1.The value of NSE, S100B and MBP in evaluating the severity and prognosis in patients with subarachnoid hemorrhage

Objective To research the significance of the levels of neuron-specific enolase(NSE), S100B and myelin basic protein(MBP) in predicting the severity and prognosis in patients with subarachnoid hemorrhage (SAH). Methods The serum levels ofNSE, SI00B and MBPwere measured within 72 hours after the injury in 36 patients with SAH(SAH group), then the severity of illness and prognosis was evaluated by world federation of neurosurgical societies(WFNS) grade and Glasgow outcome scale(GOS) respectively. Twenty healthy persons were selected as controls(control group). Results After the injury, the serum levels of NSE,SI00B and MBP in SAH group increased significantly compared with those in control group. Moreover, there were significant difference in the serum levels of NSE, S100B and MBP among different groups in different severity of illness and prognosis. Assessed the severity and prognosis by serum levels of NSE 25 μg/L, S100B 1.2 μg/L and MBP 10.0 μg/L after 72 hours of SAH, the specificity of assessing the severity by NSE, SI00B and MBP was 71.43%,61.90%, 66.67%, and the sensitivity was 73.33%,86.62% and 73.33%. At assessment of prognosis,the specificity of NSE, S100B, MBP was 69.57%, 60.87%,65.22%, and the sensitivity was 61.54%,76.92% and 69.23%. Conclusion There are higher specificity and sensitivity in evaluating the severity of illnoss and prognosis in patients with SAH by serum levels of NSE, S100B and MBP.
2.Pedigree screening of a Chinese multiple endocrine neoplasia type 2A family and prophylactic total thy-roidectomy

Objective To explore the clinical significance of integrated screening of RET in a Chinese multiple endocrine neoplasia type 2A(MEN 2A)family and to evaluate the feasibility and effectiveness of prophy-lactic total thyroidectomy to MEN 2A-related medullary thyroid carcinoma ( MTC).Methods Medical history was obtained from 10 family members in a 3-generation south China family .Systemic investigations including bio-chemical tests, imaging examinations and germline RET screening were performed .3 asymptomatic mutation car-riers underwent prophylactic total thyroidectomy .Results RET screening showed a heterozygous missense muta-tion of TGC to CGC at codon 634 on exon 11 in 6 members(p.C634R), which was completely consistent with the clinical manifestations.There were 4 males and 2 females.The initial mean diagnostic age of 33.5 years(ranging from 19 years to 65 years) and the mean maximum diameter of MTC was 2.3 cm(ranging from 0.7 cm to 5.2 cm). Among them 3 members had palpable neck masses (1 case with diarrhea).Right total thyroidectomy +right level Ⅵlymph-node dissection with modified right neck dissection in one case , and bilateral total thyroidectomy +bilat-eral level Ⅵlymph-node dissection in 2 were performed .In other 3 asymptomatic mutation carriers , prophylactic total thyroidectomy +bilateral level Ⅵ lymph-node dissection were also performed .Among them, 1 case of a-symptomatic pheochromocytom ( PHEO) underwent cortical-sparing adrenalectomy before MTC .After the first op-eration, 4 patients still presented a high value of calcitonin , among whom 1 patient( T3N 1bM 0-1) underwent re-operation for 3 times after the initial operation and presented metastasis to bone after 130 months, taking vandet-anib orally up to now;2 patients underwent reoperation at 6 and 7 months after initial operation respectively (T1N 1bM0 and T2N 1bM0), and the other one patient was closely monitored and followed up for 22 months(T2N 1b M0).Moreover, The calcitonin levels dropped to normal in the other 2 asymptomatic cases(T1N0M0) who were followed up for 20 months.Conclusions Pedigree screening can work up an early diagnosis and improve the prognosis of MEN 2A.Integrated screening of RET and pre-operative calcitonin level measurement and prophylac-tic thyroidectomy for asymptomatic RET mutation carriers are reasonable and effective .
3.Current status and perspectives of chimeric antigen receptor modified T cells for cancer treatment.

Chimeric antigen receptor (CAR) is a recombinant immunoreceptor combining an antibody-derived targeting fragment with signaling domains capable of activating cells, which endows T cells with the ability to recognize tumor-associated surface antigens independent of the expression of major histocompatibility complex (MHC) molecules. Recent early-phase clinical trials of CAR-modified T (CAR-T) cells for relapsed or refractory B cell malignancies have demonstrated promising results (that is, anti-CD19 CAR-T in B cell acute lymphoblastic leukemia (B-ALL)). Given this success, broadening the clinical experience of CAR-T cell therapy beyond hematological malignancies has been actively investigated. Here we discuss the basic design of CAR and review the clinical results from the studies of CAR-T cells in B cell leukemia and lymphoma, and several solid tumors. We additionally discuss the major challenges in the further development and strategies for increasing anti-tumor activity and safety, as well as for successful commercial translation.

4.Clinical Efficacy and Bone Turnover Markers of Buzhong Yiqitang in Treatment of Senile Osteoporotic Vertebral Compression Fracture After Operation Based on ''Spleen Governing Muscle''

ObjectiveTo study the effect on quality of life and the bone turnover markers of Buzhong Yiqitang in the treatment of senile osteoporotic vertebral compression fracture (OVCF, syndrome of Qi deficiency in spleen and stomach) after operation based on ''spleen governing muscle''. MethodA total of 135 senile patients with OVCF treated by percutaneous kyphoplasty in Rizhao Hospital of Traditional Chinese Medicine from January 2020 to January 2021 were enrolled in this study. They were randomly assigned to two groups on the basis of block randomization at a ratio of 2∶1 (90 cases in the observation group and 45 cases in the control group). Both groups were administrated with calcitriol capsules (0.5 μg·d-1) and caltrate D (1 200 mg·d-1) for basic treatment of osteoporosis. The observation group was additionally treated with Buzhong Yiqitang. Bone mineral density (BMD), procollagen type Ⅰ N-terminal propeptide (PINP), osteocalcin (OST), β cross-linked C-telopeptide of type 1 collagen (β-CTx), appendicular skeletal muscle mass index (ASMI), and quadriceps muscle strength were compared between the two groups before and 6, 12 months after treatment. Additionally, traditional Chinese medicine (TCM) symptom score and visual analogue score (VAS) before and 3, 6 months after treatment, as well as quality of life questionnaire of the European Foundation for osteoporosis (QUALEFFO) score before and 3, 6, 12 months after treatment, were compared between the two groups. ResultA total of 85 patients in the observation group and 41 patients in the control group were followed up. The general curative effect of the observation group was better than that of the control group (χ2=10.503, P<0.05). Specifically, the observation group had higher PINP, BMD, ASMI, and quadriceps muscle strength but lower β-CTx, TCM symptom score, VAS, and QUALEFFO score than the control group (P<0.05, P<0.01). No adverse reactions related to Buzhong Yiqitang were observed. ConclusionBuzhong Yiqitang can regulate bone metabolism indexes, promote osteogenesis, increase bone density, enhance skeleton appendiculare and quadriceps muscle strength, relieve clinical symptoms, and improve quality of life in patients with senile OVCF (syndrome of Qi deficiency in spleen and stomach), being worthy of promotion in clinical application.
