1.Effects of p38 MAPK kinase inhibitor SB203580 on the airway inflammation and Th2 cytokines in asthmatic mice

Aim To explore the effects of SB203580,a special inhibitor of p38 mitogen-activated protein kinase(p38 MAPK),on the airway inflammation and Th2 cytokines,interleukin 4(IL-4) and IL-5,in asthmatic mice.Methods Thirty mice were randomly divided into 3 groups: normal control,asthmatic group and SB203580 group.The expression of IL-4 mRNA and IL-5 mRNA in lung tissue and IL-4 and IL-5 protein in bronchial alveolar lavage fluid(BALF) were detected using in situ hybridization and ELISA,respectively.The number of inflammatory cells in BALF and histopathological changes were observed.Results The number of inflammatory cells and the concentrations of IL-4 and IL-5 in BALF,and the expression of IL-4 mRNA and IL-5 mRNA in lung tissue were higher in the asthmatic mice than in the normal control mice(P
2.Effect of p38 protein kinase on the activation of rat alveolar macrophages by lipopolysaccharide

AIM: To investigate the role of p38 protein kinase in the activation of rat alveolar macrophages(AMs) induced by lipopolysaccharide(LPS). METHODS: Nuclear protein was extracted, p38 protein kinase in nuclear extracts was analyzed by Western blot. The concentrations of TNF-? and IL-8 in supernatant were measured by radioimmunoassay. RESULTS: The concentrations of TNF-?, IL-8 in supernatant and p38 protein kinase in nuclear extracts were increased significantly induced by LPS and blocked by SB203580, a selective inhibitor of p38 protein kinase. CONCLUSION: The inductoin of TNF-? and IL-8 in alveolar macrophages by LPS may be mediated through the activation of p38 protein kinase.
3.Diagnosis and treatment of pancreatic trauma

Objective To investigate the diagnosis and management of pancreatic injury. Methods The clinical data of 23 patients suffering from pancreatic injury treated by surgery in recent 15 years were analyzed retrospectively. According to the pancreas organ injury scale of the American Association for the Surgery of Trauma, there were 8 cases in grade I, 8 in grade II, 5 in grade III, 1 in grade IV and 1 in grade V. Results Only 2 patients were diagnosed preoperatively out of the 19 patients.About 63.2%(12/19)of the cases had elevated levels of serum amylase. All the patients were cured.In this series,8 cases occurred external pancreatic fistula, 4 pseudocysts, 2 intra-abdominal infections and 1 hyperglycemia postoperatively. Internal pancreatic fistulas were found in all the non operation patients. Conclusions The serum amylase determinations is neither sensitive nor specific for diagnosis of pancreatic injury. Early diagnosis and correct operative pracedure are important to decrease the postoperative complications and improve the outcome.

AIM: To explore the role of p38 mitogen-activated protein kinase (p38 MAPK) in the regulation of IL-4 and IL-5 expression in asthmatic rats. METHODS: T lymphocytes were isolated and purified from blood of normal or asthmatic rats. p38MAPK in nuclear extracts, mRNA and protein of IL-4 and IL-5 in supernatants were measured by Western blotting, in situ hybridization and ELISA, respectively. RESULTS: The expression of p38MAPK, the percentage of IL-4 and IL-5 mRNA positive cells, IL-4 and IL-5 protein in supernatants were significantly increased in asthmatic T lymphocytes and was blocked by SB203580, a selective inhibitor of p38MAPK. AIM To investigate the activation of p38 protein kinase in alveolar macrophages(AMs) stimulated with lipopolysaccharide(LPS) and the effects of dexamethasone(DEX) and N acetylcysteine(NAC) on the process. Objective To investigate the clinical effect of IFNα combined with mannan peptide in treatment of patients with HBeAg-positive chronic hepatitis B ( CHB ). Methods Eighty HBeAg-positive CHB patients with HBV DNA quantity ranging from 10 to 10 eopies/mL were enrolled and randomized into the treatment group and the control group ( n = 40 for each ). Patients in treatment group were given daily subcutaneous injection of IFNα-2b 5,000,000 U for 52 weeks, and received mannan peptide 10 mg per intravenous injection or 2. 5 mg per intramuscular injection for a total of 2 to 3 treatment courses (12 weeks for each). The control group received only IFNα-2b treatment. Liver function, serum markers of hepatitis B, HBV DNA quantity and blood tests were performed before the treatment and at 2, 4, 8, 16, 26 and 52-week during the treatment; and the adverse effects were recorded. Results The rates for ALT normalization, negative HBsAg, negative HBeAg, HBeAg seroconversion and negative HBV DNA were 91. 8% , 17. 5% , 52. 5% , 27. 5 % and 47. 5% at 52nd week in the treatment group, while those in the control group were 80. 0% , 12. 5% , 30. 0% , 10. 0 % and 25. 0% , respectively. There were significant differences in HBeAg-negative, HBeAg-seroeonversion and HBV DNA-negative rates between two groups (χ2 = 4. 178, 4.021 and 4.381, P < 0. 05 ) , and these indexes in the treatment group were increased to 57. 5% , 30. 0% and 50. 0 respectively at 52nd week after drug withdraw. White blood cells began to be elevated at 4th week and were restored to the normal levels at 8th week in the treatment group, while the count in the control was lower than the normal value even at 52nd week of the treatment with the average of (3.45±1. 18)×109/L. Conclusion Alpha-interferon combined with mannan peptide therapy is effective for patients with HBeAg-positive CHB, which may restore the declined peripheral WBC counts induced by interferon and improve the compliance.
10.Curative analysis of local tissue flaps in treatment for huge mobility fistula after breast augmentation

Objective To investigate the efficacy of breast tissue flap cohesive treatment after breast augmentation injection mixture dis-placed.Methods A total of 78 patients with breast augmentation injection mixture polyacrylamide hydrogel(PAAG)shift in our hospital from January 2006 to March 2014 were divided into two groups according to different surgical methods,39 cases with the material shifted to abdominal wall in control group were treated by taking the cleaning operation to scrape residual cavity,39 patients in observation recieved new breast tissue flap design local treatment of breast fistula closed chest and abdominal wall channel after surgical elimination of the same abdom-inal wall cavities.Results The PAAG extensive deposition in the breast tissue,armpits,chest wall clearance were formed fistula communica-tion.The secretions of patients treated with tissue flap decreased 82%,the lacunar lesions reduced 80% after 2 weeks,while patients without tissue flap,the drain reduced 46%,the lesion size reduced 45%,the difference between the two groups was significant(P <0.01).The heal-ing rate of tissue flap group was higher than that of conventional group after 4 weeks.Conclusion The PAAG mixture shifting formed fistula communication in the abdominal wall is the main reason of the mass flow and a huge cavity.Flap can significantly promote abdominal wall fis-tula healing.
