1.CT-guided percutaneous transthoracic aspiration biopsy of lung lesions: factors influencing the diagnostic accuracy

Objective To investigate the factors influencing the diagnostic accuracy in CT-guided percutaneous transthoracic aspiration biopsy of lung lesions. Methods CT-guided percutaneous transthoracic biopsy was performed in 435 patients with surgical pathologic confirmation or clinical follow-up.Univariant statistical analysis and multivariant stepwise Logistic regression analysis were used to study the influence of lesion-related factors(benignancy or malignancy,size,location,depth,presence of cavity),procedure factors(laser-guidence,position of patient) and patient-related factors(sex,age,presence of emphysema);with the accuracy of the diagnosis. Results ① 289 lesions were diagnosed as malignant ones and 146 as benign ones.The accuracy of CT-guided biopsy was(83.4%)(363/435).Univariant analysis showed that the diagnostic accuracies for malignant and benign lesions were(88.9%)(257/289) and(72.6%)(106/146),respectively(?~2=(18.71),P=(0.00002)).Laser-group was superior to non-laser group((88.4%) versus(80.9%),respectively,?~2=(4.00),P=(0.0456)).Mean diameters of lesions in correct diagnostic group and non-correct diagnostic group were(3.78)?(1.64) cm and(3.02)?(1.26) cm,respectively((F=)(13.79),P=(0.0002)).②Multivariant stepwise Logistic regression analysis showed that among the various factors influencing the final benign or malignant diagnoses(Wald ?~2=(14.01),P=(0.0002)) and using laser-guidence(Wald ?~2=(3.92),P=(0.0477)) were significantly associated with the diagnostic accuracy. Conclusions Final correct diagnoses(benign,malignant) are closely related to the application of laser-guidence for determining the diagnostic accuracy in CT-guided transcutaneous thoracic needle aspiration biopsy.
2.CT-guided Percutaneous Transthoracic Aspiration Biopsy for Pulmonary Lesions:Comparison between Lesion Size and Depth

Objective To evaluate the influence of lesion size and depth on the diagnostic accuracy and the rate of pneumothorax in patients undergone CT-guided transthoracic aspiration biopsy for pulmonary lesions.Methods CT-guided percutaneous transthoracic biopsy was performed in 398 patients that had surgical resection or clinical follow-up.According to lesion size,lesions were divided into≤2 cm,2~4 cm and ≥4 cm.Lesion depth was measured from the pleura surface to the edge of the lesion.This depth was classified in three categories:≤1 cm,1~3 cm and ≥3 cm.Comparison of the diagnostic accuracy and the frequency of pneumothorax in different size and depth of lesion was performed.Results(1)The overall diagnostic accuracy of CT-guided percutaneous needle aspiration biopsy was 83.2%(331/398).Pneumothorax occurred in 58(14.6%)of 398 biopsy;(2)The diagnostic accurate rate in different size of lesion was 73.3%(44/60),81.5%(176/216)and 91.0%(111/122)respectively,?2=9.91,P0.05.The occurred rate of pneumothorax in different depth lesion was 2.8%(4/141),14.8%(21/142)and 28.7%(33/115),?2=34.03,P

4.The therapeutic effect of sertraline combined with brain function treatment for depressive episodes

Objective To observe the therapeutic effect of sertraline combined with brain function treatment instrument for depressive episodes.Methods According to sequence of hospitalization,80 patients with depression without severe physical disease randomly divided into sertraline with brain function treatment group (40 cases,group A) and only take sertraline group (40 cases,group B),and two groups were given Sertraline 100 mg/day for 8 weeks.24 Hamilton Depression Scale (HAMD-24) and 20 Self-rating Depression Scale (SDS-20) were assessed before and after 1,2,4,6,8 weekend treatment.Results The scores of HAMD-24 and SDS-20 were not significant difference between the two groups before treatment(P>0.05),but there were statistically significant after treatment (P<0.05).They were significantly decreased after 8 weeks treatment in group A and were decreased slightly in group B with HAMD-24 and SDS-20.Clinical efficiencies of HAMD of the two groups were 87.2 %,76.3 %,respectively,and those of SDS were 89.7 %,78.9 %,respectively.Clinical efficacy in group A was significantly better than that in group B (P<0.05).Conclusion Sertraline with brain function treatment instrument can shorten the onset time and improve its effect.
5.Connotation and Extension Discussion of Marine Traditional Chinese Medicine

Marine traditional Chinese medicine (MTCM) is an important part of Chinese medicine (CM),there are some differences in understanding of the current literature and the extension of the connotation of marine medicine,which leads to the definition dispute of MTCM,hindering clinical application and further development of MTCM.In this study,we explored the concept of MTCM in literature,discussed the attributes of ocean marine CM,summed up the differences between the land CM and MTCM over variety characteristics,effect of drug composition characteristics and biological activity characteristics,and discussed the connotation and extension of MTCM from three aspects of theoretical basis and the effect and source of the drug,leading to the formation of the narrow and broad concept of MTCM.The five kinds of disputes in the definition of MTCM were discriminated according to the concept and connotation,which provided a theoretical basis for the definition and the research of MTCM.Moreover,we also defined the English translation and its abbreviation as Marine Traditional Chinese Medicine (MTCM).
6.β-arrestin1 mediates the effect of MK-801 on levodopa-induced dyskinesia in Parkinson ', s disease

ObjectiveTo investigate the effect of MK-801 on levodopa-induced dyskinesia (LID)in Parkinson' s disease (PD). MethodsRat models ( n = 25) of Parkinsonism related motor complications were established and were randomly divided into levodopa-induced dyskinesia (LID) group (n = 10), MK801 treatment group (n = 10) and PD group (n =5). Another 5 rats were served as control group. The behaviors of LID rats treated with MK-801 were observed. Immunohistochemistry and Western blot analysis were used to determine the expression of β-arrestin1 in the striate of rats.Results After MK-801 treatment, abnormalinvoluntarymovementscores and peakturning weredecreasedin LIDrats.Immunohistochemistry showed that β-arrestin1-positive cells of the lesioned side ((2. 95 ± 0. 44) × 104) in LID rats were decreased compared to the contralateral side ( ( 3.78 ± 0. 37 ) × 104, t = 5. 415, P < 0. 05 ).Western blot showed that the levels of β-arrestinl in PD group ( presented as lesioned side/contralateral side) were ( 81.02% ± 2. 23% ). The levels of β-arrestin1 (64. 88% ± 3. 10% ) were deceased in LID rats compared to PD rats ( t = 9.47, P < 0. 01 ). However, the levels of β-arrestin1 ( 89. 26% ± 1.90% )were increased in MK-801-treated rats (t = 14. 82, P <0. 01). ConclusionsMK-801 reduces LID in PD rats. The beneficial effect of MK-801 may be mediated through the increased expression of β-arrestinl which in turninhibits the overactivation of glutamate receptors.
7.CT-guided percutaneous biopsy of malignant musculoskeletai tumors: an analysis of its diagnostic accuracy

Objective To evaluate CT-guided pereutaneous biopsy and fine needle aspiration in diagnosing malignant musculoskeletal tumors. Methods CT-guided percutaneous biopsy and fine needle aspiration was performed in 106 cases with suspected musculoskeletal tumor. The pathological results obtained from biopsy specimens were compared with clinical final pathological diagnoses. Results All 106 cases were finally diagnosed as suffering from malignant musculoskeletal tumors, which were proved by operation and incisional biopsy. Of 106 cases receiving CT-guided percutaneous biopsy and needle aspiration, the pathologic diagnosis obtained from this procedure was consistent with the clinical final pathological diagnosis in 89 and was "negative" in 17, with an accuracy rate of 84.0%. Conclusion CT-guided percutaneous biopsy and fine needle aspiration is a safe, simple and effective technique for the diagnosis of malignant musculoskeletal tumors, Incisional biopsy may be necessary when this procedure gives "negative" result.
8.The safety and efficiency of precise liver resection for patients with primary liver cancer

Objective To evaluate the the safety and efficiency of precise liver resection for patients with primary liver cancer. Methods 86 patients with primary liver cancer were randomized to receive conventional routine hepatectomy (n=44) or the precise liver resection (n=42). Outcomes were compared between the precise hepatectomy group and the routine group, including, the blood loss, operation time, morbidity and mortality. Results There were significant differences in morbidity rates (7.1% vs. 20.5%; P<0.001), the blood loss [(320±315) ml vs. (613±526) ml;P<0.001) , postoperative alanine aminotransferase (ALT) value (in postoperation 7 d, 82.7 U/L vs.321.7 U/L; P<0.001) and length of hospital stay (12.3 d vs. 18.6 d; P<0.001) between precise hepatectomy and routine groups. The 1 year tumor recurrence rate and 1 year survival rate were 26.2%(11/42) and 78.6% (33/42) in precise liver resection group, 38.6% (17/44) and 65.9%(29/44) in routine liver resection group,with significant difference (P=0.010;P=0.018). Conclusion Precise liver resection is safe and effective in the treatment of liver tumor without much injury to patients.
9.Lowering the rate of perfluorochemical oxygen carrier solution to preserve liver by cold machine perfusion

OBJECTIVE: To explore the association between low flow rate and reperfusion injury during the process of preserving liver by cold machine perfusing perfluorochemical oxygen carder solution. METHODS: Forty-four male adult Wister rats were randomly divided into normal, control, experimental Ⅰ and experimental Ⅱ groups. In the normal group, the removed liver was performed isolated reperfusion guided by Clavien method. In the other 3 groups, the removed liver was infused through the portal vein by the 4-5 ℃ conserved perfluorochemical oxygen carder solution. The infused rate was controlled at 0.4, 0.2, 0.1 mL/(min·g) with 18 hours perfusion, After that, isolated reperfusion was performed. The activity of aspartate aminotransferase, alanine transaminase and endotheiin mass concentration of the effluent was detected at minutes 10, 30, and 60 after reperfusion. The histopathological changes of liver under light and election microscopy were also observed. RESULTS: The activity of aspartate aminotransferase, alanine transaminase and endothelin mass concentration had no remarkable differences between the experimental Ⅰ group and control group (P > 0.05). The differences between the experimental Ⅱ group and control group were remarkable (P < 0.05). The light and electron microscopy showed that the histopathological changes of liver in the control and experimental Ⅰ groups was lightener than experimental Ⅱ group. CONCLUSION: During the process of preserving liver by cold machine perfusion, the rate of 0.1 ml/(min·g) perfluorochemical oxygen carder solution increase the injury of hepatocyte and sinusoidal endothelial cells, which eventually result in severity of reperfusion injury.
10.Clinical study on chronic pain of Parkinson' s disease

Objective To assess the prevalance and distribution of pain in Parkinson' s disease (PD) patients, and to describe the relationship between pain and development of disease and its impact on the quality of life. Methods 113 PD patients were assessed with Visual Analog Scale (VAS), Brief Pain Inventory (BPI), Unified Parkinson' s Disease Rating Scale (UPDRS), Hoehn-Yahr Scale (H-Y), Hamilton Anxiety and Depression Scale (HRSD, HAMA) and Pittsburgh Sleep Quality Index (PSQI). Patients were divided into no pain group, PD-pain group and non-PD-pain group. PD-pain group was divided into PD-pain direct group and PD-pain indirect group. And statistical analysis was performed for each group. Results The incidence of PD-pain was 42. 5% (48/113). And compared with no pain group, PD-pain group had earlier age at onset of disease, longer duration, higher HAMA, HRSD, UPDRS and H-Y score, higher daily L-dopa dose, with statistical significance. Compared with non-PD pain group, the differences in the onset of age (57.4±9.6 vs 65.9±8.7, t= -2.596, P =0.012), HRSD(12. 9±7.9 vs 8.7±3.7, t =2.605, P=0.014) and VAS (61.6±25.9 vs 38.0 ± 30. 1, U = -2.290, P =0.022) were also statistically significant. BPI was different between PD-pain direct group and PD-pain indirect group except normal work and relations with other people. Conclusion Pain is frequent in Parkinsonian population. Most pain was chronic and related to PD (PD-pain). These patients always have earlier onset age and poorer quality of life.
