2.A clinical analysis on treatment of patients who suffered from acute exacerbations of chronic obstructive pulmonary disease with typeⅡrespiratory failure

Objective To study the effect of noninvasive pressure venrilation(NIPPV) on the treatment of acute exacerbarions of chronic obstructive pulmonary disease(COPD) with typeⅡrespiratory failure. Methods 46 cases of COPD were selected to treat with NIPPV in our hospital from 2001 January to 2005 May. The data in arterial blood gas(pH,PaO2,PaCO2),the course of disease in the hospital,the rate of endotracheal intubation, the mortality of the patients in NIPPV were analyzed and compared with those of the patients without NIPPV treatment before. Results The datas in arterial blood gas had been imprived significantly in NIPPV after 24h.The couse of disease in hospital was significantly shkrter,and the rate of endotracheal intubation and mortality of the patients was significantly lower in NIPPV than those in no NIPPV. There was remarkably difference between two groups. Objective: Epi-LASIK appeared to be effective and safe in the treatment of high myopia,but some patients complain about visual symptoms postoperation.The purpose of this study was to evaluate the high order aberrations(HOAs) of the vision-decreased eyes treated by Epi-LASIK,and to analyze the causes of postoperative glare,scieropia and monocular diplopia.Methods: We evaluated 76 eyes of 49 patients treated by Epi-LASIK with a questionnaire three months after surgery,and from them selected 34 eyes of 27 patients who scored less than 80 points and complained of postoperative visual symptoms,including glare(25 eyes in 20 patients),scieropia(7 eyes in 5 patients)and monocular diplopia(2 eyes in 2 patients).Another 42 symptom-free eyes of 22 patients treated by Epi-LASIK were included in the control group.The root mean square(RMS) of total HOAs,coma and SA were determined with the iTrace Visual Function Analyzer from the central 6.0 mm zone,and the modulation transfer function(MTF) was simulated with the iTrace 3.1 software.Results: Significant differences were found in the RMS of total HOAs,coma and SA between the eyes with visual symptoms and those without,and the values of ablation zone decentration were significantly higher in the former than in the latter(P
4.Stress analysis of magnetic attachment in mandibular complete overdenture supported by nature roots and implants

Objective To analyze the stress state of magnetic attachment in mandibular complete overdenture supported by nature roots and implant, and to provide neference for designing of clinical prosthodentics. Methods Three-dimensional finite models (model,Ⅱ,Ⅲ)of three groups of mandibular overdentures depending on the different location of the implant and natural tooth root, placing three pairs of magnetic attachment, were constructed by application of CT scanning,computer photo processing system,and Solidworks finite element model building software.The natural teeth and implant neck bone stress of the models in three groups under different stress were calculated and compared.Results The stress of the bone around the implant in the area of molar teeth in model with both sides of the implants under oblique load (Model Ⅱ)was significantly increased compared with the model with one side of the implant (Model Ⅲ). The supporting bone stress of oblique load was increased compared with the vertical load;among them under the oblique load at one side’ s molar teeth, the stress of the bone around the implant in the area of molar teeth in modelⅠ,modelⅡ and model Ⅲ was increased about 30%, 43%,and 55%.Conclusion When there only one nature teeth remain,two implants should be at least added, one in the area of opposite cuspid, and the other in the area of molar teeth of the same side. The magnetic attachment is better than other kinds of attachment in preventing the damage of abutment when it suffers inclined load.
5.Microsatellite instability of D310 and D16184 located in mitochondrial D-loop region in acute leukemia

Objective To study the microsatellite instability (MSD) of D310 and D16184 located in mitochondrial D-loop region in acute leukemia (AL). Methods The HV-1 and HV-2 regions in D-loop region of 100 persons with the untreated and treated acute leukemia was amplificated and screened by PCR-SSCP,then the abnormal samples was amplificated and sequenced directly and compared with revised Cambridge reference sequence (rCRS) and mtDB. The mutation rates of D310 and D16184 was measured by SPSS11.5 statistics software, x2-test. Results The total mutation rate of D310 was found in 49.0 % (49/100) of our patients. Its mutation rates in untreated group and treated group were 32.5 % (13/40) and 60.0 % (36/60)respectively. The mutation rate of treated group is higher than that in untreated group (P < 0.05). The total mutation rate of D16184 was found in 32.0 % (32/100) of our patients. Its mutation rates in untreated group and treated group were 20.0 % (8/40) and 40.0 % (24/60) respectively. The mutation rate of treated group is higher than that in untreated group (P < 0.05). Conclusion There was a high mutation rate with various types of mutations of microsatellite D310 and D16184 located in mitochondrial D-loop region in AL, which led to a doughty MSI. Chemotherapy could cause a more doughty MSI.
6.High quality care for non-small cell lung cancer chemotherapy syn-chronous conformal intensity modulated radiotherapy patients quality of life impact analysis

Objective To study the influence of the nursing quality of non-small cell lung cancer chemotherapy pa-tients with synchronous conformal intensity modulated radiotherapy quality of life. Methods From April 2013 to May 2014, 104 cases of patients with chemotherapy in our hospital in the synchronization of conformal intensity modulated radiation therapy were randomly divided into observation group (52 cases) and control group (52 cases) by digital method. The control group was treated by routine nursing methods while the observation group patients were given with the high quality nursing care, and the satisfaction and quality of life outcomes were compared in two groups. Results The rate in the observation group at the satisfaction of the lung original kitchen complete response ratio,total effective ratio and quality of life in the a good proportion int observation group were significantly higher than those in the control group, with statistically significant differences (all P<0.05). Conclusion High quality nursing care for non-small cell lung cancer chemotherapy patients with synchronous conformal intensity modulated radiation therapy,can improve the patient's quality of life, with security,it is worthy of clinical promotions.
7.Hepatic VX2 tumor in rabbits: treated with radio frequency ablation and evaluated with enhanced CT.

OBJECTIVETo explore and evaluate the effect of radio frequency ablation (RFA) in treatment of hepatic VX(2) tumor in rabbits.
METHODSThe hepatoma was treated with RFA in rabbits. The complete necrotic rate of the tumors, pathological changes, CT images and the animals' survival time were observed.
RESULTS(1) ALT in serum increased significantly on the first day and decreased to the control level during 4th-7th day following RFA. (2) On the second week, CT scans showed that complete necrotic foci became larger, and the density of which was asymmetrical. Enhanced CT scans showed no obvious intensification inside; however, ringed intensification appeared along edges of the foci. Biopsy showed that the dark necrotic tissue was surrounded by ringed granulation tissue. Incomplete necrotic tumor foci resembled the complete necrotic foci in no enhanced CT images; however, asymmetrical intensification was observed in enhanced CT scans in the incomplete necrotic foci. Macroscopic observation showed irregular tumor tissue between the necrotic tissue and the peripheral normal tissue. Biopsy showed tumor recurrence in it. (3) Compared with the control, survival time of the animals was longer, and metastases rate in lungs and the fatality rate were lower in the treatment group.
CONCLUSIONSRFA, with little injury, is an effective method in the treatment of hepatoma. Enhanced CT scans are consistent with pathological morphometrics of the tumor foci after RFA. The enhanced CT scan is a valuable check-up, which could be used to observe the therapeutic effect after RFA.

8.Bile duct injury following blunt abdominal trauma.

Bile duct injury caused by abdominal trauma, usually accompanied with injuries of other abdominal organs, is rarely seen. For the reason of its complexity and often delayed diagnosis and treatment, bile duct injury usually leads to severe complications such as abdominal infection. This paper reports a group of 10 cases of bile duct injury treated in our center in the recent decade.
9.Promoter methylation status of SFRP genes and induced apoptosis by demethylation in Jurkat cells.

OBJECTIVETo study the promoter methylation status of SFRP genes and the effect of 5- aza- 2'- deoxycytidine (5- Aza- CdR)induced apoptosis via Wnt/β- catenin pathway by demethylation in Jurkat cells.
METHODSJurkat cells were treated with different concentrations of 5- Aza- CdR. The cell proliferation level of Jurkat cells was detected by MTT assay. Apoptosis was evaluated by flow cytometry. Methylation- spcific PCR (MSP) was used to determine the methylation status of SFRP genes. The expressions of SFRP genes were detected by real time fluorescence quantitative PCR. The mRNA expression levels of survivin, c- myc and cyclin- D1 were analyzed by RT- PCR. Western blot was used to detect the levels of β-catenin protein.
RESULTSCompared with control group, the different concentrations of 5-Aza-CdR could significantly inhibit the proliferation of Jurkat cells in a time-dose dependent manner (P<0.05). After being treated by 5- Aza- CdR for 48 hours, the cell early apoptosis rate in experiment group was significantly higher than that in control group (P<0.05). The promoters of SFRP1, SFRP2, SFRP4, SFRP5 genes were hypermethylation state in the control group, after being treated by 5-Aza-CdR for 72 hours, the brightness of SFRP1, SFRP2, SFRP4, SFRP5 genes' methylation strips weakened in a dose- dependent manner. SFRP mRNA expression increased (P<0.05) when 5- Aza- CdR concentration increased, and the level of β- catenin protein was dampened in a dose- dependent manner (P<0.05). As compared to the control group, the mRNA expressions of associated apoptosis genes survivin, c-myc and cyclin- D1, respectively were obviously down- regulated in a dose- dependent manner (P<0.05).
CONCLUSIONThe effect of demethylation could up- regulate SFRP genes expressions by reversing its hypermethylation and induced apoptosis by down-regulation of β-catenin and associated apoptosis genes.
