1.X-ray and high resolution CT Findings in pulmonary alveolar proteinosis and their relationship with pulmonary function

ObjectiveTo study X-ray and high resolution computed tomograghy(HRCT)features and tIleir relationship with pulmonary function test in pulmonary alveolar proteinosis(PAP).MethodsThe findings of X-ray and HRCT and the Resultsof pulmonary function tests in 19 patients with PAP were reviewed retrospectively.X-ray and HRCT scores were rated in the aspects of opacity density,extent,and severity.The relationship between the imaging scores and pulmonary function tests were determined.ResultsX-ray showed a symmetrical perihilar predominance appearance in patients with PAP.HRCT scan showed bilateral clear-defined pathchy areas of ground-glass opacity and interlobular septal thickening,a pattern commonly characterized as"crazy paving".Nineteen PAP respiratory function Resultswere:DLCO/Pred=(54.49±16.78)%,FEV1/FVC=(86.00±6.70)%.Chest radiographic extent score and severity score correlated with diffusing capacity(r=-0.661,P<0.01,r=-0.475,P<0.05).HRCT severity score was 16.47±5.26.Among all the HRCT scores,severity score correlated significantly with the presence of restrictive ventilatory defect and diffusing capacity defect(DLCO/Pred,r=-0.809,P<0.01;FEV1/FVC,r=0.573,P<0.05).ConclusionsIn patients with PAP,there Is close relationship between X-ray,HRCT findings and pulmonary function.HRCT correlates more closely with pulmonary function.
2.Effect of oral drugs associated with acupuncture on peripheral nervous lesions of diabetes mellitus

ObjectiveTo observe the clinical effect of oral drugs associated with acupuncture on peripheral nervous lesions of diabetes mellitus.Methods60 diabetes mellitus patients with peripheral nervous lesions were randomly divided into the treatment group and control group with 30 cases in each group. Patients of the treatment group were treated with oral drugs and acupuncture, but cases of control group were only taking oral drugs.ResultsThe effective rate of the treatment group was 86.67% higher than that of the control group (43.33%)(P<0.01).ConclusionDrugs associated with acupuncture has a good effect on peripheral nervous lesions of diabetes mellitus.
3.Application of point massage combined with physiotherapy in the treatment of insomnia for the community elderly

Objective To observe the application effect of point massage combined with physiotherapy in the treatment of insomnia for community elderly.Methods 30 elderly patients with insomnia who accepted point massage combined with physiotherapy were selected as observation group,and 30 elderly patients treated in tertiary hospital with insomnia who took alprazolamum for treatment were selected as control group.The SRSS score of the two groups before and after treatment,as well as treatment efficacy were compared.Results The insomnia treatment efficacy of the observation group was higher than that of the control group(Z =2.45,P <0.05).The SRSS rating of the observation group before treatment was (30.76 ±7.1 9)points and after treatment was (1 5.43 ±3.95)points.The SRSS rating of the control group before treatment was (32.33 ±6.98)points,which after treatment was (21 .93 ±6.73)points.The SRSS scores of both two groups after treatment were decreased (t =1 0.23,5.87,all P <0.05),with obvious differ-ences.The SRSS scores of the observation group decreased more obviously and was lower than that of the control group (t =4.56,P <0.05).Conclusion The point massage combined with physiotherapy can effectively improve insomnia of the elderly,which is also with advantages of easy operation and no obvious side effects and high cooperation degree etc,and it is suitable to be promoted in community.
4.Retroperitoneal Laparoscopic varicocele ligation

Objective: To assess the clinical results and value of retroperitoneal laparoscopic varicocele ligation. Methods: Sixty-eight cases of retroperitoneal laparoscopic varicocele ligation were analysed retrospectively. Results: Successful surgery was achieved in 67 cases,with average operation time of 30.5 minutes and mean post-operation hospital stay of 3 days.Two cases of recurrence were found during a 3-9 months follow-up. Conclusion:Retroperitoneal laparoscopic varicocele ligation is effective and minimally invasive.
5.Experimental study on biological character of rabbit lingual keratinocyees

Objective To explore the cultural method of lingual keratinocytes and make preparations for further investigation in using lingual keratinocytes as a new choice of seed cells for the reconstruction of tissue engineered corneal epithelium.Methods Keratinocytes were enzymatically dissociated from lingual tissue specimens using 0.25%disepase and 0.05%trypsin.The biological characteristics of the cells were observed through phase microscope.Meanwhile.1ingual kemtinoeytes were immtmohistochemically labeled with broad spectrum keratin antibody (AE1/AF3).Results The cells revealed the form of "road metal" and could be passed for 3 or 4 generations.Immunohistochemistry indicated that lingual keratinocytes were positive for AE1/AE3 staining.Condusion Lingual kemtinocytes of rabbits can be cultured with KSFM in vitm and magnitude quantity can be attained,laying a favorable foundation for lingual keratinocytes as a new choice of seed cells for corneal epithelium reconstruction with tissue engineering.
6.A Comparing Study of Different Anesthetic Methods for Patients during Laparoscopic Hysterectomy

Objective To study the effects of different anesthetic methods on stress response in patients during laparoscopic hysterectomy. Methods 45 patients scheduled for laparoscopic hysterectomy were randomly divided into three groups: group A (n = 15) , group B( n = 15) and group C (n = 15). blood samples was taken from patients in each group before anaesthesia, at 10 min and 40 min after pneumoperitomeum with CO_2 , 10 min after operation, to determine the glucose (BG) , catecholamine ( CA ) , C - response protein (CRP) and interleukins -6( IL -6). The changes of MAP ,HR,P_(ET)CO_2 and SpO_2 were also recorded. Results After anesthesia, the MAP, HR significantly decreased in group A (P 0.05 ). Conclusion Combined general epidural anesthesia can effectively inhibit the stress response in patients undergoing laparoscopic hysterectomy.
7.Sequential Therapy of Gatifloxacin in Elder Inpatients with Lower Respiratory Tract Infection: A Clinical Observation

0.05) between two groups in the above results.Disc agar diffusion test showed the sensitivity rates of overall clinical isolates to gatifloxacin and levofloxacin were 97.56% and 92.68%,respectively.The incidence of adverse drug reactions(ADR) of two groups were 23.91% and 43.75%,respectively.In 5 cases severe ADR were found. CONCLUSIONS Sequential therapy of gatifloxacin may get satisfactory results in lower respiratory tract infections of elder people.The irrational use of drugs is an important factor to increase ADR(including collateral damage).So we should pay attention to the ADR and grasp the indications strictly and use the drugs appropriately,especially for the elder patients.

Objective:To investigate the expression pattern of Mtsl/S100A4 in mouse spinal cord;to investigate the effects of Mtsl/S100A4 on glial cell responses.Method:The study was carried out on Mtsl/S100A4 wild type and knock-out mice.The degenerative spinal cord model was established by dorsal root or sciatic nerve injury.The de-myelinated spinal cord model was established by ethidium bromide injections.Then the expressions of S100A4,GFA P,NG2 and Mael were measured.Result:The expressions of Mtsl/S100A4 in mice spinal cord were similar to that in rats.In WT mice this protein expressed in a thin layer of fiber bundles in the tract of Lissauer,and in white matter astrocytes.There was intracellular up-regulation of Mtsl/S100A4 in white matter astrocytes of WT mice after dorsal root or sciatic nerve injury,with no difference in glial cell response between WT and KO mice.However,7 days after ethidium bromide injection,in WT mice,the astroglial reaction was restricted on operated side,where a distinct glial scar had formed.While in KO mice,no distinct glial scar formed in demyelinated area.Conclusion:Mtsl/S100A4 expression in mouse spinal cord is similar to the pattern as in rats;intracellular Mtsl/ S100A4 up-regulation does not affect glial responses in degenerative spinal cord;the presence of extracellular Mtsl/ S100A4,which entered the spinal cord after ethidium bromide induced demyelination,markedly affects the glial cell responses in demyelinative spinal cord,including glial scar formation.
9.The radical resection of hilar cholangiocarcinoma:a report of 38 cases

Objective To study the diagnosis and radical resection of hilar cholangiocarcinoma. Methods Retrospective analysis was made on the clinical feature and the effect of radical resection on 38 cases of hilar cholangiocarcinomas.Results Diagnosis was made in all of the patients preoperatively.The radical resection was perfomed on 38 patients.Of them, 3(7.9%) died after operation.After operation, 5 cases (13.2%) developed bile leakage,and 2 of the 5 cases developed subphrenic abscess,which were cured by drainage; 4(10.5%) had right hydrothorax that was cured by conservative therapy; and 3(7.9%)had incision split that was cured by resuture. Among the 35 postoperative survivors,34 (97.1%) were followed up, the 1- and 3-year survival rates were 91.9% and 35.2%.None of the patients survived for 5 years. Conclusions It′s still difficult to make early diagnosis in hilar cholangiocarcinoma.The diagnosis mainly depends on the combination of imaging examinations. Nowadays the radical resection rate of hilar cholangiocarunoma is still low, the recurrence and metastasis are common after operation, and few patients can survive for a long time. It suggests that even in radical operation for hilar cholangiocarcinoma, the regions of resection and sweep are not enough,and the operative procedure needs to improve further.
10.Progresses of interventional treatment in biliary stenosis

Interventional therapy is an important treatment for biliary stenosis.The treatment methods are different ac cording to the different causes.Conventional interventional therapy include biliary drainage,balloon dilatation and stent implantation.There are some new treatment methods such as radiofrequency catheter ablation and biliary stent loaded with 125I seeds.The applications of interventional therapy in biliary stenosis were reviewed in this article.
