1.Granrlar parakeratosis: a ease report

A case of granular parakeratosis is reported. A 31-year-old woman presented with a 23-year history of pruritic erythema and erosion in the left axilla. On examination, there was a ring-like annular erythematous patch sized 8 cm×10 cm in the left axilla. Bright mauve, cone-shaped, millet-like papules were observed in the center of the lesion, some confluenced into plaques. Erythema was present in the pedlesional region along with mild erosion, exudation and small numbers of grain-sized pustules. Scar formed in some perilesional areas. No lesions were noted at any other intertriginous regions. Fungal microscopy of lesion secretions was negative. Histological examination of biopsy specimens from the center of the left axilla revealed psoriasiform hyperplasia of epidermis and thickened stratum comeum with hyperkeratosis and parakeratosis. Most cells in the stratum comenm retained nuclei and contained numerous basophilic granules. Granular layer could be noted under the parakeratotic cells with cytoplasm vacuolization of some cells. There was a perivascular, mixed inflammatory infiltration predominated by lymphocytes and hemangiectasis in the dermis. A diagnosis of granular parakeratosis was made.
2.Efficacy analysis of S-1 combined with radiotherapy for locally advanced esophageal cancer

Objective To compare the efficacy for aged patients with locally advanced esophageal cancer who accepted 3D-CRT with or without S-1.Methods 62 aged patients with locally advanced esophageal cancer were randomized divided into three groups in Central Hospital of Kaifeng since March,2009.The S-1 combined with radiotherapy group was 26 patients as combined group,the single radiotherapy group was 20 patients and single drug group was 16 patients.S-1 combined group patients accepted 3D-CRT,and the patients were taken S-1 from the first day,the dose was 40 mg/m2 twice a day continually 14 days and then stop 7 days.There were 2 cycles during radiotherapy.The 20 patients accepted 3D-CRT in single radiotherapy group,and the patients in single drug group were taken S-1 only,the dose was 40 mg/m2 twice a day continually 28 days and then stop 14 days within 28 days.Results The response rate was 92.31% in S-1 combined group,with 13 patients CR,11 patients PR,and 1 patient SD.The response rate was 60.00 % in single radiotherapy group,with 4 patients CR,8 patients PR and 6 patients SD.The response rate was 31.25 % in single drug group,with 1 patients CR,4 patients PR and 6 patients SD.The effect of the S-1 combined with radiotherapy group was significant better than the others (P < 0.05).The 1,2 and 3 year survival rates in each group were 76.92 %,57.69 %,42.31%,75.00 %,55.00 %,40.00 % and 68.75 %,50.00 %,25.00 %,which there is no significant difference in the three groups.The adverse reaction of hematologic toxicity in S-1 combined with radiotherapy group was little higher than the others,still there was no significant difference in the incidence rate of radiation esophagitis and radiation pneumonitis between S-1 combined with radiotherapy group and single radiotherapy group.Conclusion S-1 plus three dimensional conformal radiotherapy treatments were reliable,and the adverse reactions were mild and well tolerated for elderly patients with locally advanced esophageal cancer.
3.Analysis on risk factors for deep vein thrombosis after bone trauma

Objective To investigate the risk factors for deep vein thrombosis (DVT) after bone trauma.Methods The study involved 118 patients with traumatic fractures (traumatic fracture group),21 DVT patients diagnosed by color Doppler (DVT group) and 56 healthy patients (control group).Anti-cardiolipin antibody (ACA) was determined by ELISA method.D-dimer and fibrinogen (Fib) were detected by coagulation analyzer and C-reactive protein (CRP) by rate nephelometry.Results Levels of D-dimer,Fib and CRP in traumatic fracture group were significantly increased,but were lower than those in DVT group.ACA positive rate in DVT group presented significant increase and three patients with positive ACA in traumatic fracture group all suffered from DVT.The positive rates of Fib,D-dimer and CRP in lower limb fracture group,multiple fracture group and pelvic fracture group were higher than those in upper limb fracture group (P < 0.05).Levels of Fib and D-dimer showed gradual rise with growth of age,but their levels in DVT group had different degree of reduction after thrombolytic therapy.Conclusions Positive ACA and enhancement of D-dimer,Fib and CRP are risk factors for DVT after bone trauma.Levels of Fib and D-dimer in patients with bone trauma are related with age and therefore risk of posttraumatic DVT increases with age.
4.Expression of microRNA let-7a in melanoma and its effect on the proliferation of A375 human melanoma cell line

Objective To investigate the expression of microRNA let-7a in formalin-fixed paraffinembedded (FFPE) melanoma tissue and its effect on the proliferation of A375 cell lines. Methods Tissue samples were obtained from 13 patients with malignant melanoma and 10 with congenital pigmented nevus,then fixed with formalin and embeded with paraffin. microRNAs were isolated from these samples and reversely transcripted to cDNA with stem-loop primer. Then, real time quantitative PGR was performed with Taqman MGB probe and matched primers to measure the expression of microRNA let-7a. cy3-Labeled negative control siRNA was transfected into A375 cells followed by the examination of transfection efficiency under fluorescense microscopy. Subsequently, hsa-let-7a mimics were transfected into A375 cells to observe their effect on the proliferation of these cells. Results microRNAs were successfully isolated from FFPE tissues, and quantitative PGR showed a significant reduction in the expression of microRNA let-7a in melanoma tissue compared with nevus tissue (t = 2.364, P < 0.05). The transfection efficiency was found to be about 80% ~90%. After transfection with hsa-let-7a mimics, the proliferation of A375 cells was inhibited and the inhibition rate amounted to 32.7% at 24 hours. Conclusions The expression of microRNA-let-7a is attenuated in melanoma, and overexpression of let-7a can inhibit the proliferation of A375 cells, which implies that let-7a functions as a tumor suppressor gene in melanoma.
5.Expression of Ezrin and AQP5 in carcinoma of esophagogastirc junction

Purpose To investigate the expression of the protein of Ezrin and AQP5 in the normal, dysplasia gastric and carcinoma of esophagogastirc junction tissues and to find out their relationship with the biologic behavior of the carcinoma of esophagogastirc junction in the southern area of Hebei Province. Methods Ezrin and AQP5 proteins were detected by immunohistochemical staining in 10 ca-ses of normal gastric tissue, 40 cases dysplasia and 165 cases of the carcinoma of esophagogastirc junction tissues, respectively. Re-sults All of the expression levels of Ezrin and AQP5 protein were increasing according to the order of normal-dysplsia-carcinoma. The expression rate of Ezrin was 10%, 45% and 72. 7%, while the rate of AQP5 with 20%, 32. 5% and 65. 5% in the normal, dysplasia and carcinoma subgroup. The expression rates of each protein were significantly different among the three groups (P<0. 05). The ex-pression of them in the subgroup of poorly-differentiated with serosa invasion and lymph nodes metastasis was significantly higher than the other subgroup of well-differentiated with non serosa invasion or lymph nodes metastasis (P<0. 05). There was a positive correla-tion between the expression of Ezrin and AQP5 proteins in the carcinoma of esophagogastirc junction (P<0. 05). Conclusions Over-expression of Ezrin and AQP5 is detected in the carcinoma of esophagogastirc junction, with positive relationships, which probably helps the invasiveness and metastasis in the carcinoma of esophagogastirc junction synergistically.
6.Treatment values of precise target delineation of chest MRI for lung cancer

Objective To investigate the treatment values of precise target delineation of chest MRI for lung cancer Methods From August 2011 to February 2015 , 45 non-small cell lung cancer patients were given chest CT scans and MRI scans before radiotherapy , and then active target tumor delineation , then related influencing factors were analyzed. Results All patients completed CT scans and MRI positioning. For patients that it was difficult to identify lung tissue lesions caused by lung cancer through CT , their MRI imaging showed high signal and the boundaries between the tumor and surrounding normal tissue became relatively clear. Meanwhile , 20 patients of borders were diagnosed by CT , while 25 by MRI; 36 patients with lymph node metastasis were diagnosed by CT while 40 by MRI. The difference was statistically significant (P<0.05). Univariate logistic regression analysis showed that pathological type and atelectasis were the influence factors for CT and MRI tumor target delineation differences (P<0.05), and multivariate logistic regression analysis showed the atelectasis was the main factor (P<0.05). Conclusion Compared with CT, breast MRI can precisely delineate target to improve the accuracy of target localization before radiotherapy. It can help determine lymph node metastasis and avoid the impact of atelectasis then ensure the accuracy of radiotherapy.
7.A study of event-related potential P300 in subjects with clinical high risk of psychosis

Objective To investigate the characteristics of auditory P300 amplitude and latency and cognition in patients with clinical high-risk of psychosis (CHR). Method Thirty six CHR (study group) and thirty five healthy con?trols (control group) were included. Oddball paradigm and MATRICS Consensus Cognitive Battery (MCCB) were used to record auditory P300 and to evaluate the cognition, respectively. The structured interview for psychosis-risk syndromes (SIPS) was used to evaluate the clinic symptoms of patients. Result The cognition of CHR was significantly lower than healthy controls in information processing, attention/vigilance, working memory, verbal learning, visual learning, reason?ing and problem solving and social cognition (P<0.01). The study group showed decreased amplitude in Fz, Cz and Pz and delayed latency in Pz (P<0.05). P300 latency of CHR in Fz positively correlated with positive score of SOPS (r=0.544, P=0.001), while P300 amplitude positively correlated with verbal fluency (r=0.339,P=0.043). Conclusion Cogni?tion and P300 is abnormal in CHR. The correlation between P300 and clinical symptoms, cognitive dysfunction reminds that we should put more attention on the role of P300 in CHR subjects.
8.Effects of MRE11 on Apoptosis and Proliferation of Esophageal Squamous Cancer Cells

Objective To investigate the effect of MRE11 on the proliferation and apoptosis of esophageal squamous cancer cells and its molecular mechanism. Methods MRE11 expression was downregulated by MRE11 siRNA transfection in esophageal squamous cancer cells. The AKT agonist SC79 (0, 0.1, 0.5, 1, 1.5, 1.8, 2 μg/ml) were used to treat cells with MRE11 inhibition for 24 h. Overexpression vector pcDNA.3.1-c-myc was constructed and co-transfected cells with MRE11 siRNA. Western blot method was used to detect the protein expressions of MRE11, p-AKT and c-myc in esophageal squamous cancer cells Ec9706 and TE-1. The Annexin-V FITC/PI kit was used to detect the apoptosis of Ec9706 and TE-1 cells; the activity of caspase-3 was detected by the Caspase-3 activity detection kit; the proliferation of Ec9706 and TE-1 cells was tested by the BrdU method. Results The protein expressions of MRE11 in Ec9706 and TE-1 cells were significantly increased, compared with human esophageal epithelial Het-1A cells. After MRE11 siRNA transfection, AKT phosphorylation and the protein expressions of MRE11 and c-myc were significantly decreased in esophageal squamous cancer cells. MRE11 inhibition significantly promoted the apoptosis and caspase-3 activity in Ec9706 and TE-1 cells, while inhibited the proliferation of Ec9706 and TE-1 cells. SC79 (1.5, 1.8 and 2 μg/ml) significantly increased AKT phosphorylation in MRE11-suppressed esophageal squamous cancer cells, and reversed the inhibitory effects of MRE11 inhibition on c-myc protein expression and cell proliferation and the promoting effect on cell apoptosis. Overexpression of c-myc inhibited the inhibitory effect of MRE11 down-regulation on cell proliferation and the promotion on caspase-3 activity. Conclusion MRE11 inhibition could effectively inhibit the proliferation of esophageal squamous cancer cells and promote cell apoptosis by regulating AKT and c-myc.
9.Fetal Membrane Early Broken 187 Example Clinical Analysis

Objective To discuss the ways of fetal membrane early broken and the childbirth,the relations between the maternal infant complication,and the ways to deal. Methods Making the summary and analysis on the 187 examples of fetal membrane broken pregnant woman's clinical material between January,2000 and December,2008 in the hospital I work. Results In 187 examples,there are 8 prematurely delivers(4.28%), 86 difficult labors(45.99%), 24 newborn suffocates(1Z83%) and 6 puerperal infection sign(3.21%). Conclusion The fetal membrane broken have little higher rates of premature delivery and the difficult labor formation and the newborn suffocates, which are dosed to the common maternal infant complication.We should deal with them in proper ways positively to reduces the perinatal maternal infant complication rate.
10.Ultrasonographic characteristics of the follicular variant of papillary thyroid cancer in children and adolescents

Objective To compare the sonographic features as well as clinical histopathological features of follicular variant papillary thyroid carcinoma(FVPTC) and conventional papillary thyroid carcinoma (CPTC) in pediatric patients.Methods From Jan.2006 to Dec.2017,26 FVPTC patients and 82 CPTC patients were enrolled in this study.The clinical histopathological findings and the sonographic features were compared between the two groups.FVPTCs and CPTCs were divided into PTC-like and follicular neoplasm(FN)-like based on sonographic characteristics.Results The mean nodule size of FVPTCs was larger than that of conventional PTCs.Extrathyroid invasion and cervical lymph node metastasis did not have significant difference between CPTC and FVPTC patients(53.8% vs 62.2% and 76.9% vs 82.9%,respectively).Multiple nodules(P=0.000)and distant pulmonary metastases(P=0.024) were more frequent in CPTCs than in FVPTCs(P<0.05).The rate of an ill-defined margin (P=0.000) and calcification (P=0.003)in terms of sonographic features were lower in FVPTCs than conventional PTCs(P<0.05).A Ⅴ+Ⅵ diagnosis of PTC on FNAC of FVPTCs was less common than that of conventional PTCs (P=0.014).Multifocality(P=0.000),extrathyroidal invasion (P=0.000),and lymph node metastasis (P=0.000) were significantly different between PTC-like FVPTCs and FN-like FVPTCs.Conclusion FVPTC in children and adolescents shows a relatively larger size,more benign sonographic features,and a lower diagnostic rate of PTC by FNAC compared with conventional PTCs in pediatric patients.
