1.Photoelastic analysis of the biomechanical behavior of the bone interface of Tension More implants.

OBJECTIVETo photoelastically investigate the difference in load distribution of Tension More (TM) implants with different conical angle designs.
METHODSThe following five groups of implants of different conical angles were designed: cylinder implant, upper 1/3 TM implant (taper length of 3 mm); 1/2 TM implant (taper length of 5 mm); lower 1/3 TM implant (taper length of 7 mm); and bottom TM implant (taper length of 10 mm). The implants were centrally located in individually photoelastic models consisting of a simulated trabecular bone and a 1 mm-thick layer of cortical bone. Vertical and 45° oblique static loads were applied at the center of the superstructures. The resulting stresses were monitored photoelastically and recorded photographically. RESULTS With vertical loading, the cylinder implant showed higher stress levels in the cortical bone and trabecular bone than the upper 1/3 TM implant, 1/2 TM implant, and lower 1/3 TM implant. The four groups of TM implants showed lower stress levels in the cortical bone than the cylinder implant under oblique loads. The least favorable stress concentration in cortical bone was observed in the upper 1/3 TM implant under vertical and oblique loads.
CONCLUSIONTM implants of rational conical angle designs seem to be effective in stress distribution. For all designs and load directions, the upper 1/3 TM implant is the most favorable around the crest.

2.Effects of transcutaneous electrical acupoint stimulation assisting sodium nitroprusside controlled hypotension on Glu,ATⅡ and SOD

Objective To observe the effects of transdermal acupoint electric stimulation(TEAS)assisting sodium nitroprus-side induced controlled hypotension on serum glucose (Glu),angiotensin Ⅱ (ATⅡ)and superoxide dismutase (SOD),and to inves-tigate the protective effect of TEAS under controlled hypotension anesthesia.Methods 60 cases undergoing elective endoscopy si-nus surgery by adopting sodium nitroprusside induced controlled hypotension under general anaesthesia maintained the mean arterial pressure(MAP)in 50-60 mm Hg and were randomly and equally divided into two groups.The group Ⅰ conducted TEAS,while the group Ⅱ did not conduct TEAS.The controlled hypotension time and surgery time were recorded in the two groups;Glu and ATⅡ values were detected before anesthesia (T0 ),30 min after hypotension (T1 ),hypotension stopping(T2 );SOD was detected at T0 ,T2 ,30 min after hypotension(T3 ).Results The operation time and controlled hypotension continuous time had no statistically significant difference between the two groups(P >0.05).The Glu value in the group Ⅰ had no statistically significant difference a-mong the 3 time points,while which at T1 ,T2 was higher than that at T0 in the group Ⅱ(P <0.05),and which at T1 ,T2 in the group Ⅱ was higher than that in the group Ⅰ (P <0.05);the ATⅡ value at T1 was higher than that at T0 in the group Ⅰ (P <0.05),while which at T1 ,T2 was higher than that at T0 in the group Ⅱ(P >0.05),which at T1 ,T2 in the group Ⅱwas higher than that in the group Ⅰ (P <0.05);the SOD value at T2 was lower than that at T0 in the group Ⅰ,which at T2 ,T3 in the group Ⅱwas lower than that in the group Ⅰ(P <0.05).Conclusion TEAS assisting sodium nitroprusside controlled hypotension can better in-hibit the stress response.
3.Improvement on isolation and culture of human endometrial glandular epithelial cells and stromal cells in vitro

Objective:To explore the isolation and culture of a better yield of purified human endometrial glandular epithelial cells and stromal cells. Methods:Digestion with high concentrated collagenase and DNase,filtratinn and adhesion purification were used to isolate,purify human endometrial glandular epithelial cells and stromal cells in 30samples. And the two kinds of cells were reproduced. Results:The endometrial cell culture was successfully established in 29 of 30 endometrium samples. From one gramme of endometrial tissue,the yields of glandular epithelial cells and stromal cells were (40~50)?10~6 and (40~50)?10~6 respectively. The purities of glandular epithelial cells and stromal cells were 96% and 98% respectively. Stromal cells could be reproduced into several passage. However,glandular epithelial cells could not be reproduced. Conclusion:A better yield of purified human endometrial glandular epithelial cells and stromal cells can be obtained by the modified culture procedure.

5.Prognostic Value of MELD 3.0 Based Model for Survival Outcomes in Alcoholic Cirrhosis Patients

ObjectiveTo explore the value of the Model for End-Stage Liver Disease (MELD) 3.0 in predicting survival outcomes for patients with alcoholic cirrhosis and to establish an effective mortality prediction model. MethodsClinical data of 788 hospitalized patients who were first diagnosed with alcoholic cirrhosis at the Third Affiliated Hospital of Sun Yat-sen University between January 1, 2011 and December 31, 2019 were analyzed. Patients were followed up until December 31, 2023 and divided into survival and mortality groups based on the survival outcomes at 30 days, 90 days, 1 year, and 3 years after admission. The prognostic values of the MELD 3.0, MELD, MELD-Sodium (MELD-Na) for survival in alcoholic cirrhosis patients were assessed and compared by using the receiver operating characteristic (ROC) curve and the area under the curve (AUC). Additional risk factors associated with mortality in alcoholic cirrhosis patients were identified, and a novel mortality prediction model based on MELD 3.0 was developed. ResultsThe AUC of the MELD 3.0 score in predicting 30-day, 90-day, 1-year, and 3-year survival was 0.823, 0.730, 0.686, and 0.658, respectively, which were superior to those of the MELD-Na (0.802, 0.708, 0.666, and 0.645, respectively) and MELD scores (0.698, 0.668, 0.654, and 0.633, respectively) (all P < 0.05). MELD 3.0 demonstrated better performance at 30 and 90 days (AVC=0.823,0.730; both P < 0.05) than at 1 year and 3 years (AVC=0.686,0.658; both P < 0.05). Binary logistic regression combined with LASSO regression indicated that the independent risk factors associated with the 1-year outcome included MELD 3.0, baseline ascites and hepatocellular carcinoma. A survival prediction model was then established with AUC of 0.748, sensitivity of 0.695, and specificity of 0.775. ConclusionsMELD 3.0 has a superior predictive ability for 30-day, 90-day, 1-year, and 3-year survival in patients with alcoholic cirrhosis than MELD-Na and MELD. The prediction model incorporating MELD 3.0, ascites and hepatocellular carcinoma improves the prediction of 1-year survival outcomes for alcoholic cirrhosis patients.
