1.Effect of Liver-Soothing and Spleen-Strengthening Herbal Medicine for Metabolic Syndrome:An Observation of 51 Cases

Objective To observe the therapeutic effect of liver-soothing and spleen-strengthening herbal medicine for metabolic syndrome(MS).Methods Ninety-three MS patients were randomized into group A(N=51)and group B(N=42).The two groups received the basic treatment including dietary control and exercises.Group A received liver-soothing and spleen-strengthening herbal medicine(such as Radix Bupleuri,Rhizoma Atractylodis Macrocephalae,Radix Paeoniae Alba,Radix Angelicae Sinensis,Poria,Radix Rehmanniae,Radix Astragali,Rhizoma Alismatis,Semen Coicis,Cortex Moutan,Rhizoma Dioscoreae,Radix Glycyrrhizae,etc.)additionally.The treatment course lasted 3 months.After treatment,the therapeutic effect was evaluated,and the changes of blood pressure,body weight index(BWI),blood glucose(BG)and insulin levels,insulin sensitivity index(ISI),and the blood lipid levels were observed.Results After treatment,23 patients were cured,26 effective,2 ineffective,and the total effective rate was 96.08% in group A;in group B,12 were cured,21 effective,9 ineffective,and the total effective rate was 78.57%.The therapeutic effect in group A was better than that in group B(P0.05).Fasting BG,2-hour BG and BWI were decreased in the two groups(P0.05).Total cholesterol(TC)and triglyceride levels decreased,and high density lipoprotein cholesterol(HDLC)increased in the two groups(P0.05).Conclusion Liver-soothing and spleen-strengthening herbal medicine exerts an effect for metabolic syndrome,not only decreasing blood pressure,blood lipid levels and body weight,but also improving insulin resistance and increasing ISI.

Correspondence analysis was used for determining the important factors from the six factors influencing analysis of cephalosporin in reversed phase-high performance liquid chromatography (RP-HPLC). Experiments were planed by U7(76), then the synthetical correspondence between a series of optimizations and the factors was calculated. Result indicated that that proportion of organic solvent, pH and concentration of buffer were three most important factors. It showed that the correspondence analysis was a better method to determining the important factors in RP-HPLC
3.Analysis of Status and Equity of Health Resources Allocation in Village-level Medical Institution in China

Objective:To analyze the equity of health resource allocation in village-level medical institution in China,thus to provide the basis for formulating relevant health resources allocation policy.Methods:Using Lorenz curve and Gini coefficient,the rationality and equity of allocation of village health resources in China were evalua-ted by the equity of population distribution and the equity of geographical distribution.Results:The Gini coefficient of village health workers and health institutions was 0.19 and 0.20 respectively according to population distribu-tion,and was 0.65 and 0.63 respectively according to geographical distribution.The Gini coefficient's difference of village-level health resources in the eastern,central and western regions was small according to population distri-bution,but the difference was larger according to geographical distribution among different areas.Conclusion:The equity of village-level health resource allocation in China was preferable according to population distribution,but the equity according to geographic distribution was poorer.The formulation of health policy should pay more atten-tion to the equity of geographic distribution and allocate rationally.
4.Study of the gene expression pattern of type Ⅱ diabetes related gene in Kkay mouse

Objective: To investigate the genes differently expressed in the livers of Kkay diabetic and normal mice, providing data to prevent human from diabetes and its chronic complications. Methods: cDNA microarray chips containing 8 192 cDNAs were used to investigate the gene expression pattern of the liver in Kkay mouse. Results: One hundred and fifty four genes were screened out, comprising 68 complete cDNAs and expressed sequence tags, among them 40 genes were up regulated and 114 genes were down regulated. Conclusion: The pathogenesis of type Ⅱ diabetes is complicated, including the disorder of the metabolism of carbohydrate, fat and protein, and many functional abnormalities of a number of vital proteins. 5.T, B and NK lymphocyte subgroups in the pathogenesis of chronic urticaria

Objective To study the relationship of T, B and NK lymphocytes with the pathogenesis of chronic urticaria. Methods Flow cytometry was applied to assess the proportion of T, B and NK lymphocyte subgroups in the peripheral blood of 51 patients with chronic urticaria and 30 sex and age-matched human controls. The CD4:CD8 ratio was calculated. Moreover, the symptoms, disease course and response to antihistamines of these patients were evaluated by one physician. Results The percentage of CD8+ T and NK cells, CD4:CD8 ratio were (27.20±8.22)%, (21.20±10.84)% and 1.48±0.62, respectively, in these patients,(29.9±3.74)%, (17.5±3.56)%, 1.24±0.27, respectively, in the controls; the differences were significant between the two groups (all P<0.05). Decreased levels of CD3+ T cells, CD8+ T cells and B cells were noted in patients resistant to antihistamines compared with those responsive to antihistamines[(61.81±11.70)% vs (75.74±2.36)%, (24.00±7.79)% vs (34.22±9.30)%, (10.78±2.07)% vs (15.25±4.10)%, P<0.05, 0.01, 0.05, respectively)], while the CD4:CD8 ratio and percentage of NK cells were increased in antihistamine-resistant patients compared to those in antihistamine-sensitive patients [1.67±0.76 vs 1.17±0.41, (28.61±12.62)% vs (12.78±6.02)%, both P<0.01 ]. In these patients with chronic urticaria, the percentages of CD3+ T and CD8+ T cells were negatively correlated with the symptom scores (R = -0.31, -0.28, respectively, both P<0.05 ), while the percentage of B cells was positively correlated with the symptom scores and disease course (R = 0.53, 0.55, respectively, both P<0.01 ). Conclusions There is an abnormality in the proportion of T, B and NK lymphocyte subgroups in patients with chronic urticaria,which indicates that humoral immunity may be involved in the pathogenesis of chronic urticaria and the mechanism for responsiveness to antihistamine.
6.Clinical feature analysis of premenopausal and postmenopausal female adults with kidney stones

Objective To analyze clinical characteristics of premenopausal and postmenopausal female patients with kidney stones.Methods The clinical data of 1 194 female adults with kidney stones from December 2004 to December 2014 were analyzed retrospectively.The incidence of kidney stones in different age groups was analyzed firstly.The post-hysterectomy patients or those taking estrogen for breast cancer were excluded, and then 1 166 patients were enrolled.The patients were divided into premenopausal group (464 cases, 39.8%) and postmenopausal group (702 cases, 60.2%), and clinic features of different groups were evaluated.Results The peak age of female patients with kidney stones is between 50 and 59 years old.There were 105 (22.6%) patients in premenopausal group and 204 (29.1%) patients in postmenopausal group complicated with urinary infection.Fifty-eight (12.5%) patients had genitonrinary malformations such as horseshoe kidney, medullary sponge kidney, ectopic kidney, duplex kidney or pyeloplasty for ureteropelvic junction (UPJ) stricture in premenopausal group, as well as 36 (5.1%) in postmenopausal group.Twenty patients (4.3%) had renal insufficiency in premenopausal group, and 66 (9.4%) in postmenopausal group.These differences were statistically significant (P < 0.05).The patients complicated with hydronephrosis (251 vs.346) and ureteral calculi (63 vs.110) of two groups were not significant different (P > 0.05).Multiple factors Logistic analysis indicated that age and urinary infection were statistically significant factors in the occurrence of renal insufficiency (OR =1.037,1.772, P < 0.05).Conclusions The occurrence of genitourinary malformations was higher among premenopausal female patients with kidney stones, while urinary infection and renal insufficiency were more common for postmenopausal patients.Age and urinary infection were the independent factors for the occurrence of renal insufficiency.
7.Clinical Observation of Hyperbaric Oxygen Combined with Flupentixol and Melitracen in the Treatment of Post-stroke Depression

OBJECTIVE:To investigate the effects of hyperbaric oxygen combined with flupentixol and melitracen on depres-sion improvement,extremity motor function and ability of daily living and activity in patients with post-stroke depression (PSD). METHODS:60 PSD patients were divided into control group and observation group according to random number table,with 30 cases in each group. Both groups received routine clinical treatment,comprehensive rehabilitation therapy and psychotherapy. The control group was additionally given Flupentixol and melitracen tablets,orally,one tablet each time,in the morning;3 days later, one tablet each time,in the morning and noon,for 4 weeks. Other anti-depressive agents were not given during treatment. Observa-tion group was additionally given hyperbaric oxygen,0.12 MPa,for 90 min,qd,5 times a week,for 4 weeks,on the basis of control group. Depression degree [Hamilton depression scale (HAMD) and Self-rating depression scale(SDS)],extremity motor function [Fugl-Mayer motor function assessment (FMA)] and ability of daily living and activity [modified Barthel index (MBI)] were scored in 2 groups before and after treatment,and ADR was observed. RESULTS:After 4 weeks of treatment,HAMD and SDS of 2 groups were decreased significantly compared to before treatment,while FMA and MBI were increased significantly;the improvement of observation group was significantly better than that of control group,with statistical significance(P<0.05). No ob-vious ADR was found in 2 groups. CONCLUSIONS:Hyperbaric oxygen combined with flupentixol and melitracen can effectively improve PSD,relieve negative emotion and improve extremity motor function and ability of daily living and activity.
8.Risk factor analysis of systemic inflammatory response syndrome in type 2 diabetics after percutaneous nephrolithotomy

Objective:To evaluate the risk factors of systemic inflammatory response syndrome (SIRS) in the patients with type 2 diabetes diagnosed with kidney stone after percutaneous nephrolithotomy (PCNL).Methods:In this study,461 patients with type 2 diabetes who received PCNL in Peking Uni-versity People’s Hospital from June 2006 to December 2015 were reviewed.There were 281 males and 180 females with an average age of 57 years were included,of whom,137 were diagnosed with SIRS after PCNL.The demographic data,clinical features,and test results were compared between the patients with SIRS and without SIRS,trying to identify the correlation between their clinical characters and the occur-rence of SIRS.Results:The SIRS was significantly correlated with the patients’preoperative white blood cell counting (×109 /L)[7.76 (4.00 -17.96)vs.6.31 (2.00 -17.40),P <0.001 ],preopera-tional blood glucose level (mmol /L)[7.30 (3.08 -19.90)vs.6.40 (3.42 -16.78),P <0.001], operative time (min)[75 (20 -270)vs.60 (20 -200),P <0.001 ],length of stay (d)[12 (2 -46)vs.11 (3 -29),P =0.019],staghorn stones [38.8% (33 /85)vs.27.7% (104 /376),P =0.042],and preoperational urinary tract infection [36.8% (50 /136)vs.26.6% (81 /304),P =0.032].There was no significant correlation between the SIRS and the patients’age,body mass index, preoperative hemoglobin level,preoperative serum creatinine,and transfusion.In multivariate analysis, abnormal preoperative white blood cell counting (OR =3.194,95% CI:1.531 -6.666,P =0.002), operative time longer than 60 min (OR =1.635,95% CI:1.088 -2.456,P =0.018),and preopera-tional blood glucose level higher than normal 7.1 mmol /L were significantly correlated with the presence of SIRS.Conclusion:The high level of preoperational blood glucose,abnormal preoperative white blood cell counting,and long operative time were significantly correlated with the presence of SIRS in patients with type 2 diabetes after PCNL.
9.Determination of coagulation status as well as circulating levels of complement and inflammation markers in patients with chronic urticaria during acute attack and in remission stage

Objective To determine the coagulation status as well as circulating levels of complement and inflammation markers in patients with chronic urticaria (CU) during acute attack and in remission,and to estimate the relationship of coagulant and anticoagulant factors as well as fibrinolytic markers with the development of chronic urticaira.Methods This study included 40 patients with CU (22 during acute attack and 18 in remission) and 40 healthy blood donors from the Guangzhou Blood Center.Venous blood samples were obtained from these subjects,and enzyme-linked immunosorbent assay (ELISA) was performed to measure the plasma levels of prothrombin fragrnent 1 +2 (F1 +2),tissue factor (TF),thrombomodulin (TM),high molecular weight kininogen (HMWK),tissue-type plasminogen activator (t-PA),C5a and serum levels of C3,C4,antistreptolysin O antibodies (ASO),rheumatoid factor (RF) and C-reactive protein (CRP).Erythrocyte sedimentation rate (ESR) was also determined in these patients.Comparisons of these parameters were carried out by using t test,and the correlation of these factors with CU was evaluated by using Spearman correlation coefficient.Results Compared with the healthy controls,the patients with CU showed significantly higher plasma levels of F1+2 and HMWK (both P < 0.01),but lower levels of TF,TM and t-PA (all P < 0.01).The plasma levels of F1 +2,HMWK,t-PA were significantly correlated with the symptom scores in patients with CU (r =0.81,P < 0.01; r =-0.39,P < 0.05; r =0.35,P < 0.05).A significant increase was observed in the plasma concentration of F1 +2 in patients during acute attack compared with those in remission (P < 0.01),whereas no significant differences were noted in the plasma levels of TF,TM,HMWK,t-PA,C5a,serum levels of C3,C4,ASO,RF and CRP or ESR between the two groups of patients (all P > 0.05).Conclusions It seems that coagulation,anti-coagulation and fibrinolysis are all involved in the development of urticaria.There is an obvious difference in the plasma level of prothrombin F1 +2 between patients with CU during acute attack and in remission,suggesting that coagulation factors play a certain role in the initiation and progression of CU.
10.Laparoscopic aortofemoral bypass for diffuse aortoiliac occlusive diseases

Objective To investigate the feasibility and safety of laparoscopic aortobifemoral bypass (LABF) for diffuse aortoiliac occlusive diseases.Methods Clinical data of 18 cases who underwent LABF were retrospectively analyzed.The intraoperative procedures and postoperative outcome were evaluated.Results LABF was performed successfully in 17 of 18 patients.The mean operation time was (280 ±57) minutes,with a mean aortic cross-clamp time of (117 ±32) minutes.Estimated blood loss was(524 ±45) ml.Mean ICU stay was ( 1.0 ± 1.2) days.The mean postoperative hospital stay was ( 10 ±5) days.Mean time to resume diet was (3 ± 3 ) days.Postoperative myocardial infarction developed in 1 case,lung infection in 2 cases.There was no perioperative mortality.Graft thrombosis developed in 1 case and embolectomy surgery was successfully performed.All patients were followed up from 3 to 27 months,with mean follow-up of (18 ± 8) months,grafts were all patent.Conclusions Laparoscopic aortic bypass is feasible and miniinvasive in patients with diffuse aortoiliac occlusive disease.Short-term outcomes are comparable to those with open conventional aortic bypass.
