1.The Comparing Reseach on Different Diagnostic Methods for Chest Sarcoidosis

Objective To evaluate the clinical features and various diagnostic methods for sarcoidosis.Methods The data of 28 cases with sarcoidosis confirmed by pathology were reviewed and analized,and different diagnostic methods were compared.The disease was evaluated by regular serological tests,chest X ray film,chest high resolution computed topography(HRCT),bronchofibroscopy,and pulmonary function test.Results These patients shared a typical clinical presentation,consisting of dyspnea,cough,subskin nodes,enlargement of peripheral lymphnodes and splenomegaly.Laboratory test revealed elevated serum blood sedimentation,alkali phosphatase,Angiotensin-converting,and urine calcium.The saricoidosis was confirmed by pathological biopsy for bronchial mucosa,lung tissues,peripheral lymphnodes,subskin nodes biopsies and spleen.HRCT showed its superiority to traditional X ray film examination in fingding early staging chest sarcoidosis.Conclusions Chest HRCT and bronchopulmonary biopsy through bronchofibroscopy possess significant clinical value for sarcoidosis diagnosing and staging.And extropulmonary lessions biopsy also plays a complementary role to some extent.
2.Relationship between the effect of docetaxel inducing apoptosis of non - small - cell lung cancer cell and the intracellular level of reactive oxygen

Purpose: To explore the relationship between the effect of docetaxel inducing apoptosis of non-small-cell lung cancer cell and intracellular level of reactive oxygen. Methods: SPC-AI lung cancer cells were treated with 10 -8mol/L docetaxel for 24 hours to observe the apoptotic morphological change under the electromicroscope and fluorescence microscope in ritro. The treated cells were harvested to analyze the cell cycles using flow cytometry and to estimate the intracellular reactive oxygen levels by staining with 2', 7-dichlorodihydrofluorescein diacetate and dihydroethidine. Results: After SPC-AI lung cancer cells were exposed to 10 ~ mol/L docetaxel for 24 hours, the typical apoptotic morphological changes were observed under the electromicroscope and fluorescence microscope. There was a higher reactive oxygen level in docetaxe-treated cells group than in control group. After 24 hours docetaxel exposure, there was a significant G2 ~'M phase arrest of SPC-AI lung cancer cells. Conclusions: Docetaxel can induce apoptosis in SPC-AI lung cancer cells through the mechanism of G2 ~ M phase arrest and by elevating intracellular level of reactive oxygen.
3.Research on In-vitro Percutaneous Absorption and Pharmacodynamics of Chinese Herbal Compound Cremor for Eczema with Different Mass Concentrations of Synthetic Borneol

Objective To compare in-vitro percutaneous absorption and pharmacodynamic actions of anti-inflammation and inhibiting delayed-type hypersensitivity of Chinese herbal compound cremor for eczema (CHCCE) with different mass concentrations of synthetic borneol. Methods By adopting modified Franz diffusion device add with isolated BALB/cnude mice skin as a barrier, in vitro percutaneous absorption effectiveness of CHCCE with different mass concentrations of borneol was compared by in vitro percutaneous test after the content of matrine was determined with high performance liquid chromatography(HPLC). Meanwhile, the effects of CHCCE with different mass concentrations of synthetic borneol on reducing dimethylbenzene-induced auricular edema and suppressing delayed-type hypersensitivity induced by 2,4-dinitrochlorobenzene(DNCB) in mice were compared. Results Cumulative permeation amount of matrine in CHCCE with synthetic borneol was higher than that in CHCCE without synthetic borneol 2~ 48 h after administration (P < 0.05). There was no significant difference of cumulative permeation amount (P>0.05) among CHCCE groups with different mass concentrations of synthetic borneol after 48 h. In vitro percutaneous absorption behavior of matrine arrived to the steady state and the cumulative permeation amount of matrine presented a decreasing trend in all medication groups 12 h after administration. Within 12 h of the medication, the permeation rate of CHCCE with different mass concentrations of borneol was in the sequence of 3% borneol > 1% borneol > 2% borneol > 0.5% borneol > no borneol. The content of matrine was decreased with the increase of mass concentration of synthetic borneol after 12 h. The results of pharmacodynamic actions of CHCCE showed that compared with the blank control group, CHCCE with 1%, 3% synthetic borneol could significantly suppress the acute inflammation induced by dimethylbenzene and inhibit contact dermatitis induced by dinitrochlorobenzene (DNCB) in mice(P < 0.05). Compared with Triamcinolone Acetonide Acetate and Miconazole Nitrate and Neomycin Sulfate Cream, CHCCE with 1%, 3%synthetic borneol could decrease the serum level of IL-4, but the difference was insignificant(P>0.05). Conclusion CHCCE with 1% synthetic borneol has good effects on in vitro transdermal absorption, and can suppress inflammation and delayed-type hypersensitivity effectively.
4.Status quo and trend of VIP services in the tertiary public hospitals of Shanghai

Objective:To analyze status quo and trends of VIP services in the tertiary public hospitals of Shang-hai and provide references for health administrative departments. Methods:Health policies of VIP services in tertiary public hospitals were searched and analyzed, and the number of medical institutions, services, prices and service fees were analyzed from 2011 to 2013 . Results:There is a clear demand for VIP services in the tertiary public hospi-tals of Shanghai, and fees for rooms, nursing, and examinations for outpatient and inpatient care are decided by the hospitals. 89. 7% of the tertiary public hospitals in Shanghai offered VIP services, and the trend was steadily grow-ing. The four services that could be decided by hospitals varied greatly, and the service fees for inpatient care in-creased significantly. The total cost of VIP services in the tertiary public hospitals of Shanghai accounted 6. 2% of all costs, and the percentage of income from drugs was lower. Conclusions:VIP services in public hospitals have a his-torical necessity;management should be strengthened in the short term;public hospitals should strengthen their own management and provide VIP services regularly, and health administration departments should strengthen regulation. In the long run, it is suggested that public hospitals should draw lessons from international experiences to form a pat-tern of multi-level medical services and actively carry out cooperation with private medical institutions.
5.Status quo and trend of premium private health services in Shanghai

Objective:To study status quo of premium private health services and analyze the trend of its devel-opment. Method:The scope of premium private medical institutions was first defined. Then, seven indicators were used to analyze the allocation of resources;two indicators were used to analyze services;eight indicators were used to analyze costs. The entire situation of different styles of institutions through 2011 to 2013 was compared. Results:The results indicated that in the allocation of resources, the current level of premium private medical institutions is not high enough;large-scale construction is still at its early stages;and the medical personnel structure is not reasonable enough;as for service quantity, the total growth rate of premium private medical institutions is high but the service quantity is still far below that of the VIP services in public hospitals;as for medical expenses, premium private medi-cal institutions are charging high service fees, and the internal structure of the expenses is reasonable. Conclusions:Although the development of premium private health services is at an early stage, development prospects are promis-ing. Premium private health services should strengthen the aspects of medical technology, service levels, management capabilities, human resource building, and brand development.
6.Carrier detection and prenatal diagnosis of hemophilia Alpha.

OBJECTIVETo establish an effective laboratory examination system for carrier detection and prenatal diagnosis of haemophilia Alpha(HA) with a variety of molecular biological methods which are simple,rapid and easy to use.
METHODSDetection of inversion involving intron 22 in the FVIII gene was completed by long distance polymerase chain reaction (PCR) and linkage analysis was performed by using several genetic polymorphisms including an intragenic Bcl I RFLP, 2 STRs and an extragenic St14 VNTR.
RESULTSIntron 22 inversion was observed in 10 out of the 21 (47.6%) pedigrees examined. Prenatal diagnosis was completed in 3 pedigrees. A further combination of the four intragenic and extragenic polymorphic loci gave an informative rate of 94.7%.
CONCLUSIONSFemale relatives in HA families with inversion can be detected with direct diagnostic procedure. The application of long distance PCR makes the detection much more simple and rapid. For families without inversions,it is easier and more cost-effective to undertake linkage analysis of genetic polymorphism based on PCR.

7.Mucosa advancement flap anoplasty in treatment of chronic anal fissures: a prospective, multicenter, randomized controlled trial.

Background: Anal fissure is one of the most common anal-rectum diseases, and approximately 10 percent patients with chronic anal fissure ultimately receive surgery. Relieving postoperative pain and protecting functions of the sphincter are central issues for coloproctologists. Objective: To evaluate the efficacy and safety of anoplasty in the treatment of chronic anal fissures. Design, setting, participants and interventions: In this prospective, multicenter, randomized controlled trial, 120 adult patients with chronic anal fissure were referred from Department of Coloproctology of Yueyang Hospital of Integrated Traditional Chinese Medicine Affiliated to Shanghai University of Traditional Chinese Medicine, Shanghai Ninth People's Hospital Affiliated to Shanghai Jiao Tong University School of Medicine and Shanghai Municipal Hospital of Traditional Chinese Medicine. The patients were enrolled from January 2009 to April 2010 and randomly divided into study (mucosa advancement flap anoplasty, abbreviated as anoplasty) group and control (fissurectomy) group. The two groups were assessed separately, and the main outcome measures were observed for 2 weeks, with a short-term follow-up for 6 weeks. Main outcome measures: Degree of pain, haemorrhage and anal canal pressure were observed and recorded preoperatively, and on the third day, the fourteenth day and the sixth week postoperatively. The wound healing time was also recorded. Surgical complications of the two groups were recorded and compared on the third day and the sixth week postoperatively. The curative effects associated with the surgery were analyzed on the fourteenth day and the sixth week after surgery and the therapeutic results were evaluated. Results: Three patients were dropped out due to the early discharge from hospital and losing connection (1 in study group and 2 in control group). Overall the surgery showed that the anoplasty group had better results than the fissurectomy group in the curative effect on the sixth week after operation (P<0.05). Time of wound healing in the anoplasty group was (17.22±4.41) d and was better than (21.24±7.44) d of the fissurectomy group (P<0.05). Concerning the relief of wound pain, the anoplasty group achieved better results than the fissurectomy group at the third day, the fourteenth day and the sixth week after operation (P<0.05). Anoplasty reduced bleeding and had better efficacy than the fissurectomy at the third day and the fourteenth day after operation (P<0.05), however, there was no statistical difference at the sixth week after operation (P>0.05). There were no significant differences in relieving the anal canal pressure (P>0.05) and the surgical complications (dysuria, edema of anal margin, fever, infection, anal incontinence and anal deformation) between the two groups (P>0.05). None of the patients suffered postoperative complications by the sixth week after operation. Furthermore, there was no recurrence in either of the two groups at six weeks after operation. Conclusion: The results indicate that anoplasty for chronic anal fissures has advantages such as better therapeutic effects, less postoperative pain, a shorter healing time and no incidence of anal incontinence.
8.Methodological quality assessment of robot-assisted surgery guidelines and consensus

Objective To evaluate the methodological quality of clinical practice guidelines and consensuses of robot-assisted surgery. Methods The guidelines and consensuses were screened according to the inclusion and exclusion criteria by searching the domestic and overseas guidelines network and electronic database from 1 January, 2000 to 29 December, 2021. The Appraisal of Guidelines for Research and EvaluationⅡ (AGREEⅡ) instrument was used independently by two evaluators to evaluate the guidelines and consensuses. The consistency test of intraclass correlation coefficient (ICC) was carried out for two evaluators, and the score of guidelines and consensuses in the six domains of AGREEⅡ were calculated. Results A total of 34 guidelines and consensuses were included, including 10 guidelines and 24 consensuses. The ICC was all greater than 0.75, indicating that the consistency of the two evaluators was high. The average scores of the 34 guidelines and consensuses in the six domains of AGREEⅡ (scope and purpose, stakeholder involvement, rigor of development, clarity of presentation, applicability and editorial independence) were 81.0%, 43.5%, 28.2%, 81.5%, 12.7% and 51.7%, respectively. Conclusion These evaluated guidelines and consensuses of robot-assisted surgery still need to be improved in the domains of rigor of development, applicability and editorial independence. With the continuous development of robot-assisted surgery, more guidelines and consensuses based on higher level of evidence will be developed to promote the standardized use of robot-assisted surgery.
9.Research progress of details enhancement methods in medical images.

Effective medical image enhancement method can not only highlight the interested target and region, but also suppress the background and noise, thus improving the quality of the image and reducing the noise while keeping the original geometric structure, which contributes to easier diagnosis in disease based on the image enhanced. This article carries out research on strengthening methods of subtle structure in medical image nowadays, including images sharpening enhancement, rough sets and fuzzy sets, multi-scale geometrical analysis and differential operator. Finally, some commonly used quantitative evaluation criteria of image detail enhancement are given, and further research directions of fine structure enhancement of medical images are discussed.
10.Discovery of A New Prognostic Molecular Marker NKX2-3 for Acute Myeloid Leukemia

ObjectiveTo analyze the expression of molecular marker affecting the prognosis of acute myeloid leukemia (AML) patients from bioinformatics database, thus providing an experimental basis for further exploration of a novel molecular marker for the prognosis of AML. MethodsThe prognostic data of 179 AML patients from The Cancer Genome Atlas (TCGA) database were examined for differential gene analysis and survival analysis. The bone marrow samples of 74 healthy individuals (HI) and 542 de novo AML patients in the dataset GSE13159 downloaded from the Gene Expression Omnibus (GEO) database were analyzed to detect the difference in the expression levels of differential target genes. Peripheral blood and bone marrow samples were collected from 18 de novo AML patients and 20 age- and gender-matched healthy controls, and real-time fluorescent quantitative PCR was used to validate the expression levels of the differential genes in the AML patients. ResultsBioinformatics data analysis showed that the optimal cut-off value of Homo sapiens NK2 homeobox 3 (NKX2-3) calculated by R language was 0.051. Survival analysis revealed a statistically poorer overall survival in de novo AML patients with high NKX2-3 expression than in those with low NKX2-3 expression (P = 0.0036). NKX2-3 was highly expressed in patients with de novo AML than in HI and the difference was statistically significant (P < 0.001). Real-time fluorescence quantitative PCR verified the expression levels of the NKX2-3 gene in AML patients and confirmed that compared with those in HI, in the de novo AML patients, NKX2-3-1 and NKX2-3-2 were highly expressed and were significantly correlated (P = 0.000, P = 0.000). ConclusionNKX2-3 is highly expressed in de novo AML patients, and the AML patients with high NKX2-3 expression have poor overal survival. NKX2-3 may be closely related to the clinical outcome and prognosis of AML.
