1.Study on compensation standard of basic public health service in Fujian province

To study the compensation standard of basic public health service a series of meetings were carried out,university faculty,scholars and public health workers as well as health administrators and policy makers actively joined the discussion.Based on the cost calculation of each items of public health service a preliminary proposal was developed.The proposal was disseminated for soliciting opinions widely and finalized.Finally the compensation standard of basic public health service both for urban and rural conununity health service centers and health stations were issued by Provincial Health Bureau.The compensation standard will provide support for implementation of basic public health policy in Fujian province.
2.Aggregation Behavior of Sodium Dodecylbenzene Sulfonate with Ca2+ by Resonance Light Scattering Method

A resonance light scattering(RLS) method for the determination of critical micelle concentration(CMC) of sodium dodecyl benzene sulfonate(SDBS) was proposed. Under room temperature, the RLS intensity of the SDBS system increased with increasing SDBS concentration. And when the concentration of SDBS approached CMC, the RLS intensity had increased sharply. The RLS peaks were appeared at 330 nm and 396 nm, respectively. The plot of the RLS intensity at 396 nm versus SDBS concentration was S-Curve. The concentration of SDBS at the intersection point of two tangents to S-curve was considered as SBDS CMC. This result was consistent with the results of the pyrene probe fluorescence spectrometry and electrical conductivity method. OBJECTIVETo investigate the relationship between angiotensin I converting enzyme gene insertion/deletion polymorphism and Alzheimer disease (AD), as well as the effect of hypertension on the relationship.
METHODSThis case-control study, included 96 AD patients meeting the DSM-IV diagnosis, and 96 subjects as controls coming from the same area and in the same environmental condition. Using the polymerase chain reaction (PCR) amplified the DNA segments, and the PCR products were identified by 2% agarose gel and visualized by ethidium bromide staining.
RESULTSThere was significant difference between AD patients and controls in ACE genotypes and alleles distribution, as well as between AD patients with high blood pressure and controls with high blood pressure. But between normotensive AD patients and normotensive controls, there was no significant difference in ACE genotypes distribution (P>0.05).
CONCLUSIONACE genotypes associated with the risk of AD, but II genotype as risk genetic factor only restricted in subjects with high blood pressure.

4.Effect of transforming growth factor beta1 on proliferation and secretion of osteoblasts on porous tantalum/MG63 osteoblast-like cell composites

BACKGROUND: Previous studies have demonstrated that the Chinese porous tantalum made in China has non-toxicity and good biocompatibility, which can promote osteogenesis. OBJECTIVE: To investigate the effects of transforming growth factor β1 on proliferation, cel cycle and secretion of osteoblasts on porous tantalum/MG63 osteoblast-like cel composites. METHODS: Passage 3 MG63 osteoblast-like cel suspension (1×109/L) was seeded onto the porous tantalum, then the cel composites were inoculated in the medium with 0, 0.5, 5 and 10 μg/L transforming growth factor β1, respectively. The proliferation of osteoblasts was detected by cel counting kit-8 assay at 1-13 days after inoculation; the cel morphology and ultrastructure observed by scanning electron microscope and transmission electron microscopy; and level of col agen type I detected by enzyme-linked immunosorbent assay. RESULTS AND CONCLUSON: 0.5, 5, 10 μg/L transforming growth factor β1 could promote the osteoblast proliferation, and cel proliferation in the 5 μg/L transforming growth factor β1 group was higher than that in the other groups; in the 5 μg/L transforming growth factor β1 group, laminated osteoblasts adhered on the surface and grew into inner of porous tantalum, which extended more pseudopodia toward the scaffold; osteoblasts-secreted matrix could cover the scaffold and numerous rough endoplasmic reticulum, free ribosomes, dense mitochondria, Golgi apparatus as wel as matrix vesicles could be found in the cytoplasm. In addition, the level of col agen type I in the 5 μg/L transforming growth factor β1 group was significantly higher than that in the other groups (P < 0.05). These results indicate that transforming growth factor β1 can promote proliferation, and col agen type I secretion of osteoblasts on porous tantalum/MG63 osteoblast-like cel composites, and the optimum mass concentration of transforming growth factor β1 is 5 μg/L.
5.Expression and significance of osteogenic genes on porous tantalum-bone interface during osteogenesis

BACKGROUND:Previous studies have shown that home-made porous tantalum has non-toxicity and good biocompatibility, and can promote osteogenesis. Herein, we explore the mechanisms of tantalum-bone interface osseointegration.OBJECTIVE:To observe the morphological characteristics and expressions of integrin β1 and fibronectin on the interface between porous tantalum and bone tissues after implantation into the right rabbit femur, and to evaluate the biological mechanisms of tantalum-bone interface osseointegration.METHODS: Animal models of bilateral femoral condyle defects were made in Japanese big ear rabbits. Porous tantalum rod and allogeneic bone were respectively implanted into the left (experimental group) and right (control group) femur of rabbits. The animal specimens at the bone defect region were taken and made into paraffin sections and hard tissue sections at postoperative 2, 4, 8 weeks for morphological observation of new bone at the junction between the tantalum rod and host bone under light microscope, for osteogenic observation of the tantalum-bone interface under scanning electron microscope, and for immunohistochemical detection of integrin β1 and fibronectin expression.RESULTS AND CONCLUSION:Porous tantalum was bonded closely with the host bone. The loose and thick fibrous capsule was observed in the early stage and became thinner in the late stage shown by hematoxylin-eosin staining. The new bone was visible on tantalum-bone interface. Hard tissue slicing observation showed that the new bone was seen on the porous tantalum-bone interface, blood capillaries grew into the pores at postoperative 2 weeks and the pores were full of new bone tissues at postoperative 4 and 8 weeks. Under the scanning electron microscope, the osteoblasts appeared on the tantalum surface and in the pores at the early stage, and bone maturation and lamelar bone were seen at the late stage. The immunohistochemical results showed that the expression of integrin β1 in the experimental group was significantly lower than that in the control group at postoperative 2 weeks (P < 0.05), but the expression of fibronectin had no significant difference between the two groups (P > 0.05). In addition, there was a decline trend in the expression of integrin β1 and fibronectin atpostoperative 2, 4, 8 weeks. To conclude, the porous tantalum material is beneficial to enhance adhesion of osteoblasts on the surface and inside the micro-pores. Increased expression of integrin β1 and fibronectin on the tantalum-bone interface at early stage may promote early osteogenesis, while their decreased expression at bone maturing stage can promote osseointegration and bone remodeling.
6.Effects of Shengxue Tongbian Granules on Colonic Myoelectric and Ca2+/CaM/MLCK Signaling Pathway in Rats with Slow Transit Constipation of Blood-deficiency and Intestinal Dryness Syndrome

Objective To observe the effects of Shengxue Tongbian Granules on colonic myoelectricity and Ca2+/CaM/MLCK signaling pathway in rats with slow transit constipation(STC)of blood deficiency and intestinal dryness syndrome;To explore its mechanism for the treatment of STC.Methods The STC model of blood deficiency and intestinal dryness syndrome was established by intragastric administration of loperamide combined with tail bloodletting.The rats were divided into control group,model group,mosapride citrate group and Shengxue Tongbian Granules group,with 8 rats in each group.The administration group was given corresponding drugs by gavage for 14 days.The general condition of rats before and after treatment was observed,the fecal water content was detected,the slow wave frequency,amplitude,and coefficient of variation of colonic electromyography were detected using a biological function experimental system,and intestinal propulsion rate was detected.HE staining was used to observe the pathological changes of colon tissue,the concentration of Ca2+ in colonic smooth muscle cells was detected by colorimetry,the expression of Cx43,calmodulin(CaM),myosin light chain kinase(MLCK)and p-MLC20 in smooth muscle tissue were detected by Western blot.Results Compared with the control group,the body mass,fecal water content and intestinal propulsion rate of rats in the model group decreased(P<0.01),the slow wave frequency of colonic electromyography slowed down,the coefficient of variation of frequency increased(P<0.01),and the amplitude and coefficient of variation of slow wave increased(P<0.01);colonic mucosal structure was damaged,with visible inflammatory changes and significant erosion,and the concentration of Ca2+ and the expressions of Cx43,CaM,MLCK,p-MLC20 proteins in colonic smooth muscle cells were significantly decreased(P<0.01).Compared with the model group,the body mass,fecal water content and intestinal propulsion rate of the rats in the mosapride citrate group and the Shengxue Tongbian Granules group significantly increased(P<0.05,P<0.01),the slow wave frequency of colonic electromyography increased and the coefficient of variation of frequency decreased(P<0.01),and the slow wave amplitude and amplitude variation coefficient decreased(P<0.05,P<0.01);the colonic mucosal structure was relatively intact,the erosion situation was improved,and the Ca2+ concentration,Cx43,CaM,MLCK and p-MLC20 protein expressions in colonic smooth muscle cells significantly increased(P<0.01).Conclusion Shengxue Tongbian Granules can improve defecation symptoms and promote colonic motility in STC rats with blood deficiency and intestinal dryness syndrome,which may be related to regulating colonic myoelectric rhythm and activating Ca2+/CaM/MLCK signaling pathway.
7.Design and preliminary application of 3D-printed vertebral bodies in spinal tumor surgery

Objective:To explore the significance of digital orthopedic technology in surgical plan for spinal tumor and the preliminary outcomes of 3D printed vertebral bodies in spinal tumor surgery.Methods:The clinical data of 2 patients were retrospectively analyzed who had had a 3D printed vertebral body implanted at Center of Orthopaedics, Shenzhen Hospital from June 2018 to December 2019. One was a 32-year-old male, diagnosed with cervical neurinoma; the other was a 27-year-old female, diagnosed with giant cell tumor of lumbar bone. 3D virtual reconstruction of tumor and surrounding structures was established via Mimics software for surgical plan. Virtual osteotomy was simulated, their disease models and guide templates were 3D printed, and their metal artificial vertebral bodies were 3D printed after personalized design of the vertebral body diameter, porosity and procedures of reconstruction and fixation. Lesion resection and prosthesis implantation were carried out in accordance with the preoperative plan. After operation, the motor function of cervical or lumbar vertebrae, tumor recurrence, and spinal stability reconstructed were regularly observed.Results:Resections and reconstructions went uneventfully in both cases. The 2 patients were followed up for 21 and 13 months respectively. Their postoperative images showed that their 3D printed vertebral bodies fitted the neighboring vertebral bodies well. The spinal stability was reconstructed without any loosening or periprosthetic osteolysis, and the tumors were removed completely with no recurrence in both cases. Their spinal motor function was satisfactory.Conclusions:Digital orthopedic technology can offer accurate guidance in the treatment of spinal tumors. It is necessary to consider local physiological anatomy in personalized design of a metal vertebral body 3D printed. Clinical application of 3D printed metal vertebral bodies is a new strategy for spinal reconstruction following spinal tumor resection.
8.Self?management of blood pressure among elderly hypertensive patients in new rural communities of Shanghai

Objective To investigate the status of self‐management of blood pressure (BP) among elderly hypertensive patients living in new rural communities of Shanghai. Methods We enrolled a total of 2 241 hypertensive patients over 60 years old who participated in the National Key Research and Development Project "Cohort study on hypertension" of the 13th Five‐Year Plan in Langxia, Shanghai from May to July 2018. Questionnaires, physical examinations, and laboratory tests were conducted. We collected participants' demographic information, cardiovascular history, and self‐management behaviors related to hypertension. The status of self‐management of BP among participants with hypertension was analyzed. Pearson's chi‐squared test was used to explore the differences between males and females. Results Participants'mean age was (70.0±6.8) years, and males accounted for 38.8% of all participants. The systolic and diastolic BP levels of participants were (147.8±15.4) mmHg (1 mmHg=0.133 kPa) and (82.8±9.8) mmHg, respectively. The systolic blood pressure level of women was higher than that of men, and the diastolic blood pressure level of men was higher; the difference was statistically significant (all P<0.05). The awareness, treatment, and control rate of hypertension were 79.0%, 78.0%, and 25.6%, respectively. Males had a significantly higher control rate than females (P<0.05). The status of self‐management of blood pressure was as follows: (1) As to lifestyle, 84.4% of participants lacked exercise, and 69.2% were overweight or obese. The prevalence of smoking and drinking was higher in males (40.7% and 46.2%, respectively), and the proportion of females (12.4%) with anxiety and/or depression was higher than that of males (P all<0.001). (2) Concerning BP‐monitoring, only 10.1% of participants measured BP every week, 22.9% had BP measurement instruments at home, and 4.6% recorded their BP levels. There was no significant difference in self‐blood pressure monitoring behavior between men and women. (3) Regarding adherence to medication treatment, 97.6% took antihypertensive drugs regularly, and 21.6% were on combination therapy. The proportion of men taking combined drugs was higher than that of women, but there was no significant difference. Conclusions The rates of awareness, treatment, and control of hypertension in elderly hypertensive patients in new rural communities of Shanghai were above the national average. However, the status of self‐management of BP was still not satisfactory. More attention should be paid to healthy lifestyle, home BP monitoring, and effective pharmaceutical treatment strategies for hypertension.
9.Clinical progress of internal fixation in the treatment of femoral neck fracture in middle-aged and elderly patients

Middle-aged and elderly patients with femoral neck fracture often suffer from basic diseases. Conservative treatment will significantly increase the incidence of complications. At present, surgical treatment is mostly advocated. Internal fixation is one of the effective treatment methods for middle-aged and elderly patients with femoral neck fracture. It has the advantages of improving hip joint function, accelerating patient recovery, and improving patient quality of life. At present, there are many choices of internal fixation in the world, each has its own advantages and disadvantages, and there is no unified standard. Different surgical methods can be selected according to various factors such as fracture type, patient′s physical condition and surgical auxiliary technology. This article reviews the clinical progress of internal fixation for femoral neck fractures in middle-aged and elderly patients from four aspects: the characteristics of femoral neck fractures in middle-aged and elderly patients, common internal fixation methods, 3D printing guide plate auxiliary technology and artificial intelligence auxiliary technology.
