1.Distribution of GST-pi single nucleotide polymorphism in idiopathic epilepsy patients and its association with electroencephalogram

Objective To study the distribution patterns of the SNPs for the 3 sites (Ⅱe105Val, Ala114Val and Asp147Tyr) of glutathione S-transferase pi (GST-pi) in epilepsy patients without definite etiological factors. Methods At the same time, the possible relationship of GST-pi gene mutation with the vulnerability of drug-resistant epilepsy, drug-responsive epilepsy and EEG feature were explored. The SNPs of GST-pi for healthy people, drug-responsive epilepsy patients and drug-resistant epilepsy patients were genotyped by sequence-specific primers (SSP)-based PCR technologies (PCR-SSP). Results In drugresponsive epilepsy group, the frequency for 3 sites of mutated SNP of GST-pi was 59.62%, 55.32% and 50.94%, while it was 58.33%, 51.19% and 45.92% in drug-resistant epilepsy group. The difference of genotype and allele between normal group and foregoing epilepsy group was significant ( P<0.01 ), but no difference was found between drug-respensive epilepsy group and drug-resistant epilepsy group ( P>0.05 ). There was a difference of genotype distribution between groups with typical and untypical epilepsy EEG ( F = 0.0294, 8.867 × 10-6, 1.366 × 10-5, P<0.05 ). Conclusions The results indicate that the SNPs of GST-pi are associated with an increased risk of epilepsy, but not associated with an increased risk of drugresistant epilepsy. The patients present EEG characteristic of typical epilepsy.
2.Adeno-associated viral vector mediated and cardiac-specific delivery of CD151 gene in ischemic rat hearts.

Our previous studies demonstrated that CD151 gene promoted neovascularization in ischemic heart model. To improve the delivery efficacy and target specificity of CD151 gene to ischemic heart, we generated an adeno-associated virus (AAV) vector in which CD151 expression was controlled by the myosin light chain (MLC-2v) promoter to achieve the cardiac-specific expression of CD151 gene in ischemic myocardium and to limit unwanted CD151 expression in extracardiac organs. The function of this vector was examined in rat ischemic myocardium model. The protein expression of CD151 in the ischemic myocardium areas, liver and kidney was confirmed by using Western blot, while the microvessels within ischemic myocardium areas were detected by using immunohistochemistry. The results showed that MLC-2v significantly enhanced the expression of CD151 in ischemic myocardium, but attenuated its expression in other organs. The forced CD151 expression could increase the number of microvessels in the ischemic myocardium. This study demonstrates the AAV-mediated and MLC-2v regulated CD151 gene is highly expressed in the ischemic myocardium and cardiac-specific delivery that is more efficiently targets CD151 to the ischemia myocardium after myocardial infarction.
3.The measurement and clinic significance of tissue Inhibitors of metalloproteinase-1 on children with myocardltis

Objective To evaluate the relationship between serum tissue inhibitors of metaUoproteinase-1 and children with myocarditis,and provide evidences to support the measurement of serum tissue inhibitors of metallopro- teinase-1. Methods Tissue inhibitors of metalloproteinaee-1 and bioehem indexes of the harvested specimens and e -chocardiogram were measured from children with myocarditis(age: 1~14 years old, n = 9) and healthy young chil- dren(age: 1~14 years old, n = 9). Results Children with myocarditis showed reduced ejection fraction and dilated left ventricular compared with healthy children. Tissue inhibitors of metalloproteinaee-1 in children with myocarditis was significantly reduced compared with healthy children. Aspartie acid transanainase, lactate dehydrogenase,creatine kinase, hydroxybutyrie acid dehydrogenase, MB isoenzyrne of creatine kinase, cardiac troponin Ⅰ , C reactive protein showed no difference between two groups. Conclusion The reduced serum level of tissue inhibitors of metallopro- teinase-1 may show the severity of children with myocarditis and maybe one of the mechanisms of children with di- lated cardiomyopathy following myocarditis.

5.On the auto-reference, auto-correlation and adaptive interference cancellation theories and techniques for single extracting flash visual evoked potential.

On the basis of analyzing the defects in traditional averaging theory for extracting evoked potential (EP), and by realizing the characteristic of spontaneous electroencephalo-signal (S-EES) as well as the special environment for extracting EP, we propose an auto-reference, auto-correlation, adaptive interference cancellation (AAA-ICT) for use in the single trial of flash visual evoked potential (FVEP). Firstly, the segment of reference signal, which has the best correlation with evoked electroencephalo-signal (E-EES), was obtained using the method for calculating the sliding correlation point by point between E-EES and reference signal; then, the cancellation factor between E-EES and the most correlative reference signal segment was derived by the least square method; at last, the single trial of FVEP was acquired by interference cancellation. By this method, FVEP can be extracted perfectly and the FVEP variability of individual inter-stimulation can be obtained.

6.Hemiarthroplasty with Wagner SL prosthesis for failed internal fixation of intertrochanteric fractures in elderly patients

Objective To investigate the clinical outcomes of hemiarthroplasty with Wagner SL prosthesis in treating failed internal fixation of intertrochanteric fractures of elderly patients.Methods Aretrospective case series study was conducted on the clinical data of 14 patients with failed internal fixation of intertrochanteric fractures admitted between March 2015 to April 2018.There were seven males and seven females with a mean age of 82.2 years (range,75-94 years).The primary internal fixators were proximal femoral nail anti rotation in nine patients,dynamic hip screw in three patients,and proximal femoral locking plate in two patients.The main causes of internal fixation failure were screw loosening in six patients,cutting out of screws in five patients,nonunion in two patients,and plate breakage with nonunion in one patient.Affected limb shortening and coxa varus deformity to various degrees were seen in all the patients before operation.The mean interval from internal fixation to hemiarthroplasty was 6.6 months (range,2-12 months).All patients received hemiarthroplasty using Wagner SL prosthesis,of which nine patients had additional trochanter claw plate with titanium cable fixation and two patients had tension band wire fixation.The operation time,intraoperative,and postoperative complications were recorded.Harris hip scores were measured before operation and at the last follow up to evaluate hip function.Radiographs were performed to assess the fracture healing,prosthesis position,and biological fixation.Results All patients were followed up for 6-30 months (mean,14.5 months).The mean operation time was (108.0 ± 12.4) minutes (range,88-130 minutes).No iatrogenic fracture occurred during the operation.One patient was seen contralateral intertrochanteric fracture during follow-up,and no complications such as periprosthetic fracture,prosthetic sinking,and infection occurred in the 13 patients.The Harris score increased from (26.1-±7.5)points before operation to (83.1 ±4.1)points after operation (P < 0.01).All patients were able to stand and walk,of which four needed crutch assistance.At the last follow-up,X-ray film showed that the prosthesis was well positioned and that all of them reached bony fixation.Fourteen cases of greater trochanteric fracture were healed,with two cases of slight displacement.Conclusions Hemiarthroplasty is an effective remedial treatment for failed internal fixation of intertrochanteric fractures in the elderly,which can improve hip function and enhance the life quality of patients.Wagner SL prosthesis can obtain stable fixation.

Traumatic cerebrospinal fluid leakage commonly presents in traumatic brain injury patients, and it may lead to complications such as meningitis, ventriculitis, brain abscess, subdural hematoma or tension pneumocephalus. When misdiagnosed or inappropriately treated, traumatic cerebrospinal fluid leakage may result in severe complications and may be life-threatening. Some traumatic cerebrospinal fluid leakage has concealed manifestations and is prone to misdiagnosis. Due to different sites and mechanisms of trauma and degree of cerebrospinal fluid leak, treatments for traumatic cerebrospinal fluid leakage varies greatly. Hence, the Craniocerebral Trauma Professional Group of Neurosurgery Branch of Chinese Medical Association and the Neurological Injury Professional Group of Trauma Branch of Chinese Medical Association organized relevant experts to formulate the " Chinese expert consensus on the diagnosis and treatment of traumatic cerebrospinal fluid leakage in adults ( version 2023)" based on existing clinical evidence and experience. The consensus consisted of 16 recommendations, covering the leakage diagnosis, localization, treatments, and intracranial infection prevention, so as to standardize the diagnosis and treatment of traumatic cerebrospinal fluid leakage and improve the overall prognosis of the patients.
