1.The protective role of erythropoietin induced by hypoxic preconditioning on cerebral injury and cognition after global cerebral ischemia

Objective To observe the protective effects of hypoxic preconditioning(HPC) on the improvement of the cognitive dysfunction(learning and memory) and the damage in hippocampus induced by global cerebral ischemia/reperfusion(I/R) in CA1 and CA3 for 5 days in rats,and on the regulation of expression of erythropoietin(EPO) protein to approach the mechanism of the protection.Methods One hundred and twenty adult male Sprague-Dawley(SD) rats were divided into four groups randomly: sham group,I/R group,HPC24 group(hypoxia for 24 hours before I/R) and HPC48 group(hypoxia for 48 hours before I/R).Hang(motor function),passive avoidance and Morris water maze tests were carried out on the 5th day after I/R to measure the motor and cognition functions;hematoxylin-eosin(HE) staining was used to detected histopathological changes in hippocampus tissues;and the contents of EPO were tested by immunohistochemistry at 1 hour and 4 hours after I/R from hippocampus CA1 and CA3 regions.Results Hang,passive avoidance and Morris water maze tests showed that I/R can injure rat cognition;the improvement of cognition was marked in HPC groups, and it was shown that the effects were more significant in HPC48 group than those in the HPC24 group(P
2.Protection of electroacupuncture and compound salviae in coordination on ischemia-reperfusion myocardium

BACKGROUND: Both electroacupuncture and salviae act on prevention and treatment ofis chemia-reperfusion injury, but, whether such action is achieved or not by coordination of both? OBJECTIVE: To probe into the effects of electroacupuncture and salviae on expression of myocardial cellular heat-shock protein 70mRNA that acts on protecting myocardium and on dopamine level that is the mark for myocardial injury after ischemia-repeifusion as well as the interaction between electroacupuncture and salviae. DESIGN: Factorial design of two factors. SETTLNG: Department of Anesthesia of Renji Hospital Affiliated to Shanghai Second Medical Scientific University. PARTICIPANTS: The experiment was performed in Animal Experimental Room of Renji Hospital from September 2001 to December 2002, in which 24 healthy New Zealand white rabbits were employed and randomized into 4 groups, 6 rabbits in each, named ① ischemia-reperfusion group (IR group), ② electroacupuncture group (EA group), ③ salviae group (SA group), ④ electroacupuncture + salviae group (ES group). METHODS: ① IR group: ischemia-reperfusion model was prepared by clipped the anterior descend branch of cardiac coronal artery for 30 minutes and released for 2 hours. ② EA group: modeling was done as the previous method. Twenty minutes before ischemia, electroacupuncture was done on Neiguan (PC 6), Yunmen (LU 2) and Lieque (LU 7), 0.8 V in voltage, 3.0-4.0 Hz in frequency. ③ SA group: modeling was done as the previous method. Before ischemia, salviae was injected intravenously once at 1.5 mg/kg, and was injected before and after reperfusion once respectively at 1.0 mg/kg. ④ ES group: modeling was done as the previous method. Both electroacupuncture and medication were applied in the group. The content of dopamine was determined in blood before ischemia,30 minutes after ischemia and 2 hours after reperfusion successively in rabbits of every group. Reverse transcription-polymerase chain reaction (RT-PCR) was used to determine myocardial heat-shock protein 70mRNA expression in ischemic and non-ischemic areas. RESULTS: After supplemented, 24 rabbits entered result analysis. ① Expression of myocardial heat-shock protein 70mRNA: that after ischemia and reperfusion was increased significantly compared that before ischemia in every group (P < 0.01). That in the rest 3 groups was all higher than IR group,and that in ES group was higher than EA and SA groups (F=4.48, P <0.05). CONCLUSION: ① Both electroacupuncture and salivae stimulate expression of heat-shock protein 70 mRNA after ischemia and reperfusion, enhance protein stability of heat-shock protein 70 and alleviate myocardial damage. ② Both electroacupuncture and salivae inhibit increased dopamine content in the body after ischemia and reperfusion, reduce dopamine mediated injury and protect myocardium. ③ Coordination is present between electroacupuncture and salivae.
3.The relationship between syndrome differentiation of TCM and clinical test results of 196 cases with chronic ;hepatitis B

Objective To establish the corresponding relationship between syndrome differentiation of TCM and clinical test results in chronic hepatitis B patients,and thus to provide objective evidence for TCM treatment. Methods 196 patients with chronic hepatitis B,according to the Chinese medicine dialectical typing results were divided into five groups(liver kidney yin deficiency syndrome type 35 cases,Shire Zhongzu syndrome type 24 cases, Yang deficiency of spleen and kidney syndrome type 26 cases,damp heat resistance type and blood stasis syndrome type 42 cases,liver stagnation and spleen deficiency syndrome type 69 cases).HBV -DNA,hepatitis B,TBIL,ALT, TTT were detected.Results In the HBV -DNA test,the majority of the five groups were positive,the overall compar-ison,the difference was not statistically significant (P =0.937).But in the second liver five,TBIL,ALT and TTT detection,the detection results were reflected in the corresponding relationship between TCMsyndrome differentiation. Such as in TTT detection,liver kidney yin deficiency syndrome type and liver stagnation and spleen deficiency syn-drome type group of patients,most patients with TTT detection results between 7 and 10,two groups sequentially com-pared with the other three groups,the differences were statistically significant (all P =0.000);damp heat resistance type and blood stasis type in the patients group,TTT detection results were generally less than 6,two groups in turn compared with the other three groups,the differences were statistically significant (all P =0.000);Yang deficiency of spleen and kidney in the patients group,TTT detection results were generally more than 10,and compared with the other four groups,the differences were statistically significant (all P =0.000).Conclusion TCMsyndrome differen-tiation of chronic hepatitis B patients has a certain correlation with the clinical test results.
4.Intramedullary nails for correction of deformity in the lower limbs in children with osteogenesis imperfecta

[Objective]To explore the application of the intramedullary nails for correction of deformity in the lower limbs and decrease the opportunity of refracture in children with osteogenesis imperfecta.[Method]From Jan.2000 to Aug.2004,8 patients(3 males and 5 females),with 13 femurs and 3 tibias totally,with deformity in lower limps were corrected by multi-osteotomy and fixed with intramedullary nails,2 for K-wires;3 for Ender nails;and 3 for flexible intramedullary nails.[Result]All deformities were perfectly corrected.All patients were available at final follow up,for 9 months to 60 months,mean 30 months.None of the complications occurred postoperatively,such as infection in the incision or pin tract,nonunion,and refracture.One patient can walk by a stick from wheel-chair help preoperatively and one K-wire was disrupted near the end.[Conclusion]Multi-osteotomy and fixed intramedullary nails,can correct the deformity in the lower limbs perfectly and decrease the opportunity of refracture in children with osteogenesis imperfecta,which has been proved to be a reliable method.
5.Effect of Hypoxia on in Vitro Invasion of Lung Carcinoma Cells and Its Molecular Basis

Objective To explore the effect of hypoxia on the in vitro invasion of lung carcinoma cells and its molecular basis. Methods Small cell lung carcinoma cells(H128) were exposed to hypoxia(5%O 2,5%CO 2,90%N 2) or anoxia(95%N 2,5%CO 2)condition for 48 hours. The invasive ability of the cancer cells was assayed with HABM-HEM model. The expression of E-cadherin and ? 1-integrin on the cells was also assayed by flow cytometry. Results Compared with normoxia group, the invasive ability of the cancer cells increased, the expression of E-cadherin decreased, and the expression of ? 1-integrin increased in hypoxia group. In anoxia group, the invasive ability and the expression of E-cadherin and ? 1-integrin all decreased compared with normoxia group. Conclusions Moderate hypoxia can down-regulate the expression of E-cadherin, up-regulate the expression of ? 1-integrin, and increase the invasion of carcinoma cells. But serious hypoxia can decrease the expression of adhesive molecules and the invasion of the cancer cells.
6.Effect of compound glycyrrhizin on airway inflammation in asthmatic rats

Objective To investigate the effect and mechanism of compound glycyrrhizin on airway inflammation in asthmatic rats. Methods Forty wistar rats were randomly divided into health group (group Ⅰ), asthma group(group Ⅱ), treating group (group Ⅲ) and treating control group (group Ⅳ) (n=10 in each group). Group Ⅱ, group Ⅲ and group Ⅳ received intraperitoneal injection of ovalbumin (OVA) and aluminum hydroxide gel for sensitization and OVA aerosol by ultrasonic nebulization for challenge, in addition, group Ⅲ re-ceived intraperioneal injection of 200 μg of compound glycyrrhizin 1 hour before OVA aerosol challenging every time, and group Ⅳ were in-jected with normal saline instead of compound glycyrrhizin. Group Ⅰ were injected and nebulizated with normal saline instead of OVA and alu-minlum hydroxide. All rats were sacrificed at the 6th hour after the last challenging, venous blood and bronchoalveolar lavage fluid(BALF) were collected, the IgE in venous blood were determined and the concentration of IL-4, IL-5, IL-13 and IFN-γ in BALF were detected, the cells in BALF were counted and detached. Results The concentration of IgE, IL-4, IL-5, IL-13 and the count of neutrophilic granulocytcs, lymphocytes, cosinophils in group Ⅱ were significantly higher than those in group Ⅰ and group Ⅲ(P<0.01), and the concentration of IFN-γ in group Ⅰ and group Ⅳ were significantly lower than those in group Ⅰ and group Ⅲ(P <0.01). There was no significant difference be-tween group Ⅱ and group Ⅳ(P >0. 05). The concentration of IgE, IL-4, IL-5, IL-13 and the count of neutrophilic granulocytes, lympho-cytes, cosinophils in group Ⅲ were higher than those in group Ⅰ, and the concentration of IFN-γ and the count of mononucleat macroplile cells in group Ⅲ were lower than those in group Ⅰ, but there were no significant difference(P>0.05). Conclusion Compound giycyrrhiz-in alleviated the airway inflammation and reduced the airway hyperresponsiveness in asthmatic rats, which mechanism may be related to regu-lating the balance of Th1/Th2 cytokines, suppressing the immune ability of Th2 and promoting the immune ability of Th1.

Objective:To compare influence of valsartan (Val) combined amlodipine (Am) or hydrochlorothiazide (Hyd) on blood pressure variability and quality of life in aged patients with hypertension .Methods:A total of 127 aged patients with hypertension stage 2 and 3 were randomly divided into Val + Am group (n=64) and Val + Hyd group (n=63) .Circadian rhythm and variability of blood pressure and quality of life were observed in both groups before and after treatment .Results:(1) After treatment ,the 24hSBP ,DBP ,SBPV , DBPV and daytime SBP , DBP ,SBPV ,DBPV ,and nighttime ,morning surge of SBP and DBP all significantly reduced in both groups;(2) Compared with Val+ Hyd group ,there were significant reductions in 24hSBP [(120.6 ± 10.2) mmHg vs . (110.9 ± 11.3) mmHg] ,daytime SBP [(120.6 ± 11.3) mmHg vs .(111.6 ± 11.37) mmHg] ,nighttime SBP [(118.5 ± 11.6) mmHg vs .(108.6 ± 11.9) mmHg] ,morning surge of SBP [ (26.2 ± 13.7) mmHg vs .(23.0 ± 10.4) mmHg] (P<0.05 or <0.01);24hSBPV [ (10.7 ± 2.2) mmHg vs .(8.2 ± 2.0) mmHg] ,daytime SBPV [ (10.4 ± 1.9) mmHg vs .(8.1 ± 2.1) mmHg] (P<0.01 all);significant rise in improved percentages of physical health ,mental health and total function in quality of life (P<0.05 or <0.01) in Val + Am group .Conclusion:Valsartan combined am-lodipine or hydrochlorothiazide can both effectively improve blood pressure variability and quality of life in aged pa-tients with hypertension ,and the effect of the former is even better .
10.Changes of Pathogens and Antimicrobial Resistance of Nonfermenting Gram-negative Bacilli in Intensive Care Unit

OBJECTIVE To study the isolation status and antimicrobial resistance of nonfermenting Gram-negative bacilli collected from intensive care unit(ICU) of our hospital so as to instruct the rational clinical application of antibiotics.METHODS The antimicrobial resistance of nonfermenting Gram-negative bacilli isolates collected from patients in ICU from Jan 2003 to Dec 2007 was analyzed.Antimicrobial susceptibility of clinical isolates were tested by Kirby-Bauer method.RESULTS Total 384 nonfermenting Gram-negative bacilli isolates were collected in 5 years.The most common species were Acinetobacter baumannii(219),Pseudomonas aeruginosa(117) and Stenotrophomonas maltophilla(36).The antimicrobial resistance rate of nonfermenting Gram-negative bacterial to most antibiotics were much higher.The antimicrobial resistance rate of Acinetobacter spp to imipenem,cefoperazone/sulbactam and piperacillin/tazobactam was 3.7%,28.3% and 42.9%.But the resistance rate of Acinetobacter spp to imipenem was increased in recent 2 years(58.0%).The antimicrobial resistance rate of P.aeruginosa to cefoperazone/sulbactam was the lowest.That of imipenem-resistant P.aeruginosa to cefoperazone/sulbactam was 34.0%.S.maltophilla was relatively susceptible to ceftazidime,cefoperazone/sulbactam and piperacillin/tazobactam.CONCLUSIONS Nonfermenters Gram-negative bacilli are the important pathogens in ICU.Surveillance of their prevalence and drug resistance may provide evidences for rational antibiotic choices.
