1.CT manifestations of AIDS patients with opportunistic infections of the lungs

Lung opportunistic infections are most common pulmonary complications in patients with advanced AIDS .A variety of pathogens can cause lung infections .Lung CT findings show the complexity and diversity for these complications .Analysis of CT fea-tures of pulmonary infection caused by different pathogens has an important clinical significance on strengthening the awareness of AIDS complicated with pulmonary opportunistic infection and improving the clinical treatment effect .
2.Correlation Between CT Manifestation Types and CD4+T Lymphocytes in AIDS Combined with Pneumocystis Pneumonia

Purpose To investigate the CT features of different pneumocystis pneumonia (PCP) types in AIDS as well as the relationship between different PCP types and CD4+ T lymphocyte count,in order to deepen the understanding and improve the diagnosis and treatment of PCP.Materials and Methods The CT examination data of 102 AIDS patients with PCP were retrospectively analyzed,the CT findings were divided into 6 types,and CD4+ T lymphocyte count of the 6 PCP types was statistically analyzed.Results ① The CT findings of PCP were divided into the following 6 types:transparent ground-glass type (42 cases,41.2%),muddy ground-glass type (17 cases,16.7%),solid variant (11 cases,10.8%),cystic variant (6 cases,5.9%),interstitial type (19 cases,18.6%),and mixed type (7 cases,6.9%).② The CD4+ T lymphocyte count of different PCP types was analyzed,and the difference was statistically significant (F=2.672,P<0.05).Further pairwise comparison showed that the CD4+T lymphocyte count of transparent ground-glass type was higher than that of solid variant and cystic variant (P<0.05),and there was no significant difference between the other groups (P>0.05).Conclusion CT findings of AIDS with PCP have certain characteristics,and it is helpful to deepen the understanding of PCP through imaging classification.There is a certain correlation between different PCP types and CD4+T lymphocyte count,and it is helpful to improve the diagnosis and treatment of PCP by analyzing the correlation.
3.Study on Countermeasures to Managing Shuangjiantiao(Double Duties) Cadres in TCM Colleges

Objective To analyse the problems and reasons in the management of Shuangjiantiao cadres in TCM col eges, put forward scientific management countermeasures and suggestion. [Method] Through documentary study, individual interview and questionnaire, understand the current situation and problems of Shuangjiantiao cadres in TCM col eges. [Result] The problems in management of the said cadres are: not very reasonable post distribution, not very scientific post evaluation, not very fair post treatment and not enough post efficacies. The analysis shows the main reasons are: single subject and specialty, smal schooling scale, not enough recognition to thought and lagging team construction. [Conclusion] To improve the scientific level to the said cadres management, we shal strengthen plan and design, optimize configuration mode, insist on person fitting to post, improve competence, perfect stimulating system, improve post efficacy, pay attention to education and training and improve team qualification.
4.The effects of arsenic trioxide on the expression of autoantibody and interleukin-10, interleukin-12 in MRL/lpr mice

Objective To investigate the effects of arsenic trioxide (ATO) on the expression of autoan-tibody and interleukin (IL)-10 IL-12 in MRL/lpr mice. Methods MRL/lpr mice wereseparated into 3 different groups. The 3 groups received arsenic trioxide (ATO, 0.4 mg·kg~(-1)·d~(-1)), cyclophosphamide (CTX,50 mg/kg) and sodium chloride (NS, volume weight-determined) abdominal injection respee-tively. The treatment stopped 2 months later. Afterwards, the rates of CD3~+(T) cells, CD3~+CD4~+(Th) cells and the CD3~+CD4~+cells which produced IL-10 and IL-12 were detected using single-cell measurement of intr-acellular eytokines by flow cytometry after polyclonal stimulation with PMA and ionomycin for 4 hours in 5% CO_(2.)Serum levels of IL 10 and IL-12 were assessed using the Mouse cytokines ELISA Kit. One-way ANOVA LSD test and paires t test were used for statistical analysis.Results ①The level of anti-dsDNA antibody after treatment was 0.92±0.06, while it was 1.14±0.58 before treatment. So the ds-DNA antibody level was significantly decreased in ATO group (P<0.01), while it was dramatically increased in the NS groups (P<0.05) after the treatment;②ATO group had significantly less CD3~+ cells and CD3~+CD4~+ cells[(44±4)% and (20±4)%]compared withNS group [(59±5)%and(30±3)%](P<0.01).③The serum level of IL-12 in the ATO group was (84±12) pg/ml,while it was (103±13)pg/ml in the NS group (P=0.018).④The intracellular levels of IL-10 and IL-12 produced by CD3~+CD4~+ (Th) cells in the ATO group were ( 1.5±0.4)% and (2.43±0.42)%, which was significantly lower than those in the NS group respectively (2.5±0.5)% and (3.24±0.40)%(P<0.01). Conclusion Arsenic trioxide can reduce the production of anti-dsDNA antibody,inhibit the activation and proliferation of both T cells and Th subsets in the MRLApr mice, and hence decrease the serum levels of IL-12 and the levels of IL-10, IL-12 produced by Th cells.
5.Determination of the Main Component of Captopril Tablets by Derivative Spectrophotometry

OBJECTIVE:To establish derivative spectrophotometry for the determination of the main component of Captopril tablets.METHODS:Under the alkaline condition of sodium hydroxide,2,4-dinitrofluorobenzene was used as derivatization reagent to react with captopril at 40 ℃ in the water bath.Then UV spectrophotometry was adopted to detect the content of captopril with detection wavelength set at 342 nm.RESULTS:The linear range of captopril was 1.0~14.0 ?g?mL-1(r=0.999 9) with an average recovery of 99.61%(RSD=1.27%,n=9).The RSD of intra-day and inter-day both were less than 2%.CONCLUSION:This method is stable,reliable,rapid and simple for the determination of the main components of Captopril tablets.
6.Co-expression of guanylyl cyclase-C and caudal-type homeobox transcription factor 2 in human gastric cancer and precursor lesions

Objective To investigate the expressions of guanylyl cyclase-c(GC-C) and caudal-type homeobox transcription factor 2 (CDX2) in human gastric tissues and precursor lesions and its significance. Methods The cancerous and paracancerous (5 cm from cancer lesion )samples from 30 cases of gastric cancer and 32 samples including 23 intestinal metaplasia and 9 dysplasia were collected. The mRNA expressions of GC-C and CDX-2 were detected by reverse transcriptase-polymerase chain reaction (RT-PCR) and proteins of GC-C and CDX-2 were measured by using Western blot and immunofluorescence methods. Results The mRNA expressions of GC-C and CDX-2 were absent in paracancerous tissues, but were 66.7% and 63.3% in cancerous tissues, respectively(P=0. 000). The Western blot indicated that expressions of GC-C and CDX-2 were 19/30 and 17/30 in cancerous tissues, but absent in paracancerous tissues(P=0. 000). The immunofluorescence examination revealed that GC-C and CDX-2 expressions were 39.1% and 39.1% in intestinal metaplasia, 55.6% and 55.6% in dysplasia, and 56.7% and 60.0% in cancerous tissues, respectively, but absent in paracancerous tissues. Moreover, expressions of GC-C and CDX-2 showed a statistical difference between intestinal-type and diffuse-type of gastric cancer (P< 0.05) ,but had no correlation with age, sex, size of the lesion, clinical stage and lymphnode metastasis. The positive correlation was found in expressions of GC-C and CDX2 between intestinal metaplasia and cancerous tissues(r=0. 4524 and 0. 3845, P= 0. 037 and 0. 0408, respectively). Conclusions The over expressions of GC-C and CDX2 in human gastric cancer is associated with precursor lesions and may play an important role in the carcinogenesis of gastric cancer. The examination of GC-C and CDX2 expressions will be helpful in diagnosing gastric cancer and precursor lesions.
7.Hepatolithiasis concurrent with intrahepatic cholangiocarcinoma

Objective To investigate the clinical features and management of hepatolithiasis associated with intrahepatic cholangiocarcinoma. Methods Data of 84 patients of hepatolithiasis associated with intrahepatic cholangiocarcinoma in our hospital from 1990 to 2009 were retrospectively analyzed.Results The incidence of intrahepatic cholangiocarcinoma in patients of hepatolithiasis was 4. 6%(84/1840), among them only 47 patients got a definite diagnosis before operation. All cancer located in the bile duct containing cholelith. In 20 patients intrahepatic cholangiocarcinoma was identified 6 - 16 years after lithotomy. The clinical manifestation of hepatolithiasis associated intrahepatic cholangiocarcinoma included:refractory hepatic abscess, incurable infection of intrahepatic biliary tract, and progressive obstructive jaundice. Only 35 patients received radical excision, 26 patients received palliative excision, 4 patients received radiofrequency ablation therapy, 19 patients received biopsy only. Conclusions There has been a considerable high coincidence between intrahepatic cholangiocarcinoma and hepatolithiasis. Resection of the lobe containing intrahepatic stones may help to prevent the development of intrahepatic cholangiocarcinoma.
8.Selective decongestive devascularization shunt of gastrosplenic region for treatment of portal hypertension

Objective To evaluate the curative effect of selective decongestive devascularization shunt of gastrosplenic region(SDDS-GSR) for the treatment of portal hypertension. Methods From September 2000 to June 2008, 44 patients with portal hypertension had received SDDS-GSR in our hospital. Twenty-nine of them had been followed up for 12-85 months (mean=44months). Results Operative mortality was 0 %. Mesenteric area pressure(33.82±5.12 cm H_2O) was higher than splenic area pressure(24.57±4.63 cm H_2O)soon after the operation finished(P<0.01). No re-bleeding ca-ses were found, and the encephalopathy occurred in 2.27% of the patients in the early stage of post-operation. However, the rates of 3.45% for re-bleeding and 3.45% for encephalopathy were noticed in long-term follow-up. The 1-, 3- and 5-year survival were 100%, 95% and 95%, respectively. Dur-ing the long-term follow-up, the platelet counts markedly increased from (49.2±21.8 × 10~9/L) of preoperative value to (77.2±29.5×10~9/L) (P<0.01), while spleen size was significantly reduced.Conclusion SDDS-GSR is a reliable and reasonable surgical procedure for the management of portal hypertension.
9.PI_3 K/Akt signal effects Ephrin-A1 mediated malignancy of HCC cells

Objective To investigate the role of PI_3 K/Akt signal pathway in Ephrin-Al gene mediated invasion,metastasis of Huh-7 cells.Methods Western blot was used to test the protein expression of phosphatidylinositol 3-kinase(PI_3 K)and mitogen-activated protein kinase(MAPK)after Huh-7 cells were treated with Ephrin-A1/Fc fusion protein.According to the protein expression,LY294002 was used to block PI_3 K/Akt pathway specifically,then p-Akt protein expression,mobility and invasive ability of Huh-7 cells were examined.Results In Huh-7 cells actived by Ephrin-Al/Fc fusion protein,p-Akt expression was higher than that in control group(t=4.564,P<0.05),but there was no difference of p-p38MAPK expression between Ephrin-Al/Fc fusion protein group and IgG/Fc fusion protein group(P>0.05).PI_3 K/Akt pathway was specifically blocked by LY294002,the p-Akt protein expression decreased in Huh-7 cells,and the mobility and invasive ability mediated by Ephrin-Al in Huh-7 cells decreased(P<0.05).Conclusions PI_3 K/Akt pathway effects an important role in mobility and invasive ability of Huh-7 cells mediated by Ephrin-A1.
10.Effects of selective shunt plus devascularization on splanchnic hemodynamics

Objective To evaluate the changes of splanchnic hemodynamics after selective decongestive devascularization shunt of gustrosplenic region (SDDS-GSR) in the treatment of patients with portal hypertension. Methods All these 41 portal hypertensive patients underwent a combination surgery including partially occlusion of the splenic artery, Warren distal splenorenal shunt and devascularization. Postoperative patients were followed-up by ultrasonography for changes of splanchnic hemodynamics. Results were compared with that of 21 healthy volunteers. Results The thickness of spleen 2 weeks and 1 year after surgery (47±8) mm, (46±8) nun decreased from preoperative (60±9) mm (P<0.01). The diameter of portal vein (1.13±0.19) cm and splenic artery (0.49±0.08) cm 2 weeks after surgery decreased (P<0.05) and that of hepatic artery (0.40±0.07) cm increased (P<0.05). Patients' preoperative portal vein blood flow volume (1716±1262) ml/min and splenic artery (1269±570) ml/min were larger than that of normal group (P<0.05), while that of hepatic artery (321±126) ml/min was significantly less than that of normal group (P<0.05). The portal blood flow (649±294) ml/min and that of splenic artery (446±254) ml/min 2 weeks after surgery decreased significantly (P<0.01). The hepatic artery blood flow (612±295) ml/min increased significantly (P<0.01). When reevaluated at 1 year the hepatic artery blood flow (401±152) ml/min was not significantly different compared with that before surgery and that in normal group (P>0.05). Conclusions There are significant alterations in hepatic and splenic hemodynamics in patients with portal hypertension, and that SDDS-GSR can partially reverse the chaos of the hepatic and splenic hemodynamics in cirrhotic portal hypertensive patients.
