1.The application value of sinogram affirmed iterative reconstruction (SAFIRE) algorithm in the coronary computed tomography angiography(CCTA) with the tri-low scheme of dual source CT

Objective To investigate the application value of sinogram affirmed iterative reconstruction (SAFIRE) algorithm in the coronary computed tomography angiography(CCTA) with tri-low scheme (low tube voltage,low dose contrast agent,low flow rate injection) of the dual-source CT.Methods Totally 120 patients with body mass index (BMI) of 18.6 to 24.9 kg/m 2,heart rate < 65 b/min were selected continuously and the contrast agent iohexol (350 mgI/ml) was used.The patients were divided equally into two groups:FBP group,using 120 kV tube voltages,contrast media dose of 0.9 ml/kg,and injection rate of 5.0 ml/s to carry out prospective ECG-trigger scanning,and image reconstruction based on filtered back projection (FBP) algorithm;SAFIRE group,using 80 kV tube voltage,contrast media dosage 0.7 ml/kg,injection rate of 3.5 ml/s to carry out prospective ECG-trigger high-pitch scanning,and iterative reconstruction technique.CT values of enhanced images and noise of two groups of arterial lumen and left ventricular bottom muscle were measured,and the signal-to-noise ratio (SNR) and contrast to noise ratio (CNR) were calculated.The method of double blind was used to evaluate coronary artery in 4-point scale (1 =non-diagnostic).The average volume CT dose index (CTDIvol) and the dose length product (DLP) for each patient were recorded and the effective radiation doses were calculated.Results The difference in the CT values,image noise,SNR,CNR between the two groups was not statistically significantly(P > 0.05).The effective dose in SAFIRE group was (0.39 ± 0.02) mSv significantly lower than that in FBP group of (4.99 ± 1.36) mSv (t =26.31,P<0.01).Conclusions For the patients with body mass index (BMI) 18.6 to 24.9 kg/m2,HR < 65 b/min,the application of prospective ECG-trigger high pitch tri-low scheme combined with the iterative reconstruction algorithm in dual-source CT coronary angiography,can obtain the qualified image with lower radiation dose.
2.Clinical distribution and drug resistance analysis of AmpC β-lactamase and ESBLs producing Enterobacter cloacae

Objective To investigate the status of AmpC β-lactamase and extended spectrum β-lactamases (ESBLs) production and drug-resistance characteristic of Enterobacter cloacae.Methods Enterobacter cloacae was isolated and cultured from variety of specimens from April 2005 to July 2007. VITEK32 automatic microbial analytical system was applied to performing identification of bacteria and susceptibility test. ESBLs were confirmed by NCCLS double disc assay, and three-dimensional test was used to detect AmpC.Results A total of 63 strains of Enterobacter cloacae were isolated, including 13(20.6%) single AmpC producing strains, 24(31.8%) single ESBLs producing strains, 6(9.5%) both AmpC and ESBLs producing strains, and 20(38.1%) neither AmpC nor ESBLs producing strains. The resistance of lactamases producing strains was significantly higher than that of lactamases non-producing ones. Especially in both AmpC and ESBLs producing strains, the drug resistance was serous.Conclusion The prevalence of AmpC β-lactamases and/or ESBLs producing strains is a higher level in Fuyang area. The drug resistance of AmpC β-lactamases producing Enterobacter cloacae isn′t identical to that of ESBLs producing one. Different categories of antibacterials should be chosen according to the different types of producing lactamases.
3.Modified antegrade colonic lavage for left obstructing colorectal carcinoma

Objective To analyze the outcome of patients with acute obstruction of left colorectal cancer treated by modified antegrade colonic lavage with primary tumor resection and anastomosis.Methods From April 2002 to April 2007,112 patients with acute obstruction of left colorectal cancer underwent surgery.During the operation the left colon was exteriorized and placed into a sterilized plastic bag to protect the surgical field from contamination,then a catheter was inserted via the appendix,and after antegrade colonic lavage,primary resection and anastomosis was performed.Results Tumor resection and primary anastomosis was successfully done in the 112 cases.Postoperatively,1 case had anastomotic leak which healed after reoperation with proximal colostomy,and one patient died.Conclusions Modified antegrade colonic lavage is a simple procedure,the bowel can be rapidly decompressed with essentially no contamination,and has a high level of bowel cleansing.It is possible to safely perform primary resection and anastomosis for left colon cancer after the modified antegrade lavage.
4.Treatment of unstable angina pectoris using low molecular weight heparin

Objective To observe the treatment effect of low molecular weight heparin (LMWH) in the treatment of unstable angina pectoris.Methods LMWH (0.4~0.6 ml Bid) was used through subcutaneous injection in 36 cases of unstable angina pectoris for 14 days on end.Results The clinical effective rate was 66.67%.The drug was effective in the decreasing of the frequency and time of angina pectoris,and also in the decreasing of nitroglycerin consumption as well as in the improvement of the scope and severity of cardiac ischemia by ECG.Conclusion LMWH is an effective and safe drug for the treatment of unstable angina pectoris.

AIM:To gain the molecular mechanism of active ingredients of EGb 761(ginkgo flavones and ginkgo lides) for the inhibition against A?_(1-42) aggregation and fibrogenesis.METHODS:Circular dichroism spectra(CD) and FI-IR were used to curve-fit and analyze the change in A?_(1-42) second structure under A? aged condition and intervention of ginkgo flavones and ginkgo lides.RESULTS:Analysis that was made of 1700-1600 m(-1) amide-Ⅰ-band and curve fitting indicated that after 30 min to 72-incubation,increase of ?-sheet in A?_(1-42) by(18.5%),but the reduction of ?-sheet by(50.95%) and(36.09%) in 72 h incubation with ginko flavones and ginkgo lides,respectively.And increase of ?-turn by(56.56%) and(46.56%) in sane condition.CONCLUSION:Obviously,?-sheet turned into ?-turn.-CH_2 and-CH groups of ketones and alkanes engaged in A?_(1-42) molecular change.
7.Pay attention to follow-up and monitoring of patients with gastric epithelial dysplasia associated with chronic atrophic gastritis.

Because of the close relationship with stomach cancer and higher rates of carcinoma, gastric epithelial dysplasia associated with chronic atrophic gastritis is considered the most important precancerous lesion of stomach cancer. Due to the absence of recognized treatment methods, periodical follow-up and monitoring were considered an effective method to prevent the occurrence of gastric cancer. Currently, there are numerous areas requiring attention; the authors provide some suggestions on the recognition and key technologies for follow-up and re-examination.
8.ROC analysis and one of its applications to fMRI data.

The purpose of this study is to show the basic principle of receiver operating characteristic (ROC). Some advantages of ROC were given. A simple ROC curve was made to explain the method of plotting. An example of ROC applied to fMRI data was displayed, which showed that ROC can be effectively used in fMRI data.

9.Analysis on 21 cases of chylous fistula after neck lymph nodes dissection

Objective To discuss the prevention and management method for chylous fistula after neck lymph node dissec‐tion .Methods Totally 1 793 cases of neck lymph node dissection in this department from January 2005 to September 2014 were retrospectively analyzed .The clinical data in the cases of chylous fistula occurred after operation were summarized .Results Twenty one cases of chylous fistula occurred ,accounting for 1 .17% ,in which 13 cases were cured by the local compressed bandaging and continuous negative pressure drainage;5 cases adopted the conventional method for 2-3 d ,but under the ineffective condition ,then they were treated by combining with somatostatin pumping (somatostatin 6 mg+0 .9% normal saline 48 mL ,2 mL/h ,lasting for 24 h ,for successive 2-3 d) and finally cured;3 cases were cured after reoperation .Conclusion Prevention is the best treatment for chylous fistulas ,local compression bandage plus continuous negative pressure drainage is the main method for treatment of chylous fistulas after neck dissection .The combined therapy with somatostatin can increase the close rate of chylous fistulas;for the patients with long persistent time ,large drainage volume and invalid conservative therapy should adopt the remedial measure of operation .
10.Expression profiles of genes in wild-type DJ-1 and A39S DJ-1 mutant cells

Objective: To elucidate the role of A39S mutation of DJ-1 in the onset of Parkinson’s disease (PD) and identify genes for which expressions are abnormally regulated by A39S DJ-1 mutation. Methods: We established HEK293 cell lines which stably expressed empty vector, wild-type DJ-1 and A39S mutated DJ-1 respectively. DNA microarrays were used to identify genes for which expressions change in wild-type DJ-1 cells and A39S DJ-1 mutant cells. Results: Compared with the cell line expression empty vector, we identified 42 differentially regulated genes (including 14 up-regulated genes and 28 down-regulated genes) in the wild-type DJ-1 cells and 8 differentially regulated genes (including 6 up-regulated genes and 2 down-regulated genes) in the A39S DJ-1 mutant cells. Compared with the wild-type DJ-1 cells, only the expression of UGT2B7 gene was down-regulated in A39S DJ-1 mutant cells. hTese differentially regulated genes were mainly related to signal transduction, regulation of transcription, apoptosis and metabolism. Conclusion: A39S mutated DJ-1 may disturb the transcriptional activities of DJ-l and involve in the pathogenesis of PD.
