1.THE GROWTH OF THE FEMUR OF CHINESE FETUS AND IT'S CORRELATION-RATIO WITH THE C-R LENGTH

The femurs of 98 Chinese fetuses were observed and measured with radiographic method. A series data of femur-length and -width of different fetal-age groups was obtained. Calculated femur index (width/length?100)showed that the proportion between the femural length and the femural width remained considerably constant dur- ing the development of the fetus. The positive correlation between the length of the femur and the C-R length was revealed by statistical analysis; a regression equation was obtained. The authors suggest that it is possible to take the femural length as a criterion parameter to estimate the fetal age.
2.The clinical significance of pyloroplasty in the proximal gastrectomy

Objective To explore the clinical effects of pyloroplasty on proximal gastrectomy in patients with lesions in cardia of the bottom of stomach.Methods One hundred cases of the proximal gastric diseases who were enrolled in the second affiliated hos-pital of Chongqing medical university from the January 2010 to December 2013 were selected in this prospective randomized con-trolled experimental study.Among them,6 cases were excluded,and the other 94 cases were divided into two groups:the experimen-tal group (A),in which 54 patients were underwent additional pyloroplasty surgery;the control group (B),in which 40 patients were underwent with no strings attached pyloroplasty.All patients were followed up.Then,the clinical indicators during hospitaliza-tion,term complications and repeated endoscopy prompted pyloric stenosis and reflux esophagitis situation three months after dis-charged from hospital were observed.Results There was no significant difference in the incidence rate of gastroplegia (P >0.05);there were significant differences in other clinical indicators during the postoperative hospitalization and the incidence rate of compli-cations (P <0.05).Conclusion Proximal gastrectomy additional pyloroplasty could reduce gastric retention,decrease the incidence of reflux esophagitis,shorten the hospital period and could be helpful for recovery,thus it is recommended to be further promoted.
3.Experimental study of anteversion of femoral prosthesis in hemiarthroplasty for treating femoral in-tertrochanteric fracture in the elderly

Objective To study the accuracy of the anteversion of femoral prosthesis in hemiarthro-plasty for treating femoral intertrochanteric fracture in the elderly.Methods Twenty femoral neck antever-sions of specimens were measured, which were control group(A).The femoral specimens were made into the model of femoral intertrochanteric fracture, which were installed femoral prosthesis according to minor trochanter(B),identifying the max diameter of cavum medulla(C),ocular estimating femoral condylar line ( D) ,combination of identifying the max diameter of cavum medulla and ocular estimating femoral condylar line( E) respectively.All the anteversion of femoral prosthesis were measured.Results The anteversion were:A group(8.98 ±4.31)°,B group (9.12 ±4.87)°,C group (9.57 ±6.49)°,D group (13.18 ± 5.83)°,E group (10.37 ±4.56)°respectively.The difference among the five groups was statistically sig-nificant.There was difference between D and A, there were no differences between B and A, C and A, E and A respectively.Conclusions In hemiarthroplasty for treating femoral intertrochanteric fracture, the an-teversion of femoral prosthesis is according to minor trochanter possibly; if the minor trochanter can not be got reduction, it is a good option according to combination of identifying the max diameter of cavum medulla and ocular estimating femoral condylar line to make sure the accuracy of the anteversion of femoral prosthe-sis.
4.A hybrid technique of laser intracavity obstruction and foam sclerosant in treating varicose vein of the lower extremity

Objective To evaluate the clinic effect of a hybrid technique of laser and foam sclerosant in treating varicose vein of the lower extremity.Methods From April 2009 to April 2013,hybird operation of endovenous laser combined with foam sclerosant was carrird out in 126 patients with lower extremity varicosities.A total 175 diseased legs were treated.Catheter guidewire techniques was performed first in placing optical fiber below saphenous vein flap,which was followed by laser ablation for the great saphenous vein,then foam sclerotherapy were used for greater saphenous vein trunk 4 cm below tibial tubercle and the superficial varicose veins with diameter less than 6 mm.All patients were followed up for 3-24 months.The operation time,average hospitalization days,patients quality of life score,curative effect and complications were analyzed.CT angiography in the evaluation of occlusion of varicose vein of lower limbs trunk and recurrence were performed at the time of discharge and three months later respectively.Results Hybrid operation of endovenous laser treatment and foam sclerosant was successfully performed in all cases.The procedure time for a single leg was (32 ± 7) minutes; the quality of life score and the hospitalization after procedure was (4.2 ± 1.8) days and (40 ± 7) score respectively.CT angiography at discharge and 3 months later revealed that 97 (55.42%) and 111 (63.42%) limbs varicose veins disappeared respectively,78 (44.57%),64 (36.57%) lower limb varicose veins was reduced respectively,143 (81.71%),158 (90.28%) great saphenous vein trunk were in emphraxis respectively and 32 (18.28%),17 (9.71%) great saphenous vein lumen were narrowed.No vascular recanalization was found.The complications included saphenous nerve injury in 5 cases,soft tissue infection in 5 cases,calf skin burn in 11 cases,local funicular patches nodules in 32 cases,edema in 16 cases,subcutaneous hematoma in 10 cases.No complications such as pulmonary embolism,and deep venous thrombosis happened.Conclusions For the treatment of lower extremity varicosities,hybrid operation of laser and foam sclerosant ablation have high limb cure rate,less operation time and complications.
5.FasT -Fix technology vertical mattress suture meniscal tear Ⅲ level of clinical patient outcomes

Objective To investigate the clinical results of arthroscopic FaxT -Fix technology vertical mat-tress suture for patients with meniscal tear Ⅲ level.Methods 50 cases with the level of meniscal Ⅲ orthopedic treatment of torn were divided into two groups using random number table,25 cases in each group.The control group received a simple meniscectomy,the observation group was given arthroscopic FaxT -Fix technology vertical mattress suture.Surgery and prognosis function were observed and compared in the two groups during recovery.Results The operation time (26.53 ±7.34)min,postoperative functional activity recovery time (25.01 ±9.55)d and hospital stay (5.68 ±2.01)d of the observation group were significantly shorter than the control group[(46.66 ±12.28)min, (39.53 ±11.28)d,(9.98 ±3.34)d],the differences were statistically significant (t =7.04,4.91,5.52,all P <0.05).3,6,12,18 months after operation,the improvements of Lysholm and IKDC scores of the observation group were significantly better than the control group (t =3.60,2.64,2.81,3.03,4.94,2.12,2.28,2.32,all P <0.05). During the postoperative follow -up period,the incidence rate of complications of the observation group (4.00%) was significantly lower than the control group (24.00%)(χ2 =4.15,P <0.05).Conclusion For patients with level of Ⅲ meniscus tear,arthroscopic FaxT -Fix technology vertical mattress suture can effectively reduce the surgery and hospitalization time,promote functional recovery of the patients 'knee,significantly improve the postoperative Lysholm knee score and IKDC,but also significantly reduce postoperative wound infection,hematoma,nerve damage vascular complication rate.
6.Clinical evaluation of arthroscopic rotator cuff repair using improved-press-ift double-row technique for patients of large rotator cuff tear

Objective To investigate the clinical outcomes of arthroscopic rotator cuff repair using improved-press-ift double-row technique for patients of large rotator cuff tear.Methods From December 2013 to November 2014, 52 patients (20 males, 32 females) with a full-thickness large rotator cuff tear underwent arthroscopic improved-press-ift double-row repair were retrospectively analyzed. The mean age of the patients was 65.6 years (range 51 to 76 years). The visual analog pain scale scores (VAS), the range of motion (ROM), University of California at Los Angeles (UCLA), and American Shoulder and Elbow Surgeons (ASES) were used for clinical and functional evaluations before surgery and at the time of 6 months after arthroscopy.Results The mean duration of follow-up was 10.9 months (range 6 to 17 months). At the time of 6 months after arthroscopy, the mean subjective pain score (VAS) was (1.6 ± 0.9), the mean active forward flexion was (145.6 ± 10.7)°, whereas the mean external rotation at the side was (30.8 ± 8.5)°. The mean UCLA score improved to (32.3 ± 3.5), the mean ASES score improved to (81.8 ± 8.7). There was significant difference postoperatively (P < 0.05). No re-tear occurred.Conclusion The improved-press-ift DR technique is effective in arthroscopic large rotator cuff repairing. Compared with suture-bridge technique, this technique can decrease operation time, costs, and is much easier to process.
7.Arthroscopic rotator cuff repair using suture bridge technique as treatment of traumatic shoulder dislocation combined with rotator cuff injury in older patients

Objective To compare the clinical results of arthroscopic rotator cuff repair using a suture bridge technique and conservative therapy as treatment of traumatic shoulder dislocation combined with rotator cuff injury in older patients. Methods From August 2010 to August 2013 a total of 63 patients were prospectively enrolled. In the case of documented tears of the rotator cuff in combination with symptoms persisting after conservative therapy, patients were free to decide between arthroscopic rotator cuff repair using a suture bridge technique and conservative treatment. Patients were monitored for changes in visual analogue scale (VAS) scores, range of motion, the Simple Shoulder Test (SST), the Constant score and recurrences over a minimum follow-up period of 1 year. Results 60 pa-tients (30 in surgical group and 30 in conservative treatment group) were follow-up for at least 1 year. There was no significant difference between the two groups at baseline. The surgical group resulted in better improvements in pre-operative to postoperative VAS scores, range of motion, SST scores and the Constant score. There were five recur-rences in the conservative treatment group, while no recurrences were documented in surgical group. Conclusion In this selected patient group, we believe arthroscopic repair using a suture bridge technique for the treatment of symp-tomatic rotator cuff tear can improve shoulder function and decrease recurrences in older patients suffered from trau-matic shoulder dislocation combined with rotator cuff injury. The long-term outcomes remain to be determined.
8.Diagnosis and treatment of gastric cancer with ovarian metastasis:a report of 17 cases

Objective To investigate the clinical and pathological characteristics, diagnosis and treatment of gastric cancer with ovarian metastasis. Methods The clinical data of 17 cases of gastric cancer with ovarian metastasis, confirmed by surgery and pathology, were analyzed retrospectively. Results The average age of the patients was 48.41 years, and the first appearance of symptoms and signs often were of metastatic ovarian cancer. The main ultrasonographic findings were either a complex type of mass with both solid and cystic characteristics or only solid, and most of them were accompanied by intraperitoneal fluid accumulation. Bilateral metastatic ovarian cancer was more common(13 cases). The preoperative accurate diagnosis of this disease was difficult, so that the misdiagnostic rate was 64.7% in this series. Operation was done in all the patients, but prognosis was poor. The median survival time was only 11.6 months. Conclusions The prognosis of gastric cancer with ovarian metastasis is poor. It is of importance to inspect the stomach in cases of bilateral ovarian cancer. Radical resection of the primary disease focus together with hysterectomy and bilateral adnexectomy should be performed. Postoperative comprehensive therapy is conducive to improve the prognosis of gastric cancer with ovarian metastasis.

10.Evaluation the treatment effect of synovectomy under arthroscope on elbow rheumatoid arthritis

Objective To study the treatment effect of synovectomy under arthroscope on elbow rheumatoid arthritis.Methods 38 patients with elbow rheumatoid arthritis were selected as the research subjects,received arthroscopic synovial treatment,surgical procedures require more than one direction into the intra -articular,cleaning elbow joint synovial,complete removal of the foreign body cavity.Postoperative follow -up for 30 months,and com-pared the patients'Mayo elbow performance score and joint activity situation before and after surgery.Results After surgery,Elbow pain score[(30.12 ±8.11)points],activity level[(17.01 ±2.88)points],joint function[(20.67 ± 5.01)points]and total score[(73.04 ±12.01)points]were significantly higher than the pre -operative[(3.98 ± 3.89)points,(9.78 ±5.02)points,(13.77 ±3.69)points,(33.04 ±10.01)points](t =18.38,P <0.05;t =7.90, P <0.05;t =7.01,P <0.05;t =16.18,P <0.05 ).After surgery,elbow flexion activity score [(113.12 ± 15.11)points]was significantly higher than the preoperative flexion elbow activity score[(93.03 ±19.89)points] (t =5.09,P <0.05 ).Conclusion Treated elbow rheumatoid arthritis by synovectomy under arthroscope could effectively improve the disease status of patients,relieve pain,enhance mobility,allowing patients to gradually return to normal life,and it can be widely used in clinical practice.
