1.Neoadjuvant therapy for rectal cancer

The neoadjuvant therapy for rectal cancer included neoadjuvant radiotherapy, neoadjuvant chemotherapy, neoadjuvant chemoradiotherapy and neoadjuvant molecular targeted therapy. Recent reports on neoadjuvant therapy clearly showed a downstaging and downgrading effect on the primary rectal cancer. The rates of R0 resection and sphincter preservation were improved and local recurrence was decreased, but the long term survival was not improved. The results of using new chemotherapeutics and molecular targeted drugs in neoadjuvant therapy for rectal cancer were expected.
2.Expression of ER and PR in hyperplasia of mammary glands and breast cancer

Objective To investigate the expression of ER and PR in hyperplasia of mammary glands and breast cancer. Methods The expression of ER and PR in 68 hyperplasia of mammary glands cases and 168 breast cancer cases were quantified by immunohistochemical method. Results The expression of ER in breast cancer(30%) was significantly higher than that in hyperplasia of mammary glands(10 %)(P <0.05) and there was no significant difference in PR between them. The expression of ER in postmenopausal breast cancer was significantly higher than that in postmenopausal hyperplasia of mammary glands (P <0.05) and there was no significant difference in ER between two premenopausal groups. The expression of PR in invasive lobular carcinoma was significantly higher than that in invasive ductal carcinoma and other types (P=0.005).Conclusion The expression of ER and PR may identify the characteristics of patholobiology in breast disease.
3.The Clinical Value of CEA mRNA、CEA Protein of Peripheral Blood in the Prediction of Colorectal Cancer

Objective Colorectal cancer is one of the most common malignancies with high incidence rate. Due to the progress made in diagnostic and therapeutic techniques, even about 30%~50% of patients with no evidence of metastasis on the time of diagnosis have recurrences. The present study is to investigate the carcinoembryonic antigen(CEA) mRNA,CEA protein expression in blood and to evaluate its clinical significance. Methods Peripheral blood samples were collected from 60 patients with colorectal cancer and 40 patients with colorectal polyp and 40 healthy volunteers. CEA mRNA was detected by reverse transcription polymerase chain reaction (RT-PCR) and serum CEA was detected by Enzyme-linked immunosorbent assay. Results The positive rate of CEA mRNA expression in blood samples of 60 patients with colorectal cancer was 68.3%. And one of the healthy volunteers was positive. But three of the 40 patients with colorectal polyp(7.5%) were positive. The positive rate increased with the pathological staging and liver metastasis, but there were no significant relations between the rate and the malignant extent of cell differentiation. Conclusion The expression of CEA mRNA in peripheral blood cells correlates with the pathological staging and liver metastasis of colorectal cancer, and it may be one of potential uses to monitor the micrometastases of tumor. In order to discover the precancerous change,we can detect CEA mRNA expression in blood with polyp of colon.
4.Lightweight polypropylene mesh in tension-free hernia repair

Objective To evaluate the effect of lightweight polypropylene mesh in tension-free repair of inguinal hernia.Methods A total of 60 patients were divided into 2 groups to receive lightweight polypropylene mesh(experimental group)or standard polypropylene mesh(control group)repair procedures.Data collected included:operation time,postoperative highest temperature and the length of hospital stay;leucocyte count and differential count,CRP,AST,ALT,BUN,Cr at the first day preoperatively and postoperatively.Postoperative complications including wound infection,hernia recurrence,testicular swelling or atrophy,scrotal hydrocele,chronic pain and foreign body feeling were compared with each other.Data were analyzed by statistical method.Results No significant differences were found between the two groups in the operation time,postoperative highest temperature,postoperative hospital stay,and cases of elevated postoperative CRP(c-reactive protein)level.No significant differences were found in the leucocyte count,differential count,and AST,ALT,BUN,Cr level perioperatively between the two groups.During the follow up period of 12~16 months,there was no wound infection,hernia recurrence,testicular swelling or atrophy in the two groups;There were 2 cases of scrotal hydrocele and 1 case of postoperative chronic pain in experimental group,there were 3 cases of scrotal hydrocele and 3 cases of postoperative chronic pain in control group;Six patients complained of foreign body feeling in control group,but no one in experiment group,the difference was statistically significant(P<0.05).Conclusions Lightweight polypropylene mesh in tension-free herniorrhaphy is safe and effective.It ameliorates postoperative foreign body feeling thanks to the feature of light weight,high porosity and excellent flexibility.
5.Experimental study on the tropism of umbilical cord blood mesenchymal stem cells for gastric carcinoma xenografts

Objective To investigate the possibility of human umbilical cord blood mesenchymal stem cells' migrating to the gastric carcinoma xenografts. Methods Gastric cancer cells SGC-7901 were injected subcutaneously into one side of the inguinal groove in the hairless mice to establish the animal model.Gastric cancer-bearing mice were divided randomly into two groups and five in each group (n =5). Then mesenchymal stem cells and the fibroblast HFL-Ⅰ labeled with fluorescent dye SP-DiI were injected at the opposite side. Ten days later, the mice were put to death, and the gastric carcinoma xenografts, liver, spleen,lung and the tissue of injected point were obtained. Cryosections from frozen tissues were processed for fluorescent microscopy and the distribution of MSC and fibroblasts in different organs were observed. Adjacent sections were stained with HE. Results Human umbilical cord blood mesenchymal stem cells were mainly distributed in gastric carcinoma xenografts (0.0150±0.0079), but were found only a little in liver (0.0010±0.0005), spleen (0.0015±0.0012), lung (0.0014±0.0008) and injected point (0.0043±0.0039). Fibroblasts were mainly observed in the tissue of injected point, but none in other tissues (P <0.01). Conclusion Human umbilical cord blood mesenchymal stem cells have the tendency of migrating to tumor sites, suggesting that it can be hopefully used in the diagnosis and treatment of gastric cancer.
6.Effect of pancreatic stellate cells on invasion and metastasis of human pancreatic cancer cell line AsPC-1

Objective To investigate the effect of pancreatic stellate cells on invasion and metastasis of human pancreatic cancer cell AsPC-1,and to determine the role of SDF-1 in this process.Methods PSCs were routinely isolated and cultured,and PSCs conditioned media(PSC-CM) was collected and concentrated.Different concentrations of PSC-CM,anti SDF-1 and their combination were used to treat AsPC-1 cells,and MTT assay was applied to detect the proliferation of pancreatic cancer cells.Transwell chamber migration assay was employed to detect the migration of AsPC-1 cells.In vitro invasion assay was used to determine the invasion of AsPC-1 cells.Results A490 values of AsPC-1 cell in control group and 0.25,0.5,1 μg/lμl PSCCM group were 0.437 ±0.041,0.472 ±0.048,0.553 ±0.057,0.690 ±0.051,and PSC-CM promoted cell proliferation in a dose dependant manner.The difference between 0.5,1 μg/μl PSC-CM group and control group,and between 1 μg/l and 0.5 μg/μl PSC-CM group was statistically significant (P<0.05).A490 values of control group,anti SDF-1 group,PSC-CM group and PSC-CM ± anti SDF-1 group were 0.407 ±0.028,0.416 ±0.030,0.629 ±0.048,0.481 ±0.049.The numbers of penetrating cells were 35.3 ±7.1,34.8±5.6,140.9 ± 12.7,56.5±5.9,and the numbers of invasive cells were 27.1 ±2.9,29.1 ±4.2,81.5 ±8.2,46.4 ± 4.4.The difference between anti SDF-1 group and control group was not statistically significant.The proliferation,migration and invasion of pancreatic cancer cells in PSC-CM group was significantly higher than those of control group (P <0.05 or P <0.01).The proliferation,migration and invasion of pancreatic cancer cells in PSC-CM ± anti SDF-1 group was significantly lower than those of PSC-CM group,but they were significantly higher than those of control group (P < 0.01).Conclusions PSCs can promote proliferation,migration and invasion of pancreatic cancer cells AsPC-1,and the mechanism may be partly due to SDF-1/CXCR4 receptor ligand axis.

8.MRI Study of Traumatic SCIWORA Analysis of 67 Cases

The authors retrospectively analysed 67(63 in cervical cord)cases of traumatic SCIWOAR with positive MR abnormalities. It was deduced that the cases of SCIWOAR might be related with (1)those traumatic forces resulting in SCI being not so heavy and severe, (2)the cases orignally with cervical spondylopathy,(3)the limitation of the conventional X-ray examination for SCI,and (4)the cases of children relativly rare in our country and hospital.The ALL hematomas are the pathognomonic signs in the case of the acute cervical SCI within one week.The analysis demonstrated that MR investigation of the traumatic spine could give the clinical activities the correct diagnosis of different degree of SCI and dependable reference.
9.Clinical safety and efficacy of shortened period of clopidogrel treatment after BuMA stent ;implantation in elderly coronary heart disease patients over 75 years old

Objective To evaluate the clinical efficacy and safety of domestic BuMA biodegradable drug eluting coronary stent in elderly coronary heart disease patients over 75 years old with shortened duration of clopidogrel treatment. Methods 100 elderly patients who received coronary angiography and PCI were included, and they were randomly divided into the observation group ( n=50, received oral clopidogrel for 9 months) and the control group (n=50, received oral clopidogrel for 12 months). The occurance of angina pectoris, AMI, bleeding events and the results of control angiography were compared between the two groups after 12 months of follow-up. Results All the 100 patients were followed up in 12 months after discharge. 4 patients ( 8. 0%) in the observation group and 3 patients ( 6. 0%) in the control group had recurrent angina. Control coronary angiography 12 months later showed no restenosis in the stents. There was no significant difference between the two groups in the recurrence of angina pectoris and coronary stent restenonsis. No acute myocardial infarction,cliniacl bleeding events and late stent thrombosis occurred in the two groups. Conclusions The application of the domestic BuMA biodegradable drug eluting stent for the treatment of coronary heart disease patients over 75 years old is safe and effective with shortened duration of clopidogel treatment.
10.Analysis of complications using the Ilizarov technique in the correction of limb deformities

Objective To analyze the causes of the complications in the treatment of limb deformities with the Ilizarov technique and to discuss the management and prevention of these complications.Methods From January 2000 to October 2010,846 patients with limb deformities (16 upper limbs and 830 lower limbs) who had been treated with Ilizarov technique were retrospectively analyzed,including 508 males and 338 females with an average age of 25.7 years (range,1.2-72).Statistical analysis was used to compare the type,severity,treatment methods and results of complications.Results Postoperative follow-up lasted for 6 to 30 months,with an average of 18 months.There were 81 cases of pin tract infections,36 cases of restricted joint movements,6 cases of skin thermal damage,7 cases of nerves and vessels injury,8 cases of bone delayed union,2 cases of osteofascial compartment syndrome,7 cases of dislocation,5 cases of fixed needle breakage,8 cases of secondary joint deformities,5 cases of serious osteoporosis,6 cases of skin eruption,4 cases of femoral fractures near to proximal external fixator,3 cases of subsequent fracture after external fixator dismantled,11 cases of deformity recurrence and 1 case of the others.Combining with anti-infection,functional training,physiotherapy,and the second operation therapy and so on,a permanent disability was finally left in 13 patients.Conclusion If the Ilizarov technique was used improperly,there would be multiple complications during the preoperative preparation,surgical procedures and postoperative management.Effective preventive measures should be taken,such as following the principle of the Ilizarov technique,standardizing operation procedures,strictly postoperative observation,correcting postoperative rehabilitation exercise guidance,regular follow-up radiography and postoperative adaptability dynamic adjustment of the external technique.
