1.The Observation of Clinical Effect of Using Ambroxol in the Treatment of Acute Exacerbation of Chronic Obstructive Pulmonary Disease

Objective To compare the efficacy of two doses of ambroxol in the treatment of the elderly patients with acute exacerbation of chronic obstructive pulmonary disease (COPD).Methods A hundred and ten cases of the elderly patients with an acute exacerbation of COPD were divided into low-dose group and high-dose group.Low-dose group (56 cases) were given Ambroxol 30 mg,2 times per day for 10 day and high-dose group (54 cases) were given Ambroxol 60 mg,2 times per day for 10 days.Results The effective rate in high-dose group was significantly higher than that in low-dose group.Conclusion High-dose ambroxol has significant effect in the treatment of acute exacerbation of chronic obstructive pulmonary disease.
2.Advances in Isolation and Synthesis of Xanthone Derivatives

Xanthone and its derivatives occupy a large part of the family of natural polyphenolic compounds with various biological and pharmacological activities.In recent years (from 2006 to 2011),it was reported that 127 xanthones were discovered from plants and fungi using various modem separation methods including silica gel/polyamide column chromatography,HPLC,high-speed counter-current chromatography,high-performance centrifugal partition chromatography,etc.Since total synthesis and structure modification for xanthone and its derivatives have been given attention worldwide,we introduced the synthetic methods of xanthone skeletons as well.Unfortunately,to date,there are still weaknesses in current methods of separation and synthesis,which need to be improved.This review,to a certain extent,provides necessary foundation for the further research and development of medicines containing xanthone and its derivatives.

Objective To screen a gastric carcinoma subcell line with higher metastatic potential and to explore the relationship between the liver metastatic potential and liver microenvironment with the screened subcell line Methods A gastric carcinoma subcell line was screened via procedures of orthotopic transplantation of subcutaneous tumor of its parent cell line SGC 7901 in nude mice and primary tissue culture of its liver metastatic focus Expressions of bFGFR, TGF?R 1 and c Met(hepatocyte growth factor receptor) in both the subcell line and its parent cell line were determined by immunohistochemistry Results A gastric carcinoma cell subcell line(named SGC 7901LM 2) was established from its parent cell line SGC 7901 with higher metastatic potential, and the expression of c Met increased significantly in the screened subcell line Conclusion The screened gastric carcinoma subcell line with higher metastatic potential is helpful for the further studies of liver metastasis of gastric carcinoma The elevated expression of c Met may be helpful to the liver metastatic potential of SGC 7901LM 2
4.Changes in corneal morphology after seawater soaking following explosive injury in rabbits

Obiective: To observe the changes in corneal tissue after seawater soaking following explosive injury in rabbits and the effects of seawater soaking on the healing of cornea after explosive injury. Methods:Make 20 similar explosive injury models of rabbit's eyeball using 10 adult grey rabbits. For each rabbit, the right eye was in experiment and the left one was for comparison. After the injury, the experimental eyes were soaked in seawater for 30 minutes and the comparable eyes were soaked in 0.9% sodium chloride injection for the same time. All eyes were underwent slit lamp examinations, fluorescein dyeing, and ultrasonic cornea thickness metering on the corneas before the injury and 1,2,3,4,5,7,9,10,15 and 20 days after the injury respectively. At the same time, 2 rabbits were killed and the eyes were examined pathologically. The cornea thickness numbers were compared using self- comparison t test and grouping t test. Results: After seawater soaking following explosive injury, slit lamp microscope examination found that the corneas were edema and turbidity, the experimental eyes were stricter than the control eyes. The corneas of the experimental eye were vascularized 10 days after the injury. Fluorescein dyeing showed that more apparent pigmentations were found on the experimental eyes than that on the control ones.Comparative examination under the optical microscope showed that the corneal tissues of the experiment eyes changed distinctly, their inflammations were patency and healing was delayed and the vascularization degree became high after the injury. Cornea thickness of the experimental eyes became apparently thicker than that of the control eyes. The difference was statistically significant. Conclusion:Scars and vascularization of various degrees will appear in corneal stroma after the explosive injuries. Seawater soaking will have bad effects on the healing of the cornea injured in explosions.
5.Relation of plasma secreted frizzled-related protein 5 to obesity and type 2 diabetes mellitus

To investigate the relationships among plasma secreted frizzled-related protein ( sfrp) 5 level and body fat parameters, glucolipid metabolism, insulin resistance index, and inflammation. 89 subjects with normal glucose tolerance(NGT) and 87 patients with type 2 diabetes mellitus (T2DM) were enrolled and each group was divided into no-obese and obese subgroups. Obesity was defined as body mass index ( BMI)≥25 kg/m2 according to the World Health Organization -Western Pacific Region diagnostic criteria ( 2000 ) . Body fat parameters were measured and BMI, waist-hip ratio were evaluated, meanwhile, the levels of blood glucose-lipid parameters and fasting insulin were also determined. Insulin resistance index ( IR) was assessed by homeostasis model assessment ( HOMA) . The concentrations of plasma sfrp5 and interleukin 6 were detected by enzyme-linked immunosorbent assay. Plasma sfrp5 level in T2DM group was significantly lower than that in NGT group [(8. 35±3. 38 vs 11. 35±3. 69)ng/ml, P<0. 01]. The levels of plasma sfrp5 in subjects with obesity were also lower than those in subjects with no-obesity in both NGT and T2DM groups [(9. 46±2. 70 vs 13. 12±3. 62)ng/ml and(6. 70±2. 34 vs 10. 12±3. 45) ng/ml, both P<0. 01]. Plasma concentrations of sfrp5 in T2DM-obese group were significantly lower than that in NGT-obese group(P<0. 01). Correlation analysis showed that plasma sfrp5 levels were negatively correlated with waist-hip ratio, HbA1C, fasting insulin, triglycerides, waist circumference, fasting plasma glucose, interleukin 6, natural logarithm of HOMA-IR [ln(HOMA-IR)], and BMI(P<0. 01 or P<0. 05). Multiple linear regression showed that ln(HOMA-IR), BMI, triglycerides were independent related factors in influencing the levels of plasma sfrp5 (r2=0. 216, 0. 177, 0. 113, all P<0. 05). Plasma sfrp5 levels were decreased in obesity and T2DM subjects and were correlated with body fat disposition, glucose-lipid metabolism, insulin resistance and inflammation. Lack of sfrp5 may contribute to the pathophysiology of obesity and T2DM.
6.Relationship between plasma progranulin and insulin resistance in patients with type 2 diabetes mellitus and obesity

Objective To detect plasma progranulin (PGRN) level in subjects with newly diagnosed type 2 diabetes mellitus and to investigate the relationship of plasma PGRN level with glycolipid metabolism,inflammation,and insulin resistance.Methods Eighty patients with newly diagnosed type 2 diabetes (T2DM) and 88 subjects with normal glucose tolerance (NGT) were recruited in the study.Both of them were divided into normal weight (NW)subgroup and obesity (OB) subgroup.Obesity was defined as body mass index (BMI) ≥ 25 kg/m2 according to the World Health Organization-Western Pacific Region diagnostic criteria(2000).Body fat parameters were measured and BMI,waist-to-hip ratio were determined.Fasting plasma PGRN and interleukin-6 (IL-6) levels were determined by ELISA,fasting plasma glucose (FPG),2 h plasma glucose after glucose loading (2hPG),HbA1C,fasting insulin (FINS),and lipids were also detected.Insulin resistance and pancreas β cell function were assessed by homeostasis model assessment (HOMA-IR,HOMA-β).Results Plasma PGRN level was significantly higher in T2DM group than that in NGT group(P<0.01).Within groups of T2DM and NGT,plasma PGRN level in OB subgroups was higher than that in NW subgroups [(225.22 ± 34.39 vs 195.59 ± 50.47 and 183.79 ± 61.63 vs 148.69 ± 55.27) ng/ml,P<0.05].Bivariate correlation analysis showed that plasma PGRN level was positively correlated with weight,waist circumference,BMI,systolic blood pressure,FPG,2hPG,HbA1C,triglyceride(TG),IL-6,FINS,and HOMA-IR (P<0.05 or P<0.01),and was negatively correlated with HOMA-β (P<0.05).Multiple linear regression analysis showed that BMI,HbA1C,IL-6,and TG were independently related to plasma PGRN level(P<0.05).Conclusions Plasma PGRN level was increased in patients with type 2 diabetes as well as in obesity,and was closely related with glycolipid metabolism,inflammation,and insulin resistance.

Objective To investigate the relationship between watching television time and impaired glucose regulation (IGR) , type 2 diabetes mellitus in Chongqing City .Methods Population-based cross-sectional study was conducted to investigated the local permanent staff(lived in Chongqing more than 5 years) who were 40 years old or elder in Chongqing City .Results The overall prevalence rate of IGR and T 2DM was 6 .3% ,5 .6% respectively .The average weekly watching TV time of the samples was (12 .3 ± 10 .1) h .After adjusting for possible confounding factors ,the prevalence rate of IGR and T2DM in patients watching TV time >14 h per week was significantly higher than those watching TV time ≤ 7 h per week(Adjust .OR = 1 .528 ,95% CI = 1 .034 - 2 .121 ;OR = 1 .482 ,95% CI = 1 .133 - 2 .047 ,respectively ) .Conclusion Siting and watching TV time were positively correlated with the risk of IGR ,T2DM .So ,we should actively encourage and promote healthy lifestyles to reduce siting and watching TV time .
8.Clinical diagnosis value of endoscopic uitrasonography in upper gastroin-testinal tract bulgy lesion with smooth surface

Objective To investigate the clinical diagnosis value of endoscopic ultrasonography (EUS) in upper gas-trointestinal tract bulgy lesion with smooth surface. Methods All 98 cases of patients with upper gastrointestinal tract bulgy lesion with smooth surface found by routine gastroscopy examination. They were examined by EUS and compared with pathologic examination. Results 65 cases were polypuse,10 cases were stromal tumor,5 cases were leiomyoma,7 cases were lipoma,4 cases were heterotopic pancrease,2 cases were cyst, 3 cases were venous aneurysm,2 cases were external pressure change. The diagnostic accordance rate of EUS and pathology examination were 93.68%. Con-clusion EUS can accurately determine the origin of upper gastrointestinal tract bulgy lesion with smooth surface. It is very important for treatment and evaluating prognosis.
9.Effect of age on the pharmacokinetics of polymorphic nimodipine in rats after oral administration.

The previous investigation has proved that their existed pharmacokinetic difference between the different crystal forms of the polymorphic drugs after oral administration. However, no systemic investigations have been made on the change of this pharmacokinetic difference, resulted either from the physiological or from the pathological factors. In this paper, we used polymorphic nimodipine (Nim) as a model drug and investigated the effect of age difference (2- and 9-month old) on the pharmacokinetics after oral delivery in rats. As the results shown, for L-form of Nim (L-Nim), the AUCin 2-month-old rats was 343.68±47.15 ng·h/mL, which is 23.36% higher than that in 9-month-old rats. For H-form of Nim (H-Nim), the AUCin 2-month-old rats was 140.91±19.47 ng·h/mL, which is 54.64% higher than that in 9-month-old rats. The AUCratio between H-Nim and L-Nim was 2.44 in 2-month-old rats and 3.06 in 9-month-old rats. Since age difference could result in unparallelled change of the absorption and bioavailability of the polymorphic drugs, the results in this experiment are of value for further investigation of crystal form selection in clinical trials and rational clinical application of the polymorphic drugs.
10.A cross-sectional survey of coagulation factor VIII inhibitor in children with hemophilia A.

OBJECTIVETo study the current situation of coagulation factor VIII (FVIII) inhibitor development in children with hemophilia A (HA) through a cross-sectional survey, and to explore the risk factors of inhibitor development in order to provide evidence for further prevention and management strategies.
METHODThe clinical data of outpatients with hemophilia A in Beijing Children's Hospital seen from November 2012 to May 2013 were collected, FVIII inhibitor was screened and analyzed its risk factors.
RESULTA total of 102 HA children were enrolled, 5 were mild cases, 32 were moderate, and 65 were severe cases; the median age on enrollment was 55.5 (3.0-200.0) months:19(18.6%) of patients had inhibitors and 9 (8.8%) had low-titer inhibitors, 10 (9.8%) had high-titer inhibitors. Receiving FVIII treatment for life-threatening bleeding (P = 0.03) ,OR 4.10 (95%CI:1.17-14.32) was a risk factor for inhibitor generation and patients within 20 exposure days have more chances of inhibitor development (P = 0.04) ,OR 3.32 (95%CI:1.02-10.86) . High and intense FVIII exposure within short term was the risk factor for high titer inhibitor development (P = 0.01) ,OR 5.25 (95%CI:1.45-21.92) .
CONCLUSIONIntense FVIII exposure for severe hemorrhage was the risk factor of inhibitors development especially of high titer inhibitors.
