1.Advances in Studies on Endoscopy for Diagnosis of Chronic Atrophic Gastritis

Chronic atrophic gastritis( CAG)is a commonly seen digestive tract disease with the histopathological findings of mucosa inflammation,gland atrophy,intestinal metaplasia and dysplasia. Intestinal metaplasia is considered as a typical sign of CAG. In recent years,with the rapid progress of endoscopic technique,new technology has been applied for the diagnosis of CAG. This article reviewed the advances in studies on ordinary endoscopy,narrow-band imaging technique,magnifying endoscopy,narrow-band imaging-magnifying endoscopy and confocal laser endomicroscopy for the diagnosis of CAG.
2.Analysis of the infectious complications following nonmyeloablative allogeneic peripheral blood stem cell transplantation

Objective To investigate the infectious complications after nonmyeloablative allogeneic peripheral blood stem cell transplantation (NASCT). Methods Ten patients with leukemia received NASCT from HLA-identical or 1-3 antigen mismatched sibling donors with conditioning regimens of Flu/Cy+BU+ALG. Five patients were acute leukemia (AL) and all of them is in complete remission (CR1). Cyclosporine in combination with methotrexate was administered for GVHD prophylaxis. Results WBC recovered to more than 0.5 ?10 9/L during postoperative 10 day to 38 day and platelet recovered to more than 20?10 9/L during postoperative 8 day to 16 day. Bacterial infections occurred in 3 patients (30 %) and cytomegalovirus (CMV) infections in 2 patients (20 %). Varicella zoster virus (VZV) infections occurred in one patient (10 %). No fungal infections were documented. No patients died as a result of infection . Conclusion NASCT is a safe and effective therapeutic method for leukemia. It reduces acute GVHD,regimen-related toxicity and early neutropenia associated with traditional allo-HSCT.
3.Value of Narrow-band Imaging Magnifying Endoscopy in Diagnosis of Chronic Atrophic Gastritis

Background:Currently,chronic atrophic gastritis( CAG)is diagnosed by endoscopy combined with pathological examination. Narrow-band imaging magnifying endoscopy( NBI-ME)has been widely applied to diagnose CAG. However, it is a issue how to optimize the examination technology. Aims:To investigate the clinical value of NBI-ME in diagnosis of CAG,intestinal metaplasia and intraepithelial neoplasia. Methods:One hundred CAG patients reexamined gastroscopy were enrolled,sequential examination was performd,i. e. the extent of disease was observed under convention white light endoscopy( C-WLI ) followed by NBI-ME mode to observe locally,morphological changes of gastric mucosa pit was analyzed,endoscopic findings and histological results was compared. Results:Sensitivity of sequential examination for the diagnosis of CAG was 89. 7%,specificity was 63. 3%;sensitivity and specificity for the diagnosis of intestinal metaplasia were 89. 4%,89. 3%,respectively,for intraepithelial neoplasia were 84. 2%,95. 9%,respectively. Conclusions:Sequential examination can significantly improve the diagnostic accuracies of CAG,intestinal metaplasia and intraepithelial neoplasia,and effectively guide targeted biopsy,and easily to operate.
4.Gene polymorphisms of esxA, esxH and fbpB in clinical Mycobacterium tuberculosis isolates from tuberculosis children

Objective To sequence the antigen-encoding genes esxA,fbpB and Rv2660c from clinical isolates of Mycobacterium tuberculosis (MTB) in tuberculosis children and study the polymorphisms.Methods A total of 135 isolated strains were amplified for gene sequences of esxA,fbpB and Rv2660c by PCR,and the products were sequenced respectively.The results of sequencing were compared with the corresponding gene sequences of standard strain H37Rv and human T and B cell epitope sequences from immune epitope database (IEDB).The genetic diversities of the gene sequences of esxA,fbpB and Rv2660c were analyzed.Results Among the 135 strains,no gene polymorphisms were found in esxA and Rv2660c.But fbpB had genetic diversities in 66 clinical isolates (48.89%).The 5 single nucleotide polymorphism (SNP) mutations in fbpB affected 8 (14.81%) T cell epitopes and 9 (24.32%) B cell epitopes.The dN and dS values of whole fbpB gene were 0.000 120 and 0.002 126,respectively.The dN and dS values of T epitopes areas were 0.000 067 and 0.002 382 respectively,those of non-T epitopes area were 0.000 408 and 0.000 470,respectively,those of B epitopes areas were 0.000 063 and 0.00 2380,and those of non-B epitopes areas were 0.Conclusion The gene sequences of esxA and Rv2660c are conservative,and that of fbpB contains genetic diversities.Its polymorphisms may influence the function of antigen Ag85B,and thus affect the efficacy of vaccine H56.
5.Silencing gene of TLR4 down-regulates the effect of TLR2 signal transduction in RAW264.7 cells to anti-Aspergillus fumigatus conidia stimulation

Objective To study the role of TLR2 and TLR4 signal transduction in RAW264.7 monocyte-macrophages stimulated by Aspergillus fumigatus conidia,and to investigate the expression of TLR2 signal transduction after silencing gene of TLR4.Methods Macrophages were randomly divided into normal group ( N group),normal+stimulated with Aspergillus fumigatus conidia ( N +Af group ),normal + transfected with TLR4-siRNA [ TLR4 (RNAi) group ],normal+transfected with TLR4-siRNA +stimulated with Aspergillus fumigatus conidia[ TLR4(RNAi) +Af group].RT-PCR and Western blot were used to assay expression levels of TLR2,TLR4,MyD88 mRNA and pro-inflammatory cytokines TNF-α protein when macrophages were stimulated 12 h by Aspergillus fumigatus conidia after tranfected 24 h with TLR4-siRNA by technology of RNAi.Results ( 1 ) Compared with N group,the expression of TLR2,TLR4,MyD88 mRNA and TNF-αprotein in N+Af group significantly increased before silencing gene of TLR4.(2) Silencing efficiency of macrophates was up to 83% after transfected with TLR4-siRNA.(3)The expression of TLR2,MyD88 mRNA in TLR4 (RNAi) group significantly decreased contrast with normal group.Meanwhile the expression of TLR2,MyD88 mRNA and TNF-α protein also obviously reduced in TLR4(RNAi) +Af group when compared with N +Af group.Compared with TLR4 (RNAi) group,the expression of MyD88 mRNA in TLR4 (RNAi) +Af group significantly increased.However,the expression of TLR2 mRNA and TNF-α protein have no significant change after silencing gene of TLR4.Conclusion Signaling pathway of TLR2 and TLR4 in macrophages was activated by given stimulus of Aspergillusfumigatus conidia and exerted the effect of anti-Aspergillus fumigatus spores stimulation through the release of pro-inflammatory cytokines TNF-α.Meanwhile,silencing gene of TLR4 down-regulate the effect of TLR2 signal transduction in RAW264.7 cells to anti-Aspergillus fumigatus conidia stimulation,and it found that TLR4 played an more important role by contrast with TLR2.
6.Protective effect of okra seed oil on acute gastric ulcer in mice

To investigate the protective effect of the seed oil of Abelmoschus esculentus on gastric ulcer,two acute gastric ulcer mice models were established by intragastric administration of aspirin or absolute ethanol,respectively.Clinical index of ulcer area,ulcer index,gastric volume,gastric pH value,free acidity,total acidity,and histopathological assessment were measured to evaluate the injuries of gastric ulcer and the protective effect of okra seed oil In order to comprehensively uncover the possible underlying mechanism,a series of biochemical assays were also performed,including serum TNF-α,IL-6,IL-10 and Tbil,NO,MPO and SOD in the stomach included.Moreover,the ALT,AST and ALP in the liver of mice were also tested to evaluate the possible hepatic toxicity of the seed oil.The results indicated that the seed oil of A.esculentus exerted protective effect in ethanol-induced gastric ulcer mice by reducing the ulcer area and ulcer index,declining the free and total acidity,and increasing the pH value of gastric content.Histopathological observation showed the gastric mucosa of the acute gastric ulcer mice induced by alcohol was incomplete and severely damaged,with submucosal edema and nuclear pyknosis,as well as glandular structure disappearing,compared with that of normal mice.What's more,a number of inflammatory cell infiltration occured in the gastric mucosa of alcohol-model mice,with messes of neutrophils,lymphocytes,eosinophils and plasma cells.Okra seed oil could improve the damaged structure of the gastric mucosa and gland caused by ethanol,but could not ameliorate the condensation of nucleus and infiltration of inflammatory cells.Biochemical analysis revealed that the seed oil of A.esculentus could counteract the damage induced by ethanol via decreasing Tbil and TNF-α in serum,decreasing NO and myeloperoxidase,and increasing SOD in stomach.Meanwhile,okra seed oil exhibited protective effect in aspirin-induced gastric ulcer mice by increasing the gastric content pH,and reducing free and total acidity.Compared with the control group,the gastric mucosa of aspirin-model group showed multifocal coagulation necrosis,sheet edema and infiltration of inflammatory cells by histopathological assessment.Compared with the aspirin-model group,the soybean oil group and okra seed oil group could ameliorate the inflammatory cell infiltration.Biochemical analysis revealed that okra seed oil could counteract the injury induced by aspirin via decreasing TNF-α and IL-6,and increasing IL-1O in serum,decreasing NO and MPO and increasing SOD in stomach.In a word,the okra seed oil exerted protective effect on acute gastric ulcer by anti-inflammation,anti-oxidation and hepatocyte protection.The okra seed oil deserves further development and utilization.
7.The action mechanisms of Morus alba leaves extract for the treatment of diabetes based on plasma metabolomics.

In order to evaluate the effect and mechanism of the mulberry leaf alkaloid, flavones, and polysaccharide intervention on diabetes, the overall metabolite profiling characteristics for the plasma of diabetic mouse was performed by using an ultra-performance liquid chromatography/electrospray-tandem mass spectrometry (UPLC-ESI-MS). The 8 potential biomarkers were found in diabetic mice plasma based on the data of MS/MS characteristics obtained from the UPLC-OrbitrapMS analysis, which mainly involved in sphingolipids, amino acid metabolic pathway. The principal component analysis showed that the normal group and model group were obviously distinguished and implied that metabolic disturbance was happened in diabetic mice plasma. The extracts of mulberry leaf flavonoids, polysaccharide, alkaloid had exhibited the effects of callback function for diabetic mice through regulating the amino acid metabolism and sphingolipid metabolism.
8.Clinical value of serum gastric function, Hp detection combined with narrow band imaging magnifying endoscopy in the diagnosis of gastric ulcer

Objective To investigate the clinical value of serum gastric function(pepsinogen I, II and PGR, gastrin-17), Helicobacter pylori (Hp) infection combined with narrow-band imaging magnifying endoscopy for the diagnosis of gastric ulcer. Methods From October 2015 to December 2016, we selected 113 patients diagnosed as gastric ulcer and gastric cancer by narrow-band imaging endoscopy (NBI-ME) and diagnosed by pathological examination. 65 patients were divided into gastric ulcer group. 48 patients were divided into gastric cancer group, 65 healthy subjects who had passed the physical examination were selected as the control group. Enzyme-linked immunosorbent assay was used to measure serum gastric function parameters such as serum pepsinogen I / Ⅱ, PGR, and gastrin-17, and C14 breath test was used to detect Hp. Results The Hp infection rate in the gastric ulcer group and the gastric cancer group was significantly higher than that in the control group, the difference was statistically significant (P<0. 05). Compared with the control group, serum PG I and PGR levels in the gastric ulcer group were significantly increased (P<0. 05); serum PG I and PGR levels in the gastric cancer group were lower than those in the control group(P<0. 05). The PGI and PGR levels in the gastric ulcer group were significantly higher than those in the gastric cancer group, the difference was statistically significant (P<0. 05). Compared with the control group, the serum G-17 concentration in the gastric ulcer group and the gastric cancer group was higher than that in the control group(P<0. 05). Conclusion PG I, PG Ⅱ, G-17 and other serum gastric functional parameters combined with Hp indicators have been further measured by NBI-ME in gastric ulcer and gastric cancer diagnosis and prognosis has a high reference value.
9.Clinical characteristics of 130 bacterial pneumonia children and the changes of the levels of serum WBC, CRP and PCT

Objective To observe the clinical characteristics of bacterial pneumonia children,and the changes of the serum levels of white blood cells(WBC),C-reactive protein(CRP) and procalcitonin(PCT) before and after anti-bacterial therapy,and to explore the predictive value in early diagnosis and therapy.Methods 130 bacterial pneumonia children were enrolled prospectively as pneumonia group.The clinical data were collected and the serum CRP,PCT and WBC were detected before anti-bacterial therapy (within 24h after admission) and after anti-bacterial therapy (the seventh day after admission).34 healthy children were enrolled as control group.The general clinical characteristics of the children in the pneumonia group were observed.The levels of serum CRP,PCT and WBC between the pneumonia group and the control group were compared.The levels of serum CRP,PCT and WBC before and after anti-bacterial therapy in the bacterial pneumonia children were compared.The clinical value of PCT,CRP and WBC in early predicting bacterial pneumonia was identified.Results Compared with the control group,the sex and age of the bacterial pneumonia children demonstrated no statistically significant differences (t =1.012,P =0.395;x2 =0.003,P =0.959).The mean course of the disease before admission was (5.34 ± 1.27) d,with mean temperature of (38.27 ± 0.96) ℃,and hospital days of (8.92 ± 3.93) d.35 cases were cured,and 95 cases were improved,with no death.The serum levels of CRP,PCT and WBC in pneumonia children on admission were (12.24 ±6.35) mg/L,(0.18 ± 0.15) ng/mL and (14.25 ± 7.59) 109/L,respectively,which were higher than those of the control group,the differences were statistically significant (t =4.650,5.867,2.548,all P < 0.05).The serum levels of CRP,PCT and WBC in pneumonia children before anti-bacterial therapy were higher than after bacterial therapy,showed statistically significant differences(t =8.165,7.232,5.112,all P < 0.05).The area under the curve (AUC)of the PCT,CRP and WBC in early predicting bacterial pneumonia were 0.928,0.834 and 0.718 respectively by the relative operating characteristic (ROC) curve analysis (P < 0.05).The sensitivity and specificity of PCT in predicting bacterial pneumonia were higher than CRP and WBC.Conclusion The statistics efficacy of PCT in early predicting children bacterial infection was obviously higher than CRP and PCT.The combined detection of CRP,PCT and WBC was benefit to improve the diagnostic sensitivity and specificity of children with bacterial pneumonia.

OBJECTIVETo study the water-soluble non-alkaloid chemical constituents of Corydalis yanhusuo.
METHODThe 80% alcohol extracts of C. yanhusuo passed through DA201 macroporous resin. Eluted fractions were collected and passed though 732 # cation exchange resin. Water eluate was collected, dried and derived with trimethylsilane. Gas Chromatography/Mass Spectrometry and NIST 2005 library were adopted for MS/MS mass spectrogram to infer the compound structure.
RESULTSixteen compounds were tentatively identified from about fifty peaks detected by GC-MS and identified as hydroxyl and carboxyl polar compounds.
CONCLUSIONThese sixteen compounds were found for the first time in C. yanhusuo. The results provide scientific basis for in-depth development of C. yanhusuo.
