1.Therapeutic effect evaluation of phloroglucinol combined with diazepam during active phase of first stage of labor

Objective To observe the therapeutic effect of phloroglucinol combined with diazepam in prolonged or protracted active phase of first stage of labor,and the outcome of perinatal phase.Methods Two hundred vaginal delivery primipara with prolonged or protracted active phase after dilatation of cervix 2-3 cm were selected.The patients were divided into 2 groups by random digits table method with 100 cases each,the patients in observation group were given phloroglucinol 80 mg and diazepam 10 mg,and the patients in control group were given diazepam 10 mg.The cervical dilatation,the degree soft hardness and edema of cervix,time of active phase,time of second stage of labor,mode of delivery,vaginal bleeding in 2 h postpartum and outcome of perinatal phase in the 2 groups were observed.Results In observation group,the cervical dilatation rate was (2.31 ± 0.27) cm/h,the time of active phase was (187.27 ± 33.22) min.In control group,the cervical dilatation rate was (1.82 ±0.48) cm/h,the time of active phase was (251.32 ± 45.33) min,there were statistical differences between 2 groups (P < 0.05).The rate of uterineincision delivery in observation group was significantly lower than that in control group [18.0%(18/100) vs.32.0% (32/100)],there was statistical difference (P < 0.05).There were no statistical differences in time of second stage of labor,vaginal bleeding in 2 h postpartum,rate of neonatal asphyxia between 2 groups (P >0.05).Conclusion The effect of phloroglucinol combined with diazepam in prolonged or protracted active phase is remarkable,it can promote the progress of labor,speed up delivery progress,improve the rate of vaginal delivery and decrease the rate of uterine-incision delivery,and no adverse effects on the outcome of perinatal phase,it is worth popularizing.
2.Analysis of SLC37A4 gene in 3 cases of glycogen storage disease type Ⅰb

Objectives To analyze SLC37A4 gene mutations in glycogen storage disease type Ⅰb patients and to investigate the correlation between genotype and phenotype. Methods The clinical data and SLC37A4 gene detection results of 3 cases of glycogen storage disease type Ⅰb were analyzed retrospectively. Results Two males and one female aged 6 years, 9 years, and 16 years respectively were presented with hepatomegaly, fasting hypoglycemia, slactic academia, hyperlipidemia, and granulocytopenia. The analysis of 6 alleles in SLC37A4 gene by direct sequencing of peripheral blood DNA found 4 mutations, including 2 missense mutation (p. Leu23Arg and p.Pro191Leu), one shear mutation (c.870+5G>A), and one deletion mutation (c.1042_1043 del CT). The genotypes of these 3 cases were p.Pro191Leu, p.Pro191Leu;p. Leu23Arg, c.870+5G>A;p.Pro191Leu, p.Leu347ValfsX53 respectively. Conclusions There were 4 mutations detected among these 3 cases of glycogen storage disease type Ⅰb. All of those were known mutations. The most common mutation was p.Pro191Leu. It can not be excluded that P.Gly149Glu homozygous mutation is associated with repeated infections.

4.Protective effect of sCR1-SCR15-18 on cerebral ischemia/reperfusion injury in rat via inhibition of complement

AIM: To explore the effect of complement on the cerebral ischemia/reperfusion injury in rat and the protection by sCR1-SCR15-18. METHODS: 75 male SD rats were randomly divided into three groups: sham operation group (SO, n=15), middle cerebral artery occlusion and reperfusion (MCAO) without treatment group (I/R, n=30); MCAO treated with sCR1-SCR15-18 group (sCR1-SCR15-18, n=30). After the MCAO for 2 h, then reperfusion for 24 h, the scores of neural behavioral functional deficits were determined. Infarction area was measured by TTC staining. Activity of MPO in cerebral cortex was detected. C3b deposition and pathological change were observed by immunohistochemial staining and HE staining, respectively. RESULTS: After reperfusion for 24 h, the neurological deficits score, infarction area and activity of MPO in sCR1-SCR15-18 group were decreased compared to I/R group. In sCR1-SCR15-18 group, C3b deposition in ischemic area was decreased and pathological injury was improved compared to I/R group. CONCLUSION: Complement plays a role in cerebral ischemia-reperfusion injury and sCR1-SCR15-18 exerts a protective effect by inhibiting the excessive activation of complement.
5.Effects of electric acupuncture on blood-flow volume in local tissues of rabbits with hormonal necrosis of femoral head

BACKGROUND: Necrosis of femoral head induced by hormone is the iatrogenic disorder. It is doubt whether such problem is improved or not by increasing blood-flow volume in the local necrosis tissues.OBJECTIVE: To observe the effects of electric acupuncture on blood-flow volume in local tissues of femoral head so as to understand the mechanism of acupuncture on hormonal necrosis of femoral head.DESIGN: Randomized controlled study in which the experimental animals were taken as the objects.SETTING: Department of integrated traditional chinese and western medicine in a college of traditional chinese medicine.MATERIALS: The experiment was performed from May 2002 to June 2003in Animal Experimental Room of Clinical Hospital of Integrated Traditional Chinese and Western Medicine affiliated to Anhui College of Traditional Chinese Medicine. Thirty female and male(half-half) New Zealand white rabbits were employed, in clean grade, which were randomized into blank control, model group and treatment of electric acupuncture group.METHODS: The model of hormonal necrosis of femoral head in rabbits was prepared. In the treatment group, PCE-88A type controlled electric acupuncture apparatus developed by Institute of Acupuncture and Meridians and Collaterials in Anhui College of Traditional Chinese Medicine was applied on Huantiao(GB 30) and Biguan(ST 31) . No any treatment was applied in the model and blank groups.MAIN OUTCOME MEAURES: The curve of blood-flow volume in local necrosis tissues at the moment of acupuncture and 2 weeks after treatment.RESULTS: The blood-flow volume in local necrosis tissues at the moment of acupuncture in the treatment group(0.50 ±0.08) mL/g per minute was reduced significantly compared with that of blank control group[(8.63± 0. 83) mL/g per minute] and the model group[ (4.30 ±0. 89) mL/g per minute], indicating significant difference ( P < 0.01). The blood-flow volume in local necrosis tissues 2 weeks after treatment[ (6. 19 ± 0.61) mL/g per minute] was remarkably increased compared with the blank control [ (8.33 ± 0.65) mL/g per minute] and the model group[ (4.44 ± 0. 72) mL/g per minute], indicating significant difference( P < 0.01).CONCLUSION: Electric acupuncture can improve local blood supply in hormonal necrosis of femoral head immediately, which is probably related to the regulation in the net of nerve-endocrine-immunity.
6.Clinical value of examination of p16 protein in lung lesions through fibrobronchoscopic brushing and biopsy

Purpose:To study the clinical value of preoperative examination of P16 protein , immunohistochemical method was used to examinate the expression of the protein in tissues obtained through the fibrobronchoscopic brushing and biopsy.Methods:All of the patients who found lesions of lung by radiology were gotten the fibrobronchoscopy to get the spicements. All of the specimens were examinted for expression of P16 protein. The expressions of P16 protein in lung carcinoma group , benign lung disease group and blind biopsy group were compared.Results:The positive expression rates of p16 protein of lung carcinoma group , lung benign disease group and blind biopsy groups are : 55.0%, 95.0% and 62.5%. Conclusions:Difference was found between malignant and benign lung diseases.Examination of p16 protein expression for blind biopsy groups has some values in diagnosis of lung carcinoma.
7.Construction, expression, putification and bioactivity analysis of a two functional domains containing small molecule CR1 derivative

Objective To construct and express a small complement receptor type 1 (CR1) deriv-ative which contained two functional domains. Methods Total RNA was isolated from the peripheral blood mononuclear cell. The functional fragment Ⅰ of CR1 was amplified using the RT-PCR. The functional frag-ment Ⅱ was amplified with the plasmid of pET-32a-CR1-SCR15-18 as template which had been already con-structed in our laboratory. Then the chimeric gene that contained the two fragments was constructed with spli-cing overlap extention PCR. The chimeric gene was then inserted into the plasmid of pET-32a (+) and transformed into E. coli Rosetta(DE3). The inserted gene was verified by enzyme digestion and DNA se-quencing. After induced by IPTG, the expressed protein was analyzed by SDS-PAGE and Western blot. Then the fusion protein was purified by Ni-NTA affinity column and renatured through dialysis. The comple-ment inhibition activity was determined by CH50 method. Results The chimeric gene was successfully cloned into pET-32a(+). The result of SDS-PAGE and Western blot confirmed the expressed protein and showed that the molecule mass(Mr) of the expressed protein was 63×103. The purity of the recombinant protein was up to 92% after Ni-NTA column affinity chromatography. The bioactivity assay showed the fusion protein had a concentration-dependent complement inhibition activity within the concentration range of 0-200 μg/ml. Conclusion The two functional domains contained small CR1 derivative was successfully construc-ted and expressed in E. coli Rosetta. The fusion protein had a relative high bioactivity, providing a basis for further function experiment in vivo.
8.Preventive effect of cefradine plus heparin sealing catheters on central venous catheter-related infections

Objective To explore the preventive effect of Cefradine plus heparin sealing catheters on central center venous (CVC)catheter-related infections.Methods One hundred and twelve patients with central venous catheter were randomly divided into control group(n=57)and intervention group(n=55).In the control group,heparin sodium was used to seal the catheters and in the intervention group Cefradine plus heparin were used.The two groups were compared in terms of central venous catheter(CVC)-related infections.Results The CVC-related infection rates are 12.28%and 0%in the control and intervention groups respectively, with statistical difference between them(P<0.01).Conclusions Cefazodine and heparin used to seal the catheters in the treatment of patients with central venous catheter can effectively reduce incidence of catheter-related infections.
9.Neuroprotective effect of astrocyte protein phosphatase 2 A up-regulation on APP/PS1 double transgenic mice

AIM:To investigate the protective effects of astrocyte protein phosphatase 2A (PP2A) up-regula-tion on APP/PS1 double transgenic mice.METHODS:An eGFP-wtPP2A lentivirus with glial fiber acidic protein promoter was constructed to specifically increase PP2A expression in the astrocytes.The mice were divided into wild -type mice +vector virus group (Con), APP/PS1 transgenic mice +vector virus group (APP/PS1) and APP/PS1 transgenic mice +eGFP-wtPP2A lentivirus group (PP2A) by lateral ventricular injection of the lentivirus.Four weeks after injection of the vi-rus, the immunofluorescence of brain slices were used to detect the level of β-amyloid protein ( Aβ) .Golgi staining was used to detect the changes of dendritic spine density and morphology.Electron microscopy was applied to detect the thickness of postsynaptic density (PSD).The Morris water maze test was applied to examine the learning and memory abilities of the mice.RESULTS: Up-regulation of PP2A in the astrocytes attenuated Aβlevel increasing in APP/PS1 group.Up-regulation of PP2A in the astrocytes significantly attenuated both decreases in the dendritic spine density and the percentage of mushroom-like dendritic spines in the hippocampal CA3 region of APP/PS1 mice.Up-regulation of PP2A in the astrocytes significantly attenuated the reduced thickness of PSD in APP/PS1 group.Up-regulation of PP2A in the astro-cytes attenuated the escape latency extending in APP/PS1 group .CONCLUSION: Up-regulation of PP2A in the astro-cytes reduces AD-like pathological changes, and attenuates synaptic impairment, synaptic plasticity deficits and cognitive impairment in the APP/PS1 double transgenic mice.
10.Case report of hepatic adenoma with hemorrhage in glycogen storage disease type Ⅰa

Objective To analyze and summarize the clinical characteristics of patients with spontaneous hemor -rhage of hepatic adenoma in glycogen storage disease type Ⅰa.Methods Reporting 1 case in our hospital and making a summary about general situation , category, etiology, diagnosis and treatment of the hemorrhage of hepatic adenoma with glycogen storage disease type Ⅰa through checking literatures .Results The patient was a 27 year old male who had been diagnosed as glycogen storage disease for 14 years, as well as was first found hepatic adeno-ma at the age of 17 .He once was diagnosed as intra-adenoma bleeding with persistent abdominal pain and dizziness and was underwent selective hepatic artery embolization at the age of 22.Hepatic adenoma in glycogen storage dis-ease typeⅠa generally appeared at the age of puberty .One common complication of this disease was hemorrhage of hepatic adenoma , which can be found by ultrasonography and CT .Clinical management includs observation , selec-tive hepatic artery embolization , radiofrequency ablation , surgical resection and liver transplantation .Conclusions Glycogen storage disease type Ⅰa is an autosomal recessive genetic disease with hepatic adenoma as a common complication of GSD Ⅰa, serious liver adenoma's hemorrhage can be life threatening , the radiological examination can be helpful to detect hepatic adenoma .Then appropriate intervention can improve the life quality and prognosis .
