1.Effect of midazolam as premedication on stress response in patients undergoing cervical plexus block

Objective To investigate the effect of midazolam as premedication on stress response in patients undergoing cervical plexus block.Methods Sixty female patients undergoing selective thyroid surgery were equally randomized into groups of A and B.The patients in group A were intramuscularly injected midazolam 0.08 mg/kg before cervical plexus block and those in group B were not as the controls.Cervical plexus block was performed with 20 ml mixeture of 1% lidocaine and 0.25% ropivacaine 20 min later.BP and HR were recorded,rate-systolic pressure product (RPP) was calculated,and serum glucose(Glu),cortisol (Cor) and angiotensin Ⅱ (AT-Ⅱ) were detected before enter(T_0),before cervical plexus block(T_1),at 5 min(T_2),15 min(T_3) and 25 min(T_4) after cervical plexus block injection.Results In group B BP,HR,RPP,Glu,Cor and AT-Ⅱ all higher at T_1-T_4 than those at T_0 BP,HR,RPP,Glu,Cor and AT-Ⅱ were all lower in group A than those in group B at T_1-T_4 (P<0.05).Conclusion Intramuscularly premedication with midazolam is effective in reducing stress response in patients undergoing cervical plexus block.
2.ASSESSMENT OF VITAMIN A STATUS BY CONJUNCTIVAL IMPRESSION CYTOLOGY

Ocular signs and serum vitamin A concentration are commonly used tor the diagnosis of vitamin A deficiency. Recently, impression cytology was suggested as a method for early detection of vitamin A status. Twenty patients with normal eyes and digestive disease, and 111 children with normal eyes were evaluated by serum vitamin A concentration, relative-dose-response and conjunctival impression cytology test. About 90% of the patients with normal conjunctival impression cytology has normal vitamin A status. About 70% of the patients with abnormal conjunctival impression cytology has abnormal vitamin A status despite their seemingly normal clinical ocular examination, and so did the children. These results suggest that impression cytology can identify individuals with preclinical vitamin A deficiency.
3.The clinicophatholgic report of antivirus therapy in patients with liver cirrhosis resulting from Hepatitis B

Objective To evaluate the clinicophatholgic benefits and safty of antivirus therapy in patients with liver cirrhosis resulting from hepatitis B.Methods 80 patients with HBV-ralated liver cirrhosis were divided into three groups by the histopathology of liver:group of lamivudine treated with lamivudine 100 mg once daily;Adefovir group treated with Adefovir 10 mg once daily;control group treated with liver protective treatment only.Liver and renal function,PTA and HBV DNA were regularly measured.The Child push-Turotte sore and histopathology wag compared before and after treatment.All courses of treatment were 36 weeks.Results The scores of Child Pugh-Turotte sore in groups of lamivudine and Adefovir were lowered sinificantly (3.9 and 2.1 respectively),the load of HBV-DNA was decreased also[(4.1±0.9) copies/ml and(2.8±1.0) copies/ml],liver inflammmation decreased by more than 2 scores and liver fiber was improved by more than one score,with obviously significant difference(P<0.05) as compared with control group.Conclusion Patients with HBV-related cirrhosis treated with lamivudine and adefovir for antivims are improved and antivirus is important and safe to those during cirrhosis decompensation.
4.A Comparing Study of Different Anesthetic Methods for Patients during Laparoscopic Hysterectomy

Objective To study the effects of different anesthetic methods on stress response in patients during laparoscopic hysterectomy. Methods 45 patients scheduled for laparoscopic hysterectomy were randomly divided into three groups: group A (n = 15) , group B( n = 15) and group C (n = 15). blood samples was taken from patients in each group before anaesthesia, at 10 min and 40 min after pneumoperitomeum with CO_2 , 10 min after operation, to determine the glucose (BG) , catecholamine ( CA ) , C - response protein (CRP) and interleukins -6( IL -6). The changes of MAP ,HR,P_(ET)CO_2 and SpO_2 were also recorded. Results After anesthesia, the MAP, HR significantly decreased in group A (P 0.05 ). Conclusion Combined general epidural anesthesia can effectively inhibit the stress response in patients undergoing laparoscopic hysterectomy.
5.Comparative analysis of immediate and selective interlocked intramedullary nailing for treatment of open tibial shaft fractures

Objective To compare the therapeutic effect and complication of immediate and selective interlocked intramedullary nailing in treatment of open tibial shaft fractures. Methods A total of 123 open tibial shaft fractures of 102 cases were treated with immediate fixation with interlocked intramedullary nailing after debridement (Group A) and with selective fixation with interlocking intramedullary nailing after debridement and closing the wound (Group B). Results The follow up for more than half a year showed that all fractures were healed, with excellent joint function of knee and ankle. Of all, 12 cases with skin necrosis and defect but without deep infection were healed after pedicle flap reconstruction. The result of Group A was much better than that of Group B in regard of total operation time, hospital stay, cost, amount of antibiotics used and partial weight bearing. No statistical difference in therapeutic effects and complications was found between two group. Conclusion Infection is not the contra-indication of interlocked intramedullary nailing in open tibial fractures. Immediate operation can be done after thorough debridement.
6.Mifepristone results in abortion by increasing Th1 bias at materno-fetal interface

Objective:To probe into effects of Mifepristone on the Th1/Th2 balance at materno-fetal interface.Methods:Sixty three healthy women in early pregnancy were randomized into two groups,30 women used mifepristone 200 mg,and 33 women as control.Their decidual tissue was collected by curettage in 48 hours of taking the drug.The Th1/Th2 cytokine expressions in decidua were analyzed by immunohistochemistry.Results:In the decidual tissue of normal early pregnancy,there were significantly higher densities of Th2 cytokines,IL-4 and TGF-?2,and lower density of Th1-type cytokine,IFN-? and IL-2.After having treated by Mifepristone,the decidual tissue appeared to significantly higher densities of Th1-type cytokines(IL-2, IFN-?).Conclusion:Our findings suggest that Mifepristone can up-regulate expression of the Th1-type cytokines,and cause a shift towards Th1 bias,which leads to abortion.
7.Detection of Gene Mutations in a Family of Congenital Atrichia with Papular Lesions

Objective To detect gene mutations in a family of congenital atrichia with papular lesions (APL). Methods Polymerase chain reaction and DNA sequencing were used to search for mutations in the HR gene (a causative gene of APL), the CJB6 gene, and the CDSN gene. Results No mutation was found in these three genes except for single nucleotide polymorphisms (SNPs) in the HR and CDSN genes. Conclusion No mutation is identified in the HR, CJB6 or CDSN gene in this family affected by congenital APL.
8.Construction of HaCaT cell lines stably expressing the human GJB6 gene by using a Tet-On lentiviral vector and their identification

Objective To construct HaCaT cell lines stably expressing the wild type human GJB6 gene or its mutant by using a Tet-On lentiviral vector, and to lay an experimental foundation for studies on pathogenesis of hidrotic ectodermal dysplasia. Methods The wild-type human GJB6 gene and its mutant (A88V)were amplified by PCR, and then inserted into the Tet-on lentivirus plasmid to construct recombinant lentivirus vectors. The recombinants were identified by gene sequencing and enzymatic digestion. Cultured HaCaT cells were classified into three groups to be transfected with a negative control lentiviral vector (NC group), the lentivirus vector expressing the wild-type human GJB6 gene (WT group), or the lentivirus vector expressing the mutant human GJB6 gene (MU group). Puromycin was used to select HaCaT cell clones stably expressing the GJB6 gene which encodes the connexin 30 (Cx30)protein. The selected HaCaT cell clones were cultured with or without tetracycline for 48 hours, thereafter, real-time PCR(RT-PCR) was performed to detect GJB6 gene mRNA expression, Western-blot analysis to measure expressions of Cx30 and FLAG-tag proteins, and cell counting kit 8 (CCK8)assay to evaluate cellular proliferative activity. Results Enzymatic digestion and gene sequencing showed that recombinant lentivirus plasmids were successfully constructed. RT-PCR showed evidently increased mRNA expression of the GJB6 gene in stably transfected HaCaT cells. Moreover, the expression abundance of the GJB6 gene was 112.369 times higher in the WT group induced by tetracycline than in that without tetracycline treatment (P < 0.05), and 2.249 times higher in the MU group induced by tetracycline than in that without tetracycline treatment (P < 0.05). Western-blot analysis showed that Cx30 and FLAG-tag proteins were stably expressed in the WT group and MU group after induction with tetracycline, while neither of them was observed in the WT group or MU group without tetracycline treatment, or in the NC group. Significant differences were noted in cellular proliferative activity (expressed as the absorbance value at 450 nm)between the MU group with and without tetracycline treatment and between the WT group with and without tetracycline treatment at 4, 8, 12, 24, 36 and 48 hours (all P <0.05), but not between the NC group with and without tetracycline treatment at any of the above time points (all P >0.05). Conclusion HaCaT cell lines which stably express the wild-type GJB6 gene or its mutant(A88V)are successfully constructed.
9.Effects of narrow-band ultraviolet B(NB-UVB)on the expression of Gadd45α and proliferation of human HaCaT keratinocytes

Objective To investigate the effects of NB-UVB on the expression of Gadd45α as well as cell apoptosis and cycle of human HaCaT keratinocytes.Methods Cultured HaCaT cells were exposed to various doses (100,200,400 mJ/cm2)of NB-UVB followed by an additional culture of 6,12 and 24 hours,respectively.Reverse transcription PCR and Western blot were performed to detect the mRNA and protein expression of Gadd45α respectively in HaCaT cells,cell counting kit 8 (CCK8)to measure the proliferation of cells,and flow cytometry to determine the cell cycle distribution of HaCaT cells before and after the exposure to NB-UVB.Results Gadd45α was expressed in HaCaT cells.After exposure to NB-UVB of the three doses,the mRNA and protein levels of Gadd45α increased at 6 hours and 12 hours,but declined at 24 hours,and significant changes were observed in HaCaT cells at the three time points after exposure to NB-UVB of the three doses (all P<0.05).The Gadd45α/β-actin mRNA ratio was 1.4360±0.6551.1.8633±0.0979,1.9266±0.1724 in HaCaT cells 12 hours after irradiation to NB-UVB of 100,200 and 400 mJ/cm2,respectively,significantly higher than that in unirradiated cells(0.6000±0.1276,all P<0.05).Also,increased Gadd45α/β-actin protein ratio was noted in HaCaT cells 12 hours after irradiation to NB-UVB of 100,200 and 400 mJ/cm2 compared with unirradiated cells (0.0773±0.0005,0.1936±0.0015,0.2373±0.0015 vs.0.0290±0.0010,all P<0.05).NB-UVB inhibited the proliferation of HaCaT cells in a time-and dose-dependent manner.Flow cytometry showed that irradiated HaCaT cells were blocked in G2 phase of the cell cycle.and the percentage of HaCaT cells in G2 phase was 13.53%±1.03%,17.77%±2.25%,30.03%±4.29%afler exposure to NB-UVB of 100,200 and 400 mJ/cm2,respectively,compared to 9.24%±0.97%in unirradiated cells (all P<0.05).Conclusions The expression of Gadd45α is increased in HaCaT cells after exposure to NB-UVB,and Gadd45α may be involved in the NB-UVB-induced suprression of cell proliferation of and cell cycle arrest in HaCaT cells.
