1.Effect of trimetazidine in the short-term treatment for dilated cardiomyopathy patients with heart failure

Objective To analyze the clinical efficacy of trimetazidine in short-term treatment for dilated cardiomyopathy accompanied by heart failure. Methods Sixty seven cases of dilated cardiomyopathy patients with heart failure were recruited and divided into two groups randomly,the control group(n = 33) had conventional therapy and the treatment group (n = 34) had conventional therapy plus oral tid 20 mg trimetazidine administration for 3 months. Results The heart function improved significantly in both groups. The overall effective rate was significantly lower in the control group than the treatment group(69. 7% vs. 88.2%,P < 0. 05). In the treatment group, before treating the left ventricular ejection fraction(LVEF), stroke volume (SV), cardiac output per minute (CO), 6 min walk test(6-MWT), were(28.7 ± 13. 6) %,(31.0 ± 8. 7) ml,(3. 10 ± 0. 49) L/min,(138.0 ± 30. 4) m respectively, and after treazing these indices were(38. 5 ± 9. 7) %,(48. 5 ± 6. 8) ml, (4. 90 ± 0. 98) L/min,(350. 0 ± 20. 4) m respectively, which increased significantly after treatment(Ps <0. 05). However,in the treatment group ,before treatment left ventricular. End-systolic dimension (LVESD), left ventricular end-diastolic dimension (LVEDD) and brain natriuretic peptide (BNP) were (68. 0 ± 8. 0)mm,(70. 0 ± 7. 8) mm,(669. 0 ± 71.4) μg/L respectively, whereas after treatment these indices were(59. 0 ± 6. 7) mm,(68.0 ± 7. 9) μg/L,(340. 0 ± 56. 0) μg/L respectively, which decreased significantly after treatment(Ps < 0. 05). While in the control group, before treatment LVESD,LVEDD and BNP were (67.0 ± 9. 0) mm, (70. 0 ± 8.0) mm,(666. 0 ± 70. 8) μg/L respectively, whereas after treatment these indices were(60. 0 ± 5. 5) mm,(66. 0 ± 5.7) mm,(350. 0 ± 55.3) μg/L respectively, which decreased significantly after treatment (Ps < 0. 05). Moreever, the improvement of these indices in the treatment group were significantly higher than those in the control group (Ps < 0. 05). Conclusion The additional administration of trimetazidine to conventional anti-failure treatment can significantly improve the heart function in patients with dilated cardiomyopathy,which is worth to be generalized clinically.
2.Nasal jejunal tube and nasogastric tube of two different catheter ways in improving the severe craniocerebral injury and the low temperature condition compared the curative effect of early enteral nutrition support

Objective To investigate the application of nasal jejunal tube and nasogastric tube of two different catheter ways in improving the severe craniocerebral injury and the low temperature condition and the role of early enteral nutrition support and compare the difference between its curative effect.Methods A total of 120 patients with severe craniocerebral injury underwent normal and low temperature treatment were selected,and they were divided into nasal jejunal tube enteral nutrition(nasal jejunal tube group,60 cases)and nasogastric tube enteral nutrition(nasogastric tube group,60 cases)by simple random sampling.After implement nasogastric tube and nasal jejunal tube,normal pressure using enteral nutrition elemental diet and/or enteral nutrition multi polymerization agent produced the best nutrient solution implementation of early enteral nutrition support,two groups of subjects under the condition of bowel function exists,within 48 h starting enteral nutrition,compared two groups of various nutrition indicators as well as adverse reactions and complications,and Glasgow Coma Scale(GCS)score,Acute Physiology and Chronic Health Evaluation (APACHE Ⅱ)score,etc.Results The patients with severe craniocerebral injury after early nutrition,the various nutrition indicators had improved in two groups.The rise extent of triceps skinfold thickness,arm muscle circumference,serum albumin,prealbumin,transferrin,hemoglobin and other indicators in nasal jejunal tube group was higher than that in nasogastric tube group on the 14th,28th day after admission,and there was significant difference(P < 0.05).The incidence of diarrhea,bloating,vomiting,and stomach contents reflux retention in nasal jejunal tube group was lower than that in nasogastric tube group [5.0%(3/60)vs.18.3%(11/60),10.0%(6/60)vs.21.7%(13/60),5.0%(3/60)vs.21.7%(13/60),15.0%(9/60)vs.16.7%(10/60),11.7%(7/60)vs.48.3%(29/60),8.3%(5/60)vs.21.7%(13/60)],and there was significant difference(P < 0.05).The incidence of complications in nasal jejunal tube group was lower than that in nasogastric tube group [16.7%(10/60)vs.76.7%(46/60)],and there was significant difference(P <0.05).The changes of GCS score,APACHE score in nasal jejunal tube group was more obvious than that in nasogastric tube group,and there was significant difference(P < 0.05).Conclusions Early nasal jejunal tube enteral nutrition for the severe craniocerebral injury after the low temperature condition in clinical enteral nutrition implementation is feasible and reasonable,compared with a nasogastric tube feeding,can obviously reduce the complications of enteral nutrition and reach goals earlier nutritional therapy,can improve the prognosis.
3.Clinical Observation of Diphenhydramine Combined with Prednisone in the Prevention of Iodine-containing Contrast Agent Allergic Reactions

OBJECTIVE:To observe the success rate and safety of diphenhydramine combined with prednisone in the preven-tion of iodine-containing contrast agent allergic reactions. METHODS:1 day before surgery,42 patients with positive iodine aller-gy test was given 40 mg Prednisone tablet,orally,3 times a day,50 mg diphenhydramine was given by intramuscular injection and 1 ml iodine contrast by intravenous injection(allergy test performed again)1 h before surgery. After all patients used iodine con-trast in 15 min,vascular interventional treatment was conducted if there was no bronchospasm,angioedema,leather ball sample itchy rash,hypotension,itching and other allergic reactions. Prevention success rate were observed,and the incidence of adverse re-actions was recorded. RESULTS:Prevention success rate was 90.48%,the incidence of adverse reactions was 7.14%,and it self-improved after stopping drugs. CONCLUSIONS:Diphenhydramine combined with prednisone has high success rate in the pre-vention of iodine-containing contrast agent allergic reactions,with good safety.

Objective:To investigate the clinic effect of r-MHT and hematoma aspiration on the traumatic intracerebral hematoma.Methods:89 cases with traumatic intracerebral hematoma were given hematoma aspiration,47 of them were given r-MHT and hematoma aspiration,the clinic effect on the 1st,7th,14th day after treatment were evaluated by NIHSS,the hematoma volume before treatment on the 1st,7th,14th day after treatment were counted by Dotian formula.Results:The effective rate of treatment group was 93.6%,which was significantly higher than that of the control group (P<0.05).The NIHSS score of treatment group was significantly higher than that of the control group(P<0.05) on the 1 st day,1st,2nd week after treatment (P<0.05).Conclusion:r-MHT and hematoma aspiration couldn effectively reduce the brain damage,improve the patient's neurological fumction in treating traumatic intracerebral hematoma.
6.Experiment study of the acellular bovine pericardium treated by dye-mediated photooxidation used as engineering heart tissue

ObjectiveTo evaluate the feasibility of constructing tissue engineering cardiac patch with photooxidationfixed acellular bovine pericardium.MethodsFresh bovine pericardia were treated by dye-mediated photooxidation after decellularization.Some of them were seeded with bone marrow stromal cells(MSCs) isolated from male SD rats to construct cardiac patches.Myocardial infarction(MI) model was made in female SD rats by left anterior descending coronary ligation(LAD).One week later, the confirmed MI rats were divided into three groups randomly, group MI (n = 15)without any treatment; group P (n = 18) with photooxidated pericardia implantation ; group P + C (n = 18) with seeded pericardia implantation.A sham group (n = 10) was also performed with opening and closing chest twice only.The heart were explanted at 2 or 4 weeks after implantation, and examined histologically and immunohistochemically.The heart function was evaluated by echocardiography at 4 weeks before excising the rats.ResultsThere were no cells or cell debris remained in bovine pericardium tissue.The fiber structure became condensed after photooxidation.The seeded cells formed a continuous layer on the surface of the tissue.The pericardial degradation level and newly formed microvessel density were larger in group P + C than in group P after 2 [ (13.7 ±5.2)个/mm2 vs (7.1 ±3.1)个/mm2, P<0.05]and4 [(22.6 ±4.9)个/mrn2 vs (14.1 ±5.3)个/mm2, P<0.05]weeks.Four weeks after transplantation, cardiac echocardiography showed left ventricular ejection fraction(LVEF) was lower in group MI (44.8 ± 4.4) % and group P (48.4 ± 5.0) % compared with group P + C (49.3 ± 4.8) %, left ventricular fractional shorterning(LVFS) was lower in group MI (18.0 ± 2.2) % and group P (19.8 ± 2.5) % compared with group P + C (20.4 ±2.5) %, the difference between P + C and MI was significant.ConclusionTransplantation of the tissue engineered bovine pericardial patches with dye-mediated photooxidation can improve heart function in MI rats.This kind of patches demonstrates a promising prospect in the future.

METHODSA lentiviral library was transfected into the recombinant cell line MCF-7. The cells were screened with cytotoxic agents before extraction of the genome for amplification of the miRNA precursors using PCR. The screened miRNAs were identified with sequence analysis and their expressions were analyzed quantitatively with real-time PCR in breast cancer cells and tissues.
RESULTSSeven miRNAs were screened from the recombinant MCF-7 cells, namely miR-15b, miR-16-2, miR-181b2, miR-217, miR-224, miR-329-1, and miR-487b, all of which failed to be predicted by bioinformatics software. Real-time PCR showed that miR-217, miR-329-1, and miR-487b were over-expressed in MCF-7 cells, and the expression of miR-15b and miR-16-2 was significantly increased in cancer tissues compared with the adjacent tissues (P<0.05).
CONCLUSIONNovel targeted miRNAs that can not be predicted by bioinformatics software were successfully screened from MCF-7 breast cancer cells over-expressing EZH2 3' UTR. These miRNAs are expressed differentially between normal breast cells and breast cancer tissues.
