International Eye Science 2014;(8):1519-1521
AIM:To evaluate the efficiency of vitrectomy combined with inferior scleral buckling for a special kind of retinal detachment.
METHODS: Nineteen eyes of special retinal detachment had following features: 1 ) the course was more than 6mo;2) there were at least one hole located in the 5 : 00 - 7 : 00 of the marginal retina; 3 ) the detachment range of retina more than 270 degrees and retinal proliferation in retinal detachment. The 19 eyes who received vitrectomy combined with inferior scleral buckling, retinal anatomic reattachment and visual function recovery was observed.
RESULTS: Among these 19 eyes, all eyes retinal anatomic reattachment and best corrected visual acuity ( BCVA ) was improved in various degrees. The BCVA was 0. 01-0. 1 in 5 eyes, 0. 12-0. 3 in 9 eyes, and ≥0. 4 in 5 eyes.
CONCLUSION: It is an effective method to vitrectomy combined with inferior scleral buckling for the special kind of retinal detachment which has following features:1) the course was more than 6mo;2) there were at least one hole located in the 5:00-7:00 of the marginal retina;3 ) the detachment range of retina more than 270 degrees and retinal proliferation of cable in retinal detachment.

Objective To improve the diagnosis and treatment of testicular masses associated with congenital adrenal hyperplasia(CAH).Methods A 25-year-old man presented with bilateral testicular nodules.Physical examination revealed palpable multiple nodules in the bilateral testes and epididymides as well as enlarged and hard right testis with uneven surface.Biochemical examination showed testosterone(T) of 18.7 nmol/L,17?-(OH)P>20.0 ng/ml,positive results of rapid ACTH stimulation test and dexametha- sone suppression test.B-ultrasound demonstrated diffuse testicular lesions with nodules in parts of the bilater- al testes.Bilateral testicular nodules were biopsied, and pathological diagnosis was interstitial cell hyperplasi- a.Positive staining for?-inhibin was observed immunohistochemically. The clinical characteristics of testicular masses associated with CAH were reviewed in combination with the literature. Results Based on medical history, imaging, laboratory and pathologic examinations, the patient was diagnosed with testicular masses as- sociated with CAH.He received oral cortisol at dose of 40 mg daily for 2 weeks, and then at dose of 30 mg daily as maintenance therapy. After treatment of 8 months,B-ultrasound showed complete regression of the testicular tumors.Follow-up of 24 months showed no recurrence of the tumor. Conclusions For patient with bilateral testicular masses, endocrinological evaluation and clinical history taking are indicated to exclude testicular interstitial cell hyperplasia associated with CAH.By oral steroid replacement therapy,hormone-sen- sitive testicular masses can regress.

Objective To investigate the clinical effectiveness and safety of Chinese medicine fumigation-washing therapy in treating talipes equinus denervated ulcer induced by tethered cord syndrome(TCS) . Methods A total of 21 qualified patients were randomized into fumigation-washing group (N=12) and antibiotics group (N=9). Patients of fumigation-washing group were given fumigation-washing therapy with the self-prescribed Chinese medicine, and antibiotics group was given intravenous dripping of sensitive antibiotics. The treatment lasted for 7 continuous days. The clinical efficiency of the two groups was evaluated after treatment, and the period of preparing for surgery was recorded. The changes of ulcer area and ulcer surface exudate volume in the two groups were observed before and after treatment. And the changes in blood routine examination, hepatic and renal function, and immune function of the fumigation-washing group were monitored. Results(1) The total effective rate of fumigation-washing group was 83.3%, and that of the antibiotics group was 77.8%, the difference being insignificant (P>0.05). (2) The ulcer area and ulcer surface exudate volume in both groups were significantly decreased after treatment (P<0.05 compared with those before treatment), and the decrease in the fumigation-washing group was superior to that of the antibiotics group(P < 0.05).(3) The period of preparing for surgery in the fumigation-washing group was(7.75 ± 1.71)d, shorter than(11.56 ± 4.39) d in the antibiotics group, the difference being significant(P < 0.05).(4) There were no statistically significant differences in the hepatic and renal function, and immunoglobulin and complement levels in the fumigation-washing group before and after treatment (P > 0.05). (5) The average daily hospitalization fee in the fumigation-washing group was (47.98 ± 5.17) yuan, less than (227.88 ± 43.24) yuan in the antibiotics group, and the difference was significant between the two groups (P < 0.05). Conclusion Chinese medicine fumigation-washing therapy is effective, safe and practical in treating talipes equinus denervated ulcer induced by TCS, and the therapy has the advantages of decreasing exudation, promoting ulcer healing, reducing hospitalization fee, shortening the period of preparing for surgery, and decreasing the incidence of infection.
10.Research progress on mechanism of bone marrow relapse in pediatric acute lymphoblastic leukemia.
Journal of Experimental Hematology 2013;21(1):231-235
The mechanism of bone marrow relapse in pediatric acute lymphoblastic leukemia has been deeply researched in recent years. The roles of hematopoietic stem cells, some gene abnormalities including IKZF1, JAK, CRLF2 and CREBBP, as well as mutation of genes influencing drug resistance have been confirmed to related with relapse of disease. The microRNAs, gene expression profile of patient, microenvironment of bone marrow including bone marrow fibrosis, mesenchymal stem cells and matrix metalloprotease, also play an important role in the process of leukemia recurrence. This article reviewed the details mentioned above.
