1.Valsartan combined amlodipine or hydrochlorothiazide improving blood pressure variability and quality of life in aged patients with hypertension

Objective:To compare influence of valsartan (Val) combined amlodipine (Am) or hydrochlorothiazide (Hyd) on blood pressure variability and quality of life in aged patients with hypertension .Methods:A total of 127 aged patients with hypertension stage 2 and 3 were randomly divided into Val + Am group (n=64) and Val + Hyd group (n=63) .Circadian rhythm and variability of blood pressure and quality of life were observed in both groups before and after treatment .Results:(1) After treatment ,the 24hSBP ,DBP ,SBPV , DBPV and daytime SBP , DBP ,SBPV ,DBPV ,and nighttime ,morning surge of SBP and DBP all significantly reduced in both groups;(2) Compared with Val+ Hyd group ,there were significant reductions in 24hSBP [(120.6 ± 10.2) mmHg vs . (110.9 ± 11.3) mmHg] ,daytime SBP [(120.6 ± 11.3) mmHg vs .(111.6 ± 11.37) mmHg] ,nighttime SBP [(118.5 ± 11.6) mmHg vs .(108.6 ± 11.9) mmHg] ,morning surge of SBP [ (26.2 ± 13.7) mmHg vs .(23.0 ± 10.4) mmHg] (P<0.05 or <0.01);24hSBPV [ (10.7 ± 2.2) mmHg vs .(8.2 ± 2.0) mmHg] ,daytime SBPV [ (10.4 ± 1.9) mmHg vs .(8.1 ± 2.1) mmHg] (P<0.01 all);significant rise in improved percentages of physical health ,mental health and total function in quality of life (P<0.05 or <0.01) in Val + Am group .Conclusion:Valsartan combined am-lodipine or hydrochlorothiazide can both effectively improve blood pressure variability and quality of life in aged pa-tients with hypertension ,and the effect of the former is even better .
2.The value of MCV,MCH in the screening of thalassemia

Objective To evaluate the value of the tests of mean corpuscular volume (MCV ) ,mean corpuscular hemoglobin (MCH) in the screening of thalassemia .Methods 603 cases were performed MCV and MCH tests and gene detection of thalasse-mia .The results of gene detection were used as reference standard .The sensitivity ,specificity ,accuracy ,positive predictive value and negative predictive value of single or combined results of MCV ,MCH tests were calculated .Results The sensitivity and specificity of single MCV test were 91 .2% and 76 .1% ,single MCH test were 91 .2% and 70 .7% .The sensitivity and specificity of parallel combined tests of MCV and MCH were 94 .8% and 61 .4% ,serial combined tests of MCV and MCH were 88 .4% and 75 .6% .Con-clusion The sensitivity of single MCV or MCH test in the screening of thalassemia is high .Parallel combined tests of MCV and MCH can improve the sensitivity ,and will be widely used in the in the screening of thalassemia .
3.Expression of CapG in gastric cancer and its relationship with clinicopathological features and prognosis

Purpose To investigate the expression of gelsolin-like actin-capping protein (CapG) in gastric cancer and adjacent normal gastric tissues and its relationship with clinicopathological features and prognosis.Methods The immunohistochemical (IHC) assay was performed to detect the expression of CapG protein in 125 specimens of gastric cancer (study group) and 30 specimens of matched adjacent normal gastric tissues (control group).No patients had received chemotherapy or radiotherapy before surgery.Immunostaining was scored semiquantitatively by two independent observers who were blinded to the patients' outcome and other clinicopathological parameters.The correlation of the expression of CapG with clinicopathological characteristics and prognosis was analyzed.Results Cytoplasmic and nuclear CapG protein staining was detected in all gastric cancer tissues.CapG protein expression was absent or low in matched adjacent normal gastric tissues but significantly higher in gastric cancer tissues.Expression of CapG in the tumor was not significantly associated with gender,age,tumor size,tumor location (P > 0.05).However,elevated CapG expression was strongly correlated with the depth of invasion (P =0.044),lymph node metastasis (P =0.026),distant metastasis (P =0.001),and AJCC stage (P =0.012).Significant poorer overall survival was observed in the high CapG expression group compared with that of the low CapG expression group (P <0.001).Multivariate analysis showed overall survival correlated with the patients' tumor location,lymph node metastasis,distant metastasis,AJCC stage and the expression of CapG protein.Conclusion CapG is highly expressed in gastric cancer.Tumors with CapG up-regulation tend to have poorer prognosis,suggesting overexpression of CapG may contribute to the prediction of poorer prognosis and may be a potential therapeutic target for gastric cancer.
4.Experience of accidentally injured patients participating cognitive-behavioral group intervention: a qualitative study

Objective To understand the experience of accidentally injured patients participating in cognitive-behavioral group intervention, so as to provide empirical evidence for optimizing the psychological intervention program and fulfill more patients' needs. Methods Using the method of phenomenological study, semi-structured interview was conducted on 10 cases of accidentally injured patients participating in cognitive-behavioral group intervention selected by purposive sampling method. Results The main experience of these accidentally injured patients participating in cognitive-behavioral group intervention was positive oriented mood, including more experience happiness, harvest emotional feelings, changed the focus of attention; and seeking their own growth, including recognition of self-value, adjusting coping strategies, and cherishing interpersonal benefits. Conclusions Accidentally injured patients reported a rich and deep feeling and experience after participating cognitive-behavioral group intervention, which provides valuable basis for optimizing the psychological intervention program of posttraumatic patients.
5.Plasmablastic lymphoma:clinical and pathological features of 9 cases

Purpose To investigate the clinicopathological features,treatment,prognosis and differential diagnosis of plas-mablastic lymphoma(PBL).Methods Collect clinical data of 9 cases of PBL,use HE and immunohistochemistry EnVision staining,detect EB virus using in situ hybridization,detect B lymphocyte gene rearrangement using PCR,analyze its clinical and pathological characteristics,and review relevant literature.Results Among the 9 patients,there were 7 males and 2 fe-males,with a median age of 54 years.Two cases were found in the nasal cavity and sinuses,two cases in the lymph nodes,one cases in the gum,one cases in the tonsil,one cases in the skin,one in the colon and one in the anal canal.Among the 9 cases,4 were HIV positive.The skin lesions are plasma like cells,and the remaining areas are plasma like/immune like cells.Immuno-phenotype:CD138,MUM1 and C-myc were all positive.CD20,CD3 and CD5 were negative.Some of them expressed CD38(7/8),CD79a(6/9),CD56(3/7)and PAX5(1/8).Ki67 prolif-eration index was high(70%-95%).EBER in situ hybridiza-tion was positive in 8 cases.Conclusion The diagnosis of PBL is difficult and requires a combination of clinical manifestations,histological features,immunophenotype,and molecular patholo-gy to make the final diagnosis.The prognosis of PBL is very poor and there are no effective treatment options.

Klebsiella pneumoniae carbapenemases-producing Klebsiella pneumoniae (KPC-Kp) has caused a global public health crisis, and the severity of its infection is associated with high mortality in hospitalized patients. Therefore, the KPC-Kp prevention methods and the corresponding treatment strategy exploration are imminent. The risk factors and the treatment progress of KPC-Kp colonization or infection are reviewed in this paper to explore corresponding preventive measures and treatment strategies for clinical prevention and treatment.
7.Exploration of action and significance of-source point for clinical diagnosis based on literature metrology.

Modern literature on the physical property of the-source point were collected from Chinese National Knowledge Infrastructure(CNKI) and China Biology Medicine(CBM) databases. The physical property,relevant diseases and-source acupoints were analyzed through statistical analysis of literature metrology. It is considered that articles on the electrical resistance of acupoint account for the largest part,which are mainly related to hyperthyreosis and the change of menstrual cycle. The second part is radiation spectrum,which are mostly relevant to the coronary heart disease and then the physiological change of healthy people. As to the diseases,articles of cardiovascular diseases are taken the most proportion,which were treated with the 12-source points,Shenmen(HT 7) and Daling(PC 7). Also,the results present the physical property of-source acupoints in themeridians is more sensitive to diseases and the physical property is specific to diseases. Besides,the-source acupoint can show the pathological changes of its own meridian.

Acute pancreatitis is a common acute abdominal disease of the digestive system characterized by multiple etiologies and rapid progression, and early diagnosis and treatment are closely associated with the prognosis of patients. Among various radiological examinations, ultrasound can perform real-time dynamic comprehensive scans of the pancreas and the biliary system and thus plays an important role in etiological diagnosis, grading, and treatment. This article reviews the current status and prospects of ultrasound in acute pancreatitis, in order to provide a reference for the diagnosis and treatment of acute pancreatitis.
9.Expression and clinical significance of Flotillin-2 in gastric cancer tissues

Objective: To investigate the expression of Flotillin-2 (Flot-2) protein in gastric cancer tissues and its relationship with clinicopathological features and prognosis of gastric cancer (GC) patients. Methods: 112 samples of gastric cancer tissue and the corresponding paracancerous tissue that resected at the gastrointestinal surgery department of the Second Affiliated Hospital of Nanchang University between January 2009 andApril 2010 were collected for this study. The expression of Flot-2 protein in tumor tissues was detected by immunohistochemistry. The survival data were analyzed by Kaplan-Meier and Log-Rank test, and the survival curve was plotted. Spearman correlation analysis was used to examine the relationship between Flot-2 protein expression and clinicopathological characteristics and prognosis of GC patients. Results: In gastric cancer tissues, Flot-2 was primary stained in cytoplasm. Level of Flot-2 was significantly higher in gastric cancer tissues compared with that in paracancerous tissues (53.57% vs 46.43%, P<0.05). Expression of Flot-2 in tumor tissues was significantly associated with tumor size, depth of invasion, lymph node metastasis, distant metastasis and AJCC stage (all P<0.01), but not with gender, age, differentiation degree and tumor location (P>0.05). Moreover, survival analysis showed that the overall survival of patients with low Flot-2 expression was significantly higher than that of the patients with high level (P<0.01). Cox regression analysis indicated that distant metastasis, AJCC stage and Flot-2 expression were the independent risk factors for the prognosis of GC patients. Conclusion: Flot-2 protein was highly expressed in gastric cancer tissues and closely correlated with the poor prognosis of GC patients; Flot-2 is an independent risk factor for GC prognosis and may be served as a potential therapeutic target for gastric cancer.
