2.Effect of arthroscopic debridement combined with platelet rich plasma in treatment of supraspinatus calcified tendinitis

Objective To investigate the clinical efficacy of arthroscopic debridement combined with platelet rich plasma(PRP)in treatment of supraspinatus calcified tendinitis.Methods 23 patients in the experimental group were treated with shoulder arthroscopic calcification clearing with PRP injection,while 23 patients in the control group were treated with simple arthroscopic clearing,and the curative effects were observed and compared.Results All the patients were followed up effectively.Of the 23 cases in the experimental group,5 cases were treated with rotator cuff repair(3 cases were diagnosed rotator cuff tear before operation,2 cases were diagnosed with large calcification,and rotator cuff repair was performed after cleaning),and 3 cases were treated with rotator cuff repair(2 cases were diagnosed with rotator cuff tear before operation,1 case was diagnosed with large calcification,and rotator cuff repair was performed after cleaning).The visual analogue scale(VAS)of the two groups one month after surgery was significantly lower than that before surgery,and the American Association of Shoulder and Elbow Surgeons(ASES)score of the two groups one month after surgery was significantly higher than that before surgery,with statistical significance(P<0.05).The VAS of the experimental group was significantly lower than that of the control group one month after surgery,and the ASES score was significantly higher than that of the control group one month after surgery,with statistically significant differences(t =-5.52,t = 8.73,P = 0.001).Conclusion Arthroscopic debridement combined with PRP is an effective and minimally invasive method for the treatment of supraspinatus calcified tendinitis.The calcium salt can be removed as much as possible under arthroscopy,and the scope of rotator cuff injury can be accurately evaluated.For the obvious rotator cuff injury with large damage,the rotator cuff repair with thread anchor nail can be performed.At the same time,combined with PRP,it can accelerate the repair of rotator cuff injury,relieve pain,and accelerate the functional recovery of shoulder joint.
3.Cancellous bone wrapping grafting for segmental defects of long bone

Objective To investigate the principles and effects of cancellous bone wrapping grafting for treatment of segmental defects of long bone.Methods From January 2008 to December 2015,50 patients with segmental long bone defect were treated by various wrapping grafting with rich autogenous cancellous bone.They were 31 males and 19 females,aged from 13 to 69 years (average,34.6 years).The cancellous bone was wrapped by titanium mesh in 8 cases,by wire mesh in 10,by line binding in 13,and by induced membrane in 19.The bone defect was located at tibia in 22 cases,at radius in 10,at humerus in 8,at ulna in 7 and at femur in 3.The length of bone defect ranged from 3 to 9 cm,averaging 5.9 em.Bone healing,complications and functionary recovery of adjacent joint were recorded.The bone defect healing and functionary recovery of adjacent joint were evaluated according to the Paley criteria.Results The incisions healed by the first intention in 48 cases and by the second in 2.All were followed up for 12 to 48 months (average,19.1 months).All the bone defects healed by the first intention but one treated by induced membrane wrapping which was healed 15 months later by the secondary grafting due to nonunion at ends.The total clinical healing time ranged from 3 to 16 months (average,6.1 months).The last follow-ups showed that all the affected limbs resumed weight-bearing activities.The healing of bone defects was graded as excellent in all but one in the group of induced membrane wrapping.Totally,the functionary recovery of adjacent joint was excellent in 18,good in 22,fair in 7 and poor in 3 cases (an excellent and good rate of 80.0%).Conclusions Cancellous bone wrapping grafting can avoid or significantly reduce loosening and absorption of cancellous bone graft after traditional bone grafting.Although it is effective for treatment of large segmental bone defect,its methods should vary according to the specific conditions of the patient.
4. Therapeutic effects of endoscopic foam sclerotherapy for bleeding internal hemorrhoids

Objective:
To evaluate the safety and efficacy of foam sclerotherapy for bleeding internal hemorrhoids.
Methods:
Consecutive adult patients (aged 18-60 years) with bleeding hemorrhoids were prospectively enrolled between March 2017 and March 2018. All patients were randomly assigned to the foam sclerotherapy (FS) group and the liquid sclerotherapy (LS) group. The foam and liquid sclerotherapy of hemorrhoids nucleus was performed by cap-assisted colonic endoscopy.
Results:
A total of 108 patients were enrolled, including 62 males (57.4%) with mean age of 44.0±7.6 years. There was no significant difference in gender, age, or body mass index between the FS (n = 55) group and the LS (n =53) group. 5.Construction of integrated platform for emergency clinical scientific research based on big data.

OBJECTIVE:
To explore clinical rules based on the big data of the emergency department of the Second Affiliated Hospital of Guangzhou Medical University, and to establish an integrated platform for clinical research in emergency, which was finally applied to clinical practice.
METHODS:
Based on the hospital information system (HIS), laboratory information system (LIS), emergency specialty system, picture archiving and communication systems (PACS) and electronic medical record system of the Second Affiliated Hospital of Guangzhou Medical University, the structural and unstructured information of patients in the emergency department from March 2019 to April 2022 was extracted. By means of extraction and fusion, normalization and desensitization quality control, the database was established. In addition, data were extracted from the database for adult patients with pre screening triage level III and below who underwent emergency visits from March 2019 to April 2022, such as demographic characteristics, vital signs during pre screening triage, diagnosis and treatment characteristics, diagnosis and grading, time indicators, and outcome indicators, independent risk factors for poor prognosis in patients were analyzed.
RESULTS:
(1) The data of 338 681 patients in the emergency department of the Second Affiliated Hospital of Guangzhou Medical University from March 2019 to April 2022 were extracted, including 15 modules, such as demographic information, triage information, visit information, green pass and rescue information, diagnosis information, medical record information, laboratory examination overview, laboratory information, examination information, microbiological information, medication information, treatment information, hospitalization information, chest pain management and stroke management. The database ensured data visualization and operability. (2) Total 140 868 patients with pre-examination and triage level III and below were recruited from the emergency department database. The gender, age, type of admission to the hospital, pulse, blood pressure, Glasgow coma scale (GCS) and other indicators of the patients were included. Taking emergency admission to operating room, emergency admission to intervention room, emergency admission to intensive care unit (ICU) or emergency death as poor prognosis, the poor prognosis prediction model for patients with pre-examination and triage level III and below was constructed. The receiver operator characteristic curve and forest map results showed that the model had good predictive efficiency and could be used in clinical practice to reduce the risk of insufficient emergency pre-examination and triage.
CONCLUSIONS
The establishment of high-quality clinical database based on big data in emergency department is conducive to mining the clinical value of big data, assisting clinical decision-making, and improving the quality of clinical diagnosis and treatment.
