1.Influence of periodical health education on self-care capability and living quality of patients with connective tissue diseases complicated with interstitial lung disease

Objective To explore the influence of periodical health education on self-care capability and living quality of patients with connective tissue diseases complicated with interstitial lung disease. Methods A total of 92 cases of patients with connective tissue diseases complicated with interstitial lung disease who were received and treated from January 2013 to October 2015 were selected as objects of study. The patients were divided into the observation group and the control group with 46 cases in each group according to the admission time. Both of the two groups were given conventional oxygen uptake therapy, non-steroidal anti infection therapy, expansion of blood vessels therapy and immune regulation therapy. The control group adopted usual care, while the observation group adopted periodical health education. The clinical efficacy, self-management ability, living quality and other indicators of the two groups were compared. Results The effective rate of observation group was 93.48%(43/46), and the effective rate of control group was 78.26%(36/46), The difference between the 2 groups was statistically significant (χ2=4.389, P < 0.05). The observation group related disease knowledge, self-care behavior, attitude adjustment, oxygen inhalation and respiratory function exercise with 5 self-care scores were higher than control group, the difference between the 2 groups was statistically significant (t=3.468-4.348, P<0.05). The observation group related physiological function, physiological rate function, physical pain, overall health, vitality, social function, emotional function, mental health 8 quality of life scores were higher than control group, the difference between the 2 groups was statistically significant (t= 2.261-5.456, P < 0.05). Conclusions Periodical health education is conductive to improving the self-care capability and living quality of patients with connective tissue diseases complicated with interstitial lung disease.
2.Effects of ceftriaxone on glutamate and glutamate transporter subtype-1 in hip-pocampus following traumatic brain injury in rats

Objective:To sduty the effect of β-lactam antibiotics efiriaxone on the levels of glutamate and glutamate transporter subtype-1 in hippocampus following traumatic brain injury in rats.Methods: All rats were divided randomly into three groups:sham group;TBI group and CTX group.The rat model of TBI were made by modified Feeney method ,and treated with ceftriaxone immediately after injury ( 200 mg/kg ).Wet-dry weight method was used to evaluate brain edema.The content of glutamate in hippocampus was measured with the high performance liquid chromatography.The expression of GLT-1 in hippocampus areas was tested by immunohisto-chemical and Western blot methods.Results:Compared with TBI group ,TBI-induced cerebral edema was significantly attenuated ( P<0.05 ) , the content of glutamate in hippocampus was were significantly decreased ( P<0.05 ) , the level of GLT-1 were significantly increased in CTX group ( P<0.05 ).Conclusion: β-lactam antibiotics ceftriaxone can block the excitatory neurotoxicity , reduce the extent of brain edema.
3.Clinical analysis of therapy of 46 cases systemic lupus erythematosus associated with pulmonary arterial hypertension

Objective To study the theraputic effect of cyclophosphamide,glucocorticoid combined with sildenafil on pulmonary hypertension in systemic lupus erythematosus(SLE?PAH) patients,and to analyze the prognosis adverse factors. Methods Forty?six cases patients with SLE?PAH from Tangshan Gongren Hospital from January 2009 to January 2015 were selected as research subjects. All patients were treated with prednisone, cyclophosphamide and sildenafil,and the course of treatment was 24 weeks. The treatment effect was observed, the systemic lupus erythematosus disease activity score(SLEDAI),6 minutes walk distance and heart function before and after the treatment were compared. The effective and uneffective patients'clinical and test information were compared,and the prognosis adverse factors were analyzed. Results After 24 weeks treatment,the total ef?fective rate was 78. 3%(36/46). Pulmonary artery systolic pressure(PASP) dropped from (59±16) mmHg be?fore treatment to (45±15) mmHg after treatment,there was significant statistical difference(t=-4. 539,P<0. 01). SLEDAI dropped from (6. 6±2. 3) points to (2. 4±0. 8) points,,there was significant statistical differ?ence(t =11. 680,P<0. 01). The 6 minutes walk distance rose form (402±96) minutes to (465±97) minutes, there was significant statistical difference( t=-3. 137,P<0. 01) . Cardiac function improved significantly,there was significant statistical difference( P<0. 01) . There were 10 cases uneffective and deterirated patients,with the age of (42±6) years,36 cases effective patients with the age of (33±4) years,the difference was statistically significant( t=-5. 423, P<0. 01 ) . Uneffective and deterirated patients' anti?RNP antibody positive rate was 90. 9%( 9/10 ) , significantly higher than the effective patients ( 52. 8%( 19/36 ) ,χ2 = 4. 552, P<0. 01 ) . Conclusion Cyclophosphamide, glucocorticoid combined with sildenafil has obvious effect on patients with SLE?PAH. PASP,heart function and 6 minutes walk distance are the important indicators of prognosis. Anti?RNP antibody and age are the adverse prognostic factors.
4.Bone marrow mesenchymal stem cell transplantation for the treatment of systemic lupus erythematosus

BACKGROUND:The non-specific immune suppression method is generaly used for treatment of systemic lupus erythematosus, but poor prognosis, such as infection and high recurrence rate, exists.
OBJECTIVE:To evaluate the therapeutic effect of bone marrow mesenchymal stem cel transplantation on systemic lupus erythematosus in mice.
METHODS:Sixteen mice with systemic lupus erythematosus were equivalently randomized into control and experimental groups, or then subjected to passage 3 bone marrow mesenchymal stem cel transplantation or the equal volume of normal saline via the tail vein, respectively. Mouse urine samples were colected to detect urine protein levels by Bradford method. Blood samples from the tip of the mouse tail were extracted to detect serum anti-ds-DNS antibody concentration by radioimmunoassay. Mouse kidney tissues were taken and observed pathohistologicaly through hematoxylin-eosin staining and immunohistochemistry staining under microscope. Flow cytometry was used to detect the expression of CD4+CD25+T cels in the inner canthus blood, fresh spleen and thymus.
RESULTS AND CONCLUSION:Within 10 weeks after cel transplantation, the urine protein levels in the two groups were gradualy increased, and the rising velocity was higher in the control group than in the experimental group. From the 4th to 10th week, the urine protein levels in the experimental group were significantly lower than those in the control group (P < 0.05). In the control group, lymphocyte infiltration was visible in the kidney tissues with a few of plasmocytes, and pathological findings showed the mice presented with interstitial nephritis; in the experimental group, the mice had no pathological changes in the kidney. In the two groups, immune complexes were found in the mesangial area, which showed a patch-like distribution in the control group and a punctate distribution in the experimental group; the relative proportion of the occupied area in the experimental group was significantly lower than that in the control group. The expression level of CD4+CD25+T cels in the blood and thymus were significantly higher in the experimental group than the control group (P < 0.05), and the expression level of CD4+CD25+T cels in the spleen was slightly higher in the experimental group than the control group with no significant difference (P > 0.05). The serum anti-ds-DNA antibody concentration in the experimental group was significantly lower than that in the control group (P < 0.05). Taken together, bone marrow mesenchymal stem cel transplantation can improve the pathological damage in systemic lupus erythematosus mice, and has a certain therapeutic effect on systemic lupus erythematosus.
5.Clinical Observation of Breviscapine Combined with Double Shock in the Treatment of Lupus Encephalopa-thy

OBJECTIVE:To observe the efficacy and safety of breviscapine combined with double shock in the treatment of lu-pus encephalopathy. METHODS:70 patients with lupus encephalopathy were randomly divided into control group and observation group. Control group was treated with Methylprednisolone sodium succinate for injection 500 mg for continuous 3 d,iv,once a day,Cyclophosphamide for injection 1.0 g in the 4th day,iv,and Methylprednisolone sodium succinate for injection 80 mg was used for maintaining treatment in the 5th day for continuous 15 d,once a day. Observation group was treated with Breviscapine for injection 50 mg for continuous 20 d based on the treatment of control group,iv,once a day. The SLE activity(SLEDAI) score, IL-1β,IL-6,IL-8 and IFN-γ were observed,cases of neurological and psychological symptoms,the fatality rate and incidence of adverse reactions were recorded. RESULTS:After treatment,the SLEDAI,IL-1β,IL-6,IL-8 and IFN-γ in 2 groups were signifi-cantly lower than before,and observation group was lower than control group(P<0.05);the cases of neurological symptoms(ex-cept for aseptic meningitis) and psychological symptoms (except for auditory hallucinations) were significantly less than control group(P<0.05). There was no significant difference in the mortality rate and incidence of adverse reactions(P>0.05). CONCLU-SIONS:Breviscapine combined with double shock has good efficacy and safety in the treatment of lupus encephalopathy,it can re-duce the SLEDAI score,improve neurological and psychological symptoms.
6.Frequency changes of peripheral blood interleukin-22+CD4+CD161+ T cells in rheumatoid arthritis

Objective To analyze the frequency of interleukin (IL)-22+CD161+CD4+ T cells in the peripheral blood mononuclear cells (PBMCs) in rheumatoid arthritis (RA) patients compared with healthy control subjects and investigate the relationship of IL-22+CD4+CD161+ T lymphocyte frequency changes with RA disease activity.In addition to explore the pathogenesis of RA,and to look for new treatment targets for RA.Methods Twenty-one RA cases were included in the Department of Rheumatology of Tangshan Gongren Hospital from 2017 to 2018.Fourteen patients were female and 7 were male with the age ranged from 36 to 74 years old.The average age of this group of patients was (55±10) years,the average disease course was (60±50) months.All patients fulfilled the classification criteria of American College of Rheumatology [American College of Rheumatology (ACR)].Twenty-one subjects were enrolled as the control group,all of them came to Tangshan Gongren Hospital for regular health check-up.Fifteen subjects in the control group were female and 6 were male.Their age ranged between 40-78 years old with the average age of (55±9) years.IL-22+CD4+CD161+ T cells in PBMCs were detected by flow cytometry.The frequency variation of different CD4+CD161 + T was compared between case and control groups.The correlation was studied between the frequency and RA disease activity score (DAS28),tender joints number,swollen joints number,red blood cell sedimentation rate,high sensitive C reactive protein and white blood cell counts,red blood cell counts,platelet counts,IgG,IgA,IgM,complement C3 level,complement C4 level.T-test or Mann-Whitney U test were used for single-factor analysis,Pearson's test was used for correlation analysis.Results The percentage of RA group secreted CD4+ T cells (0.33± 0.20)% of INF-γand IL-22,CD4+ T cells (0.51±0.29)% of IL-22,and CD4+CD161+ T cells of IL-22 simultaneously.The number (0.55 ±0.28)% was.significantly higher than that of the healtby control group [(0.22±0.14)%,(0.25±0.18)%,(0.36±0.24)%],and the differences were statistically significant [P=0.002,P=-0.0.45,P=0.026].Conclusion The percentage of IL-22+CD4+CD161+ T lymphocytes in the peripheral blood monocytes in RA patients is significantly higher than that in the healthy controls.The results of this study suggest that IL-22+CD4+CD161+ T lymphocytes in RA patients maybe related to RA disease activity and joint lesions.
