Objective To investigate the important role of bronchial provocation test with hypertonic saline and induced sputum in the diagnosis and treatment of chronic nonproductive cough by assessing its feasibility and the side effects which may occur during the procedure. Methods The diagnosis procedure was based on the anatomical protocol for diagnosing chronic nonproductive cough, howerer, hypertonic saline bronchial provocation test and induced sputum were conducted for unconfirmed 38 pafients who suffered chronic congh. Results 23(23/38,60.5%)patients with airway hyperresponsiveness, the forced expiratory volume in one second(FEV1)was decreased 27%?5.8)%,Side effects occurred in 3 patients(3/38,7.8%).Appropriate sputum samples were obtained after hypertonic saline challeng,The percentage of eosinophi1 counts in induced sputum was high in 24 patients(24/38,63.2%),including cough variant asthma(CVA)(17/23,73.9%)and eosinophilic bronchitis(EB)(7/38,18.4%).After treatment, the percentage of eosinophi1 counts in induced sputum of patients was significantly reduced in CVA and EB.Conclusion Bronchial provocation test with hypertonic saline and induced sputum were an effective method for diagnosing chronic nonproductive cough and little side effects occurred during the procedure.

Objective The purpose of this study was to describe the clinical and CT features in Cryptogenic organizing pneumonia(COP),and method of managemant. Methods We reviewed characteristics of clinical, laboratory, chest CT and pulmonary function test of 6 patients with histologically proven COP,and evaluated their therapeutic response to corticosteroids. Results Most patients of 6 cases manifested as a persistent nonproductive cough, exerting dyspnea, pyrexia, malaise, and weight loss clinically.Less common symptoms include pleuritic chest pain and hemoptysis.Chest CT manifestations included ground-glass opacity, consolidation, diffusing reticular infiltration of parenchyma, pleural effusion, cavitation and migration. The patients had high level of peripheral WBC number, erythrocyte sedimentation, and C-reactive protein.Pulmonary function tests showed a impairment of restrictive pattern and diffusion capacity. All but one Patients responsed well to corticosteroids. Conclusions COP is a rapidly developing pneumonia-like illness characterized by lung inflammation,The clinical doctor must always be aware of the full view of COP so as to achieve successful diagnosis. Most of the patient can recover with corticosteroids therapy.

Objective To evaluate the clinical features and various diagnostic methods for sarcoidosis. Methods The data of 28 cases with sarcoidosis confirmed by pathology were reviewed and analized, and different diagnostic methods were compared. The disease was evaluated by regular serological tests, chest X ray film, chest high resolution computed topography(HRCT),bronchofibroscopy, and pulmonary function test. Results These patients shared a typical clinical presentation, consisting of dyspnea, cough, subskin nodes, enlargement of peripheral lymphnodes and splenomegaly.Laboratory test revealed elevated serum blood sedimentation, alkali phosphatase,Angiotensin-converting,and urine calcium. The saricoidosis was confirmed by pathological biopsy for bronchial mucosa, lung tissues, peripheral lymphnodes, subskin nodes biopsies and spleen.HRCT showed its superiority to traditional X ray film examination in fingding early staging chest sarcoidosis. Conclusions Chest HRCT and bronchopulmonary biopsy through bronchofibroscopy possess significant clinical value for sarcoidosis diagnosing and staging. And extropulmonary lessions biopsy also plays a complementary role to some extent.

Journal of Xi'an Jiaotong University(Medical Sciences) 1982;0(04):-
The cytogenotoxic effect of Na_2SeO_3 on lymphocytes in human whole blood culture was studied. This was done by means of analysis of sisterchromatid exchange (SCE) and determination of unscheduled DNA synthesis (UDS) with the use of scintillometric procedure and autoradiographic observation. The experiments showed that Na_2SeO_3induced a significant increase of SCE value and higher levels of UDS. These results indicate that Na_2SeO_3 has DNA-damaging potential and cytogenotocity, its threshold concentration was 6.48?10~(-7)mol/L.

Apoptosis, an intrinsic cell death model which is regulated by organisms, is essential for the maintenance of tissue homeostasis in multicellular organisms. Recently, researchers on cell apoptosis have paid more attention on mitochondrion than on nucleus. Different death stimulus induces opening of PT pore, degradation of mitochondrial transmembrane potential, activation of caspase and inducing cell apoptosis. Bcl-2 and Bcl-XL inhibit cell apoptosis via mitochondrion while Bax, Bak and Bad induce apoptosis by means of regulation of mitochondrion.

Objective To observe the effect of combined continuous femoral nerve blockade (CFNB)with epidural morphine postoperative analgesia on total knee arthroplasty (TKA)in elderly patients. Methods Eighty elderly patients scheduled to undergo TKA were randomly divided into two groups:groups FN and FNM.Groups FNM and FN were received 0.1% morphine 2 ml and normal saline 2 ml through epidural space respectively at the end of surgery. After surgery, all patients were implanted catheter around femoral nerve guided by ultrasound, and were infused 0.2% ropivacaine 25 ml.Continuous infusion of 0.2% ropivacaine for femoral never block was administrated at a rate of 5 ml/h for 48 hours to all patients.VAS scores and bending degree during rest, active and passive exer-cise were recorded at time point of postoperative 24,48,72,96 h.Ambulation status on the fifth day after operation were observed.Satisfaction of hospitalization were graded before hospital discharge. Results Compared with group FN,VAS scores during rest, active and passive excercise were signifi-cantly decrease at 24 h in group FNM (P <0.05 or P <0.01).The bending degree of knee joints in group FNM were significantly greater compared with group FN at 24 h after surgery (P <0.01).Am-bulation status on the fifth day after surgery were significantly better in group FNM compared with group FN (P <0.05).The level of patient satisfaction was much higher in group FNM (P <0.05). Conclusion After TKA,CFNB combined with single epidural morphine injection can provide better analgesia and bending degree with a high level of satisfaction.

OBJECTIVE: To study the effects of Shengji Huayu Recipe (a traditional Chinese medicine compound recipe for resolving stagnation and promoting granulation) and its decomposed formulas (Huayu Recipe for resolving stagnation and Shengji Recipe for promoting granulation) on the synthesis of collagen types I and III in granulation tissue of rats in early wound healing. METHODS: Twenty-four male Sprague-Dawley (SD) rats with full-thickness skin lesion were randomized into 4 groups: Shengji Huayu Recipe-treated group, Shengji Recipe-treated group, Huayu Recipe-treated group and untreated group. Collagen types I and III in granulation tissue of the rats were tested with immunohistochemical methods and image analysis. RESULTS: On the third day of wound healing, collagen I of the rats in both Shengji Huayu Recipe-treated group and Shengji Recipe-treated group was higher than that in the untreated group, and collagen I of the rats in Huayu Recipe-treated group was lower than that in the untreated group (P<0.05). Collagen III of the rats in the three treated groups were lower than that in the untreated group (P<0.05). On the seventh day of wound healing, Collagen I of the rats in both Shengji Huayu Recipe-treated group and Shengji Recipe-treated group was higher than that in the untreated group (P<0.05), and collagen III of the rats in both Shengji Recipe-treated group and Huayu Recipe-treated group was higher than that in the untreated group (P<0.05). CONCLUSION: Resolving stagnation and promoting granulation therapy can promote the wound healing in rats.
