1.The clinical application research of enzymatic cycling assay for detecting serum Hcy concentration

Objective To discuss the value of clinical application with Hcy enzymatic cycling assay.Methods To compare the results between enzymatic cycling assay and FPIA.The precision test and correlation test of enzymatic cycling assay were evaluated at Hitachi 7600-DDP automatic Biochemistry analyzer.Serum Hcy concentration of patients with diabetes, hypertension, coronary heart disease, myocardial encephalon infarction or chronic renal function failure had been detected in parallel with enzymatic cycling assay and FPIA and the results had been compared to the healthy.Results The within-run imprecision (CVs) of patient serum is 3.07%(low value ), 3.99%(mid value) and 4.21%(high value ). The between-run imprecision (CVs) is 4.00%(low value), 4.79%(mid value) and 4.58%(high value). The correlation of enzymatic cycling assay and FPIA was excellent(r=0.994,n=53).The averages of Hcy concentration in healthy, diabetes, hypertension, coronary heart disease, myocardial or encephalon infarction or chronic renal function failure respective were (8.15?1.89) ?mol/L, (8.88?1.36) ?mol/L, (10.66?1.60) ?mol/L, (14.02?3.42) ?mol/L, (14.45?3.83) ?mol/L and (15.21?3.50)?mol/L. There is no significant statistic difference between enzymatic cycling assay and FPIA.Conclusion The variation profile of Hcy concentration in different diseases detected by enzymatic cycling assay were similar to FPIA.The results indicated that en{ymatic cycling assay could be used to detect serum concentration of Hcy for clinical laboratory rapidly.

Objective To analyze the clinical significance and correlation factors influencing the serum levels of CA125 and CA199 in patients with chronic nephropathy.Methods Clinical data of 649 hospitalized patients with chronic nephropathy from Jan.to Dec.in 2007 were retrospectively analyzed.The criteria of enrolment were: patients with chronic nephropathy aged below 70 years,tumor excluded by CT and ultrasound,and virus hepatitis and tuberculosis excluded by microbiological examinations.The serum levels of CA125,CA199,albumin,total cholesterol(TC),triglyceride(TG),24h urine protein and urine creatinine(Cr) were determined.The patients were divided into CA125/CA199 positive group and negative group according to the cutoff value of serum CA125 and CA199,and the clinical differences were analyzed by t or ?2 test.The correlation factors influencing the serum levels of CA125 and CA199 were analyzed by multiplicity.Results The ratio of female,nephrotic syndrome(NS),membranous nephropathy(MN) and hydrops of serous cavity were significantly higher in CA125/CA199 negative group than that in positive group(P
4.REMOVAL OF RESIDUAL STONES IN HEPATIC DUCTS THROUGH THE AFFERENT LOOP OF ANASTOMOSIS BETWEEN BILIARY DUCT AND JEJUNUM

A postoperative follow-up study, through the subcutaneouslyplaced afferent loop of a choledocho-jejunostomy by retrograde cholangiography and/or choledochoscopy in 116 patients with intrahepatic lithiasis, was made. The retained stones, worms, ductal strictures, or suppurative cholangitis were found in 41 cases over a period of one month to six years postoperatively. All these conditions required further management and some even emergency intervention. The patients were all treated successfully by a Dormia basket or other instruments through this conduit with exception of four failures, the overall success rate was 90%. It is suggested that this type of Roux-en-y choledocho-jejunostomy is recommended in the cases with intrahepatic lithiasis which can provide a permanent access from outside to thebiliary tree and appropriate instruments can be passed through this route for diagnostic and therapeutic interventions such as postoperative cholangiography, retained stones removal, and biliary drainage. We believe this route has some advantages over the T-tube tract for non- operative management of intrahepatic residual lesions.
5.Stress dynamic CT myocardial perfusion imaging combined with coronary CT angiography for the detection of ischemic coronary artery disease

Objective To assess effects of stress dynamic CT myocardial perfusion imaging (CT-MPI) combined with coronary CT angiography (CCTA) on the diagnosis of myocardial perfusion defects in coronary artery disease (CAD). Methods Patients with CAD diagnosed by CCTA underwent ATP stress CT-MPI examination. Single-photon emission computed tomography (SPECT) myocardial perfusion imaging (SPECT-MPI) was performed within one week and set as the reference standard. CT-MPI results were qualitatively analyzed, and myocardial blood flow (MBF), myocardial blood volume (MBV) as well as time to peak (TTP) were quantified according to CT-MPI. Effects of CCTA, CT-MPI, and CT-MPI combined with CCTA on predicting myocardial perfusion defects were assessed in comparison with NMPI. Results Thirty patients [(54.8±8.4)years] were enrolled in our study, 20 were men (68%). MBF [(79.3±18.0) versus (135.1± 35.2) ml·100 ml-1·min-1] and MBV [(8.9±2.9) versus (13.8±8.9) ml/100 ml] were significantly decreased in hypoperfused segments compared with normal segments, while TTP was increased in hypoperfused segments [(13.9 ± 2.5)s] compared with normal segments [(9.1 ± 2.1)s] (t=0.302, 0.866 and 0.024 respectively, all P values<0.01). The sensitivity, specificity of CT-MPI for identifying segments with perfusion defects were 91.3%(147/161), 84.6%(281/332), respectively. On a per-vessel basis, the area under the receiver operating characteristic curve for predicting myocardial perfusion defects were 0.635(95%CI:0.517—0.753) for CCTA, 0.709(95%CI:0.599—0.819)for CT-MPI, and 0.837(95%CI:0.749—0.925)for CT-MPI combined with CCTA, respectively. Conclusions The performance of stress dynamic CT-MPI in the diagnosis of myocardial perfusion defects in CAD was good. One-stop examination of CT-MPI combined with CCTA improves the diagnostic accuracy for identifying flow-obstructing stenosis.
6.Current status of chronic respiratory disease management capacity in community health centers in Beijing Miyun district

Objective:To investigate the current status of chronic respiratory disease (chronic obstructive pulmary disease, bronchial asthma and obstructive sleep apnea) management capacity in community health centers in Beijing Miyun district.Methods:From November 21 to 22,2018,nineteen community health centers and 65 general practitioners in Miyun district of Beijing were selected to participate in a questionnaire survey. The self-designed questionnaire was divided into two parts: the questionnaire for medical institutions(institution questionnaire)and questionnaire for general practitioners(doctor questionnaire). The institution questionnaires were distributed by the Miyun District Health Commission,and filled in by the person in charge of the institution; the knowledge questionnaires were sent to all general practitioners of 4 community health service centers. The two independent sample t test was used to compare the measurement data in accordance with normal distribution between the two groups, and analysis of variance was used for multi group comparison. Results:Nineteen institution questionnaires were sent to all centers in the district and all 19 valid questionnaires were recovered. Among them, 18 centers thought that chronic respiratory diseases should be included in the management of chronic non-communicable diseases, and health records should be established to achieve regular follow-up monitoring, but only one center had put asthma in the record. Nine centers purchased pulmonary function instrument; 8 centers were equipped with inhaled glucocorticoid, and 1 center was equipped with β 2-receptor agonists. The effective recovery rate of knowledge questionnaire was 100.0% (65/65). There was no significant difference in the knowledge scores of three kinds of chronic respiratory diseases (chronic obstructive pulmary disease, bronchial asthma, obstructive sleep apnea) among general practitioners [(63±19), (64±23), (62±21), F=0.087, P>0.05]. The scores of general practitioners with different ages and professional titles were (57±15), (66±13), (42±16) and (54±19), (67±12), (68±11) respectively. There were significant differences in the knowledge scores of general practitioners with different ages and professional titles ( F= 8.582 and 6.079, all P<0.05). The average scores of general practitioners with age>50 years or junior professional title were lower than others. Conclusions:The leaders of each center in Miyun district have a clear understanding of the necessity of diagnosis and treatment of chronic respiratory diseases, but there are still some problems, such as insufficient attention to chronic respiratory system, insufficient investment in disease management infrastructure, and lack of professional knowledge of chronic respiratory diseases among general practitioners. It is hoped that in the future, chronic respiratory diseases can be introduced into public health service projects, investment in related disease infrastructure will be strengthened, and comprehensive respiratory knowledge and ability training courses suitable for grass-roots general practitioners can be popularized as soon as possible.
7.Synergistic effects of human tumor necrosis factor-alpha gene transfection and interferon-gamma on the growth of tongue carcinoma cells.

OBJECTIVEThe purpose of this study is to investigate the synergistic effects of human tumor necrosis factor-alpha (hTNF-alpha) transfection and interferon-gamma (IFN-gamma) on the growth of tongue carcinoma cells.
METHODSThe cultured Tca8113 tongue carcinoma cells was divided into 2 groups, one group was transferred with hTNF-alpha gene. Each of the 2 groups was then divided into 5 subgroups, and the subgroups were added IFN-gamma until the final IFN-gamma concentrations respectively were 0, 10, 100, and 1000 U/ml. After culturing for 48 hours, the survival rates of the all groups of cells were assayed by MTT enzymatic labeling technique, and the expression of hTNF-alpha in Tca8113 cells was observed with immunocytochemistry.
RESULTSIFN-gamma did not affect the growth of Tca8113 cells without hTNF-alpha, however, the transfection of hTNF-alpha with the above different concentrations of IFN-gamma synergistically inhibited the growth of Tca8113 cells, the concentrations of IFN-gamma were positively correlated with the inhibition effects (r = 0.733, P < 0.01), the transferred Tca8113 cells displayed remarkable overexpression of hTNF-alpha, compared with the non-transferred.
CONCLUSIONIFN-gamma can enhance the inhibition of hTNF-alpha transfection on the tongue carcinoma cells.

8.Suppression effect of human tumor necrosis factor-alpha gene transfection on tongue carcinoma cells.

OBJECTIVEThe aim of this study was to investigate suppression effects of the transfection of human tumor necrosis factor-alpha (hTNF-alpha) gene on tongue carcinoma cells.
METHODSThe shuttle plasmid containing hTNF-alpha gene was extracted and purified, then it was transferred into Tca8113 tongue carcinoma cells with cationic liposome DOSPER. The control group was only given equivalent liposomes, except the plasmid. After culturing for 24, 48, 72 and 96 hours, the expression of hTNF-alpha gene in Tca8113 cells was analysed by ELISA and, the survival rate of transferred cells was assayed by MTT enzymatic labeling technique.
RESULTSThe transferred Tca8113 cells displayed significantly overexpression of hTNF-alpha (P < 0.05). The survival rate of the transferred Tca8113 cells was decreased significantly (P < 0.05).
CONCLUSIONTransfection of hTNF-alpha gene in vitro mediated by cationic liposomes can induce the overexpression of hTNF-alpha and inhibit the growth of tongue carcinoma cells.

9.Analysis of risk factors related to death in type A aortic intramural hematoma with conservative therapy

Objective:To explore risk factors of death in type A intramural hematoma (IMH) patients with conservative therapy and provide important information for clinical risks stratification and decisions-making.Methods:This retrospectively study enrolled 130 patients diagnosed with type A IMH in the first CT examination at Fuwai Hospital of Chinese Academy of Medical Sciences and only received conservative therapy between September 2009 and June 2018. Baseline clinical and CT characteristics were recorded. All patients enrolled were followed up. The endpoint was aortic disease-related death, patients were divided into endpoint group and non-endpoint group according to whether or not an endpoint event occurs. Difference between two groups of normally distributed continuous variables, non-normally distributed continuous variables and categorical variables were tested by independent sample t test, Mann-Whitney U test and chi-square test or Fisher′s exact test, respectively. Independent risk factors related to outcomes were assessed with Cox regression analysis and survival analysis. Results:In baseline CT data, the mean value of the maximum aortic diameter (MAD) was (49.2±6.9) mm, and the median value of the maximum hematoma thickness (MTH) was 11.0 (8.5, 13.2) mm. There were 56 and 30 patients with ulcer-like projection (ULP) and intramural blood pool (IBP), respectively, which including 36 patients with ULP of ascending aorta, 51 patients with pericardial effusion and 50 patients with pleural effusion. During a median follow-up time of 1 050 (242, 1 949) days, 26 patients experienced aortic disease-related death. Compared with non-endpoint group patients, patients who experienced aortic disease-related death showed older age and larger MAD( t=2.363, 3.640, P=0.020,<0.001), higher proportion of aortic atherosclerosis, ULP and pericardial effusion (χ 2=5.275, 6.596, 9.325, P=0.022, 0.010, 0.002).In Cox regression multivariate analysis shows that aortic atherosclerosis [hazard ratio (HR)=3.48, P=0.043], ULP (HR=2.66, P=0.019) and pericardial effusion (HR=2.49, P=0.030) were independent risk factors for aortic disease-related death. Conclusions:Aortic atherosclerosis, ULP and pericardial effusion are independent predictors of subsequent aortic disease-related death for type A IMH patients with conservative therapy, identifying these risk factors is helpful for further risk stratification and decisions-making.
10. Infectious disease rotation for general practice residency training

The ability of prevention, treatment and management of infectious diseases is a basic requirement for general practitioners. This article introduces our experience in the infectious disease rotation for general practice residency training, focusing on the rotation management at different stages of training to explore how to improve teaching quality and the ability of trainers in a short period of the rotation.
