in both groups as compared with those before treatment (P<0.05).The NIHSS scores of Group B at 24 h,and 7 and 14 d after treatment were significantly decreased as compared with those of Group A (P<0.05).The efficacy rate of Group B was significantly higher than that of Group A (P<0.05).Intraparenchymal hemorrhage rate was 10.0% (4/40) in Group A and 3.3% (2/60) in Group B.[Conclusion] Triple antithrombotic therapy is more effective and relatively safer than standard thrombolytic therapy in treatment of patients with acute cerebral infarction.
10.A PCR-RFLP analysis to HLA-B gene among Chinese northern Han populations.

OBJECTIVE:
To set up the method for analyzing HLA-B gene polymorphism with PCR-RFLP, and to gain population data among northern Chinese Hans of HLA-B's restricted fragments after NlaIII digestion, and to achieve application in forensic medicine practice.
METHODS:
Sample DNA was extracted by the phenol/chloroform extraction method, 943 bp-long fragments containing HLA-B exon 2 and 3 were got by PCR. The endonuclease NlaIII was applied to cut the PCR products into polymorphic fragments shorter than 943bp, then PAGE and silver staining were used to detect the digestion results, finally the digestion sites were assured by DNA sequencing.
RESULTS:
Along 943bp-long PCR products, 14 length-different fragments, 20 kinds of fragment combinations were got and 6 cutting site were observed after NlaIII digestion.
CONCLUSION
HLA-B gene was highly polymorphic among Chinese northern Hans. Even with only one endonuclease, 14 restricted fragments were got and the PIC was great. Such a HLA-B PCR-RFLP analysis will have values in forensic medicine applications.
