1.Improving Medical Care Quality and Strengthening Hospital Infection Management

OBJECTIVE To understand the current situation in management of nosocomial infections in hospitals,and provide rational preventive measures in order to improve the medical care quality.METHODS An observatory study was conducted and reasonable suggestions were preferred referring to foreign nosocomial infections modes.RESULTS Problems were found to a different extent in administration of nosocomial infections.Most alarming problems were shortages of rules and regulations,and deficiency of infection propaganda and administration coordination.CONCLUSIONS Strengthening management of nosocomial infections is one of the key issues for improvement of the medical care quality and should be paid special attention.
3. Expression of CXCL12 and CXCR4 in salivary adenoid cystic carcinoma and peripheral nerve tissues

Objective: To study the relationship between CXCL12/CXCR4 expression and the neural invasion of salivary adenoid cystic carcinoma (SACC). Methods: Totally 41 SACC specimens, 30 tongue cancer specimens, 20 pleomorphic adenoma specimens and 20 normal nerve specimens were included in the present study. The specimens were made into sections of 4 μm and EnVision method was used to detect the expression of the chemokine receptor CXCR4 in SACC sections and the expression of the chemokine CXCL12 in the peripheral nerve tissue sections. Results: The chemokine receptor was highly expressed in ACC cells. The positive rate of CXCR4 was 63.41% in the SACC sections, which was significantly higher than those in the tongue cancer group (36.67%) and the pleomorphic adenoma parotid group (35%) (P<0.05). The expression of CXCL12 was not significantly different between the nerve tissues of SACC, tongue cancer and the normal nerve tissues. Conclusion: The cbemokine CXCL12 is elevated in the SACC cells and its receptor CXCR4 is elevated in the peripheral nerve tissues, indicating a role of the biological axis of CXCL12/CXCR4 in the neural invasion of salivary adenoid cystic carcinoma.

Objective To explore the clinical value of high-sensitivity C-reactive protein(hsCRP) in chronic kidney disease(CKD) and the relationship between hsCRP and the different stages of CKD in children.Methods This study population consisted of patients who was diagnosised as CKD in Beijing Children′s Hospital from Mar.2007 to Feb.2008.The total number of subjects was 62 patients with 35 male and 27 female,the mean age was(8.7?4.0)years old,the course was from 3 months to 10 years.The healthy control group contains 30 healthy children with 16 male and 14 female,the mean age was(7.8?3.2) years old.The serum levels of hsCRP,creatine and ?2-microglobulin(?2-MG) were determined.The glomerular filtration rate(GFR) was caculated.The relationship between serum levels of hsCRP and ?2-MG,blood pressure as well as the renal function in these patients with CKD,stratified according to GFR were evaluated.The relationship between hsCRP and the blood pressure was evaluated.The statistical analysis for the data was done with SPSS 12.0 software.Use the t-test,order and examination and correlation analysis,P

6. Study on chemical components of Dendrobium officinale protocorm

OBJECTIVE: To study the chemical constituents of Dendrobium officinale protocorm. METHODS: The compounds were isolated and purified by means of column chromatography, and their structures were identified on the basis of spectroscopic data and comparison with standard literature data. RESULTS: Eight compounds were isolated from the(CH3)2CO, CHCl3 and 95% EtOH fractions of the EtOH extract, and identified as 1-O-p-feruloyl-β-D-glucopyranoside(I), arillatose B(II),4-β-D-glucopyranosy-loxy)benzyl alcohol(III), 4-hydroxymethyl-2,6-dimethoxyphenyl-β-D-glucopyranoside(IV), n-hexatriacontanoic acid(V), n-heptaco-sanol(VI), β-sitosterol(VII), and cycloart-23-ene-3β,25-diol(VIII). CONCLUSION: All these compounds were isolated from the protocorms of D. officinale for the first time. Compounds I, II, III, IV, and VIII were obtained from Dendrobium genus for the first time.
7.MRI and MRA in Detecting Wrist Triangular Fibrocartilage Tear:A Meta-analysis

Purpose Accurate diagnosis of triangular fibrocartilage (TFC) tear is very important for treatment. MRI is most used for diagnosing TFC tear. This paper aims to evaluate MRI and MR arthrography (MRA) for diagnosing TFC tear by meta-analysis. Materials and Methods The articles were searched in the databases such as Wanfang, VIP, CNKI, Cochrane Library, Medline, Embase and PubMed. The QUADAS items were used to evaluate the quality of the included studies. Heterogeneity of the included articles was tested. The pooled weighted sensitivity and specificity of MRI and MRA in diagnosing TFC tear were calculated, and the pooled receiver operation curve was drawn. Results Fifteen articles met the inclusion criteria, 2 were Chinese articles and 13 were English articles. The subjects and methods of the articles were different and existed heterogeneity. The sensitivity and specificity of MRI for diagnosing TFC tear were 0.66 (95% CI 0.61-0.71) and 0.75 (95% CI 0.69-0.81), and those of MRA were 0.80 (95% CI 0.73-0.87) and 0.86 (95% CI 0.74-0.93). The area under curve and Q* index of SROC of MRI were 0.8566 and 0.7875, respectively. The area under curve and Q* index of SROC of MRA were 0.9123 and 0.8446, respectively. Conclusion The accuracy of TFC tear avulsion for MRA are higher than for MRI, when there is unclear of TFC avulsion using MRI, MRA can be used for diagnosis.
8. Research and application of Dendrobium officinale protocorm/protocorm-like bodies

OBJECTIVE: To review the research progress of Dendrobium officinale protocorni/protocorm-like bodies according to relevant references at home and abroad in recent len years, and to explore its development prospect. METHODS: Based on the searched literatures, the research progress in the aspects of culture techniques, chemical constituents and pharmacological effects were summarized. RESULTS AND CONCLUSION: There had been a lot of researches on D. officinale protocorm/PLBs culture techniques, and D. officinale protocorm/PLBs could be used for resource regeneration and production of active polysaccharides, but its material basis and pharmacological activity still need to be further studied.
9. Chemical constituents of Dendrobium devonianum

OBJECTIVE: To study the chemical constituents of Dendrobium devonianum. METHODS: The compounds were isolated and purified by means of column chromatography with silica gel, Sephadex LH-20 and prep- HPLC. Their structures were identified on the basis of physicochemical parameters and spectroscopic data comparison with standard literature data. RESULTS: Twenty-five compounds were isolated from the 95% ethanol extract of D. devonianum, and identified as 3-hydroxy-4', 5-dimethoxybibenzyl(1), tristin(2), 3, 4', 5-trihydroxybibenzyl(3), N-trans-coumaroyltyramine(4), N-trans- caffeoyltyramine(5), p-hydroxyphenylpropionyltyramine(6), N-cis-coumaroyltyramine(7), 2, 5-dihydroxy-4-methoxy-9, 10-dihydrophenanthrene(8), 2, 4, 7-trihydroxy-9, 10-dihydrophenanthrene(9), 4', 5, 7-trihydroxy-flavonol(10), dihydroconifery acetate(11), ethyl 4-hydroxycinnamate(12), p-hydroxycinnamic acid(13), p-hydroxyphenylpropionic acid(14), 4-hydroxybenzoic acid(15), protocatechuic acid(16), erigeside II(17), citrusin C(18), dihydrosyringin(19), leonuriside A(20), adenosine(21), β-sitosterol(22), daucosterol(23), (+)-syringgaresinol-O-β-D-glucopyranoside(24) and sucrose(25). CONCLUSION: Compounds 1-21 and 24-25 are obtained from this plant for the first time. Compound 1 is isolated from the Dendrobium genus for the first time. Compounds 5, 7, 11, 12 and 18 are obtained from Orchidaceae for the first time.
10.Causes and effects of medial meniscus extrusion in patients with knee osteoarthritis

Objective To investigate causes and effects of medial meniscus extrusion in patients with knee osteoarthritis.Methods A total of 120 patients diagnosed as knee osteoarthritis between January 2011 and March 2012 were enrolled in this study,including 60 patients with medial meniscal extrusion confirmed by MRI (extrusion group) and other 60 patients without medial meniscal extrusion (control group).The extrusion distance of medial meniscus and tibiofemoral angle were measured on MRI.The correlation between tibiofemoral angle and extrusion distance were analyzed.The incidences of genu varum,medial meniscus injury and cartilage lesion of medial tibiofemoral joint were compared between two groups.The effect of medial meniscal extrusion on meniscus injury and effect of genu varum on meniscal extrusion were analyzed.Results In extrusion group,the extrusion distances of medial menisci ranged from 3.76 to 11.6 mm (average,8.3±1.79 mm); all patients had genu varum,and the tibiofemoral angle ranged from 174°to 181°(average,179.0°±2.2°); the incidence of medial meniscus injury was 50.0% (30/60) in the anterior horn,93.3% (56/60) in the body,and 93.3% (56/60) in the posterior horn; the incidence of medial meniscus tear in the root of the posterior horn was 23.3% (14/60); the incidences of cartilage degeneration in medial tibial plateau and medial femoral condyle both were 100% (60/60); a significant negative correlation was observed between dimension of tibiofemoral angle and extrusion distance of medial meniscus.In control group,the extrusion distances of medial menisci ranged from 0 to 2.61 mm (average,0.57±0.80 mm); four patients had genu varum,and the tibiofemoral angle in all patients was 180°; the incidence of medial meniscus injury was 0 in the anterior horn,16.7% (10/60) in the body,and 70.0% (42/60) in the posterior horn; no medial meniscus tear was found in the root of the posterior horn; the incidence of cartilage degeneration was 26.7%(16/60) in medial tibial plateau and 30.0% (18/60) in medial femoral condyle.The odd ratio of meniscus injury and the number of genu varum (extrusion group/control group) was 6.0 and 15.0,respectively.Compared with the control group,the incidences and severities of medial meniscus injury and cartilage lesion of medial tibiofemoral joint were higher in extrusion group.Conclusion Genu varum may be one cause of medial meniscal extrusion.Medial meniscal extrusion increases incidence of medial meniscus injury and has a significant influence on generation and development of osteoarthritis in medial tibiofemoral joint.
