1.Analysis of Prognosis of Partial Seizures and Its Prognostic Factors in Children

Objective To analyze the influence of clinical status, electroencephalography (EEG) and antiepileptic drugs on prognosis of partial seizure in children. Methods A total of 233 pediatric patients with partial seizures were included. Their data including EEG records, responses of medication were reviewed. The patients were classified into three groups: seizure free, improved control and poor control groups. The significance of therapeutic efficacy in initial presentation in different age were assessed with t test. Chi-square test was used to analyze the significance of clinical data,laboratory findings and result of treatment. Logistic regression was used to investigate independent effect of each risk factor. Results Complete seizure control was achieved in 167 patients, improved seizure control in 31 patients and poor seizure control in 35 patients. Patients with poor seizure control were in much younger age group (3 times/month ) was also a predictive factor of poor outcome (?~2=8.976 P=0). The results of treatment were significantly different among the patients with simple partial seizure(SPS), complex partial seizure, and partial seizure with secondary generalized seizure. There were no significant differences in drug responsiveness among the patients treated by phenobarbital, carbamazepine, or valproate (?~2=0.107 P=0.948). Multiple logistic regression analysis showed that outcome of treatment was correlated with age of onset, seizure frequency, and type of the seizure (P=0.053,0.007,0.054). Conclusions Poor seizure control is closely associated with a younger onset age, frequent seizures. The treatment responsiveness is the worst in patients of partial seizure with secondary generalized seizures. The best outcome is in patients with SPS. Phenobarbital, carbamazepine, and valproate are equally effective for treating childhood partial seizure.
2.Determination of Oleanolic Acid and Ursolic Acid in the Fruits of Ligustrum Iucidum Ait.of Shanxi by HPLC

Objective To determine oleanolic acid(OA)and ursolic acid(UA)from Fructus Ligustri Lucidi in diffrernt habitat and various growing stages.Method We picked the fruits of Fructus Ligustri Lucidi in Otober in five cities of Shanxi,and in August,September,October,November and December in Xi'an.Removed impurities and storaged the fruits under room temperature.By HPLC with Waters 600 as its chromatographic system,and Lichrospher C_(18)(4.6mmx250mm,5?m) column was applied with CH_3CN-CH_3OH-H_2O-H_3PO_4-(C_2H_5)_3N(50:30:20:0.02:0.04)as its mobile phase,the flow rate was 1 mL/min.The standard working curve was made to determine the contents of OA and UA at different habitat and different time spot from samples.Result The contents of OA and UA were highest in Ankang city.During prolonging growing stages,the highest contents of OA and UA were October and August,respectively.They both reduced to the lowest point in December.Conclusion The contents of OA and UA changed in different habitat and diffrernt growing stages of Fructus Ligustri Lucidi. It was suggested that we should mainly base on the highest contents to select the harvest time according to our demands.
3.Extracorporeal shock wave therapy for chronic pelvic pain syndrome: an update.

Chronic prostatitis/chronic pelvic pain syndrome (CP/CPPS) is a common disease in males, which is characterized by persistent discomfort or pain in the pelvic region. As currently used drug therapies fail to produce satisfactory results, it is an urgent task to find new and effective methods for the treatment of CP/CPPS. In recent years, many reports are seen on the extracorporeal shock wave therapy (ESWT) for CP/CPPS. ESWT can significantly improve the symptoms of pelvic pain and urination disorders in CPPS patients, and its therapeutic effect is attributed to the improvement of angiogenesis and block of pain nerves.

4.Establishing a health security system for people in rural areas

The authors first describe the current situation of cooperative medical care in China, pointing out that due to low levels of guarantee, difficulties in fundraising and insufficient government reimbursement, cooperative medical care in rural areas have come across many hardships. It is, therefore, imperative to set up as soon as possible a health security system for people in rural areas. Firstly, it is necessary to break up the dual social security structure of urban and rural areas and set up a model with transitional goals based on the actual situation of the country. Secondly, it is suggested that security taxes begin to be collected, bringing the health security for people in rural areas into line with the social security framework so as to promote the health security for people in rural areas without lowering the level of social security in urban areas.
5.Clinical and Electroencephalography Features of Childhood Frontal Lobe Epilepsy

Objective To explore the clinical and electroencephalography features of children's frontal lobe epilepsy(FLE)and improve the diagnosis and therapy.Methods Medical records were reviewed and neuropsychological evaluations of patients with FLE diagnosed from 2000 to 2006.Children were examined by 24 h EEG and imaging examinations.Their charts for family medical history,seizure,aura,simultaneous phenomenon and anti-epilepic drug therapeutic effects were reviewed.Results Seizures were clonus in 24 cases,tonic in 4 cases,adversive,atonic,vomit in 2 cases,respectively,paresthesia,cephalic,visul,sensory in 1 case,Imaging finding were in 9 of 40.All the 40 cases were bought into 1 to 6 years' follow-up.Long-term control was achieved in 35 of 40.Partial control was achieved in 3 of 40.The left 2 cases had no response to the anti-epileptic drugs.Conclusions The common features of FLE included high seizure frequency,short duration and nocturnal preponderance.There is high positive rate of active EEG in detecting epileptic discharges,and active EEG shoud be necessary.Carbamazepine is recommended to be the first choice to FLE.
6.Drug-resistance and Distribution of Pathogens in Urinary Tract Infection

OBJECTIVE To investigate distribution status and drug-resistance pathogens from infected urinary tract and offer scientific evidence for reasonably clinical usage of antibiotics.METHODS A total of 286 strains of pathogenic bacteria isolated from urinary tract were identified by common methods.Antibiotic susceptibility tests were performed by K-B methods.RESULTS Escherichia coli rated the top one,accounting for 62.2%.Results of susceptibility test showed that all pathogens were resistant to the many antibiotics in different degrees except for 100.0% sensitive to vancomycin in Gram-positive cocci and to imipenem in Gram-negative bacilli.The detection rate of extensive-spectrum beta-lactamases(ESBLs) producers in E.coli and Klebsiella pneumoniae was 36.5% and 40.9%,respectively.The average detection rate of meticillin-resistant staphylococci(MRS) was 35.7%.CONCLUSIONS The drug-resistance of pathogenic bacteria from infected urinary tract is quite serious in the local area.We should adopt effective measurement to control its prevalence.
7.Comparison of cyclopentolate and atropine on cycloplegia in children

AIM: To compare the effectiveness on 10g/L cyclopentolate and 10g/L atropine on cycloplegia in children before optometry.
METHODS:Eighty eyes of 40 children among 4-12 years old with refractive error were recruited in this study. 10g/L cyclopentolate eye drops were topically administered once per 5min for 3 times and optometry was performed after 45min. Three days after that, 10g/L atropine sulfate eye gel then was used 3 times per day for consecutive 3d and again the refractive diopter was obtained at the 4th d. The differences of the results in retinoscopy refraction were compared between 10g/L cyclopentolate eye drops and 10g/L atropine sulfate eye gel.
RESULTS: Therefraction results of those given 10g/L cyclopentolate eye drops and 10g/L atropine sulfate eye gel were no statistical different in both 4-8 years group and 9-12 years group with myopia (≤-3. 00D) (P=0. 411, 0. 924). The differences of refraction results of both the drugs were significant in 4 - 8 years group with low hypermetropia, medium hypermetropia and high hypermetropia (P=0. 007, 0. 007, 0. 009). No significant difference was found in 9 - 12 years group with low hypermetropia (P= 0. 592), given 10g/L cyclopentolate eye drops and 10g/L atropine sulfate eye gel, but the differences of refraction results of both the ophthalmic preparations above were significant in 9-12 years group with medium and high hypermetropia (P=0. 039, 0. 012).
CONCLUSION:Both 10g/L cyclopentolate eye drops and 10g/L atropine sulfate eye gel have the cycloplegic effects, but their cycloplegic effects are significant different among hypermetropia children. Thus, the reasonable cycloplegic should be chosen according to the specific situation.
8.Study of high-risk corneal transplantation rejection and the expression of VEGF-C/D

AIM:To investigate the expression and the significance of VEGF-C/D in rat cornea after alkali burning as well as the role of lymphangiogenesis in the high-risk corneal transplantation rejection.
●METHODS:The model of alkali burn corneal was made. Different times corneas were taken to electron microscope for vascularization, and examined the expression of VEGF-C/D and VEGFR-3 in l, 3, 5, 7, 14, 28d. The other rat cornea after alkali burn were divided into four parts to penetrate keratoplasty, containing only blood vessels in the cornea ( group A ) , angiogenesis and lymphangiogenesis ( group B ) , lymphangiogenesis degenerating period ( group C ) , angiogenesis degenerating period ( group D ) . ln addition, there are also normal groups ( group N ) to compare the Rl values and survival time of corneal graft.
●RESULTS: Electron microscopy showed that, when the first 7d rat cornea appeared neovascularization after alkali burn, but not lymphangiogenesis. The occurrence of new blood vessels and lymphatic in 2wk. There were no obvious lymphangiogenesis in 5wk and the angiogenesis gradually subside in 8 wk. The expression of VEGF-C/D and VEGFR-3 in the corneas of rats were up-regulated in the third days after the injury, and reached its peaks at 5d. The average survival time of group N, A, B, C, D were (14.25±0.62)d, (9.35±1.02)d, (5.06±1.13)d, (8.71±0.83) d, (9. 44±1. 05)d after transplant cornea. Compared to the rest of the group, group B plant average survival time significantly shortened (P<0. 05), while compared with group B, the survival time of A, C, D groups were significantly longer (P<0. 05).
● CONCLUSION: VEGF - C/D and VEGFR - 3 are expressed significantly after corneal alkali burn. New lymphatic vessels can accelerate high - risk corneal transplantation immune rejection.
9.Xianglian External Lotion Restored the Sensitivity of Drug-resistant Candida albicans Strains to Fluconazole: a Transcriptomics Study.

OBJECTIVETo perform a transcriptomics study in differential genes after Xianglian External Lotion (XEL) induced the recovery of drug-resistant Candida albicans strains sensitive to Fluconazole.
METHODSBroth microdilution antifungal susceptibility test was used to detect minimal inhibitory concentration (MIC) of drug-resistant Candida albicans strains induced by XEL. Transcriptome sequencing (RNA-Seq) was used to determine and compare the transcription of primary drug-resistant Candida aIbicans strains and sensitive strains induced by XEL. High expressed genes and signaling pathways strains were analyzed by gene ontology (GO) method.
RESULTSXEL could induce drug-resistant strains of the 6th generations to recover sensitivity. Transcriptome sequencing showed that, as compared with primary drug-resistant strains, there were 165 genes with up-regulated RPKM index and 144 genes with down-regulated RPKM index after XEL induction. GO analyses found that all genes were mainly classified as GO:0015903 (fluconazole transport).
CONCLUSIONSXEL could induce the recovery of drug-resistant Candida albicans strains sensitive to Fluconazole. By analyzing transcriptomes, authors speculated that XEL could recover strain sensitivity to fluconazole by opening fluconazole transport pathway.

10.The clinical value of needle localization biopsy technique in clinical nonpalpable breast disease

Objective To study the clinical value of mammography and ultrasound needle localization biopsy technique in clinical nonpalpable breast disease.Methods 148 patients with negative clinical palpation were operated only because of the mammography or ultrasound findings of lesions.They were undergone localized excision biopsy and derived definite pathological diagnosis.By histopathology diagnosis,benign and malignant lesions,lesion size,complete resection rate were statistically analyzed.Results 21 cases in 148 cases were malignant lesions(14.2%),which were all early breast cancer.The other 127 cases were benign breast disease(85.8%).147 cases were successfully resected at the first time and 1 case was successfully resected at the second time.Conclusion Mammography and Ultrasound fine needle localized biopsy in nonpalpable breast disease diagnosis and treatment has great significance,which can raise early diagnosis rate of breast cancer without mass,and this method is simple and easy for promotion.
