1.Two Hours Effects of Cyclosporin A on Lymphocyte Subsets in the Body

Objective] By detecting the peripheral blood lymphocyte subsets of AML patients before and after taking cyclosporin A, preliminary discussion on making two hours effects of cyclosporin A on lymphocyte subsets in the body. [Methods] Fasting and taking cyclosporin A two hours of the expression of peripheral blood T, B lymphocytes and NK cel were detected by flow cytometry in 43 cases of patients with AML after transplantation. At the same time, application of chemiluminescence detection concentration of cyclosporin A. [Results] The results of cyclosporin A concentration: Fasting group is 125.43 ±65.94 ng·ml-1. Taking two hours group is 564.46 ±258.67 ng·ml-1; Taking two hours group compared with the fasting group, CD3+ cel is significantly reduced and NK cel is significantly increased, but CD4+, CD8+, CD19+cel and CD4+/CD8+ have no obvious change. [Conclusions] Taking cyclosporin A 2 hours can achieve distribution and affect the number of CD3+ cel in the body, but can not affect the B cel . To AML patients after transplantation, in order to achieve the appropriate concentration and keep abreast of the body's immune status of patients, it is necessary to monitor the concentration of cyclosporin A and lymphocyte subsets during their taking cyclosporin A, which helps us determine the prognosis.

3.Application effect of bandage corneal contact lens in ptosis during surgical treatment

Objective To study the application effect of bandage corneal contact lens in ptosis during surgical treatment.Methods A total of 62 patients(73 eyes) with primary congenital ptosis in the First People's Hospital of Xiantao from March 2013 to March 2016 were randomly divided into group A,group B and group C.All of the three groups were treated by shortening levator palpebrae superioris muscle.The A group[20 cases(24 eyes)] began to wear bandage corneal contact lens during the operation.The B group [22 cases (25 eyes)] began to wear bandage corneal contact lens at the end of surgery.And the C group [20 cases (24 eyes)] took conventional methods for covering operative eyes.Postoperative follow-up lasted for 3 months,the postoperative FL staining scores and ocular pain severity were compared among the three groups.Results Comparison of postoperative FL staining scores:there was no statistically significant difference between B group and C group at postoperative 1 day(q =0.537,P > 0.05),but the differences between B group and A group were statistically significant (q =10.876,11.298,all P < 0.05),the differences among the three groups were statistically significant at the 7th day after surgery (q =8.682,4.048,4.722,all P < 0.05).There was no statistically significant difference between B group and A group at postoperative 2 weeks (q =1.635,P > 0.05),but the differences between group A/B and group C were statistically significant (q =8.747,7.201,all P < 0.05).There were no statistically significant differences among the three groups at postoperative 3 months (q =1.607,0.186,1.438,all P > 0.05).Comparison of postoperative ocular pain severity:there were no statistically significant differences among the three groups at postoperative 1 day (q =0.986,0.586,0.410,all P > 0.05).At postoperative 7 days,there was no statistically significant difference between A group and B group (q =0.948,P > 0.05),but the differences between A/B group and C group were statistically significant (q =4.743,3.843,all P < 0.05).There were no statistically significant differences among the three groups at postoperative 1 month (q =3.181,0.599,2.654,all P > 0.05).Postoperative upper eyelid radian and double eyelid plasty in all of 62 cases(73 eyes) was satisfied and the width of palpebral fissure when look straight was normal and locating above superior margin of pupil,but there were 16 eyes with slightly hypophasis which were all naturally closed good after 1 to 2 months without surgical incision infection and corneal infection.Conclusion It is effective to protect the cornea since intraoperative application of bandage corneal contact lens,and it with high safety to wear corneal contact lens shortly after surgery,which can promote corneal epithelial wound healing,reducing incidence of exposure keratitis,at the same time,it can obviously relieve ocular pain in patients.

5.Exploration and primary suture of common bile duct in laparoscopic procedure (a report of 35 cases)

Objective To determine the methods and effects of common bile duct(CBD) exploration and primary suture by laparoscopic procedure after laparoscopic cholecystectomy (LC) . Methods After LC, 35 patients' CBD were explored.After hte CBD stone was removed by choledochofiberscopy completely and CBD stenosis was excluded, the CBD was sutured primarily. Results 32 cases of CBD gallstone were removed ,and 3 exploration were negative.All the 35 CBD were sutured primarily . Two patients occurred bile leakage 1~3 day after the operation ,which were cured by conservative therapy.29 patients were followed-up for 3~24 months,no residual stone or bile duct stricture was found. Conclusions Primary suture CBD is safe and effective in laparoscopic CBD exploration,if the patient selected correctly.
6.Comparison of the effect of small incision cataract surgery whether combined with nucleus chopping or not in the treatment of cataract

Objective To compare efficacy of small incision cataract surgery whether combined with nucleus chopping or not in the treatment of cataract. Methods From March 2014 to September 2015,125 cases of age-related cataract(146 eyes) in the the First People′s Hospital of Xiantao were divided into 60 cases(75 eyes) of observation group and 65 cases(71 eyes) of control group by method of random sampling. The observation group accepted small incision cataract surgery combined with chopping nucleus. The control group only underwent small incision cataract surgery. The postoperative visual acuity,corneal astigmatism and operation time,and complications and so on in the two groups were compared. Results There was statistically significant difference in postoperative visual acuity after 1 week between the two groups(Z = -2. 078,P <0. 05),but there was no statistically significant difference in postoperative visual acuity after 1 month or 3 months between the two groups(Z= -0. 960,-0. 743,all P>0. 05). The postoperative corneal astigmatism after 1 week or 1 month between the observation group and the control group had statistically significant differences[(1. 33 ± 0. 45) D vs. (1. 52 ± 0. 49) D,(1. 03 ± 0. 42) D vs. (1. 18 ± 0. 44)D,t=2. 442,2. 108,all P<0. 05)],but there was no statistically significant difference in postoperative corneal astigmatism after 3 months between the two groups(t =0. 432,P >0. 05). There was no statistically significant difference in the operation time between the observation group and the control group[(11. 28 ± 2. 32) min vs. (11. 87 ± 2. 52)min,t=1. 473,P>0. 05]. One case of posterior capsular rupture occurred in the two groups,respec-tively. But serious complications such as lens nucleus escaped into vitreous cavity, explosive haemorrhage from the choroid or corneal endothelial decompensated had not been found. Conclusion The small incision cataract surgery combined with nucleus chopping has advantages in acquiring fast visual rehabilitation acuity in the early stage after operation,lower corneal astigmatism compared to that without nucleus chopping. Therefore,the small incision cataract surgery combined with nucleus chopping is worthy of clinical popularization and application.
7.Allogeneic hematopoietic stem cell transplantation using Flu/CTX conditioning regimen for severe aplastic anemia

Objective To evaluate the efficacy of Flu/CTX conditioning regimen for the treatment of severe aplastic anemia in pa- tients receiving allogeneic hematopoietic stem cell transplantation. Methods Nine patients with severe aplastic anemia received HLA identi- cal peripheral blood hematopoietic stem cell transplantation (PBSCT) using Flu/CTX conditioning regimen, which consisted of fludarbine [30 mg/(m2 d) for5 days (-7 to -3) ], CTX [50mg/(kg d) for4 days(-5 to-2)]. All patients received cyclosporin A (CsA) and mycophenolet mofetil (MMF) for prophylaxis of graft-versus-host disease(GVHD). Results The Fiu/CTX regimen was very well toler- ated, with no severe regimen related toxicity. In all patients, the median days of neutrephil exceeding 0. 5×109/L and platelet exceeding 20 ×109/L were 12 days (range 10-16 days) and 16 days (range 14-19 days), respectively. Complete chimerism was achieved in all pa- tients at one month after PBSCT. Two patients had acute GVHD and one had chronic GVHD. In the 39-month median follow-up duration, all patients were alive in disease-free situation. Conclusion The Flu/CTX conditioning regimen may reduce transplantation-related toxicities and can achieve full chimerism and high long-term disease-free survival. Allogeneic hematopoietic stem cell transplantation using intravenous Fiu/CTX conditioning regimen is a safe and effective treatment method for the patients with severe aplastic anemia.
8.Allogeneic hematopoietic stem cell transplantation using Flu/ivBu/TT conditioning regimen for refractory or relapsed acute non-lymphocytic leukemia

Objective To evaluate the efficacy and feasibility of Flu/ivBu/Tl" conditioning regimen for the treatment of refractory or relapsed acute non-lymphocytic leukemia in patients receiving allogeneic hematopoietie stem cell transplantation. Methods Seven patients with refractory or relapsed acute non-lymphocytic leukemia received HLA identical peripheral blood hematopoietie stem cell transplantation (PBSCT) following Flu/ivBu/TY conditioning regimen, which consisted of fludarbine, busulfex and thiotepa. All patients received cyclos-porin A (CsA) and mycophenolet mofetil (MMF) for prophylaxis of graft - versus - host disease (GVHD). Results The Flu/IVBu/TT regimen was tolerated very well, without severe regimen related toxicity. In the 31-month median follow-up duration, 5 of 7 patients were a-live in disease-free situation. Conclusion The Flu/ivBu/TT conditioning regimen reduced transplantation-related toxicities and offered high long-term disease-free survival, and was tolerated very well. Allogeneie hematopoietie stem cell transplantation using Flu/ivBu/TT condition-ing regimen is a safe and effective option for the patients with refractory/relapsed acute non-lymphocytic leukemia.
9.Clinicopathological significance of expression of HIF-1α and P-gp in pancreatic carcinoma

Objective To investigate the expression of HIF-1α and P-gp protein in pancreatic carcinoma and determine their clinicopathological significance and the correlation between the expression of HIF-1α, P-gp and the clinical prognosis. Method In samples from 74 cases of pancreatic carcinoma and 10 healthy individuals, the expression of HIF-1α and P-gp were detected by immunohistochemical method. Results The positive expression rate of HIF-1α and P-gp was 75.7% and 86.5%,respectively, which were remarkably higher than that in normal pancreatic tissue (P<0.05). There was a positive correlation between the expression of HIF-1α and that of P-gp. The aberrant expression of HIF-1α and P-gp was associated with lymph node metastasis but not the location, size, clinical stages and nerve invasion of the tumor. Patients with high intensity of HIF-1α and P-gp expression showed a significantly lower median survival time than those with low intensity expression.Conclusions The expression of HIF-1α and P-gp is up-regulated in pancreatic carcinoma and there is a positive correlation between them. The expression of HIF-1α and P-gp might be related to the lymph node metastasis and poor prognosis.
10.Spleno-left adrenal vein shunt for portal cavernous transformation

Objective To evaluate the effect of spleno-left adrenal vein shunt for the treatment of portal hypertensive upper GI bleeding caused by portal vein cavernous transformation in children.Methods Spleno-left adrenal vein shunt was performed in 8 children with portal hypertension due to cavernous transformation.The clinical data was reviewed.Results Portal vein pressure decreased significantly from (30±11)mm Hg to(22±7) mm Hg after shunt.There was no mortality perioperatively and during the follow-up.There were no recurrent hemorrhage nor hepatic encephalopathy occurring in the follow-up and all the children have normal intelligence and normal liver function though blood ammonia level increased significantly from(18±7)μmol/L to (60±17)μmol/L in 4 cases.In 7 cases in which preoperative whole blood cell count significantly decreased,the postoperative WBC,RBC,Hb and PLT was (7.64 ±4.46)×10~9/L,(4.54±0.97)×10~(12)/L,(133±5) g/L and (355.40±107.36)×10~9/L respectively (all P <0.05).In one case suffering from preoperative low PLT count the postop PLT reached 333×10~9/L,which was significantly higher than that preoperatively.Esophageal varices ameliorated in 6 cases.No stenosis of anastomotic stoma and thrombosis developed.Conclusion Spleno-left adrenal vein shunt is an effective procedure to treat portal vein cavernous transformation induced portal hypertension in children.
