Objective The study was to investigate a new method of rapid expansion by biomechanical methods. Methods Dogs were set up as animal models. Six areas, 5 cm?8 cm each, were symmetrically designed on a dog. Expanders of 240 ml were implanted in each area. Four groups were divided: A. Injection of 2 weeks. B. Injection of 6 weeks. C. Sham operation group. D. Blank control group. The first two groups were subdivided into three groups respectively according to maintaining times: one week, two weeks and four weeks. The following indexes were measured on the second stage surgery: biomechanical properties of the skin, stress strength, stress strain, stress relaxation and creep, the collagen content and alignment of skin. Results After expansion, maximal stress strength fell off, and stress strain curve moved backward. Relaxation curve fell off as well, as creep curve rose up. With elongation of the maintaining period, viscoelasticity of the skin gradually approximated normal skin, and had less relation with injection time. With the maintaining period increased, the collage content increased distinctly, and the alignment became much order. Conclusion Two week maintaining period is very important for expanded skin to recover the biomechanical properties and the content of collagen and alignment.

BACKGROUND:Poly(propylene fumarate) (PPF) can crosslink at room temperature, andβ-tricalcium phosphate (β-TCP) has good biocompatibility, but PPF/β-TCP composite bone cement has not yet been systematical y studied. OBJECTIVE:To prepare PPF/β-TCP composite bone cement and to explore its in vitro bioactivity and degradability. METHODS:β-TCP and PPF were respectively synthesized by liquid-phase precipitation and a two-step method, and PPF/β-TCP composite bone cement was prepared through mixing PPF withβ-TCP. The in vitro bioactivity of PPF/β-TCP was compared with the commercial poly(methyl methacrylate) (PMMA) through the ability of forming hydroxyapatite after immersed in simulated body fluid for 7 days. The in vitro degradability of PPF/β-TCP was studied via investigating the transformation of pH values, water uptake and mass loss, compressive strength and morphology at each time point. RESULTS AND CONCLUSION:There were hydroxyapatites formed on the PPF/β-TCP material, but none on the commercial PMMA material. The pH values of the PPF/β-TCP were stable in PBS for 63 days, indicating its degradation is moderate;the mass loss was up to 13.5%after 84 days. Scanning electron microscope displayed the degraded PPF/β-TCP surface, and its compressive strength was decreased gradual y, which good for the integrity and sustainability of mechanical properties during degradation. These results suggest that PPF/β-TCP bone cement holds mineralization and degradability in vitro.

Objective To evaluate the clinical effect of the upper lip defect with lower cross-lip flap (Abbe-Estlander flap). Methods A total of 68 cases of upper lip defect underwent the reconstruction with cross-lip flaps. We applied Abbe flap in upper lip defect in the center, and Estlander flap in lateral upper lip defect. This method was a two-stage procedure:the first stage was to rotate flap 180 degrees into the upper lip defect and to suture with the created defect of the upper lip, with careful maintenance of blood supply from the pedicle; the second was to undertake the division of bridged pedicle and paid more attention to creation of the vermilion border. Results The flaps survived in all cases.Evaluation from 3 to 12 months after the operation showed that the shape of lips were obviously repaired with excellent aesthetic and functional results. Conclusions Abbe-Estlander flap could reconstruct anatomical features and function of the lip precisely. It seems that within certain limits (probably between one-third and one-half of total upper lip length),this flap appears to be the preferred method for upper lip reconstruction.
