1.Investigation of the correlative factors for osteoporosis among senile patients with chronic diseases

Objective To investigate the correlative factors for osteoporosis among senile patients with multiple cardiovascular diseases and type 2 diabetes mellitus.Methods 206 patients were divided into osteoporosis (OP) group and non-osteoporosis (non-OP) group according to the diagnosis standards of osteoporosis.The general data were recorded.Clinical history of all kinds of cardiovascular diseases and type 2 diabetes mellitus were detected.Serum levels of hemoglobin (Hb),glucose (Glu),glucosylated hemoglobin (HbA1c),alkaline phosphatase (ALP),total serum protein (TP),albumin (ALB),BUN,creatinine (Cr),calcium (Ca),phosphonium (P),total cholesterol (TC),triglyceride (TG),high-density lipoprotein cholesterol (HDL-C),low-density lipoprotein cholesterol (LDL-C) were detected.Dual-energy Xray absorptiometry was used to detect the bone mineral density (BMD) of bilateral femoral neck and lumbar spine.The related factors of BMD were analyzed.Results The morbidity of osteoporosis was much higher in elderly females than in elderly males (58.7% vs.38.6%).The osteoporosis patients were older than non-osteoporosis patients in both males and females,although there was no significant difference.The body mass index (BMI) of both gender was much lower in osteoporosis group than in non-osteoporosis group (P<0.05).In male patients,BMD was positively correlated with BMI significantly (P<0.01).In female patients,TC and LDL C were higher in OP group than in non-OP group (P<0.05).In female patients,BMD was positively correlated with BMI,and negatively correlated with TC and LDL-C.Logistic regression analysis showed that the lower BMI was an independent risk factor for osteoporosis in male patients; BMI and TC were independent risk factors for osteoporosis in female patients.Conclusions In both male and female,senile patients with lower BMI,multiple cardiovascular diseases and type 2 diabetes mellitus have higher risks for osteoporosis.In senile female patients,TC may be correlated with BMD.TC should be reduced to decrease the risks of cardiovascular diseases and osteoporosisin senile females.

4.Influencing factors of arterial stiffness in the elderly patients with hypertension and type 2 diabetes mellitus

ObjectiveTo evaluate arterial stiffness and its influencing factors in the elderly patients with hypertension and (or) type 2 diabetes mellitus.MethodsTotally 320 elderly patients were divided into four groups: control,diabetes mellitus,hypertension,coexisting hypertension and diabetes mellitus.Arterial stiffness in ankle and brachial artery was determined by brachial-ankle pulse wave velocity (baPWV).Other factors affecting baPWV were also collected and measured,including age,body mass index (BMI),sex,smoking,blood pressure,pulse pressure,mean arterial pressure,serum fasting blood glucose,lipid profile,glycosylated hemoglobin and hypersensitive C-reactive protein (hsCRP).ResultsbaPWV in the four groups were ( 1667.2 ± 279.3 )cm/s,(2158.6 ±386.9)cm/s,(1881.2±383.8) cm/s and (2165.9± 479.9)cm/s,respectively.The baPWV value was higher in groups of hypertension,diabetes mellitus,and the coexisting diseases than in controls (F=8.473,P<0.05).Diabctic patients had a higher baPWV levels than those without diabetes.There was no difference in baPWV levels between diabetic patients with or without hypertension.Pulse pressure and hsCRP were higher in coexisting diseases group than in control group (P<0.05).Stepwise backward regression analysis showed that fasting blood glucose,pulse pressure,mean blood pressure,hsCRP and low density lipoprotein cholesterin (LDL-C) were positively related to baPWV P <0.05). ConclusionsHypertension and (or) type 2 diabetes mellitus are important influencing factors for higher arterial stiffness. Hyperglycemia might play more important role in the elderly arterial stiffness.Serum fasting blood glucose,pulse pressure,mean blood pressure,hsCRP,LDL-C may be the independent risk factors of arterial stiffness.
5.Detection of COX-2 and its relationship with MVD in colorectal carcinoma tissue

Objective To investigate the expression of COX-2 and angiogenesis in colorectal carcinoma tissue.Methods The expressions of COX-2 and MVD of 88 cases of colorectal carcinoma and 20cases of control tissue were examined by immunohistochemical staining. Results Compared with normal control,positive expression rates of COX-2(3/20 vs 64/88),and CD34(7/20 vs 74/88)of colorectal carcinoma tissue increased significantly(P<0.05).MVD in COX-2 positive expressed tissue(63.24-20.4)was higher than that in COX-2 negative expressed tissue(41.2±29.8)(P<0.01).Conclusion COX-2 is overexpressed in colorectal carcinoma tissue,which may have a strong relationship with MVD.
6.Effects of Emodin on Expression of Pneumonic Cytokines in Rats with Acute Necrotic Pancreatitis

Objective To investigate whether emodin(1,3,8-trihydroxy-6-methylanthraquinone) can attenuate inflammatory response in rats' lungs with acute necrotic pancreatitis (ANP). Method Acute necrotic pancreatitis model was induced by injection of 3% sodium taurocholic acid into the subcapsular of pancreas and emodin was administered by intestine perfusion. Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis was measured together with the detection of IL-1β, IL-6 and IL-10 mRNA expressions in rats' lungs by semi-quantitative RT-PCR. Immunohistochemistry was detected the expression of IL-1β converting enzyme (ICE) in the rats' lungs. Result Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis groups are obviously high as compared with normal group(P<0.05). Plasma amylase was (1 611.20±218.72)IU/L in normal group. Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis groups were (1 981.40±56.81)IU/L, (3 287.40±612.37)IU/L and (4 914.60±746.82)IU/L. Plasma amylase of 3, 6 and 12 h with acute necrotic pancreatitis after treatment with emodin groups were obviously low as compared with acute necrotic pancreatitis groups. The plasma amylase was(1 617.20±136.80)IU/L,(2 323.40±318.19) IU/L and (2 670.20±390.03)IU/L respectively. The study showed that the mRNA expression of pro-inflammatory cytokin IL-1β and the expression of IL-6, as well as the expression of IL-1β converting enzyme(ICE) were decreased and IL-10 was increased. Conclusion The study demonstrates that emodin plays an important role in reducing plasma amylase level. Emodin exerts anti-inflammatory effects in acute necrotic pancreatitis rats' lungs by downregulating the mRNA expression of IL-1β and IL-6 and upregulating the mRNA expression of IL-10.
7.Association between activity of hand, foot and mouth disease and meteorological factors in Suzhou

Objective To study the association between activity of hand,foot and mouth disease (HFMD) and the meteorological factors in Suzhou.Methods A total of 17 653 children diagnosed with HFMD in Children's Hospital Affiliated to Suzhou University during 2008 to 2011 were enrolled.The meteorological factors in(c)luding mean temperature,relative humidity,rainfall amount,sum of sunshine and mean wind velocity during Jan 2008 to Dec 2011 were collected monthly.Numeration data were analyzed using chi square or Fisher exact test.Normal distribution data were analyzed using Pearson correlation and non-normal distribution data were analyzed using Spearman rank correlation.Results HFMD could be found all over the year and had obvious seasonality which peaked in Summer,followed by Spring and Autumn and HFMD decreased markedly in Winter.The activity of HFMD was positive correlated with mean temperature(r=0.716,P<0.01),wind velocity (r=0.630,P<0.01),rainfall(r=0.477,P<0.01) and sum of sunshine(r=0.311,P<0.05).No statistical significance was observed between relative humidity and HFMD(r =0.276,P>0.05).Multivariate stepwise regression analysis showed that only mean temperature and rainfall were associated with HFMD and mean temperature had stronger influence on HFMD than rainfall (t =4.687,P < 0.01 ; t =2.258,P < 0.05).Conclusions HFMD has obvious seasonality.Mean temperature and rainfall are the main factors affecting this seasonality.
8.Efficacy of transforaminal endoscopic nerve root decompression in the treatment of degenerative lumbar spinal stenosis

Objective:To evaluate the feasibility of transforaminal endoscopic nerve root decompression for degenerative lumbar spinal stenosis (DLSS).Methods: From July 2011 to April 2016,96 cases of single segment DLSS were involved.All the patients had unilateral lower extremity neurological symptoms,signs,neurogenic intermittent claudication of less than 500 m.Imaging examinations (CT or MRI) or diagnostic nerve root block confirmed single segment degeneration.The mean age was (71.6±5.4) years,male: 55 cases,female: 41 cases.Their intraoperative blood loss,operation time,complications,ambulation time and discharge time were recorded.Leg pain VAS,ODI were used to evaluate the pain and lumbar function of the patients.The clinical efficacy was evaluated by Nakai evaluation.Results: All the patients were performed endoscopic decompression of the lateral recess and nerve root by removing the ventral part of the superior facet joint,the ligamentum flavum and the intervertebral disc.The decompression range was from the inferior edge of the upper pedicle to the superior edge of the lower pedicle.The nerve root was detected to have no compression and the pulse of nerve root returned to normal.The patient got ambulant on the operation day and discharged if he had no discomfort symptom.In the study,68 cases got follow up.The mean follow-up time was 12.1 months (6-63 months).The VAS at dif-ferent follow-up time points was improved relative to the baseline,and the difference was statistically significant (F=491.60,P<0.001).The ODI at different follow-up time points was improved relative to the baseline,and the difference was statistically significant (F=189.91,P<0.001).The excellent and good rates of Nakai evaluation were 79.4% (excellent in 42 cases,good in 12 cases,fair in 10 cases and poor in 4 cases).The mean intraoperative blood loss was (49.29±11.86) mL.The mean operation time was (92.46±21.34) min.The mean ambulation time was 1.8 h.The mean discharge time was 2.3 days.Postoperative epidural hematoma was found in 1 case.Foot drop was found in 1 case.Second stage open surgery was performed in 6 cases.Conclusion: We can apply transforaminal endoscopic decompression for the patients of lumbar spinal stenosis who have unilateral nerve root irritation.Patients with transforaminal endoscopic decompression can get less surgical trauma,quick recovery and obtain good short-term outcome.
9.Effect of polylactic acid-polyglycolic acid copolymer/collagen type Ⅰ microspheres combined with BMSCs on bone defects in osteoporotic rats

Objective To evaluate the bone repair capacity ofpolylactic acid-polyglycolic acid copolymer (PLGA)/collagen type Ⅰ (CoI) microspheres combined with BMSCs after being injected in intertrochanteric bone defect of osteoporotic female rats.Methods Prepared PLGA microspheres.The microspheres were coated with Col.BMSCs of the third passage were cultured with PLGA/CoI microspheres.Forty 3-month-old female SD rats were ovariectomized to establish osteoporotic animal models.The osteoporotic rats were randomly divided into 5 groups,including SHAM group,PBS group,Cell group,MS group and Cell+ MS group.There were 8 rats in each group.Different material was injected into the intertrochanteric bone defect site which was made with electric drill.Four rats of each group were sacrificed at 1 month and 3 months post-operation.The fenora were taken to measure the intertrochanteric bone mineral density (BMD) with DEXA and evaluate trabecular stucture with Micro CT.Results After 7 days of coculture,BMSCs seeded on PLGA/CoI microspheres had nice adherance and proliferation.There was no difference of BMC and BMD among all groups at 1 month post-operation.Tb.Th of Cell+MS group was higher than that of PBS group and MS group at 1 month post-operation.%Tb.Ar of Cell+MS group was higher than that of Cell group and MS group at 1 month post-operation.Tb.Sp of Cell+MS group had a tendence to decrease compared with other groups but there was no statistical difference at 1 month post-operation.After 3 months of operation,the BMC of Cell+MS group had a tendence to increase compared with that of PBS group and MS group but showed no statistical difference.BMD and Tb.Th of Cell+MS group was higher than those of other groups.%Tb.Ar of Cell+MS group was higher than that of SHAM group and PBS group.Tb.Sp of Cell+MS group had a tendence to reduce compared with other groups but showed no statistical difference.Conclusion The bone defect of osteoporotic site can be repaired 1 month after the injection of the PLGA/CoI microspheres combined with BMSCs.The trabecular reconstruction and bone quality of osteoporotic site can be improved 3 months after the injection.
10.Clinical significance of nasopharyngeal bacterial colonization in children with acute bronchiolitis

Objectives To explore the effects of nasopharyngeal bacterial colonization in children with acute bronchiolitis due to respiratory syncytial virus (RSV).Methods Hospitalized children of acute bronchiolitis were enrolled to detect pathogen and bacterial colonization.Their clinical data and laboratory results were collected and analyzed.Cases of elective surgery were also obtained as control group.Results Fifty-five percent of all children with bronchiolitis had a lower positive rate of nasopharyngeal bacterial culture (55.0% vs.74.1%,P <0.01).Children with nasopharyngeal bacterial colonization had a higher rate of severe bronchiolitis (13.8% vs.4.2%,P =0.018),presented dyspnea more frequently (19.8% vs.9.5%,P =0.037) and had a longer hospital duration (8.2 vs.7.5 days,P =0.036) as compared with those without bacterial colonization.In terms of laboratory results,a higher proportion of C-reactive protein was found in children with bacterial colonization than those with non-bacterial colonizations (17.2% vs.4.2%,P =0.003).Conclusion Bacterial colonization may be a predictor for severe bronchiolitis.
