1.Analysis of laboratory critical value reporting in the department of spine surgery and management strategies

Objective To investigate the common critical value (CV) of patients hospitalized in the department of spine surgery and to determine the corresponding management. Methods Medical records of patients between 2013 January and 2014 December were retrospectively reviewed. 47 cases with CV were noted. The following characteristics of CV were recorded and analyzed, including time-point at occurrence, type, reason and management. Results Most of the CVs (43/47, 91.5%) were detected between 8 am to 12 am, and between 17 pm and 21 pm. Overall, there were 24 cases of abnormal blood electrolyte (51.1%), 13 cases of abnormal routine blood test (27.7%), 5 cases of abnormal blood coagulation test (10.6%), and 1 case of abnormal TnT (2.1%). The remaining 4 cases including 2 cases detected in the night (1 case of abnormal blood gas test and 1 case of abnormal TnT) and 2 cases detected in the afternoon (1 case of positive blood bacteria culture and 1 case of low blood glucose level). Conclusions The most common CVs were summarized in the clinical practice. The findings have important clinical implications concerning the management of these CVs. In addition, preventive modality of daily nursing and standardization of medical management should be applied so as to ensure the safety of patients and rapid recovery.
2.Progress in the relation between cleavage and polyadenylation specific factor and tumor

For human solid tumors,the preferred treatment is surgery.However most patients with clinical diagnosis of tumor is already in advanced,and lost the chance of operation.Cleavage and polyadenylation specific factor is involved in mRNA 3' end cleavage and polyadenine (A) tail,and plays an important role in mRNA maturation and intracellular singal transduction.Under certain conditions,cleavage and polyademletion specific factor can lead to abnormal activation of cancer genes,and then cause tumors.Progress in the relation between cleavage and polyademletion specific factor and tumor was reviewed in the paper.
3.Study on endoscopic nasopancreatic drainage in the prevention of post-endoscopic retrograde cholangiopancreatography pancreatitis

Objective To explore the value of endoscopic nasopancreatic drainage (NPD) in the prevention of post-endoscopic retrograde cholangiopancreatography pancreatitis (PEP) in a high-risk population.Methods From April 2009 to February 2012,a total of 105 cases of PEP high risk population were collected and divided into non NPD group (n=58) and NPD group (n=47) according to the situation during the endoscopic retrograde cholangiopancreatography(ERCP) operation.NPD was placed in NPD group,the head of the drainage passed the neck of pancreas under X-ray.Nasobiliary drainage or no drainage was assigned to the group of non NPD group according to the situation during operation.x2 test and t-test were performed for comparing and analyzing the clinical data before and post operation,the incidence and risk ratio of PEP between the two groups.Results Twenty-four hours after ERCP,the mean level of serum amylase at 24 h (t=2.419),the degree of abdominal pain (t3h=2.585,t24h=7.236) and relief time (t=4.996) of NPD group were better than those of non NPD group (all P<0.05).The incidence of PEP in NPD group was 6.7%(3/45) and non NPD group was 29.3 % (17/58),the incidence of PEP in the NPD group was significantly lower than that in the non NPD group (x2 =8.304,P<0.01),and the relative risk was 0.172 (95%CI:0.047 to 0.632,P<0.01).Conclusion The regular placement of endoscopic NPD is a practical,effective and easy way to prevent PEP in high risk population.
4.The effect of Shugan-Jieyu capsule combined with pramipexole in treatment of parkinson's disease depression

Objective Evaluating the effect of Shugan-Jieyu capsule combined with pramipexole on depressive symptoms in parkinson's disease(PD) patients.MethodsAccording to the criteria of including and excluding, we collected 63 PD depression cases who visited doctors at neurological outpatient room between June 2014 and April 2016.35 cases only treated with pramipexole, designed as pramipexole group, and 28 cases treated with pramipexole and Shugan-Jieyu capsule, designed as combined treatment group.The Hamilton depression scale(HAMD), activities of daily living scale(ADL) and unified Parkinson's disease rating scale-part III(UPDRS-III) were assessed at baseling, week 6 and week 9.ResultsAt week 9, HAMD was (17.71±6.78) in pramipexole group versus (14.32±4.81) in combined treatment group, there was significant difference between the two groups(P=0.029), As for HAMDΔ9, analysis did show a significant difference(pramipexole group (-11.06±3.27) vs combined group (-14.93±6.24);P=0.002).Analysis showed a significant difference as for ADLΔ6(pramipexole group (-2.37±1.31) vs combined group (-3.57±2.04);P=0.006) at week 6.There was no significant difference between groups for the reduction in total UPDRS-III scale.No serious adverse event was recorded during treatment of depression.ConclusionShugan-Jieyu capsule combined with pramipexole did have significant effects versus pramipexole on depressive symptoms in PD patients.
5.Expressions and prognostic value of metadherin, E-cadherin, and β-catenin in patients with hepatocellular carcinoma

Objective To study the expressions of metadherin (MTDH),E-cadherin and β-catenin in the tissues of hepatocellular carcinoma (HCC),to investigate the relationship between them and the clinical-pathological features,and to evaluate the prognostic values after surgical resection for hepatocellular carcinoma.Methods The protein expressions of MTDH,E-cadherin and β-catenin were studied by immunohistochemistry in tumor tissues of 107 HCC patients who underwent curative surgical resection.The data were correlated with the clinical-pathological data,tumor free time and recurrence rate.Results Positive expression of MTDH and nuclear β-catenin accumulation were correlated with the Edmondson grade (P<0.05) and decreased E-cadherin expression was correlated with the preoperative serum level of α-fetoprotein (AFP) (P<0.05).All these expressions were associated with a shorter tumor-free survival and a higher recurrence rate (P<0.05).Positive MTDH expression was correlated with decreased E-cadherin expression and nuclear β-catenin accumulation (P<0.05).On Cox regression analysis,MTDH was an independent risk factor of tumour recurrence (RR=3.431,CI=1.254~ 7.318).Conclusions Positive MTDH expression,decreased E-cadherin expression,and nuclear β-catenin accumulation indicated a higher recurrence rate after curative surgical resection for HCC.MTDH was an independent risk factor of recurrence.
6.Analysis on Application of Antimicrobials in Hepatic Disease Inpatients in Our Hospital from Mar. 2007 to Mar. 2008

OBJECTIVE:To investigate the use of antimicrobials in hepatic disease inpatients. METHODS:In retrospective study,200 cases of hepatic disease in our hospital from Mar. 2007 to Mar. 2008 were randomly collected and utilization of antimicrobials was analyzed. RESULTS:136 inpatients treated with antimicrobials accounted for 68.00%. 76.47% were injected by intravenous. 35 cases were performed bacteria culture (25.74%). 7 categories (24 kinds) of antimicrobials were used. The DUI of top 10 antimicrobials in the list of DDD were below 1,which indicated drug use was basically appropriate. CONCLUSION:Great importance should be attached to application management of antimicrobials in patients with hepatic disease in our hospital to reduce irrational use of drug.
7.The study on the diagnostic and prognostic value of detecting K-ras mutations in plasma of patients with pancreatic cancer by a novel nano capture probe

Objective To detect K-ras mutations in plasma by a nano capture probe system , and to explore the diagnostic and prognostic value of this method for patients with pancreatic cancer .Methods The clinical data of 62 patients with pancreatic cancer , 38 patients with benign pancreatic diseases and 31 healthy controls admitted in the Second Affiliated Hospital of Jiaxing Medical College from June 2013 to June 2015 were collected.The diagnosis of all the patients were confirmed by pathology .The DNA were extracted from all plasma samples and were detected for the codon 12 and 13 mutation of K-ras gene by nano capture probe and conventional PCR plus direct sequencing .The correlation of K-ras gene mutation with certain clinical data and the diagnostic and prognostic value in pancreatic cancer were analyzed .Results The K-ras mutation were detected by nano capture probe in 27 pancreatic cancer patients , and the mutation rate was 43.5%(27/62), including 25 cases with codon 12 mutation and 2 cases with codon 13 mutation .The K-ras mutation rate in patients with benign pancreatic diseases was 7.9%(3/38), which were all in codon 12.K-ras mutation was detected in only 17 pancreatic cancer patients by conventional PCR plus direct sequencing , and the mutation rate was 27.4%(17/62), The K-ras mutation rate of benign pancreatic diseases was 5.3%(2/38).The mutation rate detected by nano capture probe was higher than that by conventional PCR , and the difference was statistically significant (P=0.006).K-ras mutation in the plasma of patients with pancreatic cancer was related to TMN stage and liver metastasis , but there was no correlation of the factors such as sex , age, clinical symptoms, tumor size, serum CA19-9 and CEA levels with K-ras mutation.The sensitivity of K-ras gene mutation for diagnosing pancreatic cancer was 43.5%, the specificity was 92.1%, the positive predictive value was 90%, the negative predictive value was 50%, Youden index was 0.356.The 1-year survival rate of patients with K-ras mutation was 44.4%, which was lower than that (71.4%) of patients with wild-type K-ras, and the difference was statistically significant (P<0.05).Conclusions The nano capture probe system could certainly detect K-ras mutation in a small quantity of plasma DNA , and its diagnosis sensitivity for pancreatic cancer is low , but the specificity is relatively high .K-ras mutation in plasma is closely related to the TMN stage and prognosis of pancreatic cancer .
8.Prokaryotic expression and purification of GST-NRP-1 fusion protein

Objective:To construct GST-tagged human NRP-1 fusion protein expression vector and induce its expression in Escherichia coli ( E.coli) ,then carry on inclusion body refolding and purification so as to obtain GST-NRP-1 fusion protein.Methods:NRP-1 gene was amplified by RT-PCR and inserted into pCR-blunt vector.Then the reconstructed plasmid was inserted into prokaryotic expression vector pGEX-4T-1.The constructed pGEX-4T-1-NRP-1 expression vector was transformed into BL21 cells and induced by i-sopropyl-β-D-thiogalactoside ( IPTG).Bacterial bodies were disrupted by sonication.Then the soluble fraction of fusion proteins were verified by Western blot and purified by Glutathione Sepharose 4B after inclusion body refolding.Results: The NRP-1 gene fragment was amplified by RT-PCR and inserted into pCR-blunt vector.Fusion protein expression vector pGEX-4T-1-NRP-1 was constructed suc-cessfully.After transformation, GST-NRP-1 expression vector was detected in BL21 cells and obtained purifying protein after refolding.Conclusion:The plasmid GST-NRP-1 was constructed successfully and laid basis for subsequent studies.
9.Producing and quality control of fecal-derived microbiota enteric capsules

Objective To initially explore the feasibility and quality control of producing fecal-derived microbiota enteric capsules.Methods Fecal-derived microbiota was put into double layered enteric capsules.The bacteria colony numbers of fresh prepared glycerol containing fecal-derived microbiota liquid,glycerol containing and glycerol free fecal-derived microbiota after stored at -80 ℃ for 72 h were counted with standard plate count methods in order to investigate the stability after frozen.Methylene blue was taken as the standard,resistance to acid and release rate of capsules was evaluated.The t test was performed for statistical analysis.Results The preparation process of double layered microbiota capsules was simple and practicable.The data of 12 plates of each microbiota were acquired.The number of bacteria colony of fresh prepared glycerol containing fecal-derived microbiota ((5.08 ±1.37)×107 colony-forming units (cfu)/mL)was significantly more than that of the group without glycerin ((1.73±0.64)×107 cfu/mL)at -80 ℃for 72 h (t = 7.621 ,P <0.01).There was no significant difference in the number of bacteria colony between glycerin containing frozen fecal-derived microbiota ((4.67±1 .56)×10 7 cfu/mL)and fresh fecal-derived microbiota (t = 0.694,P = 0.495).Regression equations were achieved with fecal-derived microbiota containing methylene blue,and there was a good linear relation between 0.5 μg/mL and 8.0 μg/mL.Three fecal-derived microbiota enteric capsules containing methylene blue were prepared.Their resistance to acid was 96.0%,99.1 %,and 95 .5 %,while the release rate was 88.6%,95 .1 % and 86.5 %, respectively.All met the requirement of Chinese Pharmacopoeia to enteric capsules.Conclusion The preparation of double layered fecal-derived microbiota enteric capsules had feasible technology and stable quality,which could provide reference in prevention and treatment of diseases related with colonic microbiota imbalance.
10. Expression of B cell transposition gene 3 in pancreatic ductal adenocarcinoma and its prognostic value

Objective:
To detect the expression of B cell transposition gene 3(BTG3) in pancreatic ductal adenocarcinoma(PDAC), and explore its relationship with postoperative recurrence and metastasis of tumor.
Methods:
Six self-paired frozen PDAC specimens and 3 normal pancreatic tissues from the Second Hospital of Jiaxing Affiliated to Jiaxing University were collected and the expression of BTG3 was detected by qPCR. Ten normal pancreatic tissues and 52 cases of PDAC tumor and paracarcinomatous tissues from the Second Hospital of Jiaxing Affiliated to Jiaxing University were collected from June 2009 to December 2016. The expression of BTG3 and relationship among BTG3 and clinicopathological characteristics of PDAC and patients′ prognosis were detected and analyzed using immunohistochemistry.χ2 test, Kaplan-Meier method and Cox regression model were used to analyzed the data.
Results:
The results of qPCR showed that expression level of BTG3 in PDAC (0.63±0.17) was lower significantly than that in paracarcinomatous (0.96±0.04) and normal tissues (1.00)(
