1.The clinical and pathological study of hepatocellular carcinoma invading on to bile duct:a report of 39 cases

Objective To evaluate the clinical and pathological features and treatment of primary hepatocellular carcinoma invading on to the bile duct.[WT5”HZ]Methods [WT5”BZ]39 cases of hepatocellular carcinoma of icteric type were collected in our hospital.[WT5”HZ]Results [WT5”BZ]The incidence was 6% in all hepatocellular carcinoma patients.Preoperative correct diagnosis was achieved in only 33% of the 39 cases.The positive diagnostic rate of B ultrasound and CT was 88% and 80%respectively,and 49% of cases suffered from intraportal thrombi concomitantly.[WT5”HZ]Conclusion [WT5”BZ]Preoperative correct diagnosis depends on image test,hepatectomy with biliary duct drainage is the mainstay for treatment.
2.The operation on hepatocellular carcinoma with obstructive jaundice as primary symptom

Objective To study the clinical features and treatment on hepatocellular carcinoma(HCC) with obstructive jaundice as primary symptom. Method We collected 39 cases of patients of HCC with obstructive jaundice as primary symptom in our hospital,the clinical features and treatments were studyed.Results The incidence of such patients was 6.3% in all HCC patients and all patients were operaed;the positive diagnostic rate of B ultrasonud and T was 88.2% and 80% respectively ,it was 48.7% in cases with intraportal thrombi.Conclutions Preperative correct diagnosis depends on image test,hepatectomy with biliary duct drainage is the mainstay for treatment .
3.Evaluation of ARB in Anithypertersive Therapy

Objectives To evaluate the effectiveness of ARB in antihypertensive therapy and judge whether it can also protect cardiovascular. Methods A randomized double-blinded placebo-controlled study was used to survey over ten thousand patients in ARB clinical practices. The primary endpoint was stroke, acute myocardial infarction, heart failure, deadly cardiovascular diseases, total mortal. The results were given comprehensive analysis and statistical processing. Results In LIFE study, ARB was more effective than ?-blockers and ARB had the role of protecting the cardiovascular besides lowering the blood pressure. But the VALUE study showed that ARB was not more effective than Besylate. Conclusion There is no obvious distinction between ARB and the other drugs in reducing the blood pressure.
4.The efficacy of small fly spot scan exciter laser in situ keratomileusis for treatment of myopia accompanied with moderate astigmatism

Objectives:To evaluate the effectiveness of small fly spot scan excimer laser in situ keratomileusis for treatment of myopia accompanied with moderate astigmatism. Methods:Ninety six cases and 145 eyes were divided into three groups according to the preoperative refractive diopters. GroupⅠ: myopia 0~-6 D, astigmatism -2~-4 DC, groupⅡ:myopia -6.25~-10.0 D, astigmatism -2~-4 DC, and group Ⅲ:myopia≥10.25 D, astigmatism -2~-4 DC. The postoperative follow up was more than six months. Results:One month after the operation, uncorrected visual acuity equal to or better than 0.6 were 5.3% in groupⅠ, 15.8% in groupⅡ, and 66.7% in group Ⅲ. Six months after the operation, uncorrected visual acuity equal to or better than 0.6 were 4% in group Ⅰ,14% in groupⅡ,and 75% in group Ⅲ.Uncorrected visual acuity equal to or better than 1.0 were 96% in group Ⅰ, 86% in groupⅡ, and 25% in group Ⅲ.Postoperative refractive error within ?1.00 D were 8% in groupⅠ,14% in groupⅡ, and 41.7% in group Ⅲ. Postoperative refractive error within ?0.50 D were 92% in groupⅠ, 86% in groupⅡ, and 58.3% in group Ⅲ. Postoperative refractive clinder within ?1.00 DC were 8% in groupⅠ,10.5%in groupⅡ, and 50% in group Ⅲ. Postoperative refractive clinder within ?0.50 DC were 92% in group Ⅰ,89.5% in groupⅡ, and 50% in group Ⅲ. The average exciter zone diameter was (8.02?0.069)mm. Conclusions:Small fly spot scan exciter laser in situ keratomileusis is an effective and safe method to treat mild and moderate myopia accompanied with moderate astigmatism.
5.Rapid induction and expansion of HBV-specific CD8 T lymphocytes from a patient with acute hepatitis B

Objective To develop a rapid expansion assay and determine the predominant epitope for HBV-specific CD8 T cells from peripheral blood mononuclear cells(PBMC) of patients with acute hepatitis B(AHB).Methods PBMC were isolated from a patient with HLA-A2 positive AHB,and the frequency of HBV-specific CD8 T-cells was determined using 3 kinds of HLA-A2 antigenic peptide florescence-labeled pentamer agents,i.e.S183-191 and S335-343 derived from HLA-A2 restricted HBsAg epitope,and C18-27 derived from HBcAg epitope.Cytokines and corresponding epitopic peptides were used for induction of positive CD8 T-cell expansion and enrichment in PBMC for 1-2 weeks.T epitope-specific CD8 T-cells were quantified and sorted by flow cytometry.Results After cultivation with antigenic peptide stimulation and proper cultural condition,the frequency increased significantly in the pentamer positive HBV-specific CD8 T cells in total CD8 T cells.The peak value was observed in 2-week cultivation.At the time the HBV-specific CD8 T cell frequencies increased from 0.21% to 15.17% for S183-191,and from 0.44% to 1.79% for S335-343,representing 73.8-folds and 3.1-folds increase for each epitope respectively.However,the frequency of C18-27-specific CD8 T-cells was negative and without obvious proliferation after stimulation.Conclusions Under stimulation of HBV epitopic peptide in proper culture condition,the specific CD8 T lymphocytes may be induced and expanded in quantity via sort-term conditioned culture.CD8 T cells specific for S183-191 may play an important role in the control of acute HBV infection.
6.The anterior chamber angle and intraocular pressure after clear corneal incision phacoemulsification

0.05).The iris diaphragm was flat to mild concave which lead the width of anterior chamber angle increased postoperation, no peripheral synechia was found. Conclusion: Phacoemulsification and foldable posterior chamber intraocular lens implantation through clear corneal incision had no significant influence on the long term intraocular pressure in simple senile cataract patients, the width of anterior chamber angle increased postoperation.

BACKGROUND:Study on mouse models of hyperinsulinemia confirms that hyperinsulinemia can induce the occurrence of myocardial hypertrophy. OBJECTIVE:To study the effect of insulin on proliferation and hypertrophy of cardiomyocytes in neonatal mice in vitro cultured. DESIGN,TIME AND SETTING:The in vitro cytology experiment was performed at the Key Laboratory of Anatomy,Histology and Embryology of Xinxiang Medical University from January 2006 to March 2007. MATERIALS:Twenty Wistar neonatal mice aged 1-3 days were used in this study. Insulin was obtained from Sigma,USA. METHODS:Cardiomyocytes of neonatal mice were in vitro harvested,culture and purified by trypsin digestion and differential attachment technique. Cardiomyocytes were incubated in 10-8,10-7,10-6 mol/L insulin for 48 hours. Blank control group was set up. 10-7 mol/L insulin group was used for cell cycle analysis. MAIN OUTCOME MEASURES:Cell diameter,activity and counting were measured under a microscope. The expression of cyclin E in cardiomyocytes was measured using immunocytochemistry. RESULTS:Cardiomyocyte activity in each group was over 90%. Compared with the blank control group,long diameter and short diameter of cardiomyocytes in the 10-8,10-7,10-6 mol/L insulin groups were significantly increased(t=2.781 5,P 0.05) ,but the number of cardiomyocytes in the 10-6 mol/L insulin group significantly decreased(t=1.976 2,P
8.Stress analysis of magnetic attachment in mandibular complete overdenture supported by nature roots and implants

Objective To analyze the stress state of magnetic attachment in mandibular complete overdenture supported by nature roots and implant, and to provide neference for designing of clinical prosthodentics. Methods Three-dimensional finite models (model,Ⅱ,Ⅲ)of three groups of mandibular overdentures depending on the different location of the implant and natural tooth root, placing three pairs of magnetic attachment, were constructed by application of CT scanning,computer photo processing system,and Solidworks finite element model building software.The natural teeth and implant neck bone stress of the models in three groups under different stress were calculated and compared.Results The stress of the bone around the implant in the area of molar teeth in model with both sides of the implants under oblique load (Model Ⅱ)was significantly increased compared with the model with one side of the implant (Model Ⅲ). The supporting bone stress of oblique load was increased compared with the vertical load;among them under the oblique load at one side’ s molar teeth, the stress of the bone around the implant in the area of molar teeth in modelⅠ,modelⅡ and model Ⅲ was increased about 30%, 43%,and 55%.Conclusion When there only one nature teeth remain,two implants should be at least added, one in the area of opposite cuspid, and the other in the area of molar teeth of the same side. The magnetic attachment is better than other kinds of attachment in preventing the damage of abutment when it suffers inclined load.
9.Treatment of retinal detachment due to macular hole with vitreoretinal surgery in high myopia

Objective:To investigate the efficacy of Vitreoretinal surgery for retinal detachment due to macular hole in high myopia. Methods:20 cases( 20 eyes) of retinal detachment due to macular hole in high myopia from Dec. 2000 to Apr. 2004 were analysed retrospectively. 3 cases were male and 17 were female, with a following up of 2 to 30 months (15 months in average). The Pars Plana Vitrectomy with long acting gas or silicon oil tamponade were used. The patients were kept face down at least 7 days after the surgery. Results:5 eyes used silicon oil and the others used gas tamponade. The successful rate of retinal reattachment was 80% ( 16/20) with one procedure in a month after the surgery . There were 4 cases of retinal redetachment in the follow-up duration, among them 3 cases used gas and one used silicon oil tamponade. All of them refused to accept another surgery. There were 3 eyes ( 15%) of the best corrected visual acuity greater than or equal to 0.05 before surgery and 11 eyes ( 55%) equal to or greater than 0.05 after the surgery. There was a significant difference between them(P= 0.0386 ). The main complications were slight retinal bleeding in 3 eyes ( 15%); second glaucoma in 2 eyes (10 %); nuclear cataract in 4 eyes ( 20%). Conclusion:Vitreoretinal surgery for retinal detachment with macular hole is available to improve visual acuity in high myopia.
