1.Clinical effect of proximal femoral nail anti-rotation on femoral intertrochanteric fracture in the elderly

Objective To investigate the clinical effect of proximal femoral nail anti-rotation (PFNA) on femoral intertrochanteric fracture in the elderly.Methods Eighty-seven elderly patients from June 2007 to March 2012,who had femoral intertrochanter fracture and received treatment of PFNA,were enrolled in this study.Patients were comprised by forty seven people with stable fracture and forty people with unstable fracture.Results They were all followed up from 4.5months to 14 months,with the average time of 7.2 months.No death were found in hospital.The operation time was 25 min in average.During operation,losing blood was 80 ml.Ambulation time was 5.5 d,and the fracture healing time was 22 weeks in average.However,there were significant differences in intraoperative resetpostoperative position of the blade between the stable and unstable fracture patients (P<0.01).Conclusions PFNA is one of the effective methods in the treatment of intertrochanter fracture,and it has shorter operation time,less losing blood,earlier ambulation,and hardly has complications.
2.Myogenic differentiation factors and 5-azacytidine induce the differentiation of rat bone marrow mesenchymal stem cells into skeletal muscle cells in vitro

BACKGROUND:Muscle transposition is a conventional method to treat muscle tissue defects,but it results in damage to another piece of muscle.For this reason,we designed this study to search for a method to in situ repair muscle tissue defects.OBJECTIVE:To investigate the conditions for in vitro induced differentiation of rat bone marrow mesenchymal stem calls(BMSCs)into skeletal muscle cells.METHODS:Following isolation and culture,passage 3 BMSCs were induced to differentiate in vitro by a combination of5-azacytidine,myogenic differentiation factor,transforming growth factor β1,and insulin like growth factor.At 9 days after induction,cells were harvested and identified by immunohistochemistry.RESULTS AND CONCLUSION:Primary cultured BMSCs exhibited an adherent,colony-like growth.After 5-7 days,multi-synaptic calls,thin and fiat polygonal cells,polygonal cells,and triangle-shaped cells were observed.After 12 days,calls confluenced and covered the whole bottom of culture flask,with slightly altered morphology of BMSCs.After 5-azacytidine induction,some calls died and grew slowly.After 7 days,cells markedly grew and soma was gradually enlarged,presenting with an oval,spindle-shaped,or irregular appearance.After 14 days,spindle-shaped calls become more.After 18-22 days,myotubes were increased in number and enlarged in volume,and myotube nucleuses were also increased.The newly formed myotubes and spindle-shaped fibroblasts were distributed in parallel interval.The immunohistochemistry of BMSCs revealed that cells were positive for CD44,with dark brown granules in the cytoplasm,especially around the nucleus,but they were negative for CD34.The immunohistochemistry of induced BMSCs demonstrated that calls were positive for desmin and skeletal muscle myosins.These findings indicate that myogenic differentiation factors and 5-azacytidine could induce the oriented differentiation of BMSCs into skeletal cells,with the presence of positive expression of desmin and skeletal muscle myosins.
3.Clinical Observation on Diabetic Nephropathy Treated with Enalapril and Yinxingdamo Injection

Objective To explore the clinical effects of diabetic nephropathy treated with enalapril and Yinxingdamo injection. Methods 80 patients were randomly recruited into a control group and a treatment group. The control group was treated with enalapril, and the treatment group was treated with Yinxingdamo injection on the basis of the control group. Results Comparing with the control group, the treatment group could be seen a obvious decrease of 24h UAE, BUN and Cr(P<0.01). Conclusion The therapeutic effects of combined using enalapril and Yinxingdamo injection is better than only using enalapril in treating diabetic nephropathy.
4.Application of Quality Control Circle for reducing the radiation dose during chest CT scan

Objective To explore the effect of quality control circle (QCC) in reducing the radiation dose during chest CT scan. Methods QCC was founded, activity themes were selected, activity schedule was planned, the reasons of high radiation dose was analyzed, countermeasures were planned and implemented jointly by circle members, and then per capita radiation dose and image quality before and after improving was compared to confirm the effect of QCC activities. The subjects of chest CT scan before and after improvement in our center were selected, included totally 218 cases, their average age was (47.05 ± 8.58) years, 162 were male, 56 were female, 44 cases had CT scan before improvement, 174 cases after improvement, and the data were analyzed by T-test and Chi-square test. Results The radiation dose per capita has declined from 13.75 mGy to 3.45 mGy, The rate of standard was 102.08%, progress rate was 74.91%. The rate of high-quality image was 92.52%. Compared with per capita radiation dose before and after the activities, the differences were statistically significant(P<0.001). The rate of high-quality image was 93.18% before activities, the percentage remained steady at 92.52% after the activities, there was no statistically significant differences in image quality(P>0.05). Conclusions Application of QCC not only reduced the radiation dose of the client without image quality changes, but also improved thinking and learning capacity, cooperative consciousness of the circle members.
5.Establishment of a new animal model for end-to-side anastomosis of peripheral nerves

Objective To design a new animal model for end-to-side anastomosis of peripheral nerves and analyze the efficiency. Methods The experiment involved 28 Wistar rats,of which the left legs were set as experimental group and the right ones as control group based on mode of nerve anastomosis.In experimental group,the tibial nerves were cut off for anastomosis of the proximal end of the tibial nerves with the distolateral side of the common peroneal nerve.While in control group,the common peroneal nerves were cut off to anostmose the distal end of the peroneal nerve with the distolateral side of the tibial nerves.Electrophysiological examination was done three months later to observe and compare wave amplitude,latent period and maximal wave amplitude at set stimulus volume in both groups and compare the fiber number at distal and proximal ends of the common peroneal nerves. Results There was no significant statistical difference in regard of amplitude,latent period and maximal wave amplitude in both groups.However,the fiber number at distal end of the stoma was much more than that at proximal end in the experimental group,with statistical difference(P
6.In situ HYBRIZATION STUDY ON GAP-43mRNA EXPRESSION DURING NERVE INJURE

Objective To study GAP\|43mRNA expression during nerve injure and regeneration. Methods Rat sciatic nerve was crushed then, in situ hybridization technique was used to explore GAP\|43mRNA expressions in spinal cord and dorsal root ganglion (DRG). Results The neurons of spinal cord and DRG were detected to have GAP\|43 hybridization sinal by 2 days after sciatic nerve lesion. At later times(4,7 and 14 days postsurgery) the anterior horn motor neurons and DRG cells showed an increase in the number of GAP\|43mRNA positive neurons, followed by a significant rise in their content of GAP\|43mRNA. The number of GAP\|43mRNA positive neurons was decreased by 30 days postinjure, and was nearly nomal 60 days postinjure.Conclusion GAP\|43mRNA expression was increased during peripheral nerve injure and regeneration. The study showed that GAP\|43 may play a key role in nerve regeneration. [
7.Study on the inhibitive effect of mesenchymal stem cells on the immunological rejection in rats after liver transplantation

Objective To investigate the inhibitive effect of mesenchymal stem cells (MSCs) on the immunological rejection in rats after liver transplantation. Methods The recipients and donors were female SD rats and Wistar rats. All rats were randomly divided into 3 groups (28 rats in each group). Rats in group A were infused with normal saline; rats in group B received FK506 (0.25 mg/kg) every 2 days for 2 weeks after liver transplantation; rats in group C were injected with MSCs from male Wistar rats during liver transplantation. The pathological changes, expression of TGF-β1 and IL-10, Y chromosome location, changes of liver function and the survival of the recipients were detected on postoperative day 10. The levels of ALT and AST were analyzed by com-pletely randomised design analysis of variance, and the difference among the 3 groups were analyzed by LSD. Ridit was used to analyze the pathological grading. The survival was analyzed by Log-rank test after the Kaplan-Meier survival curve was drawn. Results The values of ALT and AST were (756±104)U/L and (635±134)U/L in group A, (197±49)U/L and (331±78) U/L in group B, (103±31)U/L and (150±38) U/L in group C, respectively. The difference in the level of ALT and AST among the 3 groups had statistical significance (F = 158, 265, P < 0.05). The liver function of rats in group B and C was better than those in group A (P < 0.01), and the liver function of rats in group C was better than those in group B. The mean values of ridit in group A, B and C were 0.8333, 0.4583 and 0.2083, respectively. The expression rates of TCG-β1 in group A, B and C were 18%±5% , 69%±20% and 85%±24% , with statistical difference among the 3 groups (F=191, P <0.01). There was a significant difference in IL-10 expression among group A (21%±5%), group B (75%±14%) and group C (91%±21%) (F=672, P<0.01). The TCG-β1 and IL-10 had strong positive expression in group B and C, and the expression of TCG-β1 and IL-10 was much stronger in group C than in group B; while the expres-sion of TCG-β1 and IL-10 was weak positive in group A. MSCs cells with Y chromosome were positively stained and were concentrated at the portal area in group C. The 50-day survival rate of rats in group A, B and C were 0, 10% and 90% , respectviely, with significant difference (χ~2=36, P < 0.01). The median survival time of rats in group C was 63 days, which was longer than that in group A and B. Conclusion Simultaneous injection of MSCs from donors during liver transplantation can inhibite the immunological rejection of recipients to the liver graft.
8.Development and identification of mouse anti-human S100 A9 natural protein

Objective:To prepare and identify the mouse anti-human monoclonal antibodies ( mAbs) using leukocytes as im-munogens. Methods: The mice were immunized using human peripheral blood leukocytes. Then, use of B lymphocyte hybridoma technology preparation of mAbs,followed screening by immunocytochemistry and limited dilution. The secreted mAbs were identified by immunoprecipitation,mass spectrometry,Western blot,ELISA and immunohistochemistry. Results:The 35 positive polyclonal cells were obtained,of which 11 strains secreted mAbs against S100A9. And one strain was used to prepare monoclonal antibody. The purified mAb against S100A9 were purified and identified as IgG1 subtype,with the titer,purity and affinity constant was 1∶3. 18×105,95% and 3. 54×108 L/mol,respectively. This mAb generally had 0. 12% crossed reactivity to S100A8 ,and showed little or no cross reactivity to S100A12 and S100A13. The prepared monoclonal antibodies can specifically recognizes the S100A9 antigen in human breast cancer tissues. Conclusion:Successful preparation of mAb against S100A9,which can secrete specific mAb against S100A9 protein with high titers and specificity have been established successfully,which laid the foundation for the immunology application.
9.hMSH2 aberrant expression in patients with sporadic colorectal cancer in Xinjiang.

OBJECTIVE:
To explore the significance of hMSH2 aberrant expression in patients with sporadic colorectal cancer in Xinjiang Uygur Autonomous Region.
METHODS:
Clinicopathological parameters and postoperative samples of 327 patients with sporadic colorectal cancer were collected in Xinjiang Uygur Autonomous Region. Immunohistochemistry PV-9000 two-step method was performed to measure hMSH2 expression in the postoperative pathologic specimens. Prognostic value of hMSH2 expression was evaluated.
RESULTS:
Thirty-five (10.7%) patients showed aberrant nuclear staining of hMSH2 expression. The patients with aberrant expression of hMSH2 showed better prognosis than the normal expression group, with significant difference (P<0.05).
CONCLUSION
In Xinjiang, aberrant hMSH2 expression can be regarded as an independent prognostic factor in patients with sporadic colorectal cancer.

10.Correlations of P2Y12 gene polymorphism with clopidogrel resistance and long -term outcome in patients with acute ischemic stroke

Objective To investigate the correlations of P2Y12 gene polymorphisms with clopidogrel resistance and long-term outcome in patients with acute ischemic stroke. Methods From June 2015 to June 2017, consecutive patients with acute ischemic stroke admitted to the Department of Neurology, Nanjing Jiangbei People's Hospital were enrolled. Thromboelastography was used to measure platelet inhibition rate and assess clopidogrel resistance. Polymerase chain reaction was used to assay C34T and G52T polymorphisms of P2Y12 gene. The patients were followed up at 12 months after discharge. The primary outcome was combined outcome of stroke recurrence, myocardial infarction, and death due to cardiocerebrovascular events. Results A total of 214 patients were enrolled, 51 (23.8%) had clopidogrel re-sistance and 29 (13.4%) had major outcome events. One hundred twenty-eight (59.8%) patients were C34T CC genotype and 86 (40.2%) were CT+TT genotype. The proportion of clopidogrel resistance in patients with CT+TT genotype was significantly higher than that with CC genotype ( 76.5% vs.28.8%;χ2=25.672, P=0.001). There were 131 patients (61.2%) with G52T GG genotype and 83 (38.8%) with GT+TT genotype. There was no significant difference in the proportion of clopidogrel resistance between the GT+TT genotype and the GG genotype (43.1% vs.37.4%; χ2=0.534, P=0.465). Multiple logistic regression analysis indicated that age (odds ratio [OR] 1.064, 95%confidence interval [CI] 1.009-1.115;P=0.021), diabetes ( OR 3.773, 95%CI 1.672-8.475; P=0.004), and C34T CT+TT genotype ( OR 9.087, 95%CI 4.416-22.665; P=0.002) were the independent risk factors fot clopidogrel resistance. Cox proportional hazards model analysis showed that age (Hazard ratio [HR] 1.058, 95%CI 1.001-1.121; P=0.049), hypertension ( HR 3.105, 95%CI 1.149-9.523; P=0.028), homocysteine ( HR 1.101, 95%CI 1.020-1.190; P=0.014), and C34T CT+TT genotype ( HR 2.588, 95%CI 1.121-5.967; P=0.026) were independent risk factors for the composite outcome. Conclusion C34T polymorphism of P2Y12 gene in patients with acute ischemic stroke may be a risk factor for clopidogrel resistance and is independently associated with the risk of long-term recurrence of vascular events.
